Production monitoring for an AI agent must answer two different questions: are requests flowing, and is the agent completing the intended task safely and correctly? Build one trace for each run, combine ordinary service telemetry with behavior evaluations, establish a workload-specific baseline, and alert on actionable deviations. A model-judge score is evidence for investigation—not proof of correctness by itself.
Why service health alone misses agent failures
Conventional application monitoring reports traffic, latency, exceptions, saturation, and dependency availability. Those signals can all look normal while an agent gives an ungrounded answer, calls the wrong tool, stops before completing a task, or produces an unsafe response. Conversely, a quality regression can be caused by an operational issue such as retrieval timeouts, a failing tool, a changed prompt, or a model-version rollout.

Use two connected views. The operational view answers whether the system is available and performing within its service objectives. The behavioral view answers whether each run achieved the user’s intended outcome. Connect them through a run-level trace that preserves the request or conversation identity, timing, model and agent versions, tool activity, retrieval context, and evaluation results.

1. Define success and failure before collecting data
Write a short, testable definition of a successful run for every agent. “The model produced a good answer” is too vague to monitor. A success statement should identify the observable user outcome and any safety constraints.

Examples of observable outcomes
- A support agent resolves the customer’s issue or correctly routes it to a human.
- A booking agent gathers required fields, uses the approved reservation tool, and returns the confirmed reservation identifier.
- A research agent answers from approved sources, distinguishes uncertainty, and cites the retrieved evidence.
- A coding agent produces a patch that passes the required tests and does not modify protected files.
- A safety-sensitive agent refuses disallowed requests and offers an allowed alternative.
Build a failure-mode inventory
List failures that matter to users, operators, security teams, and finance. Include incomplete task execution, incorrect or duplicate tool calls, stale or irrelevant retrieval, unsupported claims, unsafe content, policy violations, timeout cascades, authentication failures, latency spikes, and unexpected token or model spend. Map each failure to a signal that can detect it and to the trace fields needed to investigate it.
Do not copy a threshold from another workload. The official guidance from Microsoft, Microsoft Foundry, Amazon CloudWatch, and Google Cloud describes signal categories and monitoring patterns, but does not prescribe a universal quality score, minimum sample size, or cutoff. Set objectives from your baseline, user impact, and risk tolerance.
2. Trace the complete agent run
Instrument the whole run rather than only the final model request. OpenTelemetry-compatible traces and spans are a practical stack-neutral foundation. A trace can represent one user request or conversation turn; child spans should expose meaningful work such as planning, model calls, retrieval, tool invocation, guard checks, and downstream services.
Minimum identity and timing fields
- Trace and run identifiers: a globally unique trace ID plus a stable run or conversation ID that lets investigators find related turns.
- Timestamps and durations: start and end time for the run and every span, including queue and waiting time where relevant.
- Version context: agent, prompt, policy, model, tool-schema, retrieval-index, and evaluator versions.
- Outcome status: completed, failed, refused, abandoned, timed out, or escalated, with a controlled reason code.
- Workload labels: tenant or environment, use-case route, language, channel, and other low-cardinality dimensions safe for aggregation.
Span types to capture
| Span | Record | Why it matters |
|---|---|---|
| Agent orchestration | State transitions, iteration count, stop reason, and total duration | Shows loops, premature stops, and planning regressions. |
| Model call | Model identifier, prompt/template version, input and output token counts, latency, finish reason, and error | Separates model behavior from orchestration and cost changes. |
| Retrieval | Index or collection, query, result identifiers, scores, latency, and empty-result status | Links groundedness failures to missing or irrelevant context. |
| Tool call | Tool name and version, validated arguments, authorization result, response status, latency, retry count, and side-effect identifier | Exposes wrong tools, malformed arguments, failures, and duplicate actions. |
| Guard or policy check | Rule set version, decision, category, and enforcement action | Allows safety incidents to be separated from ordinary quality variation. |
| Dependency | Service name, status, timeout, and correlation ID | Connects an agent failure to a database, API, queue, or identity outage. |
Keep parent-child relationships intact so a dashboard point can be opened directly to the affected run. Amazon’s documented agent-monitoring workflow uses traces, spans, sessions, evaluators, real-run datasets, experiments, and production monitoring together. Google’s agent-observability guidance likewise treats trace-derived dimensions as the route from aggregate signals to individual behavior.
Decide what content can be stored
Evaluator context may require the user prompt, model output, system instructions, tool definitions, and retrieved passages. Google’s online-evaluation requirements illustrate why these fields can be necessary, but they may contain personal, confidential, or regulated information. Redact or tokenize sensitive values before export, store hashes or references where full content is unnecessary, and make retention and access decisions before enabling broad sampling.
3. Create a repeatable evaluation set
Assemble a maintained set of representative tasks, difficult edge cases, known incidents, and safety cases. Use real tasks only after removing or protecting sensitive data. Version the set, record expected outcomes or rubrics, and keep examples when behavior changes so a regression can be reproduced.

