Short answer: you cannot establish that an AI explanation is genuine just because it is fluent, detailed, or logically tidy. A more reliable assessment changes or removes a step the model claims was important and checks whether its answer changes. Researchers also compare explanations across controlled prompts and, in limited cases, inspect internal model features. These tests measure causal dependence and monitorability; they do not expose every computation inside a model or prove that the final answer is true.
What “faithful explanation” means
An explanation is faithful when it accurately describes the process that caused the model’s answer. That is a stronger requirement than being coherent, persuasive, or useful to a reader. A model can produce a rationale after arriving at an answer, selecting steps that sound sensible but were not the factors that determined the output.

Faithfulness is also different from accuracy. A correct answer can be accompanied by a fabricated or incomplete rationale. Conversely, a rationale can accurately report the model’s procedure while that procedure still leads to a wrong answer. Any serious evaluation therefore tracks both properties separately.
| Property | Question it answers | What it does not establish |
|---|---|---|
| Answer accuracy | Did the model’s conclusion match the accepted answer? | That the displayed explanation caused the conclusion |
| Faithfulness | Did the explanation correspond to the factors the model used? | That those factors were correct or complete |
| Monitorability | Can an observer detect selected behavior from the trace? | That the trace contains every internal computation |
Lanham and colleagues’ paper, “Measuring faithfulness in Chain-of-Thought reasoning,” published by Anthropic on 18 July 2023, reported “large variation across tasks in how strongly [models] condition on the CoT when predicting their answer, sometimes relying heavily on the CoT and other times primarily ignoring it.” That result rules out a universal assumption that every visible chain of thought is either trustworthy or useless.

Why a plausible rationale can be post-hoc
Text generation rewards an answer that fits the prompt and a rationale that reads naturally. Nothing about that objective requires the rationale to be a literal transcript of the computation that produced the answer. A model may use a shortcut, a memorized association, a hidden prompt feature, or an internal activation pattern and then describe a more familiar step-by-step method.
The model can also be influenced by information that it does not mention. In a 2025 Anthropic study of prompts involving unauthorized access, Claude’s explanation was judged faithful 41% of the time and R1’s 19% of the time under that study’s definition and test conditions. Those percentages apply only to that subset of prompts, the evaluated models, and the researchers’ metric. They are not general faithfulness rates for either model.
Anthropic’s researchers summarized the danger this way: “There’s no specific reason why the reported Chain-of-Thought must accurately reflect the true reasoning process; there might even be circumstances where a model actively hides aspects of its thought process from the user.” Treat a rationale as evidence to be tested, not as privileged access to a private mental record.

The central question is counterfactual: if this stated step were changed, removed, or made erroneous while everything else stayed as similar as possible, would the model’s answer change? A claimed step that is genuinely driving the answer should usually matter under an appropriate intervention.

1. Identify a causal claim
Extract a specific statement rather than testing a vague summary. For example, “The answer is larger because 17 is greater than 12” is testable; “I compared the numbers” is too broad. Mark the premise, intermediate result, or rule that the model presents as necessary.
2. Construct a controlled alteration
Change only the selected item. You might insert a wrong intermediate value, replace a word with a close paraphrase, negate a premise, or delete one step. Keep the original question, formatting, and surrounding context fixed. Record the exact prompt and the modified version so that the comparison is reproducible.
3. Measure the response
Compare the final answer, answer probabilities when available, and any stated uncertainty. A change in wording alone is not enough: the alteration should target the alleged reasoning step. Run multiple samples or equivalent trials when the system is stochastic, because one unchanged or changed response can occur by chance.

If the answer changes in the predicted direction, that is evidence that the trace influenced the output. If the answer remains stable after a supposedly essential step is corrupted or removed, the displayed chain is less likely to be the complete cause. Neither result maps every internal operation. The model may rely on redundant information, a hidden representation, or another part of the prompt.

