How to Review AI-Generated Code Before It Reaches Production

Review AI-generated code exactly as you would any other proposed change—but with deliberate checks for fabricated dependencies, weakened tests, unsafe assumptions, and unclear ownership. A qualified human who understands the diff must approve it; automated tests and scanners provide evidence, not a release decision.

What makes AI-generated code review different?

AI-assisted code can look polished while misunderstanding a requirement, crossing a trust boundary, or quietly changing a security control. The model may also generate a package that does not exist, use an outdated API, or alter tests so that they confirm the generated implementation instead of the intended behavior.

As an Amazon Associate I earn from qualifying purchases.

The correct standard is therefore not “does this code look plausible?” It is “does this change satisfy the requirement, meet the project’s secure-coding rules, and have a clearly accountable owner?” OWASP’s AISVS Appendix C says AI-generated code should always receive review by a qualified human engineer. The AI agent does not count as that reviewer, and the reviewer should be separate from the identity that requested generation where policy requires it.

OWASP’s Secure Coding with AI Cheat Sheet also calls for explicit developer approval before merge and a human owner responsible for correctness, security, and maintainability. Treat those as recommended controls unless your organization, contract, or regulator makes them mandatory.

The release gate: who must approve

Assign one accountable human owner

Every AI-assisted change needs a named person who understands what it does and accepts responsibility for its behavior after merge. That owner should be able to explain the data flow, important assumptions, failure modes, and test evidence without asking the model to explain its own output.

Use an independent reviewer

Where feasible, have a qualified reviewer other than the person who prompted or accepted the generated code. Separation reduces the chance that an author’s confidence in the prompt or model will substitute for examination of the actual diff. Your policy may require one reviewer for ordinary changes and a second or security-specialist review for sensitive paths.

Keep an auditable approval record

Record the approving person, the change identifier, required checks, decisions on findings, and any model or tool provenance your workflow requires. A green pipeline is evidence that selected checks passed; it is not proof that the implementation is correct.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 1: establish scope and provenance

Start with the exact proposed change, not a model summary. Identify what the change is meant to accomplish and which portions were generated or materially edited with AI, if that information is available.

  • Capture the pull request or change-set identifier and its complete diff.
  • Read the ticket, design notes, acceptance criteria, and relevant interface contracts.
  • List affected application components, services, data stores, build jobs, and deployment targets.
  • Mark whether the change includes production code, tests, dependencies, configuration, infrastructure, workflow definitions, or documentation that affects operation.
  • Check that the AI tool and the data supplied to it comply with your organization’s approved-tool list and data-classification rules.
  • Preserve prompts, generated patches, or other provenance only where your policy calls for them; do not put secrets or restricted data into an unapproved service.

OWASP AISVS recommends a written workflow that defines permitted AI use, approved tools, data classifications, and security gates throughout the software lifecycle.

Step 2: understand the implementation before judging it

Trace the requirement to behavior

For each acceptance criterion, point to the code path that implements it and to a test that demonstrates it. If you cannot explain how a branch, default, timeout, or error response follows from the requirement, stop and clarify it before approval.

Map data flow and trust boundaries

Follow untrusted input from entry point to parsing, validation, authorization, business logic, storage, logging, and output. Identify where data crosses service, process, privilege, or network boundaries. Check that validation occurs before sensitive operations and that authorization is enforced at the operation that needs it—not only in a user interface or an earlier controller.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Inspect failure and state behavior

  • Confirm errors are handled deliberately rather than swallowed, exposed to users, or converted into permissive defaults.
  • Check retries, timeouts, cancellation, idempotency, and partial-failure behavior.
  • Review state transitions, transaction boundaries, locking, and concurrency assumptions.
  • Verify logging and telemetry do not leak credentials, personal data, tokens, or untrusted content into operational systems.
  • Check compatibility with supported runtime, database, protocol, and client versions.

Comments, descriptive names, or a familiar coding style do not establish that the behavior is safe. NIST’s SP 800-218A frames review and analysis around the organization’s secure-coding standards and security requirements.

Step 3: scrutinize dependencies, configuration, and infrastructure

Validate every dependency

Generated code can invent a package name or select a real package with an unsuitable maintainer, license, version, or transitive dependency. For each addition, verify that the package exists in the intended registry, is the correct library, is maintained to your standard, and is pinned or constrained according to project policy. Review the source, version, license, known advisories, and transitive dependencies before allowing it into a build.

