How to Take Care of Your Knees in Your 20s, 30s, 40s and Beyond

The most useful way to care for your knees at any age is to move regularly, strengthen the muscles that support movement, increase activity gradually, and address injuries or persistent symptoms rather than ignoring them. There is no separate evidence-based knee routine for each decade in the guidance reviewed here: the fundamentals stay much the same, while injury history, symptoms, and other health factors affect what is right for an individual.

What knee care looks like at any age

For most adults, knee care is not about avoiding movement or trying to prevent every ache. It is about building activity into a sustainable routine, choosing options your joints tolerate, and progressing in manageable steps.

The U.S. Centers for Disease Control and Prevention (CDC) recommends adults, including adults with arthritis, aim for at least 150 minutes of moderate-intensity aerobic activity each week and muscle-strengthening activity on at least two days a week. Examples of lower-joint-stress aerobic activities include brisk walking, cycling, swimming, and water exercise. Adults 65 and older should also include balance activities.

Those are broad adult activity recommendations, not a knee-specific prescription. Your starting point can be lower if you are inactive, recovering from an injury, or living with pain. A physical therapist or health care professional can help tailor a plan, particularly if arthritis or other symptoms make it difficult to get started. See the CDC’s physical activity guidance for people with arthritis.

In your 20s and 30s: build habits and progress carefully

These decades can be a useful time to establish regular aerobic activity and strength training that you can maintain. The CDC guidance does not prescribe a special routine for people in their 20s or 30s, nor does it establish that a particular exercise prevents osteoarthritis.

Increase training in small steps

If you are starting a new activity or increasing your workload, begin slowly and build up over time. A sudden jump in duration, intensity, or frequency can make it harder to tell whether a knee is tolerating the change. Choose movement you enjoy and can keep doing; brisk walking, cycling, swimming, and water exercise are examples of activities that put little or no stress on joints.

Strengthen without provoking joint pain

Muscle-strengthening activity can be done with weights or resistance bands. Choose a resistance level that does not cause joint pain. Bands are optional equipment, not a proven way to prevent knee osteoarthritis. The CDC explains how to start physical activity and adjust it for joint comfort.

Take injuries and repetitive work seriously

Some joint injuries can raise the risk of osteoarthritis later in that same joint; the CDC gives an anterior cruciate ligament (ACL) injury as an example for the knee. Repetitive work movements such as bending and squatting are also among the exposures it identifies. That does not mean ordinary activity or any one sport inevitably damages knees. It does mean a significant injury or recurring work-related symptoms are worth discussing with a clinician.

In your 40s and beyond: keep moving, without treating age as a diagnosis

Continue the same basic approach: regular aerobic activity, strength work, and gradual progression. If you are 65 or older, add balance activity. Modify an activity if it causes substantial discomfort, and seek individualized advice when symptoms or a previous injury affect what you can do.

Age is one factor associated with osteoarthritis, alongside previous injury or repetitive stress, obesity, family history, and sex. The CDC says osteoarthritis is common among adults 45 and older, but it is not a regular part of aging. The NHS also notes that osteoarthritis cannot be prevented altogether, although avoiding injury and healthy lifestyle choices may reduce risk. Risk is not certainty, and age alone cannot explain a painful knee. Read the CDC’s overview of arthritis risk factors and its osteoarthritis information.

The NHS describes age 45 or older, pain that worsens with use, and short or absent morning stiffness as a pattern a clinician may use when considering osteoarthritis. That is diagnostic context, not a self-test. Knee symptoms can have other causes, so do not assume a diagnosis from age or one symptom.

What to consider How it helps you choose
Activity type Include aerobic and strengthening activity; adults 65 and older should also include balance work.
Joint comfort and impact Try activities that put little or no stress on joints, such as cycling, swimming, or water exercise, and use comfort to guide adjustments.
Ability to progress gradually Prefer a routine you can build in small increments rather than a sudden increase in workload.
Injury or ongoing symptoms Get individualized advice if a previous injury or current symptoms make it unclear which activity is appropriate.

Some temporary pain, stiffness, or swelling can occur when beginning activity. The CDC advises starting slowly; if joint pain does not improve over time, or symptoms are severe, contact a health care provider. For strength work, use weights or bands that do not cause joint pain. These recommendations are general, not a substitute for advice based on your health and injury history.

When knee pain needs medical attention

Do not rely on age or an online symptom pattern to diagnose persistent pain. The NHS advises arranging a GP appointment if knee pain has not improved within a few weeks. Its guidance was last reviewed December 21, 2023; see NHS guidance on knee pain.

Seek urgent advice for concerning symptoms

The NHS advises urgent medical advice if your knee is very painful, you cannot move it or bear weight, it is severely swollen or has changed shape, it locks or gives way, or you feel feverish and the knee is red or hot. Follow the urgent-care pathway where you live; NHS 111 is specific to the UK.

What the osteoarthritis numbers do—and do not—tell you

The CDC’s 2024 osteoarthritis page says about 33 million U.S. adults have osteoarthritis, using underlying data from the National Health and Nutrition Examination Survey (NHANES) for 2017 through March 2020. This is an estimate for osteoarthritis overall, not a 2026 count of people with knee osteoarthritis.

A separate American Academy of Orthopaedic Surgeons (AAOS) guideline background estimated that 32.5 million U.S. adults—14% of that population—had symptomatic knee osteoarthritis between 2008 and 2014. The estimate covers a different condition and period, so it should not be compared directly with the CDC figure. The AAOS document is a treatment guideline for people with knee osteoarthritis, not a general prevention guide. Its authors state that recommendations are principles rather than prescriptions and that care should reflect clinician judgment and individual circumstances. See the AAOS knee osteoarthritis guideline and its 2021 guideline announcement.
