Treat the access as a security incident immediately: assign an incident commander, start a UTC timeline, constrain the agent and its tools, revoke its identities and trust relationships, rotate every secret that touched the run, and preserve the complete action record. Then separate what the agent could reach from what it actually read, changed, or transmitted. Legal and privacy owners should assess notification duties while technical teams eradicate persistence and restore only a tested, narrowly scoped fallback before considering broader autonomy.

Declare the incident and establish control

Do not wait for proof of exfiltration before opening an incident. An agent that reached sensitive production data may have had authority to read, alter, export, or persist information that its final answer does not reveal. Use your normal incident-management process and explicitly include the agent identity, model context, tools, memory stores, and downstream jobs in the incident boundary.

As an Amazon Associate I earn from qualifying purchases.

Assign named owners

Name an incident commander with authority to make containment decisions. Assign a security lead, the production-system owner, a legal or privacy contact, and a communications lead. Add platform, machine-learning, identity, and database specialists as needed. Record names, roles, handoffs, and decisions so there is one accountable path instead of parallel, conflicting changes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Start a UTC timeline

Record the original alert exactly as received, including its timestamp, detector, message, and attached identifiers. Add the first known agent invocation, approvals, tool calls, credential changes, data access, containment actions, and every subsequent decision in UTC. Preserve the original alert before anyone edits or closes it.

Choose containment with business owners

Containment that simply kills a shared service can create a second outage or destroy evidence. AWS guidance recommends pre-built decision trees for each AI component, anchored to business functions, documenting containment choices, business cost, cascading effects, and approval authority. Use those trees, or convene the system owner and business lead immediately, to select the least disruptive control that stops further risk.

Stop further access without creating a larger outage

Apply the narrowest effective control first, escalating only when the agent remains capable of harmful activity. If compromise is active or data exfiltration is confirmed, prioritize stopping the action over preserving convenience, while keeping an approved fallback for essential operations.

Immediate containment sequence

  1. Disable new agent runs. Pause the scheduler, queue consumer, workflow trigger, or deployment that can start another invocation. If a running process can continue making calls, suspend or terminate it according to the business-approved decision tree.
  2. Revoke active sessions and tokens. Invalidate agent sessions, short-lived credentials, API tokens, OAuth grants, service-account keys, and delegated permissions. Remove cloud trust relationships that let the agent assume another role or account.
  3. Remove tool permissions. Detach database, repository, ticketing, deployment, file, messaging, and administrative tools. A model that can still invoke a powerful tool remains an active risk even if its user interface is hidden.
  4. Restrict network egress. Block destinations and protocols used by the workflow where feasible. Preserve required internal traffic for containment and evidence collection, and document any exception.
  5. Switch to a documented fallback. Move critical business work to a human-operated or otherwise constrained process. Record who approved the fallback, what functions it covers, and what it cannot safely do.

Do not treat hiding the agent interface as containment. Its identity, scheduled jobs, worker processes, tool credentials, memory services, and downstream automations must all be checked. CISA and its partners recommend limiting agent autonomy and avoiding broad or unrestricted access, particularly for sensitive data and critical systems.

Rotate credentials and quarantine everything the run could have changed

Rotate every secret touched by the workflow

Rotate cloud keys, API keys, repository and package-registry tokens, signing credentials, database passwords, encryption or decryption keys exposed to the process, and secrets in environment variables or mounted files. Include credentials the agent could read even if logs do not yet show their use. Revoke the old values rather than merely issuing replacements, and update dependent services through a controlled change.

OWASP AISVS control AC.14.1 calls for “revoking AI-agent credentials, rotating every secret that touched the compromised workflow run, quarantining the compromised artifacts, notifying downstream consumers, notifying regulators where applicable, and preserving prompts, responses, and audit logs for forensics.” Treat that as a minimum containment checklist, then apply your jurisdiction-specific requirements.

Quarantine artifacts and persistence

Put generated or modified code, packages, container images, infrastructure plans, configuration files, deployment jobs, scripts, prompts, policy files, memory records, vector or retrieval stores, and scheduled tasks into a controlled quarantine. Prevent execution and further replication while retaining read-only copies for analysis. Tag each item with its source run, owner, storage location, and time of isolation.

Look for persistence that is easy to miss: new users or roles, modified access policies, webhooks, cron entries, CI/CD jobs, startup scripts, altered model instructions, poisoned retrieval documents, and changes in downstream queues. Notify teams or partners that consume an affected artifact before they promote or execute it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Preserve evidence before it disappears

Final model output is only one fragment of the action chain. Preserve the inputs, decisions, tool activity, identity context, and resulting data changes in tamper-evident storage. Restrict access to the evidence repository and record who collected, copied, viewed, or transformed each item.

