How to Review and Safely Apply Changes Made by Claude Code

By Freedom251 Team · · 11 min read

Review Claude Code’s work as proposed code, not as an automatic commit. Before accepting anything, inspect the file diff, read every command it wants to run, check the permissions those actions require, and test the result in a controlled environment. Apply only the changes you understand and can explain.

Anthropic’s documentation describes explicit approval for file edits and command execution, permission controls for tools, and a --dangerously-skip-permissions option that bypasses prompts. That bypass is explicitly cautioned against. For untrusted scripts or tool calls, a development container can add an isolation boundary. Exact prompts and menu names vary by Claude Code release and configuration, so verify them against the version installed on your machine.

What a safe review is meant to prevent

A coding agent can change several files, invoke shell commands, read local configuration, contact network services, or alter generated artifacts in one session. A green-looking summary does not prove that the implementation is in scope or safe. Your review should answer four questions:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Which files changed, and were all of them expected?
  • What does each change do in normal and failure cases?
  • Which commands were run or proposed, and what can they affect?
  • What evidence shows that the result works without introducing a new security or operational problem?

Anthropic’s security guidance places responsibility for reviewing suggested code and commands with the user. Treat approval as a deliberate decision, not as a confirmation dialog to clear quickly.

Prepare a reversible workspace before asking for edits

Start from a clean checkpoint

Use a dedicated branch or worktree for agent-assisted changes. Record the starting state so you can distinguish your own work from Claude Code’s output.

  1. Run git status and note any existing modifications.
  2. Commit or otherwise save work you cannot afford to lose.
  3. Choose a branch with a clear name, such as agent/payment-validation.
  4. Confirm that generated files, local configuration, and test fixtures are either intentionally included or excluded.

If the repository already contains uncommitted work, do not ask the agent to edit until you have decided how that work will be protected. A later diff can otherwise combine unrelated changes and make review unreliable.

Limit sensitive material

Remove credentials, production configuration, private keys, and unrelated personal data from the working area when they are not needed. Review ignore rules, environment-variable files, and scripts that can print or upload their contents. A permission prompt tells you that an action is about to occur; it does not make the underlying data safe to expose.

Choose isolation for higher-risk work

For unfamiliar repositories, third-party scripts, or tools that can reach external systems, consider a development container or another isolated environment. Isolation is a separate safeguard from permission prompts: it reduces what a mistake can reach, while permissions control whether the action is requested or allowed. Match the container’s mounted directories, network access, and credentials to the minimum needed task.

Ask Claude Code to define the intended change

Before allowing edits, request a short implementation plan. Ask for the files it expects to touch, the commands it expects to run, assumptions it is making, and tests it will use to verify the result.

Describe the intended change first. List every file you expect to modify, create, or delete; explain the purpose of each change; show commands you may run; identify any network, credential, database, or migration impact; and propose tests. Do not edit files yet.

This plan is not a substitute for inspecting the diff. It gives you a scope against which to compare the actual work. If the agent identifies an ambiguous requirement, resolve it before approval rather than allowing it to guess.

Define acceptance criteria

Write concrete criteria before implementation: the behavior that must change, behavior that must remain unchanged, supported runtime versions, error handling, and the tests that should pass. Criteria prevent a plausible but incomplete patch from being accepted simply because it compiles.

Inspect the actual diff, file by file

After proposed edits, inspect the repository state independently of the agent’s summary. Typical Git commands are:

git status --short
git diff --stat
git diff -- path/to/file
git diff --check

Use your platform’s equivalent tools if the project is not managed with Git. The important evidence is the complete before-and-after content, not only a high-level description.

Check scope and intent

  • Does every changed file support the requested feature or fix?
  • Are there unrelated formatting, dependency, lockfile, generated, or documentation changes?
  • Were files created, renamed, or deleted without an explicit reason?
  • Do configuration changes alter logging, authentication, access control, network exposure, or production defaults?
  • Do tests cover the new behavior rather than merely being rewritten to match it?

Look for dangerous or accidental content

  • Hard-coded passwords, tokens, private keys, connection strings, or personal data.
  • Debug output that prints request bodies, headers, environment variables, or database records.
  • Broad file permissions, disabled certificate verification, unrestricted cross-origin access, or authentication bypasses.
  • Destructive database operations, recursive deletion, shell interpolation of untrusted input, or silently ignored errors.
  • New dependencies whose source, version range, install scripts, or licensing you cannot explain.
  • Changes to CI, release, deployment, or infrastructure files that could run with production credentials.

