How to Set Up Human Approval for AI Agents in Banking Workflows

Make approval a technical control, not a courtesy button. A banking AI agent may prepare a payment, change a customer record, open an account, or call several tools in sequence, but a gated action must remain impossible until an authorized person approves that specific request. Build the control around risk, least-privilege identities, an explicit decision record, monitoring, and a tested stop path.

What “human approval” must accomplish

Human approval is meaningful only when the reviewer can make an informed decision and the agent cannot complete the consequential action before that decision. A person clicking “approve” after the transaction has already executed is not oversight; it is a notification.

As an Amazon Associate I earn from qualifying purchases.

For an agentic workflow, approval is a control point between planning and execution. The agent can gather information, draft a recommendation, and assemble a proposed action. A separate authorization service or downstream tool then checks whether a valid approval exists for that exact request. If approval is absent, expired, revoked, or no longer matches the request, execution stops.

  • Planning: the agent interprets the task and proposes one or more actions.
  • Review: an authorized person sees the evidence, policy results, uncertainty, and consequences.
  • Decision: the reviewer approves, rejects, requests changes, or escalates.
  • Execution: a controlled service verifies the decision and performs the action.
  • Evidence: the complete chain is logged and monitored for later review.

The control should be stricter for actions that affect customers, move money, disclose sensitive data, alter records, or are difficult to reverse. Low-risk, reversible work may use sampling or post-action review, while high-impact actions generally need an individual decision before execution.

What current guidance does—and does not—require

There is no single regulator-prescribed approval pattern for every banking AI agent. The Office of the Comptroller of the Currency’s Bulletin 2026-13, issued April 17, 2026, describes revised interagency model-risk guidance from the OCC, Federal Reserve Board, and FDIC. It covers model development and use, validation and monitoring, governance and controls, and vendor or third-party products for models within its scope. It expressly says generative and agentic AI are outside that scope, and that the guidance is not prescriptive or enforceable.

The bulletin says, “Generative AI and agentic AI models are novel and rapidly evolving.” Treat that statement as a reason to apply disciplined governance, not as an agent-approval mandate. The agencies also indicated that they planned further information-gathering on generative and agentic AI, so confirm current agency positions when a control is being approved.

FFIEC’s Authentication and Access to Financial Institution Services and Systems; Interagency Guidance is relevant to the identity side of the design. It addresses customers as well as employees, third parties, service accounts, applications, and devices. Its practices include risk assessment, appropriate authentication, layered controls, and monitoring, logging, and reporting. The guidance states that it does not establish a new compliance standard or impose new requirements.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The Federal Reserve Board’s compliance plan for OMB Memorandum M-25-21 offers an example of internal federal-agency governance. It describes use-case review, risk-impact assessment, records, technical controls that can terminate noncompliant AI use, and explicit approval for high-impact uses. Its policy says, “Any high-impact AI use requires explicit CAIO approval.” That is the Board’s own process, not an automatic requirement for banks.

These sources support a risk-based design: tailor controls to the institution, workflow, users, systems, and consequences instead of copying one universal approval screen.

Start with a complete workflow inventory

Before selecting an interface or approval product, document what the agent can see and do. The inventory is the boundary for every later decision and should have an owner who can approve changes.

Record the workflow and ownership

  • Business purpose and process name.
  • Accountable business owner and technology owner.
  • Customer, employee, counterparty, or internal population affected.
  • Agent name, model family, model version, orchestration code version, and policy or prompt version.
  • Every data source, retrieval index, API, plug-in, queue, and downstream system.
  • Actions the agent may propose, actions it may execute automatically, and actions that always require approval.
  • Third-party model, platform, identity, hosting, and core-service dependencies.
  • Change-review route, approvers, and reauthorization triggers.

Give each use case a stable identifier. Put that identifier into approval records, logs, alerts, and incident tickets so an auditor can follow one decision from the original request through execution.

Map the action chain

Draw the actual sequence, including retries and branches. An agent that first looks up a customer, then calculates an amount, then calls a payment API has several opportunities for drift. Mark where data is transformed, where policy checks run, and where an action becomes irreversible. The approval boundary belongs immediately before the consequential operation, not several steps earlier in the plan.