Evidence to collect

Evidence class Capture Why it matters
Conversation and context System and developer instructions, user prompts, retrieved documents, memory entries, model responses, refusal or policy messages, and context-window metadata Shows what the agent was told, what data entered its context, and whether instructions or retrieved content redirected it
Actions and approvals Every tool call, parameters, result, approval, denial, policy decision, retry, and human override Reconstructs the sequence that final output can hide
Identity and configuration Agent and service identities, role bindings, OAuth grants, token identifiers, model and prompt versions, tool definitions, deployment settings, and policy revisions Establishes authority, version, and the exact control state during the run
System and data logs Database reads and writes, file and object access, exports, deletes, privilege changes, network flows, DNS, proxy, gateway, and cloud audit records Tests what was actually accessed or transmitted
Artifacts and provenance Checksums, container and package digests, source commits, infrastructure changes, memory or vector-store snapshots, and job definitions Links a change to its origin and supports trusted rollback

Make the record defensible

Export logs in their original format when possible, then hash or otherwise protect the copies. Keep a chain-of-custody record with collector, timestamp, source, hash, storage location, and any transformation. Do not “clean up” suspicious prompts, logs, or memory entries in place; create an analysis copy and retain the original.

OWASP identifies sensitive-data exposure in agent context and logs as a risk. Scope access to the evidence itself, redact only in controlled working copies, and document what was redacted and why. Preserve enough detail for forensic, contractual, and regulatory review.

Determine what the agent could access and what it actually did

Separate capability from activity. A role listing proves that a resource was reachable; it does not prove that records were read. Conversely, the absence of a complete log does not prove that no exposure occurred.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Build the reachable-resource inventory

  • List every account, tenant, database, table, bucket, repository, secret store, queue, model, memory or vector index, deployment target, and external service available through the agent’s identity or delegated roles.
  • Map each tool to its permissions, token lifetime, network path, and approval requirement.
  • Identify data classifications, record populations, geographic locations, and third parties associated with each reachable resource.
  • Record inherited permissions and trust paths, including roles the agent could assume indirectly.

Verify observed activity

Correlate agent traces with source-system audit logs, database query logs, object-access records, export events, network flows, and provider telemetry. Establish whether the agent read, wrote, modified, deleted, copied, compressed, encoded, or transmitted data. Check for privilege changes, new accounts, altered policies, and activity after the apparent end of the run.

Trace persistence and downstream spread

Follow artifacts into build systems, package registries, deployment pipelines, caches, queues, reports, tickets, email, customer-facing systems, and partner feeds. Identify who or what consumed each artifact and whether it can trigger additional actions. Quarantine downstream copies when provenance is uncertain.

Describe uncertainty precisely

Use separate findings such as “reachable,” “confirmed read,” “confirmed changed,” “confirmed exported,” “confirmed external transmission,” and “not yet determined.” Do not equate access with exfiltration, and do not declare the absence of exposure solely because no transmission is visible. State the log coverage, retention limits, clock differences, and unresolved gaps beside each conclusion.

Assess notification and communication duties

Legal, privacy, compliance, communications, the system owner, and relevant contract owners should review the scoped facts together. Notification depends on the data involved, affected jurisdictions, contractual terms, regulator rules, customer and employee commitments, and the quality of available evidence.

Questions for the legal and privacy review

  • What categories of personal, confidential, regulated, proprietary, or security-sensitive data were reachable or confirmed accessed?
  • Which individuals, customers, employees, tenants, partners, or countries are implicated?
  • Do contracts, data-processing terms, insurance policies, or sector rules impose notice, preservation, or cooperation deadlines?
  • Which regulators, customers, partners, or employees may need notice, and what facts can be stated without speculation?
  • What evidence supports a conclusion of access, acquisition, alteration, or transmission, and what remains unknown?

Keep communications factual and coordinated. Avoid promising that no data left the environment when the telemetry cannot establish that. Preserve privileged legal work and operational evidence according to counsel’s instructions.

Eradicate the cause and recover in stages

Remove unauthorized changes

Delete unapproved accounts, roles, grants, jobs, webhooks, policies, files, packages, prompts, memory entries, and infrastructure changes. Compare the environment with a trusted baseline and investigate every unexplained difference. Do not restore a quarantined artifact merely because it appears to work.

Rebuild from trusted provenance

Rebuild code, containers, policies, and infrastructure from verified source commits, clean dependencies, known-good images, and documented configuration. Validate signatures or checksums where available. Treat generated code and retrieval content as untrusted until reviewed by an owner who was not the sole approver of the original run.

Validate controls before re-enabling work

Confirm that credentials have been revoked or replaced, least-privilege policies match the intended task, tool calls require the right approvals, sensitive fields are protected, and egress monitoring and blocking work. Test logging from prompt through tool result, including denied actions and human approvals. Verify that alerting reaches the incident team.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use a constrained fallback first

Run a limited workflow with reduced data, read-only tools, narrow time and tenant scopes, explicit approvals, and blocked external transmission. Compare expected and observed actions. Keep the human-operated fallback available while the constrained path is monitored.

