Before you go, prepare your goals, diagnoses, allergies, complete medication list, pregnancy plans, and any semaglutide package or instructions you already have.
In the United States, semaglutide is sold in products with different FDA-approved uses and formulations. Do not assume that a product, concentration, dose, or schedule can be substituted for another. Use your appointment to identify the exact product and indication your clinician is considering.

Start the conversation with the result you want
Tell the clinician whether your main concern is blood glucose, weight, or both. That distinction determines which questions matter and which FDA-approved indication may apply. You do not need to arrive knowing the drug, dose, or injection schedule.
A direct opening can prevent the visit from becoming a vague discussion:

Try saying: “I’d like to talk about whether semaglutide is appropriate for my health goals. Can we review which product is intended for my situation, the benefits and risks for me, and what monitoring or follow-up I would need?”
Ask the clinician to explain unfamiliar terms and to write down the product name, formulation, instructions, monitoring plan, and follow-up date. If the plan changes later, that written record helps you and other clinicians distinguish one semaglutide product from another.
Know which semaglutide product is being discussed
“Semaglutide” is the active medicine name, not a single interchangeable product. FDA information identifies products with different approved purposes in the United States.

| Ozempic | Type 2 diabetes | Injection | Confirm that diabetes treatment is the intended indication and that the prescribed pen, strength, and schedule are specifically for this product. |
| Rybelsus | Type 2 diabetes | Oral tablet | Ask how its tablet instructions fit with your other oral medicines and daily routine. |
| Wegovy | Weight management | Injection | Confirm the weight-management indication, monitoring, pregnancy instructions, and the exact device and schedule. |
These products are not automatically interchangeable. A diabetes product should not be treated as a weight-loss prescription simply because weight may change during treatment, and an injection should not be substituted for an oral product. Ask, “Which product and formulation are you prescribing, and is that use FDA-approved for my goal?”
Your clinician may discuss an option outside its approved indication. NIDDK patient guidance recommends asking whether a medicine is approved for the purpose being considered and what the evidence and uncertainties are for that use. If an off-label option is discussed, ask why it is being considered, what alternatives exist, and how success and safety will be assessed.
Is semaglutide right for me?
There is no safe answer based on a single symptom, body measurement, or social-media account. The clinician needs your complete health context and must match the product to an appropriate indication.

Explain the goal and your previous treatment experience
Describe what you want to improve, how long it has been a concern, and what you have already tried. Include previous diabetes medicines, weight-management treatments, nutrition or activity programs, side effects, allergies, and reasons you stopped or continued them. This helps the clinician compare realistic options rather than assuming semaglutide is the only path.

Review personal and family history
Tell the clinician about diagnosed conditions, previous operations, digestive symptoms, pancreatitis or gallbladder problems, endocrine disorders, and any history of serious medicine reactions. Include family history, not only your own diagnoses.
The FDA Wegovy prescribing information revised in March 2026 lists a personal or family history of medullary thyroid carcinoma (MTC) and multiple endocrine neoplasia syndrome type 2 (MEN 2) as contraindications. Say explicitly if either has occurred in your family. The same label describes thyroid tumors in rodents and states that the relevance to humans is unknown; that warning should not be presented as proof that semaglutide causes thyroid cancer in people.
Bring every medicine and supplement into the discussion
Provide the names, strengths, schedules, and purposes of every prescription medicine, over-the-counter product, vitamin, herbal product, and supplement. Include medicines prescribed by other clinicians. Do not rely on memory if you can bring a current list or photographs of the labels.
The clinician will consider allergies, potential interactions, and whether delayed stomach emptying could affect absorption of an oral medicine. Ask whether any medicine needs extra monitoring and who will coordinate changes if more than one clinician prescribes your treatment.

Tell the clinician if you are pregnant, might be pregnant, breastfeeding, trying to conceive, or planning a pregnancy. For Wegovy used for weight reduction, the cited FDA label says to discontinue it when pregnancy is recognized and at least two months before a planned pregnancy. That instruction is product- and indication-specific, so confirm the current label and plan with your prescriber rather than applying it to every semaglutide product.

