Visible glitches can raise a warning, but they cannot prove that a remote identity check is genuine. During an attended video verification, watch for changing latency, audio-video synchronization problems, inconsistent skin tone or resolution, and answers that do not follow a newly issued instruction. Treat each as a reason to pause or escalate—not as a verdict.
The decisive distinction is whether the attack is presented to a real camera or injected after capture. A convincing fake can be inserted between the camera and the identity system, leaving a reviewer with plausible video that never came from a live sensor. Reliable verification therefore combines capture-integrity controls, media analysis, biometric presentation-attack detection (PAD), protected communications, unpredictable challenges and trained human review.

Why a deepfake can look convincing
Remote identity proofing has several attack surfaces. NIST’s Special Publication 800-63A-4, Identity Proofing and Enrollment (2025) describes risks at optical capture, automated biometric mechanisms and video sessions, in both attended and unattended processes. A manipulated face may be shown to a camera, or forged images, video or audio may be inserted after the camera has captured the scene.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minutePresentation attacks
In a presentation attack, the attacker presents a fake to the sensor. Examples include a replayed video on a screen, a mask, a printed image or a synthetic face displayed to the camera. The system is at least receiving something through the expected optical path, so presentation-attack detection can look for evidence that the sample is not a live person.

Injection attacks
In an injection attack, media is inserted between capture and the verification software. A virtual camera, modified application, emulator or compromised device can feed generated frames directly into the video pipeline. The reviewer may see a smooth, believable face while the genuine camera never captured that person.
This is why a face match is not the same as capture integrity. NIST states that “a biometric comparison performed with a captured sample does not prevent these attacks.” Matching a submitted face to an identity record answers whether the images resemble one another; it does not establish that the submitted sample came from an uncompromised, live sensor.
What you can look for in an attended check
Human observation is useful for triage. It is not a stand-alone deepfake detector, and modern generated media may hide the visual defects that older checklists emphasize.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Scan for outdated or missing drivers - takes under a minuteDriver Scan →| Observed signal | What it may indicate | Appropriate response |
|---|---|---|
| Latency suddenly rises, falls or changes when a challenge is issued | Processing, replay or an injected stream may be struggling to respond in real time; an ordinary network problem is also possible. | Repeat the instruction once, note the timing and escalate if the pattern persists. |
| Speech and lip movement, or audio and video generally, drift out of sync | Playback, compositing or separate media paths may be involved. Congestion and microphone delay can produce similar symptoms. | Ask a short, unpredictable follow-up and use the organization’s escalation route. |
| Skin tone, sharpness or resolution changes within one face or during one session | Compositing, face replacement, relighting or resampling may be present, although auto-exposure and variable lighting can cause benign changes. | Do not decide from the artifact alone; request another capture or manual review. |
| The applicant does not follow a newly issued movement or object-placement instruction | A replay or pre-rendered clip may not contain the requested response. Confusion, disability, language or accessibility issues can also explain a miss. | Restate the cue clearly, offer an accessible equivalent and escalate unresolved failures. |
These examples come from NIST’s guidance for attended remote sessions. They are prompts for investigation, not proof. Do not rely on blinking, smiling, turning the head or any single visual checklist: a genuine person can fail a cue, and a sophisticated forgery can imitate it.
A practical workflow for a live, attended verification
The following sequence helps a trained reviewer use visible clues without overstating what they establish. An organization should adapt it to its approved identity-proofing procedure and accessibility requirements.