Classify risk by consequence, not by the word “AI”

Classify each proposed action using the outcome if it is wrong, delayed, exposed, or repeated. The same model can require different controls in different workflows.

Risk dimension Questions to answer Control implications
Customer and financial impact Could this deny service, change a customer’s position, move funds, set a price, or create a liability? Require a qualified reviewer, transaction-specific approval, and stronger authentication.
Operational impact Could an error disrupt a core process, create a backlog, or affect many records? Use limits, staged rollout, rate controls, rollback or compensation, and an escalation owner.
Privacy and confidentiality Does the request contain personal, account, payment, or confidential institutional data? Minimize fields, restrict retrieval and export tools, and record who accessed the evidence.
Security and fraud Could the action change credentials, beneficiaries, permissions, or security settings? Use independent identity checks, separation of duties, and a hard deny path for anomalies.
Compliance and conduct Could the action affect a regulated decision, disclosure, record, or report? Route to a reviewer with the right authority and preserve the policy results and rationale.
Reversibility Can the action be undone completely, and within what time? Irreversible or hard-to-reverse actions need pre-execution approval; reversible work may support sampling.

Document the rationale for the classification. A risk assessment should explain why a particular action is auto-executed, sampled, or blocked pending approval. Reassess it when the agent gains a tool, the model changes, the transaction limit changes, or the affected population expands.

Define the permission boundary and identities

Do not give an agent a broad employee account and rely on a prompt to keep it within bounds. Create a distinct identity for the agent or service, and make the execution tool enforce what that identity can do.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Specify allowed and prohibited capabilities

  • Data classes and fields the agent may read.
  • Systems, APIs, queues, and files it may call.
  • Permitted action types, currencies, accounts, recipients, and transaction limits.
  • Time, geography, business-unit, and environment restrictions.
  • Actions that are always prohibited, such as changing its own permissions or bypassing a review.
  • Whether a human may edit the proposed amount, recipient, content, or scope before approval.

Use separate identities for the agent, the reviewer, and the execution service. Authentication should be risk-appropriate and layered. A reviewer’s identity, role, and authority must be checked at decision time, not inferred from a display name in a message.

Separate preparation from execution

The agent can create a draft object with a unique request ID. The execution service accepts that request only when its policy engine finds an unexpired approval for the same use-case ID, action type, target, material parameters, and policy version. The service should reject a request if any of those values changed after approval.

Assign approval authority and escalation

For every gated action, name the accountable reviewer role, a backup, and an escalation owner. Match authority to the consequence and avoid circular approval in which the agent selects the reviewer or a requester approves their own action.

Use a decision matrix

Action profile Typical decision path Minimum evidence to present
Low consequence, reversible, narrow scope Owner approval or sampled post-action review, if the risk assessment permits automation. Action summary, policy result, target, and undo method.
Moderate consequence or customer-facing change Named business reviewer before execution; escalate when evidence is incomplete. Source records, proposed change, uncertainty, affected customer or process, and consequences.
High consequence, sensitive, or difficult to reverse Qualified reviewer with independent authority; use a second approver or specialist when policy requires. Full evidence trail, policy checks, limits, conflicts, model or agent version, and explicit rationale.

Do not let a timeout silently become approval. Define what happens when no one responds, the reviewer is unavailable, or the request is disputed. For consequential actions, the safe default is no execution, followed by escalation or manual handling.

Design an approval request a person can actually assess

An approval screen should expose the decision, not merely the agent’s prose. Put the material facts above the decision controls and make supporting evidence inspectable.

  • Action: exactly what will happen, including amount, recipient, account, record, message, or permission change.
  • Scope: affected customer, case, account, population, and downstream systems.
  • Evidence: source records and timestamps, with links or references that the reviewer can open.
  • Reasoning summary: the agent’s recommendation and the rules or policies it applied, without presenting generated text as proof.
  • Uncertainty: missing data, conflicting records, low-confidence extraction, or unresolved exceptions.
  • Consequences: expected customer, financial, operational, privacy, and compliance effects, plus reversibility.
  • Identity and version: agent, model, policy, and workflow versions that produced the request.
  • Choices: approve, reject, request changes, or escalate, with a required reason where appropriate.

