Act as soon as an account entry looks wrong. Check whether the transaction is truly unfamiliar, contact your bank or credit union through an official channel, preserve a written record, and ask what formal dispute or error notice it requires. Deadlines and protections depend on whether the issue involves a debit-card transfer, check, credit-card charge, or a payment you were tricked into sending.
What suspicious bank-account activity looks like
Review your checking and savings activity against receipts, scheduled bills, transfers you initiated, and purchases made by authorized account users. Look at both pending and posted entries, because a fraudulent debit may appear while it is still pending.

- An unfamiliar withdrawal, transfer, debit-card purchase, ATM transaction, or electronic payment.
- A small debit that you do not recognize. The Consumer Financial Protection Bureau (CFPB) warns that thieves may test stolen card data with a small charge before attempting a larger loss.
- A check you did not write, sign, or authorize.
- A new payee, transfer destination, direct-debit instruction, or account change that you did not request.
- Several transactions clustered in a place, time, or amount that does not fit your activity.
An unfamiliar merchant name is not automatically fraud. Businesses sometimes bill under a legal parent, payment processor, or different trading name. Check receipts, subscription records, household-authorized users, and the merchant’s contact details before deciding. Do not delay reporting a transaction merely because the amount is small; verify it while preserving the option to dispute it.

What to do in the first few minutes
- Capture the facts. Record the posting or transaction date, amount, merchant or payee description, account affected, and any reference number. Save a screenshot or statement copy in a secure location.
- Use an independent contact route. Call the number printed on your debit card or statement, or open the bank’s official app or website by typing its address yourself. Do not use a phone number or link supplied in a suspicious text, email, pop-up, or incoming call.
- Tell the fraud or claims representative exactly what happened. State whether you recognize the merchant, possess the card, authorized anyone else to use the account, or were deceived into initiating the payment. Ask which transactions can be blocked and how to submit a formal error notice or dispute.
- Stop further access when appropriate. Ask whether the institution should lock the card, cancel and replace it, reset the PIN or security code, disable electronic access, or close and reopen the account.
- Create a durable record. Write down the date and time of every call, the department, representative’s name or identifier, case number, instructions, and promised follow-up. Keep copies of letters and secure messages.
- Complete written confirmation. A bank may request a signed letter or other written confirmation after a telephone report. Follow the stated method and deadline; CFPB guidance says that failing to provide requested confirmation within 10 business days can affect provisional-credit obligations during an investigation.
Ask the institution to explain what happens next, including whether it will issue temporary credit, how to submit additional evidence, and how you will receive the final decision. Never disclose a one-time passcode to an inbound caller claiming to be the bank.

Identify which kind of transaction you are reporting
The same unfamiliar dollar amount can follow different rules depending on how it was paid. Use this classification when speaking with the bank.
| Situation | First report | Important qualification |
|---|---|---|
| Lost or stolen debit card, PIN, or security code | Bank or credit union immediately; request a lock or replacement | For covered electronic transfers, CFPB describes a two-business-day reporting period after learning of the loss or theft that can limit liability in the circumstances it explains. |
| Unauthorized electronic withdrawal while the card or code remains in your possession | Bank or credit union immediately and through its formal error process | CFPB says notification should be made promptly and no later than 60 days after the statement showing the withdrawal was sent, in the applicable circumstances. |
| Unauthorized check | Bank or credit union as quickly as you discover it | Protections may differ if the check was processed as an electronic transfer. If it was not, state law may control some rights. |
| Payment you personally initiated after being deceived | Bank, payment company, or app provider immediately; describe the scam | Do not assume the payment will be treated as an unauthorized electronic transfer or that reimbursement is guaranteed. |
| Credit-card charge | Credit-card issuer through the number on the card or statement | Credit-card dispute procedures are separate from the bank-account electronic-transfer rules summarized here. |
Tell the representative if an authorized joint owner, employee, family member, or previously approved merchant could have made the entry. That fact can change the investigation even when you do not remember the specific purchase.
Reporting deadlines for electronic transfers
These periods come from CFPB consumer guidance last reviewed August 28, 2026. They describe covered electronic-fund-transfer situations, not every payment method or every type of scam.
| Event | CFPB-described period | What it means |
|---|---|---|
| Debit card or security code lost or stolen | Two business days after learning of the loss or theft | Reporting within this period limits potential liability in the circumstances described by CFPB; waiting longer can increase responsibility. |
| Unauthorized withdrawal appears on a statement while the card, PIN, or code was not lost or stolen | Promptly, and no later than 60 days after the statement was sent | Missing the statement-related period can affect protections. Report before the deadline rather than waiting for more evidence. |
A business day and the date a statement was sent can matter, so preserve the statement and ask the institution to confirm the applicable date. These are not permission to wait: prompt notice gives the bank more opportunity to block additional activity and preserves evidence.

How the bank investigation works
For an electronic-transfer error, CFPB guidance says the institution generally has 10 business days to investigate after receiving notice. The period can be 20 business days when the account has been open for less than 30 days.
If the bank cannot finish within the initial period, it generally must provisionally credit the disputed amount while continuing the investigation, subject to exceptions. CFPB’s explanation says the provisional credit may be reduced by no more than $50 in the circumstances covered by that guidance. Ask when the credit will appear and whether the bank needs written confirmation or other documents.
Ordinary resolution is generally required within 45 days. Certain foreign transactions, new accounts, and point-of-sale debit-card purchases can take up to 90 days. These are regulatory investigation periods, not promises that every claim will be approved.
If the institution concludes that the transaction was authorized, it must provide written notice before taking back provisional credit. Read that notice, request the records supporting the decision, and ask about the bank’s internal appeal or reconsideration process.

