Tell a healthcare professional what has changed, how long it has been happening, and how it is affecting your life. A primary care clinician can begin an assessment and refer you to mental-health care when appropriate. Bring notes about symptoms, medical history, medicines, stressors and safety concerns so you and the clinician can decide what to do next.
As the National Institute of Mental Health (NIMH) puts it: “Don’t wait for a health care provider to ask about your mental health. Start the conversation.”

Before the appointment: make the conversation easier
Depression can affect memory, concentration and motivation, so relying on the moment to explain everything may be difficult. A short written record is often more useful than trying to remember a typical day in the consulting room.

For each symptom, write when it began, how often it occurs, how severe it feels and what it disrupts at home, work, school or in relationships. Include changes others have noticed. This information helps a clinician judge the pattern rather than relying on a single day.
List context that may affect your health
Write down recent losses, conflicts, work or financial pressure, major changes, substance use and other stressors. Include past medical conditions, previous mental-health treatment, hospitalizations and family mental-health history if you know it. Bring the names and doses of every prescription and over-the-counter medicine, vitamin, herbal remedy and supplement. Do not stop or alter a prescribed medicine because of something you read here; raise concerns with the prescriber.
Plan practical details
Check whether you need a referral, what your insurance or health service covers, and whether there may be a fee for an assessment or therapy. Ask how long appointments usually last and whether follow-up can be in person, by telephone or by video. If language, hearing, mobility, childcare or transportation could make attendance difficult, tell the practice when booking. Ask how your information is kept private and whether a support person can join. Rules differ by location and service, so clarify them with the provider rather than assuming.

Decide what support you want
You may ask a trusted friend or relative to come with you, take notes or help you remember questions. You can also ask to speak with the clinician alone first. If speaking feels overwhelming, hand over your notes or use a prepared opening such as: “I have not felt like myself for the past six weeks. My sleep and concentration have changed, and I am struggling at work. I would like to discuss whether depression or another health problem could be involved.” You can send a message through a patient portal before the visit if that is easier, while still discussing urgent safety concerns directly.
How to start talking with your doctor
Raise the subject even if the appointment was scheduled for another problem. Say plainly that you want to discuss your mood or other mental-health symptoms. Use ordinary words; you do not need to arrive with a diagnosis or a completed online quiz. If you are unsure whether depression explains what you feel, say that uncertainty out loud.
Be honest about symptoms that feel embarrassing, including loss of pleasure, changes in sex drive, substance use, self-harm or suicidal thoughts. A clinician needs accurate information to assess safety and possible causes. If a recommendation is unclear, ask the clinician to explain it in everyday language. You can ask about alternatives, the reasons for a proposed plan and whether a second opinion is appropriate. You are allowed to say that you need time to consider a non-urgent choice and ask for written instructions.
Questions that can open the discussion
- “What could be contributing to these symptoms, and should we rule out anything else?”
- “How will you assess what is happening?”
- “What treatment choices fit my symptoms, health and preferences?”
- “What benefits, drawbacks and alternatives should I consider?”
- “What should I expect from therapy, and how will we know whether it is helping?”
- “If medicine is an option, what side effects or interactions matter for me, and when will we review it?”
- “When should we follow up, and what should I do if I get worse while waiting or in treatment?”
- “If the first plan does not help or does not feel right, what is the next step?”
- “What will this cost, and are lower-cost or publicly funded options available?”
- “How is my information protected, and who can see my records?”
What happens in a depression assessment
An assessment is a conversation about the whole picture, not a pass-or-fail test. A clinician may ask when symptoms began, how long they last, how often they occur and how they affect daily activities and relationships. They may ask about sleep, appetite, energy, concentration, mood, anxiety, substance use, living circumstances, recent triggers and safety.

Tell the clinician about good days as well as difficult days. Explain whether symptoms are constant, come in episodes, or change with work schedules, menstrual cycles, seasons, medication changes or substance use. Mention any periods of unusually high energy, very little need for sleep or impulsive behavior, because the clinician may need to consider more than one type of mood problem. Do not try to give answers you think the clinician wants; accuracy is more useful than a particular label.
Medical and mental-health history
Expect questions about current and past illnesses, medicines, previous episodes, therapy, hospital care and family history. Share information about pregnancy or recent childbirth when relevant, and mention alcohol or other substances. These details can change which explanations and treatments are sensible. Report allergies, past medication reactions and any condition affecting the liver, kidneys, heart, thyroid or nervous system. Tell the clinician if cost, work schedules, caregiving or travel could make a treatment difficult to continue.
Questionnaires and physical checks
A clinician may use a symptom questionnaire to organize information or track change. A questionnaire, app, product or online quiz does not establish a diagnosis by itself. There is no physical test that alone diagnoses depression. The clinician may examine you or order blood or urine tests to look for other conditions with similar symptoms, such as an underactive thyroid. Some medicines and medical conditions can also produce depression-like symptoms. Results are interpreted alongside your history and current functioning. The NHS diagnosis guidance describes this approach for adults in the UK.
Ask what a test is intended to find, when results will be available and how the result would change the plan. A normal laboratory result does not prove that symptoms are imaginary, and an abnormal result does not by itself explain every symptom. The clinician considers physical, psychological and social factors together.

