Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteThe safe default is constrained autonomy: give an agent only the files, commands, credentials and network access required for a task; require a human decision before high-impact actions; then review the complete diff and test evidence before merge. A prompt or repository instruction can describe that policy, but it cannot replace sandbox enforcement, scoped identities, approval gates and an audit trail.
What guardrails must control
A coding agent can read repository content, edit files, run commands, call external tools and sometimes create processes that outlive the original request. Treat those capabilities as separate control surfaces rather than one “safe mode.” A useful policy answers five questions:
As an Amazon Associate I earn from qualifying purchases.
- Execution: Which directories may the agent read or write? Which child processes may it start? Is the workspace disposable?
- Tools and identity: Which shells, package managers, APIs, MCP servers and credentials are available, and under whose identity do they run?
- Approval: Which actions stop for a person, and what happens if review is unavailable?
- Verification: How will tests, security checks and the actual diff be reviewed before merge?
- Evidence: Can you reconstruct the task, tool calls, approvals, results, reviewer and merge decision?
OpenAI describes sandboxing as the technical boundary and approval policy as the decision about crossing it in its description of Codex’s internal controls. Those mechanisms complement each other: a policy statement without an enforced boundary is only an instruction, while a sandbox without review can still permit an unsafe change inside its permitted area.
Start with a narrow execution boundary
Use a disposable workspace first
Begin in a disposable branch, worktree, container or remote development environment. Permit the agent to access the checked-out repository and a temporary build cache, not a developer’s home directory, browser profile, SSH directory, cloud credential store or production filesystem. Keep the first run read-only or narrowly writable until you know which operations the task really needs.

Write down the boundary in operational terms: repository path, writable subdirectories, temporary directories, process limits, allowed operating-system services and maximum task lifetime. A container is useful only if its mounts, users, capabilities and network are also restricted. Do not describe a sandbox as a virtual machine or account boundary. Microsoft’s Visual Studio Code security guidance says sandboxing is an added layer and “not a virtual machine or user-account boundary, a standalone security boundary, or a replacement for endpoint security.” See Microsoft’s security documentation for the editor-specific caveats.
Define filesystem and process rules
Allow writes only where the task can tolerate them. For example, a documentation task may need the docs directory and no application source; a dependency update may need manifest and lock files but not deployment configuration. Deny access to production configuration, signing keys, local secret files and unrelated repositories. Restrict child processes where the host supports it, and ensure that processes started by the agent cannot silently continue after the session ends.
Make the boundary observable. Record denied paths and commands as well as successful ones; repeated attempts to reach a forbidden location are a useful signal that the task or prompt is mis-scoped.
Control the network explicitly
Default to no network or an allowlist of required destinations. A test that downloads packages may need a package registry; a task that reads an issue may need a source-control API. Neither requirement implies unrestricted outbound access. Block internal service ranges, metadata endpoints, production APIs and arbitrary uploads unless a reviewed task specifically needs them. Remember that fetched webpages, issue text and pull-request content can contain instructions intended to influence the agent.

Apply least privilege to tools and identities
Inventory before granting access
For each task, list the files, commands, integrations, network hosts and credentials it needs. Grant the smallest useful set and set an expiry where possible. A read-only source-control token is preferable to a token that can merge, create releases or change repository settings. Keep deployment rights outside the agent’s execution identity; a generated patch should be promoted by a separate, human-controlled pipeline.
Keep secrets out of the agent process
Do not inject production secrets into prompts, environment variables or files merely because a tool might use them. Use short-lived, narrowly scoped credentials delivered only to the operation that needs them, and prefer a managed keyring or broker that does not expose the secret value to the model. Audit which child processes and extensions can inherit environment variables. Rotate credentials if an agent, log or tool result may have exposed them.
Review MCP servers and extensions
An MCP server, extension or custom tool is part of the agent’s trusted computing base. Verify its code, maintainer, update path, data access and outbound destinations before enabling it. Give it a separate identity and only the methods required for the task. Microsoft recommends reviewing repository MCP configuration and keeping permissions scoped; the exact controls differ by agent and host, so confirm the current product documentation rather than assuming a common default.
Separate routine actions from approval-required actions
Classify actions by side effect, not by whether the agent describes them as safe. Reading source, formatting a file or running a deterministic unit test may be routine inside the sandbox. Production access, secret handling, permission changes, destructive commands, broad network access and changes outside the task scope should stop for review.

Put checks at the side effect
In a multi-agent or manager-style workflow, a check on the initial prompt or final response does not necessarily observe every intermediate tool call. OpenAI’s developer guidance states: “If you need checks around every custom tool call in a manager-style workflow, don’t rely only on agent-level input or output guardrails. Put validation next to the tool that creates the side effect.” Read the guidance at OpenAI’s guardrails and human review documentation.
At the tool boundary, evaluate the proposed target, action, arguments, identity and scope. Show the human the concrete command or API request, affected resources and reason for the request. If the reviewer cannot be reached, fail closed: do not silently continue with a weaker policy.
Protect CI and repository-triggered workflows
Repository content is not automatically trusted just because it is in a project. Issue text, pull-request descriptions, branch names, comments, repository instruction files and fetched pages can influence an agent or create shell-injection risk. OpenAI’s codex-action security guidance warns against interpolating untrusted values directly into shell scripts and notes that an agent may affect later privileged workflow steps.
Recommended Free Tools
- Restrict who can trigger an agent workflow, especially from forks or untrusted branches.
- Pass untrusted values as quoted arguments or environment data, not by concatenating them into shell source.
- Use a job with no production credentials for agent analysis and patch generation.
- Run the agent before privileged downstream steps, and verify that its files and processes cannot influence those steps unexpectedly.
- Scope API tokens, quotas and artifact permissions to the individual run.
- Require a protected human approval before merge, release or deployment.
Repository instruction files can document conventions and policy, but treat them as untrusted input unless the platform enforces their authority. A malicious or compromised instruction can ask an agent to disable tests, exfiltrate files or broaden permissions; only the execution environment can prevent that request from succeeding.

