Automatic execution is appropriate only for low-consequence, reversible tasks with effective monitoring; consequential or irreversible actions should wait for human approval.
A control is not effective merely because a person can see an AI output. The reviewer must be able to understand it, challenge it, request more information, approve, modify, reject or defer it, escalate unusual cases, and safely stop or reverse the system. The workflow should then be measured and revisited whenever the model, data, process, law or risk tolerance changes.

What a human-approval control must accomplish
Human approval is a decision control, not a decorative sign-off. It sits between an AI-generated recommendation and an action that affects a person, organization, asset or regulated process. The control should reduce the likelihood or impact of an erroneous, biased, unsafe or unauthorized outcome while preserving a practical way to complete legitimate work.
A complete control answers six questions for every decision type:

- What can go wrong, who could be affected and how serious would the result be?
- What exactly may the AI do: draft, recommend, rank, decide or execute?
- Who is accountable for the final decision, and what competence, training, time and authority do they have?
- What information must the reviewer see to interpret the recommendation and its limits?
- Which outcomes are possible, including rejection, modification, deferral and escalation?
- What evidence proves what the system produced, what the reviewer saw, what they decided and what finally happened?
Design the workflow around the decision and its consequences rather than around a particular model. A highly accurate model can still require strict oversight if an error is difficult to reverse; a less capable model may be acceptable for a low-impact suggestion when a person can easily correct it.
Start with risk and operating context
Map the people and interests at stake
List everyone who could be affected directly or indirectly. Include the applicant whose eligibility is assessed, the patient whose treatment is supported, the employee whose access is changed, the customer whose account is restricted and staff who must act on the output. Note legal, safety, financial, privacy, security and fundamental-rights consequences separately; a decision may be low-risk in one dimension and high-risk in another.
Assess severity, reversibility and time pressure
Describe the outcome of a wrong decision, not just the model’s intended purpose. Record whether a mistake can be corrected before harm occurs, how long correction takes, whether evidence is lost, and whether the affected person can appeal. A reversible recommendation with a generous review window can use a lighter control than an irreversible action taken during an emergency.
Account for data sensitivity and foreseeable misuse
Identify personal, confidential, health, financial and security-sensitive information in the inputs and the review screen. Consider how a legitimate user, a rushed operator or an attacker could misuse the output. The EU AI Act’s human-oversight requirement is intended to address risks arising both from intended use and from reasonably foreseeable misuse, so the misuse analysis belongs in the initial control design rather than only in incident response.
Write a decision-specific risk statement
For each workflow, record a short statement such as: “The system ranks applications for a staff member; it does not make the eligibility decision. A mistaken ranking could delay service, so the reviewer must inspect the underlying evidence and may request additional records.” This statement gives product, compliance and operations teams a common boundary when the model or interface changes.
Choose an autonomy tier
NIST’s AI Risk Management Framework describes human-AI configurations ranging from fully autonomous to fully manual, with advisory and delegated forms between those endpoints. Treat autonomy as a deliberate control setting, not as an accidental consequence of automation.
| Configuration | What the AI does | Approval expectation | Suitable boundary |
|---|---|---|---|
| Fully manual | A person performs the decision without an AI output. | No AI approval step; ordinary human governance still applies. | Use when automation adds more risk than value or evidence cannot be reviewed reliably. |
| Advisory | The AI supplies information or a recommendation while the person decides. | Reviewer records an explicit outcome and can disregard the recommendation. | Useful where expert judgment remains central and the recommendation is easy to challenge. |
| Delegated | The AI handles a defined class of cases under human-set rules and exception handling. | Approval is required for cases outside the delegation boundary, sampled cases or triggered exceptions. | Use only when the boundary, escalation route and monitoring are documented. |
| Fully autonomous | The AI makes and executes the decision without case-by-case human approval. | Pre-approval is replaced by tested constraints, monitoring, intervention and a recovery path. | Reserve for low-consequence, reversible tasks where anomalies can be detected before material harm. |
When automatic execution can be defensible
Automatic execution should be limited to actions whose consequences are minor or readily reversible, where the system has a defined operating boundary and monitoring can detect failures promptly. Examples might include sorting an internal queue or applying a non-destructive formatting suggestion. The example is illustrative; the actual classification depends on your context, data and recovery capability.
When approval must precede action
Require a human decision before an output changes a person’s rights, access, safety, treatment, employment status, eligibility, finances or legal position; sends an irreversible instruction; or triggers an external action that cannot be reliably recalled. If a reviewer cannot intervene before the consequence occurs, redesign the sequence or reduce the AI’s authority.
Assign an accountable reviewer
Define competence and training
Name the role, team or individual responsible for the final decision. Specify the domain knowledge required, the training on the model’s capabilities and limitations, the indicators of abnormal output, and the procedure for handling uncertainty. Training should include examples of plausible but wrong recommendations so that reviewers practise challenging the system rather than merely operating the interface.
Give the reviewer authority, independence and time
The reviewer must be able to reject or reverse the output without an informal penalty for slowing automation. State who can make the final decision, who can escalate a case, who can stop processing and who can resume it. Set service-level expectations that reflect the decision’s urgency; a queue that leaves reviewers seconds per case turns nominal oversight into rubber-stamping.

