Search the nearby rooms for a named key, an inset, a note, a photograph, a diagram, or a missing statue part. Conventional locks open with the matching item or code; environmental gates require you to retrieve a statue piece and reshape the Genesis Configuration so it fits the relief.
Spoiler warning: the guide below gives the early skull-pin solution, the graveyard-house safe code, several key locations, and the demonstrated solution to a looping-room puzzle. Exact campaign coverage is based on currently published walkthroughs and the developer’s puzzle explanation, so it should not be read as a verified list of every optional or late-game puzzle.

How the puzzle system works
Hellraiser: Revival is a first-person survival-horror game for PlayStation 5, released on October 8, 2026. Its locks combine familiar survival-horror conventions with a more unusual world-manipulation tool: the Genesis Configuration. Assistant Game Director Vid Rajin of Mad Head Games described that puzzle box as the center of the game’s puzzle design. It is not merely an object you carry between fights; you rotate, transform, and fit it into parts of the environment to change what the world allows you to do.
That design creates two broad kinds of obstruction. A door may be waiting for a conventional key, code, card, or inset. Alternatively, the apparent lock may be an environmental mechanism: a statue is missing a tongue or hand, a relief needs a particular shape, or a route has to be reconfigured before you can cross it. Some puzzles have one binary answer, while others can accept more than one arrangement. Do not assume that every unusual object is a single-use key or that every door opens from the side facing you.
#1 Best Overall
- A New Story in Hellraiser's Canon – Unwittingly kickstart Pinhead’s ritual to unleash greater pain and pleasure than ever before.
- Harness the Power of the Box – Combine the unique, hellish abilities of the Configuration with an arsenal of earthly weapons to inflict bone-breaking, flesh-searing pain - or is it pleasure? - upon masochistic enemies.
- Fight Against Unholy Enemies – Take on hell's wretches, deviants, cultists, and even its most infernal priests in a story-driven horror survival game.
Rajin summarized the relationship between the puzzle box and the setting by saying, “We use the Genesis Configuration – a puzzle box – at the centre of things,” in the official PlayStation Blog interview. That is the most useful rule for difficult doors: first determine whether the world is asking for an ordinary item or for an altered configuration of the box.
Door and key puzzle categories
| Lock type | Evidence that points to the solution | Action that completes it |
|---|---|---|
| Item lock | A named key, security card, tongue, hand, or heart-shaped inset found elsewhere | Take the item to the matching door, gate, or receptacle and interact with it |
| Code lock | A note, photograph, date, desk diagram, or other nearby numerical clue | Translate the clue into the combination and enter it on the safe or lock |
| Picture or sliding lock | A visual arrangement shown in the room or implied by a diagram | Reproduce the indicated image or sequence on the lock |
| Genesis Configuration gate | A statue is missing a piece and its relief shows the required outline | Recover the piece, then rotate or reshape the Configuration to match the relief |
| Spatial or environmental puzzle | The room loops, a route is blocked, or the geometry signals an alternate path | Change your movement or manipulate the environment instead of forcing the obvious route |
The category matters because it tells you what to search for. A key lock rewards room-by-room exploration. A code lock rewards reading and inspecting. A Configuration gate rewards examining the empty relief and checking whether the relevant statue piece is still missing. A spatial puzzle may have no inventory item at all.
A reliable method for any locked door
- Inspect the door itself. Look for a keyhole, card reader, chains, a mouth, a relief, a slot, a picture panel, or a symbol that tells you what kind of lock you are facing. If the game gives you an interaction prompt, note the wording; “use,” “insert,” and a shape-specific prompt usually indicate different solutions.
- Search the immediate area before backtracking. Read notes, inspect photographs, check desks and shelves, and rotate unusual objects. The official interview specifically describes environmental diagrams serving as lock clues, so a solution may be visible rather than written as a direct instruction.
- Inspect every candidate item. A statue hand, tongue, card, or inset can look like ordinary set dressing until its description reveals a destination. Rotate skulls and box-like objects when the inspection view permits it; a pin or slot may be hidden on a rear or upper surface.
- Match names and shapes. Use a key on a door with the corresponding name, and place an inset in a relief with the same outline. If an item will not insert, look for another receptacle rather than repeatedly trying the same door.
- Test the Genesis Configuration against the relief. Gates connected to statues generally require two stages: restore the missing statue piece, then transform the Configuration to the shape shown in the relief. The missing piece alone does not finish the puzzle.
- Re-check the room after a major change. Opening one door can reveal a card, a key, or a route that advances another lock. If a mechanism appears to do nothing, walk around it and look for a newly exposed interaction point.
This process prevents the most common mistake: treating every locked door as a search for a key while ignoring a visible diagram or a Configuration-shaped socket.
Exact solution: the skull pin puzzle and fleshy mouth door
Spoilers for an early cult-compound sequence: shortly after the first save point and the clothing change, you encounter a fleshy mouth door. Its key is inside a barbed-wire skull sitting on a desk. The skull’s pins must be pressed in a particular order.