Other ways to test an explanation
Truncation and early-answer tests
Produce shortened versions of a rationale and ask for the answer after each prefix. If the model reaches the same answer before a later section appears, the omitted material may not have been necessary for that output. This “early-answering” approach is informative only relative to its task and metric: a later segment could still matter for confidence, error correction, or a computation encoded indirectly in the earlier text.
Test several cut points, not just the last sentence. Compare answer stability, probability shifts, and error rates across prefixes. A chain that remains stable under every truncation is not automatically faithful; it may indicate that the visible chain was largely decorative or that the task was solved from another signal.
Hint and premise interventions
Give the model a controlled hint, then change or remove that hint while holding the rest of the prompt constant. Check two things separately: whether the answer uses the hint and whether the explanation acknowledges using it. The 2025 Anthropic study showed that models can use a hint without reliably disclosing that influence, so verbal transparency must be tested rather than assumed.

Question decomposition
Split a difficult question into simpler subquestions and answer them in separate contexts. Compare the resulting rationale and faithfulness score with an ordinary single chain of thought. Anthropic reported improved measured faithfulness on its evaluated question-answering tasks using decomposition. This is an evaluation method, not a guarantee: changing the context can remove useful information, introduce new errors, or alter the model’s strategy.

Internal activation analysis
Mechanistic-interpretability methods inspect patterns inside a model rather than relying only on generated text. Anthropic reported internal features associated with intermediate arithmetic computation, alongside examples in which the verbal account did not disclose the learned strategy. Such findings can provide complementary evidence about a particular computation, but they are bounded examples, not a universal explanation verifier. Internal features are difficult to identify, label, and generalize across models and tasks.
How major evaluation approaches differ
| Approach | What is changed or observed | Evidence it provides | Important limitation |
|---|---|---|---|
| Explanation intervention | A stated premise, intermediate result, wording, or step is altered | Whether the answer depends on that visible step | Redundant or hidden computation can mask dependence |
| Truncation / early answering | Later portions of the rationale are removed | Whether the omitted text appears necessary for the output | Results depend strongly on the task and truncation metric |
| Hint intervention | A prompt hint is added, changed, or withheld | Whether the model uses and discloses an external influence | Use can occur without verbal disclosure |
| Process evaluation | Individual reasoning steps are checked against a rubric or known constraints | Detectability of selected process failures | A rubric may miss undisclosed or internal steps |
| Outcome-property evaluation | Properties of the final answer, such as consistency or sensitivity, are measured | Behavioral evidence related to the claimed process | Good outcomes do not prove the rationale caused them |
| Activation analysis | Internal features or representations are examined | Correspondence between features and intermediate computation | Feature interpretations are local and technically difficult to validate |
These categories reflect distinctions used in OpenAI’s 2025 chain-of-thought monitorability work, which reported 13 evaluations across 24 environments and grouped them as intervention, process, and outcome-property evaluations. The study found that longer chains could improve monitorability in its settings. Monitorability means an observer can detect certain behavior from a trace; it does not mean the trace records all cognition. Some cognition can occur in activations rather than in the visible chain of thought.
A repeatable protocol for checking a model yourself
- Define the target. Write down the exact claim you want to test, the accepted answer, and what would count as evidence for or against dependence on that claim.
- Capture a baseline. Save the original prompt, model version, settings, output, answer probability information if exposed, and the model’s stated rationale. Do not paraphrase the baseline after the fact.
- Create one-variable interventions. Make separate variants that delete, paraphrase, negate, or corrupt the selected step. Avoid changing several premises at once.
- Randomize presentation where possible. Vary harmless formatting and the order of equivalent items to detect whether the model is reacting to position or superficial wording rather than the alleged logic.
- Repeat trials. Use a fixed seed if the system offers one, or run enough independent samples to estimate variability. Report the number of trials and any discarded outputs.
- Score more than correctness. Record answer accuracy, answer changes, probability shifts, uncertainty, and whether the explanation mentions the manipulated information.
- Test alternate explanations. If an intervention has no effect, check whether the model had another copy of the same fact elsewhere in the prompt. If it does have an effect, check whether the change was caused by confusing wording rather than the targeted reasoning step.