Require the reviewer to inspect material edits. If the reviewer changes an amount or recipient, create a new request version and rerun policy checks rather than silently modifying the approved object.

Put the gate in the execution path

The strongest design is a server-side check at the tool or service that performs the action. A front-end button, prompt instruction, or workflow status is not enough if another route can call the same API.

Properties of a robust approval token

  • Unique request and approval identifiers.
  • Stable use-case and action identifiers.
  • Hash or exact representation of material parameters.
  • Reviewer identity, role, authentication event, and decision timestamp.
  • Policy and agent versions evaluated at approval.
  • Expiration time and one-time-use or replay protection.
  • Revocation state and a reference to any later amendment.

When the execution service receives a call, it should verify the token, compare the request with the approved parameters, confirm the reviewer’s authority, check that the approval is still valid, and record the result. Reject stale, duplicate, altered, or out-of-policy requests. Renew approval after a material change to amount, recipient, content, target, or context.

This hard gate is an engineering inference from access-control and technical-guardrail principles; the cited guidance does not prescribe a particular token format or expiry period. Set those values through your risk assessment and document them.

Handle timeouts, exceptions, and emergencies

No response

Set a workflow-specific deadline and escalation path. On expiry, pause the action, notify the backup owner, and create an auditable timeout event. Never convert silence into consent for a consequential operation.

Changed or malformed requests

Reject requests with missing fields, unexpected tool arguments, changed beneficiaries, altered amounts, or a different customer context. Send them back for correction and require a new approval after the corrected request passes policy checks.

Emergency stop

Provide a control that can disable the agent, revoke outstanding approvals, block a tool, or quarantine a queue. Restrict that control to authorized operators, test it regularly, and record who used it and why.

Rollback and manual fallback

For actions that can be reversed, define the compensating transaction and who may initiate it. Where reversal is impossible, route the case to a manual process before execution and preserve the original evidence.

Log and monitor the entire decision chain

Logging only the final transaction loses the information needed to determine whether oversight worked. Capture the request, decision, execution, and any deviation as one correlated record.

Record Fields to retain
Workflow context Use-case ID, business owner, environment, customer or case reference, and start and end times.
Agent context Agent identity, model and orchestration versions, policy or prompt version, tools invoked, and relevant input or evidence references.
Approval event Request ID, proposed action, material parameters, reviewer identity and role, authentication result, decision, edits, reason, and timestamp.
Execution event Token validation result, downstream response, transaction or record ID, retries, and final status.
Exception event Timeout, denial, override, emergency stop, policy violation, incident number, and remediation.

Monitor denied, timed-out, repeated, anomalous, and out-of-policy attempts. Alert on unusual approval rates, repeated changes to the same request, attempts to call blocked tools, reviewer conflicts, and execution without a matching approval record. The FFIEC guidance supports monitoring, logging, and reporting, while the Federal Reserve Board’s plan describes use-case and determination records and monitoring for anomalous activity.

Test before production and after every material change

Approval controls fail in edge cases, especially when an agent retries or when a downstream system is unavailable. Test the complete path with realistic but controlled data.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Attempt to call the gated tool with no approval.
  2. Replay an approval after successful execution.
  3. Change the amount, recipient, customer, or content after approval.
  4. Use a reviewer with the wrong role or an expired authentication session.
  5. Submit duplicate requests and concurrent approvals.
  6. Simulate a policy-service, identity-service, queue, or core-system outage.
  7. Verify timeout, escalation, emergency-stop, rollback, and manual fallback behavior.
  8. Inject malformed tool arguments and evidence conflicts.
  9. Confirm logs contain every decision and correlate to the downstream result.
  10. Review denied and anomalous events with operations, security, compliance, and the business owner.

Repeat the tests when the model, agent code, prompt or policy, tool permissions, transaction limits, reviewer roles, vendor, or downstream integration changes. Reauthorize the use case when the risk classification or affected population changes.

Include vendors and dependencies in the control design

An approval workflow can be undermined by a third-party model, agent platform, API gateway, identity provider, or core-service integration. Apply the same inventory and change discipline to every dependency.

