How to Tell If GLP-1 Weight-Loss Medication Is Right for You

A conversation with a qualified clinician is reasonable if you have obesity, or overweight with a weight-related health condition, and nutrition, activity, sleep and behavior changes have not achieved your health goals. GLP-1 medicines can produce substantial average weight loss, but the right choice depends on your medical history, other medicines, pregnancy plans, treatment goals, cost and access, and whether you are prepared for ongoing monitoring and often long-term treatment.

This is a decision guide, not a prescription. A clinician must confirm whether an approved medicine is appropriate, review contraindications and interactions, and arrange lawful prescribing and dispensing. The eligibility information below reflects U.S. Food and Drug Administration (FDA) labeling and U.S. clinical guidance; approvals and rules differ elsewhere.

What GLP-1 weight-loss medicines are

Semaglutide is a glucagon-like peptide-1 (GLP-1) receptor agonist. Tirzepatide activates both GIP and GLP-1 pathways. They are prescription medicines used with a reduced-calorie eating pattern and increased physical activity, not replacements for those measures. They influence appetite and energy intake and can also affect blood-glucose regulation, which is why the medication review must include diabetes treatment and hypoglycemia risk.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

In the United States, semaglutide is marketed as Wegovy for chronic weight management, while tirzepatide is marketed as Zepbound for chronic weight management. Brand, indication, dose, insurance coverage and availability can change, and a product approved for diabetes is not automatically interchangeable with a weight-management product.

Who the FDA labeling covers

The Wegovy label covers adults and adolescents age 12 and older with obesity, and adults with overweight when at least one weight-related comorbidity is present. Wegovy also has an indication to reduce major cardiovascular events in adults with established cardiovascular disease who have obesity or overweight. The cardiovascular indication is an additional, specific use; it does not mean every person seeking weight loss has the same risk or benefit.

BMI is a starting screen, not the whole decision

The Endocrine Society recommends considering pharmacotherapy as an adjunct to behavior modification at BMI 30 kg/m² or higher, or at BMI 27 kg/m² or higher when a comorbidity such as hypertension, dyslipidemia, type 2 diabetes or obstructive sleep apnea is present. These are clinical framework thresholds, not a guarantee of approval, insurance coverage, response or safety. BMI does not describe muscle mass, fat distribution, medical history or treatment preference, so a clinician should interpret it alongside the rest of your health picture.

Situation What it means for a discussion
Adult with obesity Meets the broad population described in FDA chronic-weight-management labeling, subject to contraindications, clinical judgment and local rules.
Adult with overweight plus at least one weight-related comorbidity May fit FDA labeling and Endocrine Society guidance; the specific condition and overall risk still matter.
Adolescent age 12 or older with obesity Wegovy labeling includes this group; treatment requires age-appropriate specialist assessment and monitoring.
Adult below those BMI frameworks Do not assume a GLP-1 medicine is appropriate for cosmetic weight loss; a clinician should evaluate another explanation for weight change and other treatment options.

How much weight loss can you realistically expect?

Clinical trials report averages for selected participants who followed a study protocol. They help compare medicines, but they cannot predict your individual result. Response, side effects, adherence and weight maintenance vary, and obesity guidance treats medication as part of continuing chronic care rather than a short course that works in isolation.

Study and medicine Reported result How to interpret it
STEP 8, semaglutide 2.4 mg Mean weight change of −15.8% at week 68 Average result reported by the American College of Cardiology/American Heart Association in 2023 for the specified trial population.
STEP 8, liraglutide 3.0 mg Mean weight change of −6.4% at week 68 Trial comparator, not a promise that one person will lose that amount.
STEP 8, placebo Mean weight change of −1.9% at week 68 Shows the difference between the study medicine and the trial’s background care and behavior program.
SURMOUNT-1, tirzepatide Dose-dependent mean weight change up to −20.9% at week 72 Reported in eligible adults without diabetes; the highest figure reflects a particular dose and trial population.

