How to Support Brain Health as You Age: Evidence-Based Habits That Matter

Regular activity, good management of health conditions, adequate sleep, and care for hearing and vision are practical ways to support brain health as you age. These habits benefit overall health, but none guarantees that an individual will avoid dementia. Evidence for specific diets, social activities, and cognitive training is promising in some areas and limited in others.

What brain health means as you get older

Brain health is broader than memory. It includes cognitive abilities such as thinking and learning, as well as movement, emotional wellbeing, touch, and the senses. Supporting it means looking after overall health and addressing risks that can be changed, not relying on a single food, app, or exercise routine to prevent dementia.

Build regular physical activity into your week

The National Institute on Aging (NIA) cites the federal recommendation that adults get at least 150 minutes (2.5 hours) of physical activity each week. Walking is one practical place to start. Activity can support strength, energy, balance, and mood, and help prevent or delay some chronic disorders; that does not establish that a particular workout prevents dementia. NIA: Real-Life Benefits of Exercise and Physical Activity.

If you have been inactive and are considering vigorous exercise, check with a health-care provider first. Choose an activity you can do safely and regularly; if walking is your choice, comfortable, well-fitting shoes may make it easier to keep up the habit.

Manage blood pressure and other health conditions with a clinician

High blood pressure can have no noticeable symptoms. NIA summarizes decades of observational research associating high blood pressure in midlife with later cognitive decline, and recommends screening and clinician-guided treatment. A SPRINT MIND finding summarized by NIA involved adults 50 and older who received five years of treatment targeting systolic pressure below 120 mmHg; the intervention reduced their risk of mild cognitive impairment. That was a clinical trial result, not a self-treatment target for everyone. NIA: High Blood Pressure and Older Adults.

Work with a health professional on conditions such as diabetes, depression, and high cholesterol. Ask whether medicines could be affecting memory, sleep, or brain function, and discuss ways to reduce risks of falls and head injuries. Do not stop or change prescribed medicine without medical advice. NIA: How to Be a Healthier Older Adult.

Choose a balanced diet, but be cautious about brain-specific claims

A balanced pattern with fruits and vegetables, whole grains, and lean protein sources—and less excess solid fat, sugar, and salt—is sound general health advice. Mediterranean and MIND-style diets have been associated with lower dementia risk or slower decline in some observational studies, but the cognitive evidence is mixed. NIA describes a clinical trial that found only small cognitive improvements, similar to those in a comparison group following a diet with mild calorie restriction. The evidence does not establish either diet as a way to prevent Alzheimer’s disease.

NIA does not recommend vitamins or supplements to prevent Alzheimer’s or other cognitive decline. A supplement should not replace a balanced diet or medical care. NIA: Cognitive Health and Older Adults.

Protect sleep, hearing, and vision

NIA generally recommends seven to nine hours of sleep each night. If sleep is persistently poor, raise it with a health professional, including whether medicines may be contributing. Have hearing or vision concerns assessed and address sensory loss with appropriate care. NIA: How to Be a Healthier Older Adult.

A hearing-aid trial summarized in NIA’s 2024 progress report followed older adults for three years. Hearing aids appeared to reduce cognitive decline among participants with specific risk factors for cognitive decline, but not among participants without those factors. This result does not show that hearing aids prevent dementia for everyone; it supports seeking a hearing evaluation when you notice a problem. Stay socially and mentally engaged for connection and enjoyment

Social contact and mentally stimulating activities are worthwhile parts of a full life, but their evidence should not be overstated as proof of dementia prevention. In an analysis summarized by NIA, more than 7,000 adults aged 65 and older with high social engagement—such as visiting neighbors or volunteering—had better cognitive health in later life. That is an association, not proof that socializing caused the difference. Early results from the I-CONECT trial, involving almost 200 adults aged 75 and older, suggested regular internet conversations could reduce cognitive decline and social isolation; these were early trial findings. NIA: Cognitive Health and Older Adults.

NIA also describes the ACTIVE randomized trial, which tested training in memory, reasoning, and speed of processing and found lasting benefits for some trained skills, including reasoning and processing speed over ten years. The findings do not mean every commercial brain-training app has the same demonstrated impact: NIA says there is not enough evidence to establish that. Learning a skill, joining a group, or pursuing a personally meaningful challenge can still be worthwhile for engagement and enjoyment. NIA: Cognitive Health and Older Adults.

When to talk with a health professional about memory

Arrange a clinical conversation if changes in memory or thinking are new, concerning, or interfering with daily life. Do not try to diagnose yourself or rely on a supplement or app as a substitute for assessment. A clinician can consider health conditions and medication effects among the possible factors and advise what evaluation is appropriate. NIA: How to Be a Healthier Older Adult.
