Stop relying on the output, verify the disputed claim, record what happened, and route it through your employer’s approved review process. If the answer has already influenced a person, decision, or external communication, notify the people who may be affected and request a documented correction. Treat a single bad response as a signal to investigate—not, by itself, proof that the system is systematically discriminatory.
1. Pause before the answer travels further
A confident tone is not evidence. The UK Information Commissioner’s Office (ICO) identifies automation bias as the risk that people assume an AI system is right by default and fail to review its output effectively. Until the claim is checked, do not forward it as verified, enter it into a customer or employee record, use it to approve or reject a request, or rely on it for a consequential decision.

Pause does not mean deleting the exchange immediately. Preserve enough context for a review, while following your employer’s retention, privacy, and security rules. If the output contains personal, confidential, regulated, or commercially sensitive information, stop copying it into other tools or sending it to people who are not authorized to see it.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteWhen the risk is urgent
- Potential immediate harm: If the answer could affect someone’s safety, pay, employment status, access to care, legal position, or essential service, stop the workflow and contact the responsible manager or emergency escalation channel without waiting for a lengthy investigation.
- Security or privacy exposure: If the tool disclosed credentials, personal data, confidential documents, or another organization’s information, use the security or privacy incident route as well as the AI-use route.
- Routine factual error: Keep the item out of downstream work, verify it, and report it through the normal owner of the tool or process.
2. Identify exactly what is wrong
“The AI was biased” is difficult to review. Rewrite the concern as a specific, testable statement. Separate a factual error from unfair framing, stereotyping, or unequal treatment.

Factual error
Check names, dates, calculations, quotations, policy language, citations, and other assertions against the primary document or the current authoritative source. Record the exact statement, what the source says instead, and whether the discrepancy could change an action or decision.
Possible bias or unfairness
Describe the mechanism rather than only the reaction. Examples include an unsupported stereotype, a recommendation that applies a rule differently to comparable people, a translation or description that omits a group’s relevant context, or a ranking that disadvantages a protected or otherwise affected group without a job-related reason. Explain what was compared and why the difference matters.
A response can be both inaccurate and biased, but the two findings should still be documented separately. A neutral-sounding error may affect everyone equally; a polished answer may also encode an unfair assumption. Keeping the categories distinct helps the reviewer choose the right test and corrective action.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →3. Verify with an independent, appropriate source
Use a source suited to the claim, not another unverified AI response. For a workplace policy, consult the current approved policy or its owner. For a legal, financial, medical, safety, or technical question, use the applicable primary authority, current official data, or a qualified professional. Ask a subject-matter reviewer to check the result when the issue is outside your expertise.
- Copy the disputed claim accurately. Include surrounding wording if it changes the meaning.
- Find the controlling source. Note the document title, version or publication date, section, and the passage that supports or contradicts the answer.
- Check currency and scope. Confirm that the source applies to your country, business unit, population, product version, and date.
- Ask for a second review when stakes are high. A qualified person should assess both the underlying fact and any impact on people or groups.
- State the result precisely. Record “unsupported,” “incorrect,” “incomplete,” “stereotyped,” or “unevenly applied,” with the evidence for that label.
Do not try to “prove” a broad pattern by repeatedly prompting an unapproved system with personal or confidential examples. If comparative testing is needed, use de-identified or synthetic cases only where your policy permits it, and ask the tool owner or governance team to design the test.

4. Assess who could be affected
Impact determines urgency and the right owner. Ask what the answer can change, who receives that change, and whether the effect is reversible.
- Decision impact: Could it influence hiring, scheduling, performance management, promotion, pay, lending, insurance, access to services, resource allocation, or a safety action?
- Population impact: Does it concern one person, a small team, customers, applicants, or a demographic group? Could language, disability, race, sex, age, religion, nationality, or another characteristic alter the result?
- Scale and repeatability: Was this a one-off conversational answer, a saved workflow, a batch output, or a model embedded in a recurring process?
- Reversibility: Can a human correct the outcome before harm occurs, or has the answer already triggered an irreversible action?
NIST’s Generative AI Profile (NIST AI 600-1), published July 26, 2024, recommends evaluating fairness with methods appropriate to the use case and examining results across demographic groups and subgroups. That recommendation supports investigation; it does not turn one example into proof of systemic discrimination. A single output can reveal a serious control failure even when its overall prevalence is unknown, so report the incident and describe the limits of what you have established.