The Wegovy cardiovascular-outcomes trial randomized more than 17,600 participants, according to the FDA’s 2024 announcement. That scale supports confidence in the trial’s cardiovascular evidence, but it still does not turn a population average into an individual forecast.

What should determine your choice?

Start with the health outcome you want

Clarify whether your priority is weight reduction, improved blood glucose, lower cardiovascular risk, relief of sleep-apnea or mobility problems, or several goals at once. Wegovy’s cardiovascular indication applies specifically to adults with established cardiovascular disease and obesity or overweight. A person without established cardiovascular disease may have a different reason to consider treatment and a different balance of benefits and risks.

Consider whether long-term treatment fits your plan

These medicines are used for chronic management. Before starting, ask what happens if weight loss slows, if side effects limit dose escalation, or if access changes. Continuing nutrition, physical activity, sleep and behavioral support is part of treatment rather than an optional add-on.

Compare the practical trade-offs

The clinically relevant comparison is not simply which headline percentage is larger. Discuss average benefit for the populations studied, glucose and cardiovascular priorities, adverse effects and contraindications, dose titration, injection or administration burden, interactions, cost, insurance coverage and local availability. A medicine that is theoretically stronger but unaffordable, unavailable or intolerable is not a practical choice.

Ask how success and failure will be defined

Agree on more than a target number on the scale. Your clinician may track weight trend, waist or metabolic measures, blood-glucose readings, blood pressure, mobility, sleep-apnea symptoms, appetite control and quality of life. Ask how long you will remain at a dose before judging response, what will happen if side effects prevent escalation, and when another treatment should be considered. A slower response is not automatically treatment failure, while a rapid loss accompanied by dehydration or inability to eat needs prompt review.

Safety checks to complete before prescribing

Contraindications in the Wegovy label

Wegovy is contraindicated if you have a personal or family history of medullary thyroid carcinoma, multiple endocrine neoplasia syndrome type 2 (MEN2), or known hypersensitivity to semaglutide or an ingredient. Tell the prescriber about these conditions rather than relying on an online questionnaire.

Conditions that require a risk discussion

The label warns about pancreatitis, acute gallbladder disease, hypoglycemia when Wegovy is used with insulin or an insulin secretagogue, acute kidney injury related to volume depletion, severe gastrointestinal reactions and other adverse effects. A history of pancreatitis, gallstones or other gallbladder disease, persistent vomiting or diarrhea, significant gastrointestinal symptoms, or kidney vulnerability can change the decision and the monitoring plan.

Diabetes medicines and low blood sugar

If you use insulin or an insulin secretagogue, reduced food intake and weight loss can increase hypoglycemia risk. The American Gastroenterological Association advises that doses may need adjustment. Do not change diabetes medicines yourself; coordinate changes with the clinician managing your diabetes and establish how glucose will be monitored.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Pregnancy and contraception planning

For people taking Wegovy for weight reduction or cardiovascular-risk reduction, the current 2026 label instructs discontinuation at least two months before a planned pregnancy because semaglutide has a long half-life. Tell your prescriber about a possible or planned pregnancy before treatment starts. Pregnancy-related decisions for any medicine should be individualized with an obstetric clinician.

Do not combine GLP-1 products on your own

The Wegovy label says concomitant use with other semaglutide-containing products or any other GLP-1 receptor agonist is not recommended. A prescriber should reconcile every injection, tablet, supplement and diabetes medicine before you begin.

Tell the clinician what you cannot reliably do

Be candid about barriers that affect safe use: difficulty obtaining refrigeration or regular supplies, trouble measuring doses, missed appointments, swallowing or eating problems, limited health literacy, or inability to afford follow-up. These are clinical considerations, not personal failures. The safest plan may involve additional education, a different medicine, or postponing treatment until monitoring and supply are dependable.