Combine deterministic checks with evaluators
- Deterministic checks: schema validation, required-field checks, exact identifier matches, permission checks, tool-argument validation, test results, citation presence, and completion-state rules.
- Reference or retrieval checks: whether an answer is supported by approved evidence, whether citations resolve, and whether retrieved content is relevant.
- Rubric-based evaluators: coherence, fluency, relevance, groundedness, task completion, and adherence to instructions.
- Safety and security checks: prohibited-content detection, data-leak tests, prompt-injection cases, unsafe tool actions, and refusal quality.
- Tool-call accuracy: correct tool selection, valid arguments, appropriate ordering, authorization, and avoidance of unnecessary calls.
Choose dimensions that correspond to the actual use case. A judge or aggregate score is a triage signal. Validate it against labeled examples and domain review, inspect disagreements, and retain the underlying rationale or evidence. Never present a single judge score as ground truth.

Pick the symptom - the matching free tool is one click away.
Evaluate continuously at three horizons
- Pre-release suite: run the stable evaluation set for every model, prompt, policy, retrieval, or tool-schema change.
- Sampled live evaluation: evaluate a privacy-approved sample of production runs to detect behavior the fixed set does not contain.
- Scheduled drift evaluation: rerun the stable set and selected fresh examples on a schedule, comparing results with prior versions.
Keep the evaluation output linked to the originating trace. When a score moves, investigators need to see the prompt, retrieved evidence, tool sequence, output, evaluator rationale, and version context—not only a time-series number.
4. Establish a baseline before writing alerts
Collect enough representative traffic to describe normal behavior for each important route, tenant class, language, and risk tier. Report distributions rather than only averages: latency percentiles, token counts, tool-call counts, completion rates, and quality measures. Separate planned releases, incidents, and unusual campaigns from ordinary traffic when calculating the baseline.
Baseline both absolute performance and change from the agent’s own history. A 90% completion rate may be acceptable for one exploratory assistant and unacceptable for a regulated workflow; a fixed industry benchmark cannot decide that. Record the sample size and confidence or uncertainty for each quality estimate so small samples do not create false alarms.
5. Build dashboards that join operations and quality
Place operational and behavioral panels in the same dashboard, with every quality series drillable to traces and examples. Microsoft Foundry documentation describes dashboards for evaluation trends, safety, hallucination or grounding, and tool use; Google documents views for model and tool performance; Grafana’s agent-observability material emphasizes OpenTelemetry traces, evaluations, guards, offline experiments, and cost or performance tracking. These are capability examples, not a mandatory vendor-specific layout.

6. Configure layered, actionable alerts
Use different alert paths for immediate service degradation, behavioral regression, and scheduled drift. Page only when a human can act; send lower-urgency changes to a ticket, chat channel, or daily report. Tune rules against normal variation and include a minimum sample or persistence condition appropriate to the workload.
| Alert class | Example condition | Context to include | Typical response |
|---|---|---|---|
| Availability | Run errors, timeouts, or dependency failures exceed the established service objective. | Route, region, dependency, first-seen time, affected version, and trace links. | Mitigate the outage, fail over, or roll back. |
| Latency or cost | Tail latency, token use, or estimated spend deviates materially from its baseline. | Percentile, cohort, model, tool, retry count, and representative traces. | Check loops, prompts, model changes, throttling, and dependency latency. |
| Task quality | Completion, required-field, groundedness, relevance, or tool-correctness measure falls below the team’s stated objective. | Score, denominator, evaluator version, affected cohorts, examples, and run IDs. | Inspect traces and compare the last known-good version. |
| Safety or policy | A confirmed harmful output, prohibited action, data leak, or policy violation occurs. | Severity, policy rule, redacted evidence, user impact, action taken, and access-controlled trace. | Contain, notify the responsible team, preserve evidence, and assess disclosure obligations. |
| Drift or regression | Scheduled evaluation changes materially from the approved baseline or release gate. | Dataset version, candidate and baseline versions, per-case failures, and evaluator rationale. | Pause release, investigate grouped failures, and add confirmed cases to the suite. |
Alert on meaningful changes from the agent’s baseline, not on every individual low score. For safety-critical events, a single confirmed incident may warrant immediate handling even when aggregate quality remains normal. The threshold and routing must reflect the consequence of the failure.
Make every alert investigable
An alert should answer what changed, where, when, who is affected, and what to open next. Include a link or identifier for the dashboard, trace IDs or session IDs, agent and model versions, tool and retrieval context, evaluator name and version, sample count, baseline comparison, and a suggested owner. If privacy rules prevent putting content in the notification, provide a controlled path to the redacted trace.