1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsQuestions about suicide or self-harm
Direct questions about wishing you were dead, thinking about suicide, self-harm, plans or access to means are a normal part of a safety assessment. Answer as accurately as you can. If you have a plan, have taken steps toward it, or cannot keep yourself safe, say so at once rather than waiting for the rest of the appointment. Ask what support is available outside office hours and who to contact if your safety changes.
Treatment choices to discuss
Depression care may include psychotherapy (talk therapy or counseling), medication, both, or—when symptoms are mild in some health systems—structured support and monitoring. NIMH emphasizes that there is “no ‘one-size-fits-all’ treatment” (NIMH depression guidance). The suitable plan depends on symptom severity and urgency, your preferences, medical circumstances, possible interactions, availability and how progress will be reviewed. Finding a workable plan can involve adjustment with a healthcare professional.
| Option to discuss | What it may involve | Important limits or questions |
|---|---|---|
| Psychotherapy | Regular sessions with a trained professional to work on thoughts, emotions, behavior, relationships or coping, depending on the therapy. | Ask what approach is offered, session frequency, expected duration, cost or coverage, privacy and how improvement will be measured. It can be used instead of or alongside medicine. |
| Medication | A prescribed antidepressant selected and monitored according to symptoms, other medicines, medical history and preferences. | Ask about likely benefits, common and serious side effects, interactions, when it may begin helping, and the review plan. Never stop suddenly or change the dose without the prescriber. |
| Combined treatment | Psychotherapy and medication used together. | Discuss whether the added appointments, cost, travel or side-effect burden is realistic and how each part will be evaluated. |
| Guided self-help or exercise | Structured written or digital support, or planned physical activity, sometimes alongside professional follow-up. | These are not substitutes for urgent assessment. Ask how the program is supervised and what to do if symptoms worsen. |
| Watchful waiting with follow-up | For some adults with mild depression, a clinician may suggest monitoring symptoms and arranging a review rather than starting intensive treatment immediately. | This is an NHS-described option for England and is not a universal rule. Confirm the review date and the symptoms that should prompt earlier contact. |
The NHS adult treatment guidance for England also discusses guided self-help, exercise, talking therapies, antidepressants and combined treatment for more severe depression (NHS treatment guidance). That pathway should not be treated as a prescription for people in other countries or as a way to determine your own severity.

Compare options on more than one factor
Ask the clinician to compare expected benefits, burdens and uncertainties rather than simply naming a preferred option. Consider how urgent the symptoms are, whether you can attend regularly, the time and travel involved, cost or insurance coverage, privacy, cultural or language fit, and your medical history. Ask how quickly a choice might be reviewed and what would count as meaningful improvement in sleep, concentration, mood, relationships or work.
Access can shape the practical choice. A therapy that requires weekly travel may be unrealistic if you provide care for someone else; a remote appointment may be useful but not available everywhere. If a recommended service has a waiting list, ask about interim support and what to do if symptoms worsen. It is reasonable to ask about another qualified therapist, a different format or a referral if the first option does not fit.
Comparing psychotherapy approaches
Rather than asking which therapy is “best” for everyone, ask what a proposed approach focuses on and what happens in sessions. Clarify whether sessions are individual, group, couples-based, in person or remote; how homework or between-session practice works; how long treatment usually lasts; and what happens if you do not connect with the therapist. The NIMH overview of psychotherapies explains that different therapies use different methods and goals (NIMH psychotherapies).
Ask how the therapist will include your goals and how progress will be checked. You can discuss boundaries, confidentiality and what happens in a crisis between sessions. A first meeting is also a chance to explain accessibility needs and to say if an approach feels uncomfortable or irrelevant. A poor fit does not mean therapy cannot help; tell the clinician and discuss a change.
Discussing medication safely
Give the prescriber your complete medicine and supplement list, including occasional products. Ask how a new medicine could interact with existing treatment or medical conditions, what side effects require a call, and when the first review will occur. Ask what to do if you miss a dose, feel worse or want to stop. Ask whether monitoring, a dose adjustment or another appointment is expected. Do not substitute supplements for an evaluated treatment plan, and do not discontinue a prescribed medicine abruptly unless a clinician gives you specific instructions.