Approved products versus compounded and online offerings

FDA has reported adverse-event cases involving compounded semaglutide and tirzepatide, including doses beyond approved labeling. Some patients and clinicians seek unapproved products when access is difficult, but a social-media seller or online listing is not equivalent to an FDA-approved medicine.

Before accepting a product, verify all four elements:

  • The exact active ingredient, strength and formulation.
  • A prescription issued by an authorized clinician who knows your medical history.
  • A pharmacy legally authorized to dispense that product in your jurisdiction.
  • Clear instructions for storage, measurement, administration, follow-up and reporting adverse effects.

Do not treat a compounded or unapproved product as interchangeable with an approved product, and do not use a product that arrives without reliable labeling or a way to contact the dispensing pharmacy.

A step-by-step appointment checklist

  1. Calculate and document your BMI. Bring your height, weight history and waist or metabolic information if available. BMI is a screening measure, so include relevant diagnoses and symptoms.
  2. Describe what you have already tried. Note nutrition changes, physical activity, sleep interventions, behavioral programs, prior medicines, duration and results. Include barriers such as shift work, mobility limits or food insecurity so the plan is realistic.
  3. Set measurable goals. Decide whether the goal is a percentage of weight, glucose improvement, cardiovascular-risk reduction, improved mobility, fewer apnea symptoms or another health outcome. Ask how success will be assessed if the scale changes slowly.
  4. Bring a complete medication and history list. Include insulin, insulin secretagogues, other diabetes medicines, prior pancreatitis or gallbladder disease, gastrointestinal symptoms, kidney problems, allergies, thyroid-cancer history and MEN2 in the family.
  5. Discuss pregnancy timing and contraception. If pregnancy is possible or planned, address the two-month semaglutide discontinuation instruction and alternatives before treatment begins.
  6. Agree on follow-up. Establish when dose changes, side effects, nutrition, activity, glucose readings, weight response and maintenance decisions will be reviewed, and identify who to contact between visits.

What monitoring looks like after starting

Early follow-up and dose changes

Follow-up is needed to assess gastrointestinal tolerance, hydration, eating patterns, glucose readings when relevant and whether the current dose is helping. Dose escalation should follow the prescribed product’s current instructions; do not accelerate it to chase a faster result. If adverse effects or poor intake occur, contact the prescriber before taking the next dose.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Keep a practical symptom and response record

Record injection dates, the prescribed dose, appetite and food intake, vomiting or diarrhea, abdominal pain, fluid intake, glucose readings when applicable, and weight trends. This gives the clinician information to distinguish expected gastrointestinal effects from a problem requiring a pause, dose adjustment or evaluation. Bring the list to visits rather than relying on memory.

Symptoms that need prompt medical attention

Seek urgent clinical advice for severe or persistent abdominal pain, repeated vomiting, inability to keep fluids down, signs of dehydration, symptoms of gallbladder disease, or suspected low blood sugar. The label’s pancreatitis, gallbladder, kidney and severe-gastrointestinal warnings make these symptoms more than routine discomfort. Emergency symptoms such as fainting, confusion, severe weakness or a serious allergic reaction require emergency care.

Maintenance is part of the decision

Ask in advance how weight and health markers will be monitored after the initial loss, what support will continue, and how the clinician will respond if weight returns or access is interrupted. A plan that includes maintenance and behavioral support is safer than treating the first months as the entire course.

Cost, coverage and availability

Prices, insurance coverage, shortages and prescribing rules were not established for a single geography in the evidence summarized here. Confirm current local coverage, prior-authorization requirements, out-of-pocket cost, lawful supply and follow-up access before committing to treatment. If a product is unaffordable or unavailable, ask the clinician about evidence-based alternatives rather than turning to an unverified seller.

Ask the clinic or insurer whether coverage depends on a documented BMI, a qualifying comorbidity, prior lifestyle treatment, specific laboratory results, or periodic proof of response. Also ask about deductibles, quantity limits, refill timing and what happens if coverage changes. A lower advertised price may not include consultation, monitoring, supplies or treatment of adverse effects.