- Establish a trusted session. Use the organization’s authenticated application or web flow, confirm that the connection is protected and record the session identifier. A reviewer should know which device and channel are expected before judging the face on screen.
- Observe a baseline before challenging the applicant. Note ordinary motion, speech, lighting, focus and network delay for several seconds. A baseline makes a later change easier to distinguish from normal camera auto-focus or network variation.
- Issue an unpredictable, human-in-the-loop cue. Ask for a movement or other action selected at the time of the session rather than one announced in advance. NIST gives examples such as requesting a movement or asking the applicant to move an object between their face and the sensor.
- Vary the cue if needed. A replay may contain one expected action but not a new instruction. Use a different cue, while avoiding needless repetition that burdens a legitimate applicant.
- Check the response across media. Compare the timing of the action, voice and image. Look for a new latency spike, lip-sync drift, abrupt resolution change or a face that fails to remain consistent when partly occluded. These are investigative signals, not a scoring formula.
- Separate fraud indicators from accessibility or technical issues. A person may need an alternative movement, a captioned instruction or more time. Network delay, low light and camera exposure can create artifacts. Document the observation and the accommodation offered.
- Escalate instead of improvising a verdict. Follow the provider’s manual-review or identity-document escalation path. Do not ask an applicant to reveal extra biometric information or continue indefinitely after a material integrity concern.
Controls an identity-proofing system should implement
For organizations, the reliable answer is layered engineering rather than a reviewer trying to recognize every generated face. NIST SP 800-63A-4 calls for controls across the capture, transmission, analysis and decision stages.
Capture-integrity checks
Controls should increase confidence that media came from a genuine sensor and expected device. Examples include detecting a virtual camera, device emulator or jailbroken device. Device attestation, application integrity checks and restrictions on unsupported capture paths can reduce injection opportunities, but they do not make an endpoint automatically trustworthy.
Media-forensics analysis
Submitted images and video should be examined for modification, manipulation, tampering and forgery. An organization should know which attack artifacts its automated analysis has been tested against and should document the corresponding false-negative rates. NIST says this information should be available to relying parties on request. A detector trained on one generation of synthetic media may not cover a later one, so coverage and test dates matter.
Authenticated, protected channels
Protect the path from capture to the service with authenticated communications and integrity checks. Encryption in transit prevents eavesdropping, but encryption by itself does not prove that the endpoint’s camera feed is genuine. Channel authentication, application integrity and capture provenance address different parts of the problem and should not be treated as interchangeable.
Biometric presentation-attack detection
For remote biometric collection and comparison, NIST requires PAD capabilities with an impostor attack presentation accept rate (IAPAR) below 0.07 and testing conformant to ISO/IEC 30107-3:2023. PAD is intended to detect an impostor presentation at the sensor; it is not a universal detector for every injected or generated video.

Automated analysis plus manual review
NIST recommends augmenting algorithmic analysis and automated decisions with manual review to address detection errors. A useful workflow specifies when an automated flag is escalated, what evidence a reviewer sees, who can overturn a decision and how an applicant can recover from a false rejection. Attended agents should be trained to notice anomalies and to use unpredictable cues, while understanding that appearance alone cannot identify every deepfake.

Keep records of the media types, manipulation techniques and devices included in testing; the dates and versions of the detection components; measured false negatives; reviewer outcomes; and the reason for each escalation. Without that record, a claim that a system “detects deepfakes” is too broad to evaluate.
How to interpret the IAPAR requirement
The threshold is often misunderstood. NIST’s requirement is:
| Measure | Value and scope | What it does not mean |
|---|---|---|
| Impostor attack presentation accept rate (IAPAR) | Below 0.07 for remote PAD capabilities used in biometric collection and comparison; testing must conform to ISO/IEC 30107-3:2023. | It is not a 7% deepfake success rate, not a probability assigned to one session and not a guarantee that every presentation or injection attack will be blocked. |
IAPAR is a system-level performance measure for tested presentation attacks under specified conditions. It does not measure all possible forged-media injections, and it cannot turn a face comparison into proof of an uncompromised camera feed. Organizations should ask what attack artifacts were tested, under which conditions and with which version of the PAD component.
Compare defenses by the attack stage they cover
| Defense | Primary attack stage | Questions to ask |
|---|---|---|
| Genuine-sensor and device checks | Injection at or before the capture pipeline | Can the system detect a virtual camera, emulator or jailbroken device? Is the expected application running with integrity protections? |
| Media analysis | Modified, manipulated or forged submitted media | Which images, videos and known generative-media signatures were tested? What false-negative rates were recorded? |
| Protected, authenticated channel | Interception or substitution while media travels to the service | Are both ends authenticated, and are integrity failures visible to the service? |
| PAD conforming to ISO/IEC 30107-3:2023 | Presentation to the biometric sensor | What IAPAR was measured, and does the test represent the organization’s capture conditions? |
| Unpredictable attended cues | Replay and pre-rendered response attempts | Can the reviewer issue a fresh instruction, provide an accessible alternative and escalate a failed response? |
| Manual review | Errors and attacks that automated tools miss | Is there a documented escalation path, trained staff and an audit trail for decisions? |
No row covers every attack. The value comes from combining controls that fail in different ways.