Manage conflicts and coverage
Define conflict-of-interest rules and a substitute process for absences, outages or surges. A single person should not be treated as available oversight if that person is also responsible for meeting a throughput target that discourages scrutiny. For high-impact work, document the separation between the team that builds or operates the system and the role that approves individual outcomes when such separation is practical.

1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsDesign the approval screen around evidence
The reviewer should not have to reconstruct the case from hidden logs or multiple systems. Put the recommendation and the material evidence in one review context, while respecting access controls and data minimization.
Show the recommendation and its scope
State what the AI proposes, what action would follow, the decision class, the model and version, and any rule that limits the recommendation. Distinguish a prediction, a ranking, a suggested text and an instruction to execute; ambiguity about the output type is a common source of unintended delegation.
Show meaningful uncertainty and limitations
Display confidence or uncertainty when it has a defined interpretation for that model and decision. Do not present a number as a probability of correctness unless it has been validated for that context. Explain known blind spots, out-of-scope conditions, missing inputs, data freshness and any reason the output may be unreliable. If no meaningful uncertainty measure exists, say so plainly and direct the reviewer to the qualitative warning signs instead.
Provide provenance and material inputs
Identify the records, data ranges, policies or documents that materially influenced the output. Show their source and timestamp where available, and mark information that was unavailable or excluded. A reviewer should be able to distinguish evidence retrieved from an authoritative record from text generated by the model itself.

Support investigation and accessibility
Include a way to request more evidence, open the underlying record, flag a suspected data error and ask for specialist review. Make controls usable with the organization’s accessibility requirements, keyboard navigation and assistive technologies. If the reviewer cannot read, interpret or operate the evidence panel under realistic time pressure, the control is weaker than its specification suggests.
Make the reviewer’s decision explicit
Use distinct decision states
Provide separate actions for approve, modify, reject, defer and escalate. A reviewer’s act of opening a case, viewing an output or allowing a timeout must not be recorded as approval. Define what each state does to the downstream workflow and which role may select it.
Capture a reason without forcing a false explanation
Require a reason for rejection, modification, escalation or override, using structured options plus a free-text explanation where needed. Do not force reviewers to choose a misleading reason merely to complete the form. For an approval, capture the policy or evidence relied on when the decision’s risk warrants it; for a routine low-impact case, a shorter rationale may be proportionate.
Prevent passive acceptance
Use an intentional confirmation step that states the proposed consequence and identifies the reviewer. Make unresolved warnings visible before submission. Where workload data shows unusually rapid approvals, repeated identical rationales or almost no use of rejection and escalation, treat that as a signal for review rather than proof of quality.

Provide override and safe-stop controls
Allow the reviewer to disregard or reverse the output
Article 14(4)(d) of Regulation (EU) 2024/1689 requires reviewers of high-risk systems to be able to decide not to use the system or to disregard, override or reverse its output in a particular situation. Implement that authority in the product: the reviewer should be able to replace the recommendation, cancel the proposed action and route the case through a manual path.

Define a stop procedure
Specify who can pause processing, which actions are blocked during a pause, how in-flight cases are handled and who authorizes restart. The stop path should work when the model produces anomalous output, an upstream data source fails, a security event occurs or the review service is unavailable. A safe state is one that prevents further consequential actions while preserving the information needed for recovery.
Test controls under realistic failure conditions
Exercise timeout, outage, duplicate submission, partial execution and anomalous-output scenarios. Verify that a stop actually prevents downstream actions, that a reversal reaches every affected system and that queued cases do not silently execute after a restart. Record the test date, scenario, observed result and remediation; do not claim a control works until it has been tested in the environment where it will operate.
Build an evidence chain for every decision
Logging should let an authorized auditor reconstruct the case without relying on memory or an unverifiable screenshot. Retain only what is necessary, protect sensitive records and set a retention period that matches legal, contractual and operational needs.