5. Preserve a reviewable record without creating a new privacy problem
Make the concern reproducible for an authorized reviewer. Capture the minimum necessary information and store it where your organization requires incident or quality records.
- Tool name, deployment or model version if displayed, and the date and time.
- The work task and intended use, without pasting unnecessary personal or confidential data.
- The prompt, relevant settings, and complete output where policy allows; otherwise record a redacted excerpt and the approved location of the original.
- The exact error, biased wording, omission, or unequal treatment you observed.
- The independent source, reviewer, or test that confirmed the concern.
- People, groups, decisions, systems, or communications that may have been affected.
- Actions already taken, such as pausing a workflow, correcting a record, or notifying a manager.
- Any uncertainty, missing context, or limitation that could change the assessment.
NIST risk-management guidance emphasizes documenting assumptions and evaluation limitations. The ICO likewise discusses retaining evidence relevant to fairness obligations. Neither instruction overrides your employer’s data-minimization, access-control, legal-hold, or retention requirements. Redact identifiers, use an approved secure repository, and do not email sensitive transcripts to a broad distribution list.

6. Choose the escalation route by urgency, data, impact, and ownership
There is no universal workplace reporting chain. Your employer may combine responsibilities, use different names, or have no dedicated AI officer. Select the route that can act on the specific risk, and use more than one route when policy requires it.
| Situation | First route to consider | Why | Additional route when applicable |
|---|---|---|---|
| Possible immediate harm or a consequential decision in progress | Decision owner or manager | Can pause the action and arrange human review | Compliance, legal, HR, or safety team |
| Personal, confidential, or regulated information exposed | Privacy officer or security incident channel | Can contain access, preserve evidence, and assess notification duties | Tool owner, compliance, or legal |
| Hiring, performance, pay, accommodation, or other employment effect | HR or the designated employee-relations channel | Can protect the affected person and review the employment process | Legal, compliance, or AI governance lead |
| Recurring output, batch process, or embedded application | System owner or AI governance lead | Can suspend the workflow, test the system, and correct controls | Security, privacy, compliance, or procurement |
| Incorrect content with no sensitive data or immediate decision impact | Manager, product owner, or support channel named in the AI-use policy | Can correct the work and log a quality issue | Governance team if a pattern emerges |
Use the organization’s written AI-use, acceptable-use, incident-response, whistleblowing, privacy, and employee-relations policies to confirm the actual contact and escalation deadline. Do not assume that a general help desk, a model vendor, or a single manager is authorized to investigate every type of concern.
7. Report the issue so someone can act
- Open with the requested action. Say whether you need a decision paused, a record corrected, a privacy review, a security containment step, or a fairness assessment.
- Give the smallest complete evidence set. Include the date, tool, task, output, independent check, and potential impact. Link to an approved secure record instead of attaching sensitive material unnecessarily.
- Distinguish facts from hypotheses. State what you verified, what you suspect, and what remains unknown. Do not label a system discriminatory when you have only shown one unverified output.
- Name downstream uses. Tell the reviewer whether anyone relied on the answer, whether an external party saw it, and whether a decision or record changed.
- Request confirmation and an owner. Ask where the case is logged, who is responsible, and when you should expect a review or interim control.
- Escalate if the route is unresponsive or conflicted. Follow the next channel in your policy, such as compliance, an ethics line, HR, privacy, legal, or security. Keep a factual chronology of your reports.
Do not publish a customer, colleague, applicant, or coworker’s information to demonstrate the problem. Publicly posting a transcript can create a second privacy, confidentiality, or defamation issue and may prevent a controlled investigation.
8. Correct work that already used the answer
If the output was shared or acted upon, containment is part of the response. Tell the people who can prevent further reliance, using the channel approved for the sensitivity of the information.
- Mark the original analysis, ticket, report, or record as under review where your system supports that status.
- Send a concise correction that identifies the affected statement and the verified replacement; do not circulate unnecessary sensitive details.
- Ask the decision owner to re-check any person or group affected by the output using the approved human process.
- Correct copies, dashboards, exports, templates, and automated downstream records, not only the message where the error was first noticed.
- Record who was notified, when, and what corrective action was completed.
If a person may have lost an opportunity, access, pay, or service because of the answer, ask the responsible team how the decision will be reconsidered and how the person will be informed. The appropriate remedy depends on the organization, the process, and applicable law; do not promise a particular outcome yourself.