Make review and testing a release gate
Review the complete diff
An accountable developer should inspect the actual patch, not accept the agent’s claim that it is complete or tested. Review source, tests, configuration, generated files, dependency and lockfile changes, migration scripts, permissions, logging and error handling. Check for unrelated edits and for code that moves data to a new endpoint. Require the agent to identify files it could not inspect or tests it could not run.
Run ordinary and security checks
Use the repository’s normal formatter, compiler, unit and integration tests, then add checks appropriate to the change: static analysis, secret scanning, dependency and license checks, infrastructure validation, and targeted security tests. Preserve command lines, tool versions, exit codes and relevant artifacts. A green test run proves only what those tests cover; it does not approve a new network destination, privilege or data flow.
Assign human ownership
OWASP’s Secure Coding with AI Cheat Sheet says each AI-assisted change should have a human owner, explicit developer approval before merging and an audit trail identifying the approver and AI tool or model version. Its direct requirement is: “Require explicit developer approval before merging any AI-generated code.” Put the approval in the same system that records the merge decision, and make the approver responsible for the resulting change.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteBuild an audit trail that is useful during an incident
Capture the task or request identifier, repository and commit, agent and model version, configuration or policy version, tools invoked, targets and arguments, approvals and denials, command results, tests, reviewer and merge decision. Record policy changes and credential grants with the same care as code changes. Protect logs because prompts, source fragments and tool results may contain sensitive information; apply retention and access controls appropriate to that data.
Best Value
OpenTelemetry export is one pattern described in OpenAI’s account of its internal deployment, but the principle is vendor-neutral: evidence must be available outside the agent’s own conversation. A transcript that omits denied calls, hidden tool invocations or the exact diff is not a sufficient audit record.
Measure whether controls work in your workflow
Compare setups using consistent axes rather than a marketing label:
- Enforcement: Are filesystem, command, network and tool limits enforced by the host?
- Scope: Can writable paths, credentials, hosts and capabilities be narrowed per task or session?
- Approval quality: Which actions stop, who can approve, and does review failure stop the action?
- Isolation: Are child processes, extensions and MCP servers covered, and what local caches or services remain reachable?
- Reviewability: Can a developer inspect the full diff, tests, tool calls, approvals and decision trail?
- CI safety: Who can trigger the workflow, what secrets are present, how is untrusted input passed to shells and what runs afterward?
- Operational friction: How often do routine tasks stall, and will frustrated developers disable controls or request broad exemptions?
Microsoft documents limitations in command auto-approval rules involving shell aliases, quote concatenation and complex shell syntax. Use sandboxing as a layer instead of relying on command parsing alone. Configuration depends on the agent, operating system, repository and risk classification.

OpenAI’s April 30, 2026 Auto-review article reports that, in its described internal use, sessions stopped for human approval roughly 200 times less often than in manual approval mode. It also reports that Auto-review approved around 99% of the small fraction of actions that needed review. OpenAI says the ratios depend on use case, environment and sandbox configuration.
The same article gives an illustrative internal snapshot: among 720 out-of-sandbox actions that would have interrupted a user in manual mode, Auto-review rejected seven; four continued through a safer path and three stopped for user input. These are observations from one vendor’s deployment, not a universal expectation for another agent, reviewer or organization. Treat them as questions to test in your own environment: which actions are escalated, how often are unsafe requests rejected, and what happens when the reviewer or policy service is unavailable?
Quick Recap
A practical rollout sequence
- Classify the workflow. Mark the repository, data, commands, network destinations, credentials and potential impact. Separate low-risk maintenance from deployment, permission and data-changing work.
- Start constrained. Use read-only access or a narrow writable worktree with no production credentials and no unrestricted network.
- Inventory requirements. Grant only the files, tools, hosts and identities listed for the task. Set expiration and resource limits.
- Define approval cases. Include production access, secrets, destructive changes, permission changes, broad network requests, new tools and unclear scope. Fail closed when review is unavailable.
- Enforce locally. Put checks immediately before the tool or API call that creates the side effect, including calls made by sub-agents.
- Verify the result. Run tests and security checks, inspect the entire diff and confirm that generated scripts, dependencies and configuration match the task.
- Approve and merge. A named human accepts responsibility only after reviewing the evidence; the agent identity cannot merge or deploy.
- Record and improve. Preserve actions, decisions and outcomes. Reassess after incidents, blocked actions, false approvals, tool additions or permission changes.
Guardrail checklist before enabling production work
- Is the agent running outside production and outside the developer’s personal credential stores?
- Are writable paths, child processes, network hosts and tools explicitly allowlisted?
- Are production credentials and deployment rights unavailable to the agent identity?
- Does every side-effecting tool perform its own target, action, identity and scope check?
- Do high-impact actions require a named human, with fail-closed behavior?
- Can untrusted issue, pull-request and branch data reach a shell without safe quoting?
- Are agent-created files and processes prevented from affecting later privileged CI steps?
- Will reviewers see the full diff, tests, security results, tool calls and approvals?
- Does the audit record identify the agent or model version and the human approver?
- Have you measured interruption rate and false approvals without weakening the boundary?