What an applicant can realistically do
If you are the person being verified, you generally cannot inspect the service’s capture pipeline or prove that its detector is effective. You can, however, reduce avoidable confusion and protect yourself from social engineering.
- Use the organization’s official verification flow rather than a link sent unexpectedly in a message.
- Check that the page or application is the expected one and that the session is using the permissions you intended to grant.
- Use steady lighting and a stable connection so ordinary exposure or delay does not resemble manipulation.
- Follow newly issued instructions, but stop and contact the organization through a separately verified channel if a reviewer asks for unrelated secrets, payment, remote-control software or an unusual recording.
- Request an approved alternative if a movement or object-placement cue is inaccessible. A failed challenge should not automatically be treated as proof of fraud.
Biometric characteristics are not secrets. NIST notes that facial images may be obtained without a person’s knowledge, so protecting the sensor and processing environment matters as much as comparing the face. Do not assume that because a system has your face image it can authenticate every future use safely.
Common mistakes that produce false confidence
“The face matched, so the video was real.”
A match evaluates similarity to an enrollment or reference image. It does not establish that the sample came from a genuine live sensor or that no media was injected.
“The applicant blinked and turned their head.”
Those actions are predictable and can be replayed or generated. They may be useful only as part of a broader, unpredictable interaction and sensor-integrity process.
“One visual artifact proves a deepfake.”
Compression, poor lighting, autofocus, network jitter and accessibility factors can mimic the same artifacts. A signal should trigger a repeat, alternate cue or escalation—not an unsupported accusation.
Best Value
“A detector’s headline accuracy applies to every attack.”
Performance depends on the media, attack artifacts, device, environment and test protocol. Ask for the tested coverage and false-negative data rather than treating one number as universal.
“A live reviewer can replace technical controls.”
Reviewers add context and can catch errors, but they cannot see media that has already been forged before it reaches their screen. Human review and technical controls address different attack paths.
Deepfake video versus a face morph
A morph attack is related but distinct. A single still image can be synthesized to combine the characteristics of two identities, such as in an enrollment or document-photo process. It is not the same as a real-time injected video in a remote attended session. NIST’s August 2025 guidance discusses operational approaches to detecting morphs; organizations should evaluate that risk separately instead of claiming that a video PAD result covers single-image morphing.
Jurisdiction, privacy and changing technology
NIST SP 800-63A-4 is U.S. federal digital identity guidance. It is an important technical benchmark, not automatically a legal requirement for every organization or country. A service must also account for applicable privacy, accessibility, employment, financial-services and data-retention obligations.
Detection methods and attack techniques evolve. When a system is updated, reassess the capture path, test artifacts, false-negative measurements, PAD conformance and reviewer training. Publish enough information for relying parties to understand the assurance level without exposing details that would help an attacker bypass the controls.
The bottom line
During a remote identity verification, treat lag, lip-sync errors, inconsistent rendering and failed unpredictable cues as warning signs only. The central question is not whether the face looks strange; it is whether the system can establish that media came from a trusted capture path and has not been altered. A defensible process combines genuine-sensor checks, media forensics, authenticated protected communications, PAD tested to the applicable standard, unpredictable attended challenges and trained manual escalation. Even that layered approach reduces risk rather than guaranteeing that every deepfake will be detected.