Review configuration as executable behavior

Configuration can grant privileges or disable protections just as surely as application code. Check authentication settings, authorization rules, network exposure, storage permissions, encryption requirements, certificate handling, feature flags, and default values. Look for debug modes, permissive cross-origin settings, disabled verification, broad service accounts, and secrets written into files or logs.

Check build and deployment changes

Inspect build scripts, container definitions, infrastructure-as-code, workflow files, migration scripts, and release settings. Confirm that a generated change cannot execute arbitrary commands in a build runner, publish artifacts to an unintended location, bypass required checks, or alter the production environment’s access controls.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 4: review tests as carefully as production code

Tests created or edited by the same agent can contain the same misunderstanding as the implementation. OWASP warns that agents may delete tests, weaken assertions, replace real calls with unhelpful mocks, or write tests that merely confirm generated behavior.

  • [Dont Hesite to Order] 24 hours customer services and professional technology team are standing by

Look for weakened coverage

  • Compare the before-and-after test files for deleted cases, narrowed inputs, removed assertions, and changed expected errors.
  • Check that mocks still exercise the integration, authorization, serialization, and persistence boundaries that matter.
  • Ensure a test fails when the requirement is violated; a test that only checks “no exception was thrown” may prove very little.
  • Review coverage changes in context. A percentage increase can coexist with missing critical paths.

Add independent adversarial cases

Write or request tests that were not copied from the generated implementation. Include invalid and boundary inputs, missing or duplicated fields, oversized values, expired credentials, unauthorized users, malformed encodings, dependency failures, timeouts, retries, concurrent requests, and partial transactions where those conditions apply. Test both the expected result and the absence of dangerous side effects.

Test the security property, not only the happy path

If a requirement says one tenant must not see another tenant’s records, create a test that attempts that access. If a parser must reject malformed input, assert the rejection and verify that no state changed. If a role cannot perform an operation, test the operation at its actual enforcement point.

Step 5: run layered automated and manual checks

Select checks according to the system and the risk of the change. NIST identifies code-review tools, static and dynamic analysis, software-composition analysis, and penetration testing as possible methods. Its recommended minimum standards for software verification provide additional context for documenting and performing appropriate testing.

Check What it can reveal What it cannot establish alone
Unit and integration tests Regressions and incorrect behavior covered by the cases That untested requirements or threat scenarios are safe
Static analysis and linters Some insecure patterns, defects, type errors, and policy violations That the design matches business intent or that every finding is exploitable
Dynamic testing Runtime behavior, configuration interactions, and reachable failures That unexercised paths and environments are correct
Dependency or software-composition analysis Known vulnerable components, licenses, and dependency changes That a package is appropriate for your architecture or correctly used
Secret scanning Potential credentials and tokens committed to repositories or artifacts That secrets were never exposed elsewhere or that access is least-privilege
Infrastructure and policy checks Unsafe permissions, public exposure, and configuration drift That application-level authorization and business rules are correct
Penetration testing Exploitable weaknesses in the tested deployment and scope That untested components, future changes, or every code path are secure

Run the project’s normal continuous-integration and security gates, investigate findings rather than blindly suppressing them, and document the disposition of accepted risks. NIST’s Notional Reference Model for DevSecOps describes AI-generated outputs as passing through established peer review, security validation, automated testing, and approval workflows rather than independently modifying or deploying production.

Step 6: raise the threshold for sensitive changes

Not all diffs deserve the same review depth. Set explicit escalation rules in policy so that the decision does not depend on a reviewer’s intuition.

Change characteristic Minimum additional scrutiny to consider
Authentication, authorization, session handling, or identity claims Second qualified reviewer; negative authorization tests; security sign-off where policy requires it
Cryptography, key management, certificate validation, or token verification Reviewer with relevant security expertise; algorithm, mode, key-storage, and failure-path analysis
Input parsing, deserialization, templating, file handling, or command execution Boundary and malformed-input tests; trust-boundary review; focused dynamic or penetration testing
Database migrations, payment or financial state, or irreversible operations Transaction and rollback review; concurrency tests; staged deployment and recovery plan
Build pipelines, infrastructure, permissions, or deployment configuration Infrastructure-policy checks; least-privilege review; independent approval before release
Shared libraries or changes with broad blast radius Compatibility analysis across consumers; expanded integration tests; controlled rollout

OWASP AISVS and the Secure Coding with AI Cheat Sheet highlight security-critical files and behaviors such as input validation, authorization, and deserialization. They recommend enhanced review and testing controls; your organization should define the exact thresholds, reviewers, and exception process.