When a GLP-1 discussion is probably not the right next step

  • You have a listed contraindication such as personal or family medullary thyroid carcinoma, MEN2 or a known serious allergy to the product.
  • You are seeking a quick cosmetic fix without willingness to continue nutrition, activity and follow-up.
  • You cannot obtain a reliably labeled, lawfully prescribed and dispensed product.
  • You have uncontrolled symptoms that need diagnosis first, such as persistent vomiting, severe abdominal pain or unexplained weight loss.
  • You are planning pregnancy soon and have not yet discussed medication timing with your clinicians.

In these situations, address the underlying safety or access issue first. A clinician can then determine whether a GLP-1 medicine, another treatment or non-medication care best matches your goals.

Bottom line

GLP-1 medication is worth discussing when obesity, or overweight with a weight-related condition, remains a health problem despite sustained lifestyle efforts and you can commit to medical screening, follow-up and often long-term treatment. Semaglutide and tirzepatide can produce large average losses in trials, but the choice must account for contraindications, diabetes medicines, pregnancy plans, cardiovascular priorities, tolerability, cost and lawful access. Use an authorized prescriber and pharmacy, and make the decision with a clinician who can monitor your response rather than relying on an online seller or a trial average.

Frequently Asked Questions

How do I know if a GLP-1 medicine is right for me?

Discuss it with a clinician if you have obesity, or overweight with a weight-related condition, and lifestyle measures have not met your health goals. The decision also depends on contraindications, other medicines, pregnancy plans, treatment priorities, cost, access and willingness to attend follow-up.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What BMI do I need for a GLP-1?

Endocrine Society guidance uses BMI 30 kg/m² or higher, or 27 kg/m² or higher with a comorbidity such as hypertension, dyslipidemia, type 2 diabetes or obstructive sleep apnea. FDA labeling and insurance rules can differ, and BMI is only a screening measure.

How much weight could I lose?

In STEP 8, semaglutide 2.4 mg produced a mean 15.8% loss at 68 weeks. In SURMOUNT-1, tirzepatide produced dose-dependent mean losses up to 20.9% at 72 weeks. These are trial averages, not personal guarantees.

What are the most important safety concerns?

Wegovy is contraindicated with a personal or family history of medullary thyroid carcinoma, MEN2 or known hypersensitivity. The label also warns about pancreatitis, gallbladder disease, hypoglycemia with insulin or insulin secretagogues, kidney injury from dehydration and severe gastrointestinal reactions.

Can I take a GLP-1 with my diabetes medicine?

Often, but insulin and insulin-secretagogue doses may need adjustment because reduced food intake and weight loss can raise hypoglycemia risk. Never change doses yourself; coordinate with the clinician managing your diabetes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How long before pregnancy should semaglutide be stopped?

The current Wegovy label instructs people using it for weight reduction or cardiovascular-risk reduction to discontinue at least two months before a planned pregnancy. Discuss pregnancy plans and alternatives with your prescribers.

Are compounded GLP-1 products safe?

FDA has received adverse-event reports involving compounded semaglutide and tirzepatide, including doses beyond approved labeling. Use an authorized prescriber and legally authorized pharmacy, and do not assume a compounded or online product is interchangeable with an approved medicine.

What should I ask at my appointment?

Ask which health outcome the medicine is intended to improve, how response and side effects will be measured, how titration works, what monitoring is required, what happens if access is interrupted, the expected out-of-pocket cost, and how long-term maintenance will be handled.

The Bottom Line

GLP-1 treatment may be appropriate for obesity or overweight with a related condition when lifestyle measures alone have not met health goals. A clinician should weigh expected benefit against medical risks, interactions, pregnancy timing, tolerability, cost and reliable access to an approved product.
