Sit fully back in the chair, use only enough support to fill that hollow, and keep your feet supported. If the support presses, pushes your torso forward, or increases symptoms, move it, reduce it, or remove it. A rolled towel is a valid temporary option when a chair has no built-in support.
This setup can improve workstation fit, but it is not a treatment or guarantee against lower-back pain. Your chair, desk height, foot support and movement habits all affect comfort.

Where should lumbar support sit?
Lumbar support belongs at the small of your back—the inward curve between your lower ribs and pelvis. The support should meet that hollow and preserve the spine’s normal curve, rather than flattening it or forcing your chest and abdomen forward. The Occupational Safety and Health Administration (OSHA) describes a suitable chair backrest as one that conforms to the spine’s natural curvature and says height-adjustable lumbar support helps it fit the user’s lower back. See OSHA’s Computer Workstations chair guidance.

There is no single correct height, thickness, firmness or shape for every person and chair. The correct position is the one that supports the hollow without creating pressure, a new ache or a feeling that you must lean away from the desk. Adjust by comfort and fit, not by forcing your back into a preset posture.

What correctly positioned support feels like
- You can sit back against the chair instead of hovering in front of the backrest.
- The support contacts the lower-back hollow, not the belt line, tailbone or middle of the rib cage.
- Your shoulders remain relaxed and your head does not have to move forward to see the screen.
- Your hips stay on the seat and your feet remain stable.
- You can breathe and change position without fighting the cushion.
What a poor position feels like
- The support drives your pelvis forward so you slide toward the front edge.
- It presses on one spot, causes numbness or creates a sharper version of your existing pain.
- Your feet lose contact with the floor because the support changes how you sit.
- You have to arch excessively or hold yourself rigidly to stay comfortable.
Those are adjustment signals. Reposition the support, reduce its bulk, or take it out. Continuing to use a painful setup does not make the back adapt safely.
How to set up lumbar support step by step
- Start with the chair back. Sit down and slide your hips all the way back until your back can rest against the backrest. Do not position the support while perched on the seat edge; that changes the distance between your lower back and the chair.
- Find the natural hollow. With your feet supported, place a hand behind your lower back. The indentation you feel is the target area. It is usually above the belt line and below the bottom of the ribs, but the exact level varies with body shape and chair geometry.
- Set a built-in support to that area. If the chair has a height-adjustable lumbar mechanism, move it until its fullest part fills the hollow. Start with the least prominent setting that makes contact, then increase only if the chair still leaves a gap. OSHA identifies height adjustment as a useful chair feature because the lower-back curve differs among users.
- Check that the support is centered. The support should contact both sides of the lower back evenly. A tilted or off-center pad can create one-sided pressure and may make you twist.
- Sit back again and assess the whole posture. Your torso should not be pushed so far forward that your elbows no longer reach the desk comfortably. Your shoulders should stay down, and you should be able to look at the screen without bending your neck.
- Make small changes. Move the support a little at a time or alter its firmness. There is no evidence in the cited guidance for one universally correct measured height or cushion dimension, so use a gradual comfort check rather than a fixed number.
- Confirm the seat and feet. Keep your feet flat on the floor when possible. If the seat must remain high to match the desk and your feet dangle, use a footrest; OSHA and Mayo Clinic both identify stable foot support as part of a workable setup.
- Test normal work movements. Type, use the mouse, read and turn slightly. The support should remain helpful while you work, not only while you sit motionless for a few seconds.
- Recheck after a short working period. If discomfort builds, do not simply tighten the support. Change its position or remove it and reassess the chair and desk relationship.
Can you use a rolled towel instead of buying a lumbar pillow?
Yes. OSHA specifically recommends a rolled-up towel or removable back-support cushion as a temporary way to provide support when a chair lacks lumbar support. CDC’s National Institute for Occupational Safety and Health (NIOSH) also describes making a lumbar support pad or roll for an ordinary chair. OSHA’s wording is: “If your current chair does not have a lumbar support, use a rolled up towel or a removable back support cushion to temporarily provide support and maintain the natural curve of the spine.”
How to make and place the towel
- Use a clean towel that can be rolled into a stable cylinder.
- Begin with a modest roll rather than a thick bundle. A large roll can push you too far forward.
- Place it across the chair at the level of your lower-back hollow, not beneath the pelvis.
- Sit back and secure it if it slips, using the chair’s backrest or a removable strap that does not create a hard pressure point.
- Unroll or reduce it if you feel focal pressure, increased pain or a need to lean away from the desk.
A towel is a fitting experiment, not a promise that a particular diameter will suit everyone. Its advantage is that you can change the roll quickly and remove it without buying equipment.

