A useful antidepressant review has four parts: an accurate list of everything you take, a concrete record of what changed, an interaction check against your exact medicines and health conditions, and a follow-up plan. Your prescriber or pharmacist can interpret those details; this guide cannot determine whether a particular combination is safe or tell you to change treatment.
Bring the questions and information below to the appointment. Do not stop an antidepressant, skip doses, or change the dose on your own. If you cannot attend in person, the same preparation can make a telephone or video visit more useful.

What a medication review should accomplish
The goal is not to match your symptoms to a generic internet list. It is to give the care team enough specific information to assess possible side effects, interactions, expected benefit, and next steps for you.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- Prescriber: evaluates the antidepressant’s benefits and risks, considers your diagnoses and other treatments, and makes treatment decisions.
- Pharmacist: can check the complete medicine list, explain how products may interact, and identify questions that should go back to the prescriber.
- You: describe what you take and what you have experienced, then agree on what to monitor and when to seek help.
FDA describes interactions in three broad groups. A single product can fit more than one group, and the significance depends on the exact product and the person taking it.

| Interaction type | What the review considers | Information to provide |
|---|---|---|
| Medicine with medicine | Prescription drugs, over-the-counter (OTC) products, vitamins, botanicals, supplements, and medicines used occasionally | Product name, strength, dose, schedule, and recent starts, stops, or changes |
| Medicine with food or beverage | Foods, drinks, alcohol, and other substances that may matter for a particular product | What you consume and how often, including anything you use recreationally when relevant |
| Medicine with a health condition | Medical conditions or circumstances that could alter effects or risks | Current conditions, allergies, pregnancy or breastfeeding status when relevant, and changes in health |
FDA summarizes the possible consequences this way: “Drug interactions may make your drug less effective, cause unexpected side effects, or increase the action of a particular drug.” That is why a general “safe medicines” list cannot replace a check of your own list.
What should I tell my doctor before taking an antidepressant?
Prepare one current list and explain how you actually use each item. A pharmacist or prescriber needs the product, not just its purpose. If possible, bring labeled containers; otherwise use a written or electronic list. Ask the office whether it prefers a particular format and whether you should send the list before the visit.
Include every product
- Prescription medicines, including products prescribed by another clinician.
- OTC pain relievers, cold and allergy medicines, antacids, sleep aids, and motion-sickness products.
- Vitamins, minerals, protein or weight-loss products, and other supplements.
- Herbs and botanicals, including St. John’s wort.
- Products used only occasionally or “as needed.”
- Alcohol and recreational substances when they are relevant to the interaction review.
Record the details
| List for each item | Why it matters |
|---|---|
| Exact name (brand or generic), strength, and formulation | Similar-sounding products can have different ingredients or release mechanisms. |
| Your dose and schedule, including as-needed use | Timing and amount can affect a potential interaction review. |
| Reason for use, if known | Helps the team understand which treatment is essential and who manages it. |
| When you started, stopped, or changed it | Timing can help compare a medicine change with a new symptom. |
Do not omit a product because it is “natural,” familiar, or taken infrequently. Tell the team if the way you take it differs from the label or prescription. Include medicines borrowed from someone else or taken from an old prescription, even if you do not plan to continue them. A notebook or a free written medicine record is sufficient; a paid organizer is not medically necessary and does not perform an interaction check.

- Check bottles, refill boxes, and supplement labels for the exact names and strengths.
- Mark anything started, stopped, or changed since your last visit.
- Write down allergies and any previous medicine reaction you remember.
- Note relevant changes in health, pregnancy or breastfeeding status, diet, alcohol use, or recreational substance use.
- Put your top two concerns and your preferred way to be contacted at the top of the list.
If another clinician or urgent-care service has recently prescribed something, include that clinician’s name and the date. The goal is a single, current list that every member of the team can use.
How to describe a suspected side effect
Use your own words and specific times rather than trying to name a diagnosis. FDA advises recording changes so they can be discussed with a health professional. A short log can make the conversation more precise.
Capture five facts
- What happened: describe the sensation, behavior, or physical change without interpreting its cause.
- When it began: include the date or an approximate time and whether it followed a start, stop, or dose change.
- How often it occurs: note whether it is constant, intermittent, or recurring after a dose.
- How it affects life: record effects on sleep, work, driving, relationships, eating, exercise, or other daily activities.
- What you tried: note any self-care measure or other change and whether it helped; do not alter the antidepressant plan without professional advice.
Also note the dose time, whether you took the medicine with food, and whether another product was taken nearby in time. These details may help the clinician decide what needs checking, but they do not prove cause and effect.