Step 7: resolve findings and make the approval decision

  1. Collect results. Bring together human review comments, test outcomes, static and dynamic analysis, dependency findings, secret scans, and infrastructure checks.
  2. Triage each finding. Classify it as a defect, vulnerability, policy violation, false positive, accepted risk, or unrelated result. Explain the reasoning in the change record.
  3. Fix or formally disposition. Do not merge merely because a warning is inconvenient. Accepted risks need the owner, rationale, scope, and expiration or follow-up required by policy.
  4. Re-run affected checks. A fix can introduce a new regression or invalidate earlier evidence. Require the relevant tests and scans to pass on the final diff.
  5. Confirm ownership. The approver should be able to summarize the change, remaining risks, rollback approach, and monitoring plan.
  6. Record explicit approval. Merge only after required human approvals and automated gates are satisfied.

A practical pull-request checklist

Use this checklist as a starting point, then tailor it to your language, architecture, and risk profile.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Does the diff implement the ticket and acceptance criteria without unrelated behavior?
  • Can the reviewer explain every new trust boundary, privilege change, state transition, and external call?
  • Are inputs validated for type, size, encoding, and permitted values before use?
  • Are authentication and authorization enforced at the operation that changes or discloses data?
  • Are errors, retries, timeouts, cancellation, concurrency, and partial failures deliberate?
  • Are secrets, personal data, and sensitive tokens protected in code, configuration, logs, and telemetry?
  • Does every new dependency exist, come from the intended source, meet license and maintenance requirements, and have an acceptable version?
  • Have build scripts, workflows, infrastructure, permissions, and deployment settings been inspected?
  • Were tests deleted, narrowed, mocked excessively, or changed to weaken assertions?
  • Do independent negative, boundary, authorization, failure, and concurrency tests cover the important risks?
  • Did the normal CI, security, dependency, secret, and infrastructure checks run on the final diff?
  • Were all findings investigated and fixed or formally accepted?
  • Has the required independent or security review occurred for sensitive files and behaviors?
  • Is a human owner explicitly approving correctness, security, and maintainability?

Common failure modes and how to correct them

“The model explained the code, so it must be right.”

A model-generated explanation is not independent evidence. Ask the owner to trace the implementation to requirements, then have the reviewer reproduce that reasoning from the diff and tests.

“All tests are green.”

Green tests show only that selected assertions passed. Compare test changes, add cases generated independently, and check requirements and threat scenarios that the suite does not cover.

“The scanner found no issues.”

Scanners have coverage limits and false positives. Combine them with manual review, dependency and configuration inspection, and risk-appropriate dynamic testing.

“It is only a small AI-generated patch.”

Small changes can alter authorization, parsing, logging, or build permissions. Size is a useful signal for review effort, not a security exemption.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“We can review it after deployment.”

Post-deployment discovery turns a review task into incident response. Keep production access behind the same approval, testing, and rollback gates as other changes.

How to institutionalize the workflow

Define permitted use and data handling

Publish which tools may be used, what data classifications may enter them, where generated output may be stored, and which lifecycle gates apply. Require contributors to disclose AI assistance to the extent needed for review and audit.

Make ownership and approvals enforceable

Configure repository rules so protected branches require the appropriate number and type of approvals, passing checks, and review of changed ownership areas. Ensure the person who generated or materially authored a change cannot satisfy an independence rule by approving their own work.

Measure process quality without pretending to measure correctness

Track review completion, escaped defects, recurring finding categories, dependency exceptions, time to remediate, and coverage of sensitive paths. Do not treat a single metric—such as test coverage or scanner counts—as a substitute for engineering judgment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Improve prompts and templates after failures

When review finds a repeated issue, add the relevant secure-coding rule, test scenario, or checklist item to the team’s guidance. Better prompts can reduce defects, but they never remove the requirement for independent human approval.

What “ready for production” means

An AI-assisted change is ready only when its intended behavior is understood, its risks have been examined, required tests and analyses have passed, findings have been resolved or formally accepted, and a qualified human owner has explicitly approved the final diff. That standard is the same release discipline used for other code, with extra attention to the ways generated output can fabricate, weaken, or obscure behavior.