| Option | Best use | Advantages | Limitations and checks |
|---|---|---|---|
| Built-in, height-adjustable lumbar support | A chair that already fits most of your body | Integrated, stable and adjustable; OSHA identifies height adjustment as useful for matching the lower-back curve | Its range may not reach your hollow; test whether it presses or pushes you forward |
| Rolled towel | Temporary support or a no-cost fit test | Free, removable and easy to reshape; specifically suggested by OSHA and CDC/NIOSH | Can flatten, slip or become too thick; it is a temporary aid rather than a chair adjustment |
| Removable lumbar cushion | A chair with no usable built-in support | Portable and easier to replace or reposition than a chair | Shape and firmness are not universally correct; it must fit the actual chair and user without causing pressure |
| Different adjustable chair | The current chair cannot support your back while allowing stable feet and workable desk reach | May offer a wider adjustment range, including backrest and seat-height changes | It is a conditional solution, not a requirement for everyone; try to confirm fit before replacing the chair |
When comparing options, assess four things: whether the support reaches the small of your back, whether it feels comfortable in your actual chair, whether it lets you sit back with feet supported, and whether you can work without holding one rigid position. A cushion that feels good in a showroom but pushes you forward at your desk is not a successful fit.
Adjust the chair and foot support around the lumbar curve
Lumbar support cannot compensate for a seat that leaves your feet dangling or a desk that forces your shoulders to lift. Once the support is in place, check the rest of the workstation.
Seat height and feet
OSHA advises that feet rest flat on the floor or on a footrest. Mayo Clinic likewise advises flat feet or a footrest, with the thighs parallel to the floor. If lowering the chair makes the desk too high for comfortable arm position, keep the chair at a workable height and add a footrest rather than letting your legs hang.

Stay in contact with the backrest while doing desk work. If you constantly slide forward, the seat angle, cushion thickness or desk reach may be working against the support. Correct the underlying fit instead of adding more lumbar bulk.

Torso-to-thigh angle
In a Mayo Clinic News Network desk-work Q&A, physical therapist Jill Henderzahs-Mason asks whether the chair allows the normal spinal curves and discusses a torso-to-thigh angle of 115 to 120 degrees. That is advice from that Q&A, not a universal target established by OSHA or a population statistic. Read the source at Mayo Clinic News Network.
Desk reach and screen position
After adding support, place the keyboard and mouse close enough that your elbows can remain near your sides. Keep the screen where you can view it without leaning your entire torso forward. If lumbar support causes you to reach farther, reduce its depth or change the workstation rather than sacrificing shoulder and neck comfort.
Why movement matters even when support feels good
A lumbar cushion is one part of a workstation, not permission to remain still. Mayo Clinic advises against sitting in one position for hours and recommends getting up, walking around and changing position. The practical goal is regular variation: stand briefly, walk, shift your sitting position and return to work with the support readjusted if necessary. Guidance from Mayo Clinic’s office-ergonomics page is available at Mayo Clinic’s office ergonomics guide.
Movement also gives you useful feedback. If a position becomes uncomfortable after several minutes, treat that as information about the chair, desk or support. Do not try to stay perfectly still to preserve an “ideal” posture.

Troubleshoot pain or pressure from lumbar support
The support feels too hard or creates a pressure spot
Move it slightly up or down and reduce its prominence. Check that you are sitting fully back and that the pad is centered. If the pressure remains, remove it and test the chair with a thinner towel or no added support.
You feel pushed toward the desk
The support may be too thick or too far forward. Decrease the roll, use a less prominent cushion or adjust the chair’s backrest if possible. Recheck keyboard and mouse reach after every change.
Your feet no longer reach the floor
Do not solve the problem by stretching your toes or hanging your legs. Lower the chair if the desk allows it; otherwise use a footrest while keeping the thighs supported. OSHA and Mayo Clinic both treat foot support as part of the setup.
The towel slips down
Use a smaller, firmer roll, place it against the chair’s backrest before sitting, or choose a removable cushion that stays in position. A slipping support cannot reliably remain at the lumbar level.

One side feels more pressured than the other
Center the support and check whether you are sitting squarely. If the chair or cushion remains uneven, remove the added support and reassess the chair rather than compensating by twisting.
Pain increases during or after use
Stop using the painful configuration. Remove or reposition the support and return to a neutral, comfortable setup. Persistent or worsening pain needs individualized health advice; chair adjustment alone cannot diagnose or treat the cause.
Lumbar support is not the same as a medical back brace
Office-chair support is an ergonomic aid intended to fit the natural lower-back curve. It is not a belt, corset or prescribed brace. NICE’s guideline on low-back pain and sciatica emphasizes self-management approaches for many people and recommends against belts or corsets as a treatment. That recommendation should not be recast as an endorsement or rejection of a particular office cushion. Read the clinical recommendations at NICE guideline NG59.
None of the cited sources provides a statistic showing that a lumbar cushion prevents or cures lower-back pain. The practical evidence-based claim is narrower: fitting the chair to the user, supporting the feet, varying position and removing an uncomfortable aid are sensible workstation measures. They do not replace an assessment when symptoms persist or worsen.
A quick daily setup check
- Lower back: the support contacts the hollow and does not force an exaggerated arch.
- Seat: your hips are back against the chair and you are not sliding forward.
- Feet: both feet are flat on the floor or a stable footrest.
- Legs: thighs are supported and the chair is not cutting sharply behind the knees.
- Arms: keyboard and mouse are close enough to avoid reaching.
- Comfort: no focal pressure, numbness or increase in pain.
- Movement: you have a plan to stand, walk or change position rather than sit motionless for hours.
If any item fails, correct that item before adding more lumbar support. The best setup is the one that lets you work comfortably, move normally and adjust when your body’s response changes.