Review both added and deleted lines. Removing a validation branch or a permission check can be more consequential than adding a large block of code.

Compare behavior, not just text

Trace the changed path from input to output. For an API, follow authentication, authorization, validation, storage, and response handling. For a command-line tool, consider quoting, exit codes, signals, and paths containing spaces. For a migration, determine whether it is reversible, how it behaves on existing data, and whether application and schema versions can coexist during deployment.

Read every proposed command before approving it

Claude Code can request permission to execute commands. Read the complete command, including arguments, pipes, redirects, substitutions, and chained operations. Do not approve a familiar command without checking what directory and environment it will run in.

Command characteristic Questions to answer Safer review action
File writes or deletion Which paths can be changed? Is the operation recursive? Can a variable expand to an unexpected location? Confirm the working directory and target paths; prefer a narrow, previewable command.
Package installation or build scripts What code runs during installation? Does it execute lifecycle scripts or modify global state? Inspect the manifest and lockfile; run in an isolated environment when provenance is uncertain.
Network access Which host is contacted? What data is sent? Is the destination expected? Verify the endpoint and payload; deny the request if the purpose is unclear.
Credentials or secret stores Which identity is used, and could output expose a secret? Use least-privilege, short-lived credentials and inspect output before continuing.
Database or infrastructure changes Is the target development, staging, or production? Is there a rollback? Require an explicit environment check and a tested recovery plan.
Shell composition Do pipes, redirections, command substitution, or eval-like behavior hide additional actions? Break the command into smaller, reviewable steps.

A command that only reads files can still disclose secrets if its output is sent to a service or included in logs. A command that appears to run tests can execute project-defined hooks. Review what the project’s scripts actually invoke.

Understand Claude Code’s permission controls

Anthropic’s CLI reference documents controls for allowing and denying tools, selecting permission modes, and bypassing prompts. The exact configuration surface can change between releases, so use the options supported by your installed version.

Control Purpose Review implication
--allowedTools Specifies tools that may be used under the selected configuration. Allow only the tools required for the task, and understand what each tool can do.
--disallowedTools Blocks specified tools. Use denies for capabilities that are unnecessary or unacceptable in the current workspace.
Permission mode selection Controls when actions require approval and how approval is handled. Choose a mode appropriate to the project’s trust level; do not assume a mode is restrictive without checking its current definition.
--dangerously-skip-permissions Skips permission prompts. Anthropic labels this option for cautious use. Treat it as unsuitable for ordinary review and especially risky around untrusted code, credentials, or production access.

Permission settings are not a code review. A tool can be allowed and still produce an incorrect or unsafe patch; a tool can be denied and still leave you with changes that require inspection.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Keep rules narrow and temporary

Scope approvals to the needed project and tool. Avoid broad rules that allow every shell command, every directory, or every network destination when the task needs only one operation. Remove temporary allowances after the task or run the work in a disposable environment.

Be careful with batch acceptance

Some configurations support accepting multiple file changes together. Batch approval is appropriate only when you have already reviewed the complete set and confirmed that the files share one bounded purpose. If the set includes generated files, configuration, migrations, and application logic, review those categories separately instead of treating one approval as proof that all are safe.

Apply changes in small, reviewable increments

Do not approve a large sequence merely because the first files look correct. Divide work by a coherent unit, such as a parser change followed by its tests, or a schema migration followed by compatible application code.

  1. Approve the smallest edit set that implements one acceptance criterion.
  2. Inspect the resulting diff immediately.
  3. Run focused checks for that unit.
  4. Record or commit the verified state.
  5. Proceed to the next unit only if the scope remains correct.

When a proposed edit is not acceptable, reject it and state the constraint precisely: file that must not change, API that must remain compatible, command that is forbidden, or behavior that needs a test. Asking for a narrower revision is safer than manually repairing an opaque large patch.

Run tests and inspect their output

Run the project’s relevant unit, integration, type, lint, and build checks after reviewing the diff. Use the project’s documented commands rather than assuming that a single default test command covers all risk.

Read failures as evidence

  • Separate failures caused by the patch from pre-existing failures by comparing with the clean checkpoint.
  • Check whether tests were skipped, filtered, marked as expected failures, or run against a different environment.
  • Inspect warnings, deprecations, coverage changes, and generated output, not only the final exit code.
  • For security-sensitive changes, add negative tests for denied access, malformed input, expired credentials, and error paths.
  • For migrations or deployment changes, test upgrade, downgrade or rollback, and mixed-version behavior where applicable.