9. Ask for a proportionate investigation
The reviewer should match the test to the use case. A writing assistant that produced one wrong date needs a different response from a screening system that ranks applicants.
For an isolated conversational error
Verify the fact, correct the work, determine why the answer was accepted without review, and update instructions or training if needed. Check whether the same prompt or source is likely to recur.
For a repeated or embedded workflow
Pause or restrict the workflow while the owner examines representative cases, input data, prompts, retrieval sources, human review points, and logging. Assess results across relevant groups and subgroups when fairness is at issue. Document the test population, method, thresholds, limitations, and decision to resume, modify, or retire the system.
For a high-impact use
Involve the functions responsible for privacy, security, compliance, legal, HR, safety, or accessibility as appropriate. Require a human decision-maker who can override the system, a way to challenge an outcome, and a record of the rationale. NIST describes trustworthiness as something AI actors should consider throughout design, deployment, use, and testing; it is not a one-time check after deployment.

NIST’s AI Risk Management Framework and its Generative AI Profile are voluntary risk-management resources, not a universal employee incident procedure or a universal legal standard. NIST has stated that AI RMF 1.0 is being revised, so organizations should check the current edition and their own policy rather than treating every recommendation as a fixed rule.
Best Value
The ICO’s material on discrimination and bias is focused on the UK data-protection context and is primarily written for AI engineers and key decision-makers. The ICO says that guidance is under review following the Data (Use and Access) Act. Its discussion of UK GDPR and fairness should therefore not be presented as settled law everywhere or as a complete answer for workplaces outside the UK.
Both sources identify causes that can produce unfair outcomes, including insufficiently diverse or inappropriate training data, historical discrimination reflected in data, and design choices. Those explanations help an organization investigate; they do not tell an individual which internal form to use or determine legal liability in a particular case.
11. Common mistakes that make the situation worse
- Forwarding first, checking later: A polished answer can acquire authority once it appears in an email, report, or system record.
- Deleting the only evidence: Removing the transcript may make correction and root-cause analysis impossible. Preserve it securely when policy permits.
- Overstating the finding: One biased-looking answer warrants review, but does not establish prevalence, intent, or cause.
- Testing with real sensitive data: Reproducing a problem in an unauthorized tool can expose more people and violate policy.
- Using the wrong owner: A model team may not control an employment decision, and HR may not be able to contain a security incident. Route by impact and ownership.
- Ignoring downstream copies: Correcting the original chat does not fix a spreadsheet, dashboard, or decision record built from it.
- Assuming a disclaimer is enough: A notice that “AI may be wrong” does not replace meaningful human review for consequential work.
12. A short decision checklist
- Have I stopped the output from being treated as verified?
- What exact claim, wording, omission, or unequal treatment is at issue?
- Which current primary source or qualified reviewer checked it?
- Who could be affected, and how serious or reversible is the impact?
- Did I preserve the prompt, output, date, version, context, and evidence safely?
- Does personal, confidential, regulated, or security-sensitive information require a special route?
- Which team owns the decision or tool, and what does the employer’s policy require?
- Has the answer already been shared or used, and who needs a correction?
- What remains unknown, and what investigation or interim control should follow?
13. What a responsible outcome looks like
A sound response is not merely a complaint about an AI tool. It is a controlled correction: the questionable output is paused, the claim is independently checked, evidence is preserved with appropriate safeguards, affected work is corrected, and the organization decides whether the issue is isolated or indicates a wider control problem. The record should show what was known, what was uncertain, who reviewed it, and why the chosen action was proportionate.

That approach protects people from automation bias without pretending that every error has the same cause or legal consequence. It also gives the employer something actionable: a reproducible case, a clear owner, and a basis for improving prompts, data, human review, training, or the decision to use the tool at all.
