How to Run an Incident-Response Tabletop Exercise for AI-Assisted Workflows

An effective AI incident-response tabletop is a facilitated rehearsal of decisions around one real workflow: who declares an incident, who can pause or constrain the AI, what evidence is preserved, when a manual fallback starts, how a provider is contacted, and what is communicated. Build the exercise around the workflow’s data, downstream consequences, people and contracts—not around a generic AI scenario.

NIST’s Artificial Intelligence Risk Management Framework: Generative Artificial Intelligence Profile (AI 600-1, published July 26, 2024) recommends rehearsing third-party generative-AI response plans, learning through post-mortems and after-action assessments, and considering legal, privacy and breach-reporting duties. The AI RMF and its Playbook are voluntary, flexible and use-case agnostic, so there is no universal script or required meeting length.

What the exercise should prove

A tabletop is successful when participants can make and record defensible decisions under pressure. It is not a product demonstration, a model-accuracy test or a presentation about AI policy. By the end, you should know whether the organization can:

  • Recognize an abnormal AI result and route it to an incident lead.
  • Decide who may stop, limit or isolate the workflow.
  • Assess which people, records, decisions or operations may be affected.
  • Preserve prompts, outputs, logs, versions, approvals and provider communications without violating retention or privacy rules.
  • Move to a safe manual or alternate process and staff it.
  • Escalate to a third-party provider, legal or privacy specialists, leadership and affected stakeholders.
  • Record corrective actions with owners and due dates.

These are exercise objectives derived from NIST’s guidance on incident ownership, dependencies, fallbacks, communication and continual improvement. Select only two or three as primary objectives; too many make the discussion unfocused.

1. Choose a real workflow and define its boundaries

Describe the workflow in operational terms

Pick one AI-assisted process, such as drafting customer messages, summarizing support cases, processing documents or assisting an internal decision. State what happens before the AI call, what the system receives, what it returns, who reviews it, and what action can follow. An exercise becomes useful when players can point to an actual queue, application, approval step or customer consequence.

Document the intended use, operating context, known limitations, data categories and third-party dependencies. Note whether the service is a hosted model, an embedded feature in another application, an internal model or a chain of services. Identify the model or feature version where that information is available; an unexpected provider change can itself be an inject.

Map people and consequences

List everyone who relies on the output: customers, employees, patients, applicants, suppliers or another internal team. Describe the worst credible consequence and whether it can be reversed. A wrong draft that is caught in review is different from an automated eligibility decision or a disclosure of confidential material.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Write down the safe operating state if the AI is unavailable or untrusted. This might be manual review, a prior approved template, a queue freeze, a different tool or a delayed service. If no safe fallback exists, make that an explicit exercise finding rather than assuming one will appear during a crisis.

Set observable success criteria

Use statements that a facilitator can verify. Examples include: “Within 15 minutes, participants identify an incident lead and a person authorized to pause the workflow”; “The team names the records to preserve and where they are stored”; “Participants decide the threshold for switching to manual processing”; and “A provider contact, contractual duty and internal approval path are identified.”

2. Assign roles, authority and handoffs

Invite the people who would actually respond, not every person who has an interest in AI. NIST recommends defining ownership and communicating third-party generative-AI response plans to relevant AI actors. Include, as applicable:

  • The workflow owner and frontline operators.
  • Incident response, security operations and IT or platform staff.
  • AI, model, data or application owners.
  • Business continuity or operations leadership.
  • Legal, privacy, compliance and records-management representatives.
  • Communications, customer support and executive decision-makers.
  • A provider contact or account owner when an external AI service is involved.

Give each role a decision right, not just a title. The exercise should establish who can declare an incident, disable an integration, reduce permissions, approve data collection, authorize a manual fallback, contact the provider, notify a regulator or communicate externally. If authority depends on a policy that is unavailable, record the dependency.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use a simple authority map

Decision Primary owner Required input Evidence to record
Declare or classify the incident Designated incident lead Workflow owner and security Time, trigger, severity rationale
Pause, constrain or isolate AI use System or service owner Incident lead and operations Change made, scope and rollback plan
Switch to fallback Business or continuity owner Workflow operator and risk owner Capacity, checks and start time
Contact the provider Vendor or service owner Security, legal or privacy as needed Ticket, contract clause and response
Communicate externally Communications or executive lead Legal, privacy and affected business Approved message, audience and time

The table is a planning aid, not a substitute for your organization’s policies. During play, ask participants to name the actual person or team for each cell.