Secure the account and related devices
Cards, PINs, and online banking
- Confirm whether the card is locked, canceled, or replaced. A temporary lock may not stop recurring debits or transactions already authorized for settlement.
- Change the online-banking password from a trusted device, and do not reuse it elsewhere. Ask whether the bank should reset security questions, tokens, or trusted-device registrations.
- Change the debit PIN when the card, PIN, or a card-reading device may have been exposed. Do not choose a number based on your birth date or another public detail.
- Review contact information, beneficiaries, linked external accounts, scheduled transfers, direct-debit permissions, and newly enrolled payment wallets.
- Update the operating system and remove suspicious remote-access software if a scammer had you install it. Contact the bank from a different device if you suspect the current one is controlled.
When closing and reopening is appropriate
Ask the fraud department whether the compromised account should be closed and replaced rather than merely monitored. Closing an account can interrupt legitimate direct deposits, bill payments, and checks, so obtain a list of pending items and move authorized payments deliberately. Keep the old account documentation until all disputes and scheduled transactions are resolved.
If personal information may have been misused
Unauthorized activity can be a symptom of identity theft rather than a single compromised card. Contact each affected financial institution’s security or fraud department, not only the institution where you first noticed the entry.
- Report the identity theft at IdentityTheft.gov. The service can produce an Identity Theft Report and a recovery plan.
- Review your credit reports for accounts, inquiries, addresses, or collection activity you do not recognize.
- Consider a fraud alert or a security freeze through a nationwide credit-reporting company. A fraud alert tells businesses to take extra steps before opening credit; a freeze restricts access to the report until you lift it for a legitimate application.
- Replace exposed identification documents when the relevant agency permits it, and document every replacement request.
A bank-account dispute and an identity-theft report solve different problems: the first challenges a transaction, while the second helps address misuse of your personal information and new-account fraud.
Recognize bank-impersonation scams while reporting
Scammers often contact victims immediately after a suspicious transaction appears. They may claim to be from the bank, a government agency, or a payment service and ask you to “move” money to a safe account, read back a verification code, install remote-control software, or provide your password.

- End an unexpected call and dial the number on your card or statement.
- Open the official banking app yourself instead of tapping a message link.
- Never transfer funds to protect them at an outsider’s instruction.
- Do not share passwords, PINs, full card numbers, or one-time codes with an inbound contact.
- Tell the bank that a scammer impersonated it, even if you did not disclose information or send money.
CFPB scam guidance says to alert the bank or payment company promptly when a money transfer or mobile-app payment may have gone to a scammer. Explain that you were deceived into initiating it; the institution can tell you which recovery and recall options exist without promising that the payment qualifies as unauthorized.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What to do about an unauthorized check
Contact your bank or credit union as quickly as you can when you discover an unauthorized payment. CFPB’s consumer guidance, last reviewed August 30, 2024, specifically uses that instruction for unauthorized checks.
Provide a copy or image of the check if available, identify the check number and amount, and ask whether it was paid as a paper check, converted to an electronic transfer, or handled through another process. Request instructions for a stop-payment order, account closure, affidavit, or replacement checks when applicable. State-law protections can apply when the item was not processed electronically, so ask which rule the bank is applying rather than assuming the debit-card deadline controls.
Recommended Free Tools
Keep evidence that makes a claim easier to resolve
Create one secure folder containing:
- Original and corrected statements, screenshots, receipts, and check images.
- A chronological call log with dates, times, departments, names, case numbers, and promised actions.
- Copies of dispute letters, secure messages, identity-theft reports, and attachments.
- Proof of when a statement was sent or when you discovered a lost card or code.
- Evidence that a payment was a recurring bill, an authorized-user purchase, or a scam-induced transfer.
Use secure bank messaging or mail instructions supplied by the institution; do not email full account numbers or identity documents to an address you cannot independently verify.
Best Value
Escalate an unresolved complaint
Start with the institution
Ask for a written explanation, the investigation result, the documents relied on, and the deadline for internal reconsideration. Keep paying legitimate obligations while the dispute is pending unless the bank instructs otherwise; a disputed debit does not automatically cancel a separate bill.
Use the CFPB complaint process
If the bank does not resolve the matter, you can submit a complaint through the Consumer Financial Protection Bureau. Include a concise timeline, transaction details, the case number, copies of correspondence, and the remedy you are requesting. Do not send passwords or unnecessary full account numbers.
Find the correct regulator
Regulatory jurisdiction depends on the institution’s legal charter. Identify the bank’s full legal name and regulator from statements, the back of the card, or the institution’s official website. FDIC insurance does not mean the FDIC is necessarily that bank’s regulator. The FDIC’s consumer complaint information explains this distinction and directs consumers toward the appropriate agency.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Clear out junk files and repair common Windows errorsFree Scan →A practical review routine to catch problems early
- Turn on transaction alerts for card purchases, withdrawals, transfers, and profile changes when your institution offers them.
- Check the account at least weekly and reconcile unfamiliar entries against receipts and subscriptions.
- Use a unique password and multifactor authentication for online banking.
- Keep contact information current so the bank can reach you about a fraud alert.
- Store checks, statements, and account numbers securely; shred documents you no longer need.
- Review authorized users and recurring debits periodically, removing permissions that are no longer necessary.
- Report even a small suspicious debit and continue monitoring after the first report for related attempts.
The most important distinction is whether you authorized the payment yourself. A stolen card, an unauthorized electronic withdrawal, a forged check, and a scam that persuaded you to press “send” may involve different deadlines, evidence, and recovery options. Describe the facts precisely, report through an independently verified channel, and keep the entire paper trail.