| Inputs and provenance | References to material input records, source, time and data-quality warnings. | Allows later verification without copying unnecessary personal data. |
| Output and evidence shown | Recommendation, uncertainty display, limitations, retrieved evidence and warnings presented to the reviewer. | Establishes what the reviewer could reasonably evaluate. |
| Reviewer action | Reviewer identity or role, decision state, timestamp and rationale. | Demonstrates that an authorized person made an explicit choice. |
| Override and escalation | Replacement decision, escalation destination, reason and response. | Reveals where the AI was not followed and whether support was available. |
| Final outcome | Action taken, later correction, appeal or incident linked to the case. | Connects approval activity to real-world results. |
Protect the audit record
Restrict log access by role, separate operational access from audit access where appropriate, and protect records against unauthorized alteration. Document retention and deletion rules, including how a legal hold or an investigation changes ordinary deletion. If a sensitive input cannot be retained, keep a stable reference or integrity record that allows an authorized party to verify what was used without exposing the data broadly.
Monitor whether oversight is working
Track outcome and workload signals
Measure approval, modification, rejection, deferral, escalation and override rates by decision type and reviewer group. Also track review latency, repeat reviews, queue age, outage-related fallbacks and incident rates. A high approval rate is not inherently good or bad; interpret it alongside sampled quality, reviewer workload and the consequences of missed errors.
Sample for automation bias and missed anomalies
Review completed cases independently or with a second assessment at a cadence appropriate to risk. Look for reviewers accepting a recommendation despite contradictory evidence, copying model language into rationales without analysis, overlooking warnings or escalating too rarely. Compare samples across teams and shifts to identify training, interface or workload effects.
Investigate drift and change thresholds
Monitor changes in input populations, missing-data patterns, output distributions and error reports. When performance or operating conditions shift, adjust the autonomy boundary, evidence shown, staffing or escalation threshold rather than assuming the original design still fits. Keep a record of the trigger, decision, owner and effective date for each change.

Reassess controls across the lifecycle
Schedule a control review after a model or prompt update, a change to input data, a workflow or user-interface change, a serious incident, a new type of foreseeable misuse, a staffing change, or a change in law or risk tolerance. Reconfirm the autonomy tier, reviewer authority, evidence display, stop behavior, logging fields and monitoring thresholds.
Maintain versioned approvals for the control design itself. When a new model is introduced, run it through the same decision scenarios and failure tests before allowing production use. If the model’s output format or uncertainty meaning changes, update reviewer training and audit queries at the same time; otherwise an apparently stable workflow may be recording incomparable decisions.