3. Write a short scenario and staged injects

Start with one paragraph containing enough facts to make an initial decision, then reveal new information in stages. Plausible prompts include sensitive data appearing in an output, harmful or unreliable generated content being used downstream, a compromised or unavailable provider, or an unexpected system change. These possibilities should be tailored to your workflow; they are not a ranking of incident likelihood.

Example sequence

  1. Opening: An operator flags an unexpected AI-generated result in a live workflow. Ask who receives the report, who becomes incident lead and what immediate safety action is taken.
  2. Inject 1—downstream use: The result has already informed a customer message, case disposition or internal recommendation. Ask who assesses impact, whether additional records or people must be identified, and who can pause processing.
  3. Inject 2—evidence and provider uncertainty: Logs or provider records may be needed, but the service status and retention period are unclear. Ask what is preserved, who requests support and which contractual contact is used.
  4. Inject 3—loss of trust or availability: The service is unavailable or cannot be trusted. Ask who authorizes fallback, how manual work is staffed, what quality checks apply and how the backlog is controlled.
  5. Inject 4—questions from inside and outside: Users and potentially affected people ask what happened. Ask who approves messages, what is known, what remains uncertain and whether legal, privacy or breach-reporting analysis is required.

Keep the scenario factual and time-bounded. Do not reveal the “correct” answer. The value comes from exposing assumptions, delays and conflicting procedures.

  • [Dont Hesite to Order] 24 hours customer services and professional technology team are standing by

4. Facilitate the tabletop for decisions

  1. Brief participants. Explain the objective, scope, confidentiality expectations and no-fault learning tone. State that the session is a discussion, not a live production test, unless a separate technical exercise has been approved.
  2. Present the opening. Give players the minimum information available at the stated time. Ask each relevant role what it would do next, what authority it has and what information it needs.
  3. Require explicit handoffs. When someone says “security handles it” or “the vendor will investigate,” ask which team, which person, through which channel and by when.
  4. Advance one inject at a time. Allow enough discussion for a decision, then introduce the next fact. Capture the decision, its owner, assumptions and any dependency that was not verified.
  5. Test communication. Ask participants to draft the substance of an internal update and an affected-stakeholder message. They should distinguish confirmed facts, uncertainty, protective actions and next update time.
  6. Pause for clarification, not coaching. If players disagree about whether the workflow can be stopped, which records may be retained or who may notify a provider or regulator, record the disagreement as a finding. Do not invent policy in the room.
  7. Close with a decision recap. Read back the declared incident, containment choice, evidence plan, fallback decision, escalation path and communication owner. Ask the group to correct the record while memories are fresh.

A facilitator keeps time and presents injects. A separate note-taker records decisions, missing contacts, delays, contradictory procedures and unresolved questions. A no-fault tone encourages participants to reveal gaps instead of defending a department.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. Exercise third-party provider dependencies

Hosted AI introduces dependencies that may be outside your operational control. NIST’s Generative AI Profile calls for documenting dependencies and fallbacks and examining provider-contract responsibilities. Before the session, locate the current contract, service description, support plan and escalation contacts. During the exercise, verify—not merely assume—answers to these questions:

  • What incident types must the provider notify you about, and through what channel?
  • What response times and critical-support coverage apply to your service and region?
  • Can the provider supply relevant logs, timestamps, model or feature versions and abuse-investigation records?
  • What data may be retained, used for service improvement or transferred to subprocessors?
  • Can you suspend access, rotate credentials, restrict data or route traffic elsewhere?
  • What happens if the provider is unavailable, changes the service unexpectedly or cannot establish whether an output was generated by a compromised component?

Include the provider contact path in the exercise, but do not treat a ticket number or status page as a complete response plan. Your organization still owns decisions about customer impact, internal containment, records, fallback and communications.

6. Decide what evidence to preserve

AI incidents can be difficult to reconstruct because prompts, retrieved documents, system instructions, outputs, human edits and model versions may be stored in different places. Have participants identify the minimum evidence needed to understand what happened without collecting more sensitive data than necessary.