Ask what is known for breastfeeding and whether a planned procedure or change in anesthesia timing affects treatment. Do not stop or delay a prescribed medicine on your own; ask for a documented plan.
Prepare for the appointment
Make the visit concrete by bringing information the clinician cannot reliably obtain from a brief conversation.
- Write your main goal. Use plain language such as “improve blood glucose,” “address weight,” or “work on both.” Note what would count as a meaningful improvement to you.
- List diagnoses and relevant history. Include diabetes type, prior pancreatitis or gallbladder disease, digestive problems, endocrine conditions, surgeries, allergies, and family history of MTC or MEN 2.
- Make a complete medication list. Include prescriptions, nonprescription medicines, supplements, insulin, and medicines taken only occasionally. Record the dose and when you take each one.
- Record pregnancy and lactation details. Note a current pregnancy, breastfeeding, contraception questions, or a planned pregnancy and its approximate timing.
- Bring product information if you already use semaglutide. Take the package, pen, vial, tablet container, or a clear photograph. Record the exact product name, concentration or strength, formulation, and instructions. Do not guess from the appearance of a device or vial.
- Ask whether glucose records would help. If you have diabetes, follow your clinician’s instructions about whether, when, and how often to monitor. A paper blood glucose logbook can organize readings if your clinician recommends one, but it does not measure glucose or replace a meter, app, prescribed equipment, or medical advice.
- Write down your questions. Leave space to record the answers, warning symptoms, contact number, and follow-up date.
If you cannot obtain every record, bring what you have and say what is missing. An incomplete list is safer than silently omitting a medicine or supplement.

Questions to ask your doctor before starting or changing treatment
- Which goal are we treating, and which semaglutide product and formulation, if any, is approved and appropriate for it?
- What personal or family history, current symptoms, or other conditions could affect whether I should use it?
- Do my prescription medicines, over-the-counter medicines, or supplements create interactions or require a monitoring plan?
- If I have diabetes, how should glucose be monitored, and might my insulin or another diabetes medicine need adjustment?
- What benefit should I realistically look for, how will we measure it, and when will we review whether the plan is working?
- What alternatives should I consider, and how do their expected benefits, risks, administration, and monitoring compare for my circumstances?
- Which side effects are common or expected, and which symptoms mean I should contact the clinic promptly or seek urgent care?
- How do pregnancy plans, breastfeeding, or a planned procedure affect timing or treatment?
- If someone has offered me compounded semaglutide, what should I verify about the product and its instructions?
Ask for answers in numbers or dates when possible: when the first review will occur, which readings to record, what phone number to use after hours, and what should trigger urgent care. Those details turn a general recommendation into an actionable plan.
Discuss benefits, alternatives, and how success will be measured
The evidence available for this appointment-preparation question does not establish a universal amount of weight loss, glucose improvement, or treatment duration for every person. Ask your clinician to define an individualized target and a time point for reassessment rather than relying on someone else’s result.

| Decision factor | What to ask |
|---|---|
| Approved indication | Is the option approved for my goal, or is another use being considered? If it is off-label, why? |
| Expected benefit | What change is realistic for my health situation, and which measurement will show whether it is helping? |
| Contraindications and warnings | Do my history, symptoms, allergies, or family history change the balance of benefit and risk? |
| Administration | Is it an injection or tablet, how often is it used, and what product-specific instructions must I follow? |
| Interactions and monitoring | Which medicines or supplements require special attention, and who will review glucose or other results? |
| Pregnancy and procedures | How would conception, breastfeeding, or a planned procedure change timing? |
| Access and cost | What coverage, prior authorization, supply, or follow-up issues could affect continuity, and what alternatives are available? |
A clinician may recommend a different treatment, additional diabetes education, nutrition support, or a staged plan. Ask what would make the team continue, modify, or stop the plan. Do not interpret a treatment target as permission to change a dose yourself.
Safety issues your clinician should review
Medullary thyroid carcinoma and MEN 2
Tell the clinician about personal or family MTC and MEN 2 before a Wegovy decision. The March 2026 FDA Wegovy label identifies either history as a contraindication. If you are unsure what a relative’s thyroid diagnosis was, say so and ask how it should be verified.
Pancreatitis and gallbladder disease
The label includes warnings about pancreatitis and acute gallbladder disease. Report any prior episode, surgery, gallstones, or current abdominal symptoms. Ask which symptoms warrant a same-day call, an emergency evaluation, or routine discussion at the next visit.
Low blood glucose with diabetes medicines
Semaglutide used with insulin or an insulin secretagogue can increase hypoglycemia risk. The label says a dose reduction may be necessary, but only the prescriber should decide whether and how to adjust another medicine. Ask who will make that decision, what glucose readings to record, and what to do if readings are unexpectedly low.
Delayed gastric emptying and oral medicines
Semaglutide can delay gastric emptying, which may affect absorption of some oral medicines. Give the clinician a complete oral-medication list and ask whether timing, monitoring, or an alternative is needed. Mention persistent digestive symptoms rather than assuming they are harmless or expected.
Do these 3 things before closing this tab:

Describe previous reactions to medicines, devices, or ingredients and any condition that could complicate treatment. Ask which symptoms indicate an allergic emergency and which can wait for a clinic call. Keep the exact product name available because formulations and inactive ingredients differ.

If you have diabetes, make glucose monitoring part of the plan
The cited FDA Wegovy labeling says to monitor blood glucose before starting and during treatment in patients with diabetes. Ask whether you should use existing meter or continuous-monitoring records, how often readings are needed, and where to send them.
Before leaving, clarify four points:
- Which readings matter and how often to take them.
- What range or pattern should prompt a call.
- What to do if you use insulin or an insulin secretagogue and glucose falls.
- Who will review results and authorize any change to the diabetes regimen.
Do not reduce, skip, or add insulin or another diabetes medicine because of an online dosing chart or a single reading. A clinician must account for meals, other medicines, kidney or liver conditions, illness, and your full glucose pattern.
What to do if compounded semaglutide is offered
Ask exactly what the product is, who supplied it, its concentration or strength, the formulation, and the written instructions. FDA communications state that compounded semaglutide is not an FDA-approved drug. Compounding may be considered in limited circumstances when an individual medical need cannot be met by an FDA-approved medicine or the approved medicine is not commercially available, but a compounded product is not automatically equivalent to an approved product.
Bring the container or prescription to your clinician and ask the prescriber or compounder to explain measuring, storage, administration, and titration instructions. Do not infer a dose by comparing a compounded vial with an Ozempic or Wegovy pen. Ask how the clinician will verify the product and monitor you, and what to do if the concentration, supplier, or instructions change.
Leave with a shared, written plan
A useful appointment produces more than a yes-or-no opinion. Before you leave, make sure the plan records:
- The treatment goal and the exact product, formulation, and approved indication being considered.
- Why this option fits your history and what alternatives remain available.
- Product-specific administration and titration instructions supplied by the prescriber.
- Which glucose or other health measures to monitor, how to record them, and who reviews them.
- Which side effects are expected, which require a prompt call, and which require urgent care.
- How insulin or another diabetes medicine will be handled if hypoglycemia risk changes.
- Pregnancy, breastfeeding, and planned-procedure instructions.
- The follow-up date, contact route, and circumstances that should lead to an earlier review.
- Any coverage, supply, or access problem and the clinician-approved alternative plan.
Read the instructions back in your own words. Ask the clinician to correct anything you misunderstood and to identify the current FDA labeling or patient information for the exact product.
After the visit
Keep the written plan, medication list, and product packaging together. Share the plan with clinicians who manage your diabetes, pregnancy care, procedures, or other medicines. Record monitoring results exactly as instructed and bring them to follow-up.

Pick the symptom - the matching free tool is one click away.
Contact the clinic promptly about symptoms or readings the clinician identified as concerning. Seek urgent care for severe or rapidly worsening symptoms, especially those involving severe abdominal illness, serious low blood glucose, or a possible allergic reaction. If you are uncertain whether a symptom is urgent, use the after-hours number supplied by the practice rather than changing treatment on your own.
Regulatory labeling and product availability can change. At each new prescription, refill, or product switch, confirm the exact name and current instructions with the prescriber and pharmacist.