  • Identify which provider hosts the model and where prompts, evidence, and logs are processed.
  • Define provider access to production data, administrative functions, and approval records.
  • Require notice and review for model, software, endpoint, or permission changes that could alter behavior.
  • Test continuity when the provider, identity service, or network is unavailable.
  • Verify that provider logs and retention support your investigation and reporting needs.
  • Ensure contract and exit plans cover data return, deletion, incident notification, and replacement of the dependency.

OCC guidance discusses vendor and third-party product considerations for models within its scope, and FFIEC access guidance remains relevant whether systems and controls are supplied by the institution or a third party.

Choose an implementation pattern by risk and operating reality

There is no universally superior pattern. Compare designs against the consequence and reversibility of the action, the agent’s autonomy and tool breadth, reviewer expertise and independence, strength of the execution gate and identity controls, auditability, monitoring quality, latency, staffing, and third-party dependence.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Pattern Best fit Trade-offs
Pre-execution approval for every item High-impact, sensitive, or hard-to-reverse actions. Strong prevention; requires reviewer capacity and may increase latency.
Threshold or exception approval Large volumes of low-risk work with clear limits and reliable policy checks. Efficient, but classification and thresholds must be monitored for drift.
Two-person approval Separation-of-duties, fraud, or particularly consequential actions. Independent control is stronger; staffing and turnaround costs are higher.
Sampled post-action review Reversible, low-consequence tasks with demonstrated performance and rapid detection. Finds errors after the fact and is unsuitable where harm cannot be undone.
Queue with specialist escalation Cases requiring legal, compliance, fraud, or operational expertise. Better judgment for exceptions; queue design and service levels become critical.

Use different patterns within one workflow when actions have different consequences. A single blanket setting is usually less defensible than a documented matrix tied to action types and limits.

Governance, records, and accountability

Assign responsibility across business, risk, compliance, security, technology, and operations. The business owner remains accountable for the outcome; technology owns implementation and availability; risk and compliance challenge the classification and controls; security owns identity, access, and incident response; operations owns queues, staffing, and fallback procedures.

Keep a current use-case register and approval-policy version history. Record why the workflow was approved, what evidence supported the decision, who accepted residual risk, and when the next review is due. Retain records according to applicable legal, regulatory, privacy, and records-management requirements.

Do not present generated explanations as independent evidence. Reviewers should be able to inspect source records and policy outputs, and the organization should be able to reconstruct what the agent saw, proposed, and attempted.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

EU and other jurisdictional considerations

A search result for Regulation (EU) 2024/1689, the EU AI Act, includes human-oversight language for high-risk systems: designated natural persons should be informed and able to decide whether and how to intervene or stop the system. The operative article, scope, application date, and amendments should be checked against the current EUR-Lex text before giving EU compliance advice. Whether a banking workflow is covered depends on the system’s role, provider or deployer status, and applicable dates.

For any jurisdiction, evaluate customer-protection, outsourcing, privacy, operational-resilience, records, anti-fraud, and model-governance obligations that apply to the particular product and process. This implementation guide cannot determine whether a specific workflow is lawful or compliant.

Production-readiness checklist

  • Every use case has an owner, identifier, risk classification, and review date.
  • The agent, reviewer, and execution service have distinct identities and least-privilege permissions.
  • Gated actions are blocked at the executing service until a valid, matching approval exists.
  • The reviewer sees material evidence, uncertainty, policy results, and consequences.
  • Edits create a new request version and require renewed checks and approval.
  • Timeout, escalation, emergency-stop, rollback, and manual fallback paths are documented and tested.
  • Logs correlate the agent request, human decision, tool call, and final result.
  • Monitoring alerts on bypass attempts, anomalies, repeated denials, timeouts, and out-of-policy activity.
  • Material model, code, policy, tool, vendor, limit, or population changes trigger review and testing.
  • Vendor access, continuity, logging, incident notification, and exit arrangements are documented.

A well-designed checkpoint is therefore a small authorization system embedded in the workflow, not a cosmetic human-in-the-loop label. It combines a risk-based decision, an informed and accountable reviewer, a server-side execution gate, and evidence that can be monitored and audited.