  • Original input, prompt or API request, with sensitive fields handled under approved procedures.
  • Generated output and any human edits, approvals or downstream actions.
  • Model, feature, policy, system-prompt and configuration versions.
  • Identity, access, timestamps, correlation IDs and relevant application logs.
  • Source documents or retrieval results supplied to the model.
  • Provider tickets, notices, status updates and contract references.
  • Copies of internal decisions, messages and approved external communications.

Ask who can collect each item, where it is stored, how integrity is protected and when retention or deletion rules apply. Legal and privacy representatives should determine whether the event triggers investigation, preservation, notification or other obligations in the relevant jurisdictions. NIST guidance is not jurisdiction-specific legal advice.

7. Compare candidate scenarios before choosing one

If several incidents could be exercised, select the scenario that best tests your most consequential gap. The following axes synthesize NIST’s focus on impact, third-party dependence, fallbacks and ownership; they are a practical comparison method rather than a formal NIST scoring model.

Axis Questions to ask Why it matters
Impact and reversibility Who could be harmed, and can the downstream action be undone? High-impact or irreversible actions require clearer authority and faster containment.
Detection speed Would a person, monitoring rule or provider report reveal the issue first? Tests whether the first alert reaches the right owner.
Provider dependency Which decisions or evidence depend on an external service? Reveals contract, support and information-sharing gaps.
Fallback credibility Is manual or alternate processing documented, staffed and quality-checked? Separates a real continuity option from an assumption.
Evidence availability Can you reconstruct inputs, outputs, versions and approvals? Shows whether investigation and notification decisions are supportable.
Coordination load How many teams, locations, providers or external parties must act? Surfaces handoff and communication failures.

8. Debrief immediately and convert findings into changes

Hold the debrief while the decisions are still fresh. Ask:

  • What worked as designed?
  • What was unclear or delayed?
  • Which information, contact or permission was missing?
  • Which decision lacked an owner or escalation threshold?
  • Was the fallback safe, staffed and measurable?
  • Could the organization explain the event and its uncertainty to affected people?

Classify observations as plan or process, people or authority, technology or evidence, third party, and communications. Turn each material finding into an action with one accountable owner, a due date, a completion test and the document or control that must change. Examples include updating an on-call roster, adding a provider escalation clause, creating a prompt-and-output preservation procedure, approving a manual queue capacity, or clarifying who may disable an integration.

NIST recommends retrospective learning, incident post-mortems and after-action assessments. A finding is not closed because a meeting occurred; verify that the corrective change exists and works in a later rehearsal.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

9. Set a practical rehearsal cadence

NIST states, “Rehearse third-party GAI incident response plans at a regular cadence.” The guidance does not prescribe one interval for every organization. Choose a cadence based on the workflow’s impact, rate of change and risk tolerance. Rehearse sooner after a material change to the model or provider, data handled, automation level, fallback process, organizational ownership or applicable legal requirements.

Use a short, focused tabletop for a single workflow, then broaden later if the first session exposes cross-team dependencies. Track open actions between exercises and begin the next session with the changes that were supposed to be completed. A recurring exercise should become a feedback loop for the incident plan, provider records, training and technical controls.

Common failure modes and fixes

The scenario is generic

Symptom: Participants debate abstract AI risks rather than making operational decisions. Fix: Name the actual workflow, data, users, downstream action and fallback before writing injects.

No one can stop the workflow

Symptom: Several people believe another team owns the kill switch or approval. Fix: Assign a named decision owner, document the technical action and test it in a controlled environment where appropriate.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The fallback exists only on paper

Symptom: Manual processing has no staffing, queue limit, review rule or service-level expectation. Fix: Define capacity, prioritization, quality checks and the authority to invoke it.

Evidence is collected too late

Symptom: Prompts, outputs or provider logs have expired or cannot be linked to the affected action. Fix: Predefine preservation steps, access rights, retention limits and a secure collection location.

The provider is treated as the incident owner

Symptom: The organization waits for a vendor response before protecting users or operations. Fix: Separate provider investigation from your own containment, impact assessment, fallback and communication duties.

The debrief produces observations but no change

Symptom: Notes are filed without owners or deadlines. Fix: Maintain an action register with accountable owners, due dates, completion evidence and a retest date.
