Use the report or feedback control attached to the unsafe answer whenever one is available. Select the most accurate safety, legal, abuse, or offensive-content category, explain what happened with enough surrounding context to reproduce it, and inspect the data that will be sent before submitting. A report can include the conversation, uploaded files, or connected-app content, so remove unrelated confidential information where the workflow allows.

What to do first when a chatbot gives an unsafe answer

Stop relying on the response and preserve the relevant context. Do not continue a dangerous instruction merely to “test” it, and do not edit the answer so heavily that the provider cannot see what the system actually produced.

  1. Identify the exact answer and feature. Note the provider, app or workspace surface, model feature if shown, and whether the content came from a normal chat, a shared link, a custom assistant, an advertisement, or a connected-app workflow.
  2. Use the response-level control. Look for a thumbs-down, Bad response, Report, Report abuse, or similar control directly beneath or beside the answer.
  3. Choose the closest category. Safety, offensive or unsafe content, abuse, legal concern, and security vulnerability are not interchangeable routes.
  4. Describe the failure precisely. Explain what the answer claimed or instructed, why it is dangerous or misleading, and what immediately preceded it.
  5. Review the submission payload. Check whether the report will include prompts, responses, files, images, documents, email, or connected-app data. Remove unrelated private information if the interface permits it.
  6. Save a private record. Keep the date, screenshots or copied text, the conversation URL if applicable, and any steps needed to reproduce the behavior. Do not publicly repost harmful instructions just to demonstrate the problem.

Reporting is an input to a provider’s review process, not a promise that a model will be removed, a conversation deleted, or a particular enforcement action taken.

What qualifies as an unsafe chatbot response?

A response can warrant reporting even when it is not illegal. Common examples include:

  • Instructions that could facilitate physical harm, self-harm, violence, biological threats, fraud, or other dangerous conduct.
  • Confident medical, legal, financial, or emergency advice that is materially wrong or omits an important warning.
  • Hate, targeted harassment, sexual exploitation, graphic abuse, or other offensive material that bypasses the service’s safeguards.
  • Personal data disclosed or inferred inappropriately, including information pulled from an uploaded file or connected application.
  • A refusal or warning mechanism that fails in a repeatable way, such as answering a prohibited request after a small wording change.
  • A security weakness or universal jailbreak that could affect many users, rather than one isolated troubling completion.

Use the narrowest accurate description. A factual mistake is not automatically a security vulnerability, and an offensive answer is not automatically a legal complaint. Correct classification helps route the report to the team equipped to assess it.

Provider routes at a glance

Provider and surface Primary control Separate routes Context that may be attached
ChatGPT web or mobile Thumbs down beneath the message → Select an issue → Safety or Legal concern Dedicated reporting options for shared links, GPT conversations, GPTs, advertisements, and forum posts; a content-reporting webform is also available The reported conversation or content, depending on the workflow
Claude In-product report control on shared Claude content, or Anthropic’s standalone reporting form Separate policy or local-law content reporting; universal-jailbreak and security issues go through vulnerability-reporting guidance The material submitted and the details needed to reproduce the issue
Gemini Apps Good response or Bad response beneath the answer → choose Offensive / Unsafe when applicable Settings & help → Send feedback for general problems; Report legal issue for legal concerns Associated prompts and responses, and potentially uploaded files, images, and connected-app content
Gemini for Workspace Report abuse beside the response or under More options Good/bad suggestion flow for ordinary response feedback Prompts, context documents, emails, and the Gemini response used in the workflow

Labels and placement can vary by app surface, account type, and locale. The instructions summarized here reflect provider documentation available in October 2026; check the relevant Help Center if a control is absent or renamed.

How to report a ChatGPT response

Use the message-level safety route

  1. Open the conversation on the web or in the mobile app.
  2. Locate the problematic assistant message and select the thumbs-down control beneath it.
  3. Choose Select an issue.
  4. Select Safety or Legal concern, then follow the prompts to submit the report.

Use the item-specific route when the problem concerns something other than an ordinary answer. ChatGPT documentation describes separate reporting paths for shared links, GPT conversations, GPTs themselves, advertisements, and forum posts. A content-reporting webform is another option when the in-product control is unavailable or the report concerns a published item.

OpenAI says it combines automated detection of potentially harmful prompts, completions, and uploads with reports from people; trained experts may review human reports alongside automated systems. Its transparency information says any moderation or enforcement decision depends on circumstances such as legal requirements, severity, and prior or repeat violations. A submitted report therefore should not be described as a guaranteed takedown or account action.

Reporting harmful or illegal content in a shared link

Treat a shared link as a published item rather than assuming the ordinary message control is the right channel. Use the shared-content reporting option or the provider’s content-reporting form, identify the link, and state whether the concern is unsafe instructions, illegal material, exposure of personal information, or another policy issue. Include only the surrounding conversation needed to establish the problem.

