If a medical bill, insurance claim, diagnosis, or treatment appears in your name but does not match the care you received, check the underlying records and contact both the provider and your insurer. The entry is a warning sign—not proof of who caused it. Follow the steps below to identify what is wrong, request corrections, and report any related credit or coverage problem.
Which signs should you investigate?
The FTC lists several kinds of activity that can indicate medical identity theft. Each is a reason to check your records, not conclusive proof that a particular person committed fraud.

- A provider bills you for care you did not receive.
- Your health plan rejects a legitimate claim because its records suggest you have used up your benefits.
- Your plan refuses coverage based on a diagnosis or condition you do not have.
- Your medical file includes unfamiliar treatments, procedures, diagnoses, medications, charges, or claims.
These warning signs are described in IdentityTheft.gov’s warning-sign guidance. Billing and coverage problems can have other explanations, so verify the entry before deciding what happened.

- Check relevant patient portals. Review portals for hospitals, clinics, doctors, and laboratories where you have received care or where a related claim may appear. Download available records.
- Request records you cannot access. Contact the provider and ask how to obtain the relevant file. Some records requests may involve fees, according to the FTC.
- Compare entries with care you actually received. Look for unfamiliar services, procedures, diagnoses, medications, charges, and claims. Note the date and the organization associated with each questionable item.
- Save the evidence. Keep copies of records, bills, claim notices, and portal messages so you can identify the exact entry when you dispute it.
The FTC notes that federal law gives you the right to know what is in your medical files. Its medical identity-theft recovery steps explain how to request records and what to do when access is a problem.

Contact the provider that controls the record. Use its patient portal if it accepts correction requests there, or send a written request. Make the request specific so the provider can identify the item and understand what needs to change.
- Name the incorrect entry and where it appears.
- Explain why it is wrong and what correction you are requesting.
- Include or point to the relevant record and supporting documents.
- If you have one, include a copy of your FTC Identity Theft Report.
- Ask the provider to notify other providers that may have received or reused the same mistaken information.
Keep a copy of everything you send. If mailing a letter, the FTC advises using certified mail with a return receipt. Sample letters are available at IdentityTheft.gov’s sample-letter page.

- Medicaid: Contact your state Medicaid agency.
- Suspected fraudulent Marketplace enrollment: Contact both the insurer and the Marketplace.
These routes are outlined in the FTC’s recovery steps. A provider’s correction may not update every insurer or provider record automatically. Ask the provider who else may hold the incorrect information, and follow up with the insurer about the claim or coverage issue.
What if the medical billing error appears on your credit report?
A provider or insurer correction does not itself remove an inaccurate item from a credit report. If medical billing errors appear there, IdentityTheft.gov directs consumers to notify all three credit bureaus using its identity-theft steps. Follow the site’s current instructions and provide the requested documentation.

The FTC also explains that identity-theft victims may dispute or block fraudulent information and may place a free credit freeze. How do you create an identity-theft report and keep track?
At IdentityTheft.gov, you can report identity theft and receive a recovery plan tailored to your situation. Save the report and plan when generated. The FTC says it does not resolve individual consumer complaints; the report helps generate recovery steps and supports law-enforcement pattern tracking.

Keep a dated case file with copies of medical records, bills, claim documents, reports, letters, emails, and responses. Record the date, organization, contact method, person or department reached, and any next steps from each call or message.
Which dispute route fits the problem?
| Where the problem appears | Who to contact | What to address |
|---|---|---|
| Medical record or claim | The provider that holds the record and the insurer’s fraud department | Request the medical-record correction from the provider and separately ask the insurer to review the claim or coverage issue. |
| Credit-report entry | All three credit bureaus through the FTC identity-theft process | Dispute or seek to block the fraudulent credit information using the current documentation instructions. |
| Medicare or Medicaid coverage | Medicare at 1-800-633-4227, or your state Medicaid agency | Report the suspected misuse through the relevant program route. |
| Marketplace enrollment | Your insurer and the Marketplace | Report the suspected fraudulent enrollment to both. |
These are separate records and organizations. Do not assume that making one report corrects every medical, insurance, or credit entry. State rules and individual plan procedures can vary; check your plan documents and the notices you received for the current dispute process and any applicable deadlines.