- Explore and fight in a story-based campaign to save America
- Pick up the skull and enter its inspection view. Use the orientation you see when it is first picked up as your reference.
- Press the pin on the right side.
- Press the pin on the back/top. This means the pin on the top of the skull’s back, not simply any pin visible from the front. Rotate the skull if necessary.
- Press the pin on the left side.
- Press the pin at the top/center.
- After the fourth press, the jaw opens. Take the Tongue Key.
- Return to the fleshy mouth door and interact with it to use the Tongue Key.
The sequence is therefore right side → back/top → left side → top/center. If the jaw does not open, reset your mental orientation: the “back/top” pin is easy to mistake for a front-facing upper pin when the model is not rotated.
Graveyard-house safe: code 9606
Spoiler: in the house in the graveyard, read the note and then examine the photograph. The walkthrough identifies the two relevant dates as 96 and 06. Enter 9606 into the safe.
The safe contains a pistol upgrade. If the combination is rejected, make sure you have inspected both the note and the photo rather than relying on the numbers from memory. This is an example of the game’s ordinary clue-chain logic: the code is not presented as a separate keypad hint; it is assembled from two pieces of environmental information.

The courtyard contains three gates built around statue mechanisms. The official puzzle discussion describes gates that need missing pieces such as a tongue or a hand. A later walkthrough describes the same two-part procedure: place the recovered Statue’s Tongue or Statue’s Hand, then match the Genesis Configuration to the relief.

Stage 1: find the missing statue piece
Search the surrounding rooms and routes for the named body part. The empty statue socket is your confirmation that you have found the correct type of item. Do not spend time trying to force the Configuration into an incomplete relief; the missing physical piece has to be restored first.
Stage 2: install the piece
Interact with the statue and place the hand or tongue in its corresponding socket. The world state should change, exposing or activating the relief that shows the required Configuration shape.
Stage 3: reshape the Configuration
Take out the Genesis Configuration and rotate or transform it until its outline matches the relief. Approach the fit as a silhouette problem: compare the broad contours, not just the direction of one protruding segment. If the shape seems close but will not register, rotate the box in another axis or inspect the relief from a different angle.

These gates illustrate why the box is a puzzle tool rather than a conventional weapon or key. The object you retrieve and the shape you create are separate requirements.

Red chained bedroom door and the house entrance
A later route uses a chained red bedroom door as part of a key-and-card sequence. First complete the poison-gas statue puzzle. That sequence leads to the Bedroom Key.
- Finish the poison-gas statue puzzle and collect the Bedroom Key.
- Use the Bedroom Key on the red chained bedroom door.
- Search the room you have opened for the Security Card.
- Take the Security Card to the house’s front door and use it on the reader.
This is a useful example of a door that does not yield its final access item immediately. The bedroom key opens an intermediate room; the security card found inside is what advances the house entrance.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Heart-shaped door and inset
The ornate heart-shaped door uses the Genesis Configuration indirectly. A monument has a relief that can be matched with the box. Solving that shape produces a Heart-Shaped Inset.
- Locate the monument and inspect its relief.
- Match the Genesis Configuration to the relief until the mechanism yields the Heart-Shaped Inset.
- Carry the inset to the ornate heart-shaped door.
- Place it in the door’s matching receptacle and interact again if the opening animation does not begin immediately.
If the door refuses the inset, check that you are at the ornate door rather than another decorative heart motif. The matching outline and the interaction prompt should agree.

The repeating-room puzzle: turn around
One demonstrated environmental puzzle takes place in a looping room. The apparent route forward keeps the character trapped in the same pattern. The solution is to turn back instead of trying to move forward. Reversing direction changes the setting and breaks the loop.
This sequence reflects a broader rule for spatial puzzles: when a corridor repeats without revealing a new clue, stop treating forward movement as the only valid input. Turn around, retrace the room, and watch for a change in geometry, lighting, objects, or exits. Rajin described the labyrinth as “intertwined with the real world,” so ordinary navigation assumptions do not always apply.
What to do when a door will not accept an item
- Check the item’s exact identity. A generic statue part may not be interchangeable with a named Tongue, Hand, or Heart-Shaped Inset.
- Inspect both sides of the door and its surroundings. A slot or relief can be offset from the visible door panel, especially on gates tied to statues.
- Look for a prerequisite state. Some mechanisms require a missing statue piece to be installed before the Configuration interaction appears.
- Rotate the inspection object. The skull puzzle hides a required pin on the upper rear surface; similar inspection objects may conceal relevant details.
- Read every note and inspect every photograph. The safe’s 9606 combination comes from two separate pieces of evidence.
- Leave and return after changing the environment. Opening an intermediate room or completing a nearby mechanism can create a new prompt.
- Do not assume a decorative symbol is a lock. If there is no interaction prompt or matching receptacle, continue searching for the actual mechanism.
How to manage exploration and saves
Save before entering an unfamiliar puzzle room when the game gives you a save point. This is especially useful for mechanisms with moving parts, rotating objects, or environmental hazards. Keep a short written record of unexplained symbols, dates, and statue sockets; the game expects you to connect clues that may be separated by a room or an encounter.
When you find a named item, note where it was collected and which door seemed to reference it. A key that does not work immediately is often intended for a nearby intermediate lock, not evidence that the item is unusable. Conversely, if a door uses a relief, stop searching inventory for a conventional key and examine the Genesis Configuration.
What this guide does and does not establish
The solutions above cover the specifically documented skull, safe, courtyard, bedroom, heart-door, and looping-room examples. They are not an independently verified catalog of every main-path, optional, or late-game puzzle in Hellraiser: Revival. Published walkthroughs can change as players discover alternate routes, and the game’s developers have said that some puzzles may permit multiple solutions.
For that reason, use the categories and troubleshooting method even when a particular door is not listed here. The game’s consistent language is evidence-based: inspect the environment, identify the object or shape the lock is asking for, and use the Configuration when the world itself—not an ordinary key—is the mechanism.