Restore broader autonomy only with approval

The business owner and security lead should approve each expansion of scope. The approval record should identify the tested model and prompt versions, identity and tools, data boundaries, monitoring, rollback method, and fallback owner. If any gate fails, keep the agent constrained and continue investigation.

Use explicit gates for turning the agent back on

Gate Evidence required Failure response
Identity and secrets Old sessions and grants revoked; exposed secrets rotated; trust relationships reviewed Keep the agent disabled and repeat credential review
Artifact integrity Code, packages, infrastructure, prompts, memory, and indexes traced to trusted provenance Rebuild or quarantine the affected component
Authorization Least-privilege roles, tool scopes, approval rules, and tenant boundaries tested Reduce permissions or require human approval
Data and egress controls Sensitive-data handling, redaction, destination allow-lists, and blocked paths verified Keep external transmission disabled and correct the policy
Observability Complete prompt, response, tool, identity, data-access, and network telemetry retained and alerted Do not restore autonomy without actionable logs
Continuity and rollback Constrained fallback works; rollback owner, steps, and recovery point are tested Operate through the fallback while remediation continues
Business approval System owner accepts residual risk and records the permitted scope Maintain the current restriction

Build controls that reduce the next incident’s blast radius

After recovery, conduct a blameless review that includes security, platform, machine-learning, legal or privacy, and business owners. Update the playbook, ownership matrix, logging retention, credential-rotation automation, and business-specific decision trees. Rehearse the revised process rather than leaving it as documentation.

Compare agent-control options on the right dimensions

Whether you are selecting an internal platform or an external service, compare capabilities rather than labels. The relevant questions are:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Control area Questions to ask
Identity isolation Does each agent, run, tenant, and environment have a distinct identity and independently revocable trust?
Least-privilege granularity Can permissions be limited by tool, action, record, tenant, time, environment, and approval state?
Token lifecycle How short are token lifetimes, and how quickly can sessions, grants, and assumed roles be revoked?
Audit completeness Are prompts, context, responses, tool parameters and results, approvals, policy decisions, and identity metadata captured together?
Tamper resistance and redaction Can logs be protected from alteration while sensitive values are controlled in authorized views?
Egress enforcement Can destinations, protocols, payload types, and external tools be blocked or approved centrally?
Rollback and provenance Can code, packages, policies, memory, indexes, and infrastructure changes be traced and reverted?
Integration Does the control feed existing identity, SIEM, SOAR, ticketing, backup, and incident-response processes?
Continuity Is there a tested human or constrained fallback, with clear ownership and recovery objectives?
Testing support Can teams run exercises, approval tests, failure drills, and business-owner tabletop scenarios?

Keep autonomy proportional to risk

Give agents read-only, narrow, and time-bounded access by default. Separate planning from execution, require explicit approval for destructive or external actions, and isolate production identities from development and evaluation environments. Sensitive data should be minimized before it enters context, and memory stores should have retention, access, and deletion controls.

Monitor the full action chain

Alert on unusual tool sequences, privilege changes, bulk reads, exports, new destinations, prompt or policy changes, and actions outside the agent’s normal tenant or time window. Retain enough context to reconstruct an incident without exposing more sensitive data than investigators need.

Set a realistic response target

OWASP’s agentic incident playbook treats active production exploitation or confirmed data exfiltration as critical and gives that category an illustrative one-hour response target. It is not a universal regulatory deadline or a guarantee that every organization can meet it. Set an organization-specific service level based on business impact, staffing, geography, and contractual obligations, then exercise it.

NIST SP 800-61 Rev. 3 places this work in a lifecycle of preparation, response, recovery, and improvement aligned with the Cybersecurity Framework 2.0. The post-incident review is therefore part of response maturity, not an optional retrospective.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Common mistakes that prolong exposure

  • Only disabling the chat or front end: revoke the underlying identity, sessions, tools, jobs, and delegated roles.
  • Rotating one API key: inventory and replace every secret that touched the run, including signing, repository, database, and cloud credentials.
  • Deleting suspicious logs or memory: preserve originals, isolate them, and analyze controlled copies.
  • Assuming capability proves access: corroborate role reachability with source-system and network telemetry.
  • Assuming no visible export proves no exposure: document telemetry gaps and continue checking persistence and downstream consumers.
  • Restoring the old agent unchanged: rebuild trusted artifacts, close the exploited path, and use a constrained fallback first.
  • Making notification promises before scoping: involve legal and privacy owners and communicate only verified facts and clearly labeled uncertainty.
  • Ignoring business dependencies: use approved decision trees so containment does not trigger an avoidable outage or unsafe manual workaround.

The practical endpoint is controlled restoration, not a quick restart: the agent’s authority is revoked and rebuilt, evidence is preserved, impact is understood as far as the logs allow, obligations are assessed, and every increase in autonomy has an owner, a test, and a rollback.