This log is a communication aid, not a validated symptom scale and not a way to identify the cause by itself. Keep it factual: “felt unsteady for 30 minutes after the evening dose” is more useful than “the medicine is toxic.”

MedlinePlus lists general possibilities such as nausea or vomiting, weight gain, diarrhea, sleepiness, and sexual problems. Not everyone experiences these effects, and a symptom listed for one antidepressant or class may not apply to another. Other effects may be described in the product’s current information. A generic list should not be used to reassure yourself that a worrying change is harmless.
Ask about the current Medication Guide for your exact product and contact a health professional about serious, new, or troubling changes. If a symptom affects driving, breathing, consciousness, hydration, or your ability to stay safe, explain that impact when you call so the office can direct you appropriately.
Questions to take to the prescriber or pharmacist
Write the questions down and leave space for the answers. Ask the pharmacist to review the same complete list that the prescriber uses. You can ask for plain-language explanations and repeat the plan back before leaving.
Side effects and warning signs
- What side effects are common with my exact medicine?
- Which changes should prompt a routine call, a prompt call, or urgent help?
- Could this symptom have another explanation that needs assessment?
- What should I monitor at home, and how should I record it?
- Are there activities, such as driving or operating machinery, I should avoid until we know how I respond?
Interactions
- Could any prescription, OTC medicine, vitamin, supplement, herb, food, beverage, alcohol, or recreational substance on my list interact with this antidepressant?
- Could one of my health conditions change the risk or effect?
- What interaction symptoms should I know, and whom should I contact if they appear?
- If another clinician proposes a medicine, should I check with you or the pharmacist before taking it?
- Does the timing of doses matter, and should any product be separated or taken with food?
Benefit, monitoring, and practical use
- What improvement should I look for, and when will we review progress?
- Do I need monitoring, measurements, or tests?
- What should I do if I miss a dose?
- What should I do if a side effect is bothersome?
- What is the plan if I want to stop or if we decide to change treatment?
- Who should I contact after hours, and how quickly should I expect a response?
FDA medicine guidance recommends asking about expected results, side effects, interactions, monitoring, and stopping instructions. Getting these answers in advance prevents an unplanned decision during a stressful symptom episode. If you receive different instructions from two clinicians, ask them to communicate or ask a pharmacist to help identify the discrepancy; do not choose between instructions on your own.

St. John’s wort
FDA specifically names St. John’s wort among herbs that should be reported to a clinician. Disclose it as you would any prescription medicine, including the brand, strength, and how often you use it. Do not infer a complete interaction judgment from its name alone; ask the pharmacist or prescriber to check your exact product and list.
OTC and occasional products
Cold remedies, pain relievers, sleep aids, and “as-needed” products are easy to forget when an appointment focuses on a daily prescription. Include the label and your actual frequency of use. Combination cold products may contain several active ingredients, so the full label is more useful than saying only “cold medicine.” If you are considering a new OTC product, ask before taking it when practical.
Food, beverages, and substances
Tell the clinician about regular beverages, alcohol, and recreational substances when they could affect the review. You do not need to decide whether a substance is relevant; disclose it confidentially and let the pharmacist or prescriber explain what matters for your medication and circumstances.
Use current information for the exact antidepressant
Antidepressant class summaries are useful orientation, but they do not contain every risk or instruction for every product. Ask for the current Medication Guide or patient information for the exact prescribed medicine. FDA’s drug-information resources provide access to Medication Guides, FDA-approved labeling, MedlinePlus drug information, and Drugs@FDA records.
Check that the product name, formulation, and strength match your prescription. A pharmacy handout or an older article may not reflect the current instructions. If the guide and your clinician’s instructions appear inconsistent, ask the pharmacist or prescriber to reconcile them rather than guessing. Keep the guide where you can find it and review it when the product, dose, or formulation changes.
Starting, changing, or stopping: safety rules
Some children, teenagers, and young adults under 25 may be more likely to think about self-harm or suicide when starting an antidepressant or when the dose changes. This is a warning to monitor and seek help, not a prediction that it will happen to every person. Family members or trusted people can help notice concerning changes when the patient agrees. New or worsening suicidal thoughts require immediate professional help. If there is immediate danger, contact emergency services or a crisis service available where you are.