7. Close the investigation loop
- Confirm the signal. Check sample size, evaluator health, telemetry gaps, deployment timing, and whether a dependency incident explains the change.
- Inspect representative traces. Compare successful and failed runs, then follow the span sequence from orchestration to model, retrieval, tools, guards, and dependencies.
- Group failures. Cluster by model, prompt or agent version, tool, retrieval collection, route, language, tenant class, and failure reason.
- Reproduce the case. Save the input, relevant context, tool definitions, version metadata, and expected outcome under the organization’s data policy.
- Test a change. Compare candidate prompts, models, policies, retrieval settings, or tool logic against the same evaluation set and selected live examples.
- Promote confirmed failures. Add the case and its expected behavior to the maintained suite; add a deterministic check where one is possible.
- Release with a gate. Require the standard suite and operational checks to remain within the team’s stated quality, reliability, safety, and cost objectives.
Amazon documents a workflow that links production monitoring, traces, evaluation datasets, and experiments. Microsoft describes evaluations as useful for regression testing and release gates. Together, these practices turn an alert from a one-time interruption into a permanent improvement in coverage.
8. Protect telemetry as production data
Telemetry is not harmless metadata. Prompts, outputs, retrieved passages, tool arguments, and traces can contain personal data, credentials, business secrets, or regulated records. Microsoft’s guidance recommends balancing forensic value with privacy, data residency, minimization, retention, legal obligations, access controls, and encryption. It also warns that AI observability is rarely "set it and forget it." It is a continuous process that incurs operational overhead.
- Define which fields are collected, sampled, redacted, hashed, or excluded.
- Encrypt data in transit and at rest; separate production access from evaluator and analyst access.
- Apply least-privilege roles and audit access to sensitive traces.
- Set retention by purpose and jurisdiction instead of retaining everything indefinitely.
- Keep evaluator datasets versioned and governed like other production data.
- Document residency, deletion, legal hold, and incident-response procedures.
- Measure the cost and latency of telemetry export, evaluation, and storage.
9. A practical rollout plan
Phase one: visibility
Instrument run and span IDs, timestamps, versions, outcomes, model calls, retrieval, tools, and dependencies. Build a dashboard for traffic, latency, errors, tokens, and tool performance. Verify that a dashboard point opens the complete trace and that redaction works.

Phase two: measurable behavior
Define success and failure for the highest-value workflow. Create a small but representative evaluation set containing normal tasks, edge cases, and safety cases. Add deterministic checks first, then rubric-based evaluators. Compare results with domain-reviewed examples.

10. How to compare observability platforms
When assessing tools, compare capabilities rather than vendor claims of superiority. The cited Microsoft, Amazon, Google, and Grafana documentation supports these as relevant evaluation axes, but it is vendor documentation rather than a controlled neutral benchmark. No comparative cost or performance ranking is established here.
| Axis | Questions to ask |
|---|---|
| Instrumentation | Does it support OpenTelemetry and the frameworks used by the agent? Can it represent complete runs, sessions, spans, and dependency topology? |
| Evaluation | Can teams define deterministic checks, custom evaluators, domain rubrics, and safety rules? Are evaluator versions and rationales retained? |
| Sampling and scheduling | Can it evaluate approved live samples and run a stable dataset on a schedule with explicit denominators? |
| Trace explanations | Can an aggregate score open the exact model, retrieval, tool, guard, and dependency spans that produced it? |
| Alerting and incidents | Can rules use baselines, cohorts, persistence, safety severity, and incident-routing integrations? |
| Performance and cost | Can dashboards break down model latency, tokens, cost, tool calls, retries, and failure rates without losing run context? |
| Governance | Are redaction, residency, retention, encryption, role-based access, deletion, and audit controls adequate for the data? |
Common mistakes and recovery steps
Monitoring only the final response
Symptom: a poor answer is visible, but there is no explanation. Fix: add parent-child spans for planning, retrieval, tools, guards, and dependencies, and preserve the run identity across retries.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteUsing one blended score
Symptom: a “quality” number hides a safety failure or tool error. Fix: keep task completion, groundedness, relevance, tool correctness, safety, latency, and cost as separate dimensions, then inspect examples.
Best Value
Alerting on tiny samples
Symptom: normal variation pages the team. Fix: display denominators, require an appropriate sample or persistence condition, and use cohort-aware baselines.
Trusting a judge without validation
Symptom: evaluator scores look good while users report failures. Fix: label examples with domain reviewers, measure evaluator disagreement, revise the rubric, and retain deterministic checks where possible.
Discarding sensitive context without a plan
Symptom: incidents cannot be reconstructed, or telemetry creates privacy exposure. Fix: classify fields, redact or tokenize them, define controlled retention, and document which evidence is required for forensic work.