Never claim that tests passed unless you actually ran them and reviewed the result. If a test cannot run because a service or credential is unavailable, state that limitation and do not treat the patch as fully verified.

Review changes that cross trust boundaries

Secrets and credentials

Search the final diff and generated artifacts for secret-like values. Confirm that logs, error messages, snapshots, and test fixtures do not capture credentials. If a secret may have been exposed, stop the review, revoke or rotate it, and determine where it was transmitted or stored.

External services

For commands that call package registries, APIs, cloud services, or remote repositories, verify the destination, authentication method, data sent, and retention implications. A successful response does not establish that the destination was authorized.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Production and shared environments

Do not let an agent operate directly against production merely because a command is convenient. Require an explicit environment check, least-privilege credentials, a backup or rollback plan, and human confirmation immediately before an irreversible action. Prefer rehearsing the operation in a disposable or staging environment.

Use a development container when isolation is justified

Anthropic identifies development containers as an option for additional isolation. A container is useful when the repository includes untrusted scripts, when dependencies have unknown install behavior, or when the task needs tools that should not access the host.

Configure the boundary deliberately

  • Mount only the project data required for the task.
  • Do not mount the host home directory, SSH agent, cloud credentials, or password stores by default.
  • Restrict network access when the task does not need it.
  • Use non-root execution where practical.
  • Persist only the files you have reviewed, not the entire container state.

Isolation reduces blast radius but does not replace code review. A malicious or incorrect change can still be merged if the final diff and tests are not examined.

Perform a final review before commit or merge

  1. Run git status --short and account for every changed, untracked, renamed, and deleted file.
  2. Read the complete final diff, including lockfiles, generated files, CI configuration, and documentation.
  3. Confirm that no credentials, private data, temporary debugging code, or unrelated edits remain.
  4. Check that tests and other validation commands were run in the intended environment and that failures are resolved or documented.
  5. Review the commands that were approved and verify that no temporary broad permission or credential remains active.
  6. Commit with a message describing the bounded change, or stop and request another review if the scope is unclear.

For a pull request, include the motivation, files changed, tests run, known limitations, migration or rollback notes, and any permission or environment assumptions. This record helps a second reviewer evaluate the same evidence instead of relying on an agent-generated summary.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Common failure branches and the correct response

The diff includes unrelated files

Stop approval. Ask Claude Code to explain each file, then revert or separate unrelated edits before continuing. Do not accept a broad diff on the assumption that the extra changes are harmless.

The agent repeatedly requests a broad permission

Identify the exact operation that requires it. Replace it with a narrower command, grant only the necessary tool for one step, or move the task into an isolated container. Do not use --dangerously-skip-permissions simply to avoid interruptions.

A command is destructive or irreversible

Do not approve it until you have a backup, a tested recovery path, and confirmation of the target environment. Ask for a dry run or a preview command where one exists.

Tests fail after the edit

Keep the failing state, capture the output, and compare it with the clean checkpoint. Ask for a focused fix rather than allowing unrelated refactoring. If the failure is environmental, document that fact and run the check in a reproducible environment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A secret appears in output or the diff

Stop the session, prevent the secret from entering version control, and rotate it if exposure is possible. Then determine whether logs, command history, remote services, or artifacts received the value.

The interface does not match an instruction

Do not infer that a differently labeled control is equivalent. Check the installed Claude Code release and its current CLI documentation, then confirm what the selected permission mode actually permits. Interface names and behavior can vary with release and configuration.

A compact approval checklist

  • Scope: I can explain every changed file and why it is needed.
  • Diff: I inspected additions, deletions, renames, generated output, and configuration.
  • Commands: I read each command, argument, pipe, redirect, and destination.
  • Permissions: Approvals are limited to the required tools and project; no broad bypass is being used without a controlled reason.
  • Secrets: No credentials or private data are exposed in code, logs, fixtures, or artifacts.
  • Tests: Relevant checks actually ran, their output was reviewed, and limitations are recorded.
  • Isolation: Untrusted code or tools run in a boundary appropriate to their risk.
  • Recovery: Destructive operations have a backup, rollback, or verified disposable target.
  • Final state: The repository is clean except for the intended work, and the change is ready for a human commit or merge decision.

The safest operating pattern is therefore consistent: ask for a plan, inspect the real diff, read commands before approval, keep permissions narrow, isolate untrusted work, test the result, and perform one final independent review. Claude Code can accelerate implementation, but the human reviewer remains responsible for deciding what enters the codebase and what systems it may affect.