FDA and MedlinePlus advise not stopping an antidepressant without first talking with a healthcare provider. Stopping suddenly or too quickly can cause problems. Do not skip, halve, or otherwise alter doses to test whether a symptom improves. Contact the prescriber or pharmacist for an individualized plan, including instructions for missed doses and any proposed transition to another medicine.
Serious problems should be discussed with a health professional. FDA’s MedWatch program accepts adverse-event reports, but a report does not replace medical care or emergency help. Ask the clinician whether they want to submit a report and what information to keep, such as the product name, lot details when available, dates, and the outcome.
How to compare alternatives with your care team
There is no universal “best” antidepressant in the guidance used here. If you and the clinician discuss another option, compare the choices against your circumstances rather than ranking medicines in the abstract.
| Decision point | Question to resolve |
|---|---|
| Intended benefit | What outcome is this option meant to improve, and how will we assess it? |
| Side effects that matter to you | Which possible effects would interfere most with sleep, work, sexuality, appetite, driving, or other priorities? |
| Interaction review | How does the option fit your exact medicines, supplements, foods, beverages, and health conditions? |
| Instructions and monitoring | How is it taken, what monitoring or tests are needed, and what should happen after a missed dose? |
| Follow-up or change plan | When will you review progress, whom do you contact with problems, and how would any change be managed? |
Ask the team to explain trade-offs in terms you can use, then repeat the agreed plan back in your own words. That “teach-back” step can reveal an unclear dose, timing instruction, or call-back threshold before you leave. Ask what would count as adequate benefit and what the next discussion would cover if improvement is limited or side effects remain troublesome.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What to do after the appointment
- Update your written list with the agreed medicine, dose, schedule, and reason for use.
- Write down what to monitor, the expected review date, and the phone number or service to contact.
- Keep the current Medication Guide with your medicine information.
- Record new symptoms and medicine changes as they occur so the next discussion is based on dates and details.
- Before starting a new prescription, OTC product, supplement, or herb, ask whether it should be checked against your list.
- Share the updated list with clinicians who prescribe or dispense your medicines, especially after an emergency or hospital visit.
If instructions are unclear, call the prescriber’s office or a pharmacist instead of making an independent change. If symptoms are severe, rapidly worsening, or involve immediate danger, use urgent or emergency help appropriate to your location.
Frequently Asked Questions
What should I tell my doctor before taking an antidepressant?
Bring a current list of every prescription, OTC product, vitamin, supplement, herb, and occasional medicine, with names, strengths, doses, schedules, reasons for use, and recent changes. Mention allergies, relevant health conditions, pregnancy or breastfeeding when applicable, alcohol or recreational substance use, and the exact symptoms or concerns you want reviewed.

Can I take this with my other medicines?
Ask the prescriber or pharmacist to check your exact list. Interaction questions should include prescriptions, OTC products, vitamins, supplements, herbs, foods, beverages, alcohol, recreational substances, and health conditions. Do not rely on a general compatibility list or make a change before receiving individualized advice.
What side effects should I ask about?
Ask which effects are common for your exact product, which changes require a routine or prompt call, and which require urgent help. Possible effects vary by medicine, and not everyone experiences them. Use the current Medication Guide and describe any new or troubling change to a health professional.
Should I stop taking my antidepressant if it causes side effects?
Do not stop suddenly, skip doses, halve tablets, or change the dose without speaking with the prescriber. FDA and MedlinePlus advise discussing stopping or changing treatment with a healthcare provider because stopping suddenly or too quickly can cause problems. Seek immediate professional help for new or worsening suicidal thoughts or immediate danger.
Can vitamins, supplements, St. John’s wort, or cold medicine interact with antidepressants?
They can be relevant, so disclose the exact product and how you use it. St. John’s wort is specifically named by FDA as an herb to report. Cold remedies and pain relievers may contain multiple ingredients. A pharmacist or prescriber must assess your particular products, dose, timing, and health conditions.
What if I miss a dose?
Ask the prescriber or pharmacist for the instruction that applies to your exact medicine. Do not guess, double a dose, or alter the schedule unless your care team tells you to. Write the missed-dose plan in your medication notes.
Where can I find reliable information for my antidepressant?
Ask for the current Medication Guide or patient information for the exact product, formulation, and strength. FDA drug-information resources link to Medication Guides, approved labeling, MedlinePlus drug information, and Drugs@FDA. If written information conflicts with your clinician’s instructions, ask them to reconcile it.
How should I prepare for a pharmacist review?
Bring the same complete list you give the prescriber, including labeled containers when possible. Add recent starts, stops, dose changes, symptoms with dates, and questions about food, beverages, supplements, missed doses, monitoring, and new products. Pharmacists can explain potential interaction concerns and direct treatment decisions back to the prescriber.
The Bottom Line
Bring a complete, current medication and supplement list; describe symptoms with dates, frequency, and daily impact; and ask the prescriber or pharmacist to check interactions against your exact products and conditions. Use the current Medication Guide, agree on monitoring and follow-up, and never stop or change an antidepressant without professional guidance. New or worsening suicidal thoughts or immediate danger require urgent help.