If you are pregnant, breastfeeding, planning a pregnancy or caring for a child, mention it before medication is prescribed. Tell the prescriber about alcohol or recreational drug use and any difficulty taking medicine at a regular time. Ask for instructions in writing and use one pharmacy or an up-to-date medication record when possible so interactions are easier to identify.
Agree on follow-up and a backup plan
Before leaving, confirm the next appointment or contact method, what improvement would look like and when progress will be checked. Ask whether you should track mood, sleep, side effects or daily functioning. Make sure you know whom to contact if the first treatment is ineffective, intolerable or unavailable, and whether a referral is needed. Write down the plan, including the medication name and instructions if one is prescribed, the date of review and warning signs.

Symptoms can change while you are waiting for therapy, a referral or a medication review. Ask for an earlier assessment if functioning declines, side effects are troubling or safety concerns appear. If you cannot attend an appointment, ask whether telephone or video care, another clinician or a different service is available. Do not assume that silence means a referral is progressing; ask how to check its status.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Finding care depends on where you live
United States
In the United States, a primary care provider can start an assessment and help coordinate mental-health care. If you are struggling or having thoughts of suicide, call or text 988 for the Suicide & Crisis Lifeline. In a life-threatening emergency, call 911. These numbers are U.S.-specific; people elsewhere should use their local crisis or emergency service.
England
In England, NHS Talking Therapies may allow adults to self-refer for anxiety and depression. Eligibility, referral routes and availability vary locally, and waits may occur. Check the official NHS Talking Therapies service finder or ask a GP about local options. NHS arrangements do not automatically apply in Scotland, Wales, Northern Ireland or other countries.
Other locations
Ask your local primary care practice, public health service or insurer how referrals, privacy, costs and emergency support work where you live. If there is immediate danger, contact your local emergency service now rather than waiting for a routine appointment.
A practical appointment checklist
- Write a one-sentence reason for the visit.
- Bring symptom notes showing onset, frequency, severity and effects on daily life.
- List recent stressors, medical conditions, past mental-health care and family history.
- Bring every prescription, over-the-counter medicine, vitamin, herb and supplement with doses when possible.
- Mark any thoughts of self-harm or suicide clearly so they are not missed.
- Choose whether you want a support person or note-taker.
- Take the questions you want answered about causes, therapy, medication, alternatives and follow-up.
- Check practical needs such as cost, coverage, transport, language access and appointment format.
- Before leaving, repeat the plan, review warning signs and confirm how to get help if symptoms worsen.
When the first plan does not work
A first treatment may need adjustment, a different therapy, a medication review or further assessment. Tell the clinician specifically what has changed, what side effects or barriers you experienced and what felt unacceptable. Ask what the next step is rather than stopping care on your own. Revisit possible medical, substance-related or situational contributors when the course is unexpected.
If symptoms improve only partly, bring that information to the review instead of assuming treatment has failed. Ask whether the goals, timing, dose, therapy format or diagnosis need reconsideration. If you feel dismissed, you can request an explanation, seek another qualified opinion or contact a different service, while arranging urgent help if safety is at risk.

Frequently Asked Questions
How do I tell my doctor I think I may be depressed?
Say what has changed, when it started and how it affects daily life. You can read from notes or use a sentence such as, “I have had low mood and poor sleep for several weeks, and I am struggling at work. Could we assess what is causing this?” You do not need to diagnose yourself first.
What should I bring to a depression appointment?
Bring symptom notes covering onset, frequency, severity and effects on work, school, relationships and self-care. Also bring recent stressors, medical and mental-health history, family history if known, and a complete list of prescription medicines, over-the-counter products, vitamins, herbs and supplements.
Can a blood test diagnose depression?
No single blood, urine or physical test establishes depression. A clinician may order tests, such as thyroid testing, to look for medical conditions or medication effects that can resemble depression. Results are considered with your symptoms, history and functioning.
Should I choose therapy or antidepressants?
There is no universal best choice. Psychotherapy, medication, both, or structured support may be considered depending on symptom severity, urgency, medical circumstances, preferences, access and cost. Ask about expected benefits, drawbacks, alternatives and how progress will be reviewed.
What if I cannot get an appointment or therapy quickly?
Ask the practice or service what interim support and follow-up are available, whether another clinician or remote appointment is possible, and what changes should prompt earlier contact. If your safety is at risk, use local emergency or crisis services rather than waiting.
What should I do if I am thinking about suicide?
Tell the clinician or emergency responder directly. If you are in immediate danger, contact emergency services now. In the United States, call or text 988 for the Suicide & Crisis Lifeline, or call 911 for a life-threatening emergency. People elsewhere should use their local crisis or emergency number.
The Bottom Line
Start the conversation, bring specific notes and a complete medicine list, answer safety questions honestly, and ask how causes, treatment choices, costs and follow-up will be handled. A clinician can help you build and adjust a plan that fits your health and circumstances.
