If you find inaccurate or incomplete information, you can ask that provider or plan to amend it. There is no separate AI-specific procedure in the federal guidance covered here: start with the organization that maintains your record, and do not assume an AI draft, transcript, recording, or vendor-held file is part of the record you can access.
How to get a copy of your medical records
Ask the health care provider or health plan that maintains the record how to submit an access request. It may accept requests through a patient portal, a written form, or a designated medical-records contact. You generally do not have to explain why you want your records.

Under the federal HIPAA baseline, individuals can generally inspect, review, and receive copies of protected health information in a covered entity’s designated record set. That can include medical and billing records, payment and claims information, and clinical notes such as SOAP notes, whether kept electronically or on paper. Some exceptions apply, so the right does not guarantee access to every item.

In most cases, you can ask for a preferred format, such as email, if it is available. A provider cannot refuse to provide a copy just because you have not paid your medical bill. A reasonable fee may apply for copying and mailing, but not for searching for or retrieving records; HHS says electronic-record fees may not be charged on a per-page basis. One notable exception is psychotherapy notes that a provider keeps separate from the medical and billing record. The general access right described in HHS’s patient guidance does not cover those notes. Other limited exceptions may also apply.
What to ask for after an AI-assisted visit
Ask for the visit record the provider maintains, such as the clinical note and related records in its designated record set. The reviewed federal guidance does not establish that a raw AI transcript, recording, draft, or a vendor’s internal file is separately accessible to the patient. Those materials may not be the same thing as the provider’s maintained record; the status of a particular artifact depends on the circumstances and applicable rules.

How to review the visit note
When the record arrives, compare it with your understanding of the visit. Focus on details that could affect care or administration, including:

- Why you came in, and the symptoms or history recorded.
- Medication and allergy information.
- Examination or test information.
- The assessment and follow-up plan.
Not every record contains all of these fields. The point is to identify a specific error or omission in the record you received, rather than to assume the note should match a transcript or a particular format.

How to request a correction
If information is inaccurate or incomplete, send an amendment request to the provider or plan that maintains the record. Identify the entry in question, explain what is wrong or missing, and state what you believe the corrected information should say. Ask the organization how it wants the request submitted, and keep a copy of your request and any response.
HHS summarizes the right this way: “If you think the information in your medical or billing record is incorrect, you can request a change, or amendment, to your record.” A request is not a guarantee that the provider or plan will agree or alter the original note. The covered entity handles the amendment process; do not assume you can directly overwrite or erase a signed clinical note.

If the entity accepts the request, it must make reasonable efforts to provide the amendment to people you identify as needing it and to others it knows might rely on the information to your detriment. An amendment may be handled through linked documentation, and electronic handling is possible where the parties agree.

The entity must give you a written denial and an opportunity to submit a statement of disagreement for inclusion in the record. The disputed documentation must be linked to the disputed information and accompany later disclosures of that information. Keep the denial and your statement. HHS’s patient guide puts it plainly: “Your doctor might not agree, but you always have the right to have your disagreement added to your record.”

How long do access and amendment requests take?
Access and amendment requests have different federal HIPAA response periods. The general periods below come from HHS guidance; check the organization’s written response if it says it needs an extension.

HHS describes 30 days as the outer limit for access and encourages faster responses when possible. The access deadline is not the amendment deadline: asking for a copy and asking the organization to change information are separate requests.
Does HIPAA cover every record after an AI-assisted visit?
The federal guidance discussed here sets out ordinary access and amendment rights; it does not create a distinct AI-specific pathway or establish that an AI vendor necessarily holds a separately accessible patient record. The practical starting point is the provider or plan that maintains your record. The guidance does not settle the status of every draft, raw recording, transcript, or vendor-held item in a particular AI workflow.