Compare alternative control designs
When choosing between designs, score each one against the decision’s actual risk rather than selecting the most automated option. The relevant dimensions are:
| Dimension | Questions to ask |
|---|---|
| Consequence coverage | Does the design address health, safety, legal, financial, privacy and fundamental-rights harms that are reasonably foreseeable? |
| Reviewer authority | Can the reviewer refuse, modify, reverse and stop the system without an obstructive approval chain? |
| Evidence quality | Are material inputs, provenance, uncertainty and limitations available at decision time? |
| Interpretability | Can a trained reviewer understand what the output means and when it is out of scope? |
| Escalation and safe stop | Is there a clear route for unusual cases, outages and anomalous output, with a verified safe state? |
| Auditability | Can an authorized person reconstruct the recommendation, review and outcome from protected records? |
| Latency and staffing | Can qualified reviewers complete the work within the required time without incentive to rubber-stamp? |
| Accessibility and privacy | Can the intended reviewers use the interface, and does it expose only necessary sensitive data? |
| Operational resilience | Does the process continue safely during model, network, data-source or staffing failure? |
Proportionality is the governing principle: a lightweight review may fit a low-impact recommendation, while the same design is inadequate for a high-risk or irreversible decision.
How the main frameworks describe human oversight
EU AI Act Article 14 and Recital 73
Article 14 of Regulation (EU) 2024/1689 requires high-risk AI systems to be designed so that effective human oversight is possible. Oversight must be proportionate to the system’s risks, autonomy and context of use. Reviewers must understand capabilities and limitations, monitor for anomalies and dysfunctions, remain aware of automation bias, correctly interpret outputs, decide not to use or to override or reverse them, and intervene or interrupt through a stop procedure.
The Act states that human oversight should aim “to prevent or minimise the risks to health, safety or fundamental rights that may emerge when a high-risk AI system is used in accordance with its intended purpose or under conditions of reasonably foreseeable misuse.” This is a risk objective, not a requirement that every AI output receive identical manual treatment.
Recital 73 adds that oversight should include operational constraints the system cannot override. Reviewers need competence, training and authority, and the system should guide them about if, when and how to intervene or stop it. Translate those principles into permissions, interface instructions and tested controls rather than leaving them as policy language.
NIST AI Risk Management Framework
NIST’s Govern function calls for transparent policies and procedures, executive ownership of AI-risk decisions, training for personnel and partners, and differentiated roles for human-AI configurations and oversight. Its Govern 3.2 formulation is explicit: “Policies and procedures are in place to define and differentiate roles and responsibilities for human-AI configurations and oversight of AI systems.” Use that separation to assign ownership for model operation, case review, incident response and control testing.
NIST’s discussion of configurations is useful when documenting the autonomy tier. It avoids treating “human in the loop” as a single architecture and instead distinguishes fully autonomous, advisory, delegated and fully manual arrangements. Your documentation should identify which arrangement applies to each decision and what boundary changes it.
FDA context for regulated decisions
For AI that supports drug or biologics regulatory decisions, the U.S. Food and Drug Administration describes a risk-based credibility assessment tied to a specific context of use. The implication for approval controls is practical: define exactly what the model is being used to support, identify the consequence of being wrong in that context, and evaluate evidence and oversight for that use rather than making a broad claim that the model is credible for every purpose.
Illustrative control patterns
Low-consequence internal triage
An AI may sort non-urgent internal requests into queues. Keep execution limited to routing, allow staff to correct the category, and monitor misroutes and aging. If a route could trigger a legal, safety or access consequence, that downstream action needs its own approval boundary even if the initial classification is harmless.
Recommended Free Tools
Eligibility recommendation with an appeal path
The AI can summarize records and recommend an eligibility outcome, but a trained reviewer must inspect material evidence, record approve, modify, reject, defer or escalate, and notify the affected person through a process that supports correction. Preserve the evidence shown and any later reversal so an appeal does not depend on an unverifiable model state.
Best Value
Safety-sensitive operational instruction
Do not let a recommendation directly execute an instruction that could create physical danger or an irreversible system change. Require an authorized operator to confirm the exact action, provide a tested stop command and define what happens if communications fail. If the operator cannot intervene in time, the AI should be limited to advisory output or removed from that step.
Common failure modes and corrective actions
“A person is present, so the system is supervised”
Presence is not oversight if the person lacks authority, evidence, training or time. Correct it by naming the decision owner, defining the permitted actions and measuring review quality and latency.
Approval is inferred from inactivity
A timeout, screen view or lack of objection can silently become authorization. Replace implicit approval with an explicit state and a safe default that prevents consequential execution when the reviewer or service is unavailable.
Recommended Free Tools
The interface hides uncertainty and provenance
Reviewers then see a confident-looking answer without knowing what data supported it or whether the case is out of scope. Add material inputs, source information, limitations and a meaningful uncertainty explanation before the decision buttons.
Override exists but is operationally punished
If overrides require a manager’s permission, add excessive paperwork or damage performance metrics, reviewers will avoid them. Make override a normal, logged outcome; review patterns for learning rather than treating every disagreement as misconduct.
The stop control is untested
A button that does not halt queued actions or cannot be used during an outage is not a reliable safeguard. Test model, network, data-source and duplicate-execution failures, then fix the downstream behavior and record the result.
Logs capture the answer but not the review context
Without the version, evidence shown, warnings and rationale, an audit cannot determine what the reviewer knew. Expand the evidence chain and protect it with access and retention controls.
Free tools Windows power users keep installed
One-click scans. No signup required.
Staffing makes careful review impossible
Backlogs and unrealistic service levels encourage rapid acceptance. Recalculate capacity from observed review time, route high-risk cases to qualified staff, and reduce autonomy or pause intake when coverage falls below the safe level.
Controls remain unchanged after a model update
New output behavior, data sources or model limits can invalidate old training and thresholds. Trigger a lifecycle reassessment for every material change and require evidence that the revised workflow passed its scenario and stop tests.
Implementation checklist
- Document the decision, affected people, consequences, reversibility, urgency, sensitive data and foreseeable misuse.
- Select and record the autonomy tier: manual, advisory, delegated or autonomous.
- Name the accountable reviewer and specify competence, training, authority, independence, coverage and service expectations.
- Design the review screen to show the recommendation, material evidence, provenance, limitations and meaningful uncertainty.
- Implement explicit approve, modify, reject, defer and escalate states; never infer approval from viewing or timeout.
- Provide tested override, reversal and safe-stop actions with a documented restart procedure.
- Log the case, system and version, inputs, output, evidence shown, reviewer, action, rationale, escalation and final outcome.
- Protect logs, restrict access and define retention and deletion rules.
- Monitor outcomes, workload, latency, anomalies, automation bias, overrides and incidents by decision type and reviewer group.
- Reassess after model, data, workflow, incident, legal or risk-tolerance changes.
Following these steps turns “human in the loop” from a slogan into a measurable control: the AI remains within a defined boundary, a qualified person can make and document a real decision, and the organization can demonstrate whether the arrangement continues to reduce risk.