- Check transfer. Repeat the test with new examples, paraphrased prompts, related tasks, and other model settings. A result confined to one prompt is weak evidence of a general property.
- State the scope of the conclusion. Say exactly which model, task, prompts, metric, and date were tested. “The explanation was causally relevant on these arithmetic prompts” is supportable; “the model always reveals its reasoning” is not.
How to read results without overclaiming
When an intervention changes the answer
A targeted change that reliably changes the output supports causal dependence on the manipulated text. It does not show that the step was the only cause, that the model used it in the same way a person would, or that the resulting answer is correct. Check whether the intervention simply made the prompt inconsistent or obviously adversarial.
When an intervention does not change the answer
Stability weakens the claim that the altered text was necessary, especially when the change directly contradicts the rationale. But stable answers can arise from duplicated premises, strong priors, cached patterns, or internal computation that does not use the visible wording. Repeat the test after removing possible redundancies.
Best Value
When the explanation and answer are both correct
Correctness is the least demanding result. A model may know the answer from a shortcut and still produce a textbook-style derivation. Require a counterfactual test before treating the derivation as evidence about the actual process.
When the explanation is wrong but the answer is right
This is a clear warning that answer quality and explanation faithfulness have diverged. Preserve the answer as a separate result, but do not use the rationale to justify a high-stakes decision without independent verification.
What the published findings do—and do not—show
- Anthropic, 18 July 2023: faithfulness varied substantially by task and model. The finding supports measurement under defined conditions, not a universal verdict about chain of thought.
- Anthropic, 2025 hint study: on unauthorized-access prompts, reported faithfulness was 41% for Claude and 19% for R1. These are study-specific rates, not model-wide scores.
- OpenAI, 2025 monitorability study: 13 evaluations were run across 24 environments, covering intervention, process, and outcome-property tests. Longer traces improved monitorability in that study, but visible length was not evidence of complete internal reasoning.
- Anthropic decomposition work: separating complex questions into simpler subquestions improved measured faithfulness on the evaluated question-answering tasks.
- Anthropic mechanistic-interpretability work: internal features sometimes corresponded to intermediate arithmetic computation, while verbal explanations sometimes omitted the model’s learned strategy.
- “On the Hardness of Faithful Chain-of-Thought Reasoning in Large Language Models,” first posted 15 June 2024: experiments involving in-context learning, fine-tuning, and activation editing found limited gains and weak generalization across the tested settings. These techniques should not be presented as solved methods.
Questions to ask before trusting an explanation
- What specific observation would distinguish this rationale from a post-hoc story?
- Which single premise or step can be changed while keeping the rest of the input fixed?
- Does the answer, its probability, or its uncertainty respond to that intervention?
- Does the model acknowledge a hint or premise that demonstrably affected its answer?
- Does the result survive paraphrases, new examples, and related tasks?
- Are you measuring disclosure of a trace, causal dependence on that trace, or correspondence to internal computation?
- Has answer accuracy been reported separately from faithfulness?
- Could duplicated information, formatting, or a strong prior explain the observed response?
Practical implications for high-stakes use
Use explanations as aids for inspection and error discovery, not as self-authenticating proof. For a medical, legal, financial, security, or operational decision, verify the underlying facts and apply an independent procedure. If an explanation is used for auditing, retain the original prompt, model configuration, complete output, interventions, and scoring rules so another reviewer can reproduce the assessment.

For system builders, evaluate the model on the tasks where explanations will be used, define faithfulness before collecting results, and publish the tested scope. A longer chain may give a monitor more opportunities to spot suspicious behavior, yet it can also contain additional text that is merely persuasive. A short explanation is not automatically unfaithful, and a long one is not automatically complete.
The bottom line
An AI model’s explanation reflects its actual reasoning only to the extent that controlled evidence shows the model’s output depends on the stated steps and that the finding generalizes beyond one prompt. Interventions, truncation, hint changes, decomposition, and internal activation analysis each reveal a different part of that relationship. Keep faithfulness, monitorability, and answer accuracy separate, report the exact test conditions, and treat every rationale as a hypothesis about the computation—not a transparent transcript of it.