How to report a Claude safety issue

Ordinary safety and user-experience problems

Anthropic says it welcomes reports about safety issues, including safety mechanisms that create user-experience problems. On shared Claude content, use the in-product report button. If that control is not available, use the standalone reporting form described in Claude’s Help Center.

Anthropic asks reporters to provide enough detail to replicate the issue. Include the prompt sequence, the concerning completion, relevant settings or feature context, and the smallest repeatable set of steps. If a refusal was bypassed, explain the wording and sequence without publishing dangerous instructions more broadly than necessary.

Policy, local-law, and security routes

Use Claude’s separate content-report route for a policy or local-law concern instead of treating it as ordinary thumbs-down feedback. Universal jailbreaks and security vulnerabilities require a different path. Anthropic’s vulnerability guidance describes universal-jailbreak submissions for ASL-3 uses of concern related to biological threats and directs security researchers to its Responsible Disclosure Policy. Those channels are intended for disclosure of a weakness with wider security implications, not for a single unpleasant answer.

How to report a Gemini response

Gemini Apps

  1. Under the answer, select Good response or Bad response.
  2. For an unsafe or offensive completion, choose the Offensive / Unsafe reason.
  3. Add a concise explanation describing the harmful behavior and the relevant preceding prompt.
  4. Submit the feedback after reviewing what information the form will attach.

For a general malfunction that is not tied to one answer, use Settings & help → Send feedback. Choose Report legal issue when the concern is a legal complaint rather than a normal safety report.

Google states that Gemini feedback includes the associated conversation, such as prompts and Gemini responses, and may also include uploaded files, images, and personal content from connected apps. Treat the preview or disclosure shown in the form as a privacy check, not as a formality.

Gemini for Workspace

In Workspace applications, use Report abuse next to the response or under More options. Google’s support guidance says this report can include the prompts, context documents, emails, and Gemini response used to produce the output. Ordinary response feedback uses a separate good/bad suggestion flow. If the answer drew on a sensitive document or mailbox, confirm that the report is appropriate for your organization before submitting it.

What to include in an effective safety report

Describe the failure, not just your reaction

“This was bad” is difficult to investigate. State the observable behavior: what the model said or recommended, which safety expectation it violated, and what a safer response would have done instead. Distinguish a false statement from an instruction that creates a realistic risk, and identify whether the answer was merely offensive, factually dangerous, or potentially illegal.

Preserve the minimum useful context

Include the prompt immediately before the problematic answer and any earlier turn that changed the model’s behavior. If a file, image, document, email, tool result, or connected application supplied important context, identify it without copying unrelated personal material. Do not crop away a refusal, warning, or adversarial prompt if that surrounding text explains how the failure occurred.

Make the behavior reproducible

  • Record the product surface, approximate date and time, account or workspace context when relevant, and the steps taken.
  • State whether the result happened once or repeatedly.
  • Note any model, mode, custom instruction, tool, extension, or connected service involved.
  • Explain what changed when the behavior stopped, if you tried a safe reproduction.
  • Attach screenshots or copied text only when they add information that the form does not already capture.

Anthropic explicitly asks for enough detail to replicate a safety issue. The same discipline improves reports to every provider, even when the form does not require every field.

Use the provider’s category labels

Select the closest available category and add a short explanation. Gemini’s Offensive / Unsafe option is more informative than a bare bad-response signal when the answer presents a safety risk. If none of the labels fits, choose the general feedback route and name the issue plainly in the description.

Check privacy before you press Submit

A report can contain substantially more than the text you type. Depending on the product, it may attach the full conversation, prompts, responses, uploaded files, images, connected-app content, context documents, or emails.

  • Read the disclosure or preview shown by the form.
  • Remove unrelated names, account numbers, health information, customer records, confidential source code, and private correspondence when the interface allows editing.
  • If the attachment cannot be narrowed, decide whether the provider needs that material to investigate. Consult your employer’s or client’s data-handling policy for Workspace content.
  • Do not upload a cleaned-up reconstruction if doing so would hide the actual failure; instead, preserve the original privately and submit only the necessary excerpt.
  • Never include passwords, authentication tokens, private encryption keys, or other credentials in a report.

Privacy review is especially important for Gemini feedback and Workspace abuse reports because Google says those workflows may include connected-app data and the documents or emails used as context.

Choose the right route: feedback, legal complaint, or vulnerability disclosure?

Situation Use What to emphasize
One answer is dangerous, offensive, or materially misleading Response-level safety or abuse feedback The exact output, preceding prompt, category, and risk
A shared link or published item contains prohibited material Shared-content or content-reporting form The item URL or identifier and why the published material violates policy or law
You believe content violates local law or requires a formal legal review Provider’s legal-content route The jurisdiction, item, and specific legal basis when you can state it accurately
A weakness can affect many users, or a universal jailbreak defeats a safeguard Security vulnerability or responsible-disclosure channel Reproduction steps, impact, scope, and safe handling of exploit details

Do not send a single-answer complaint through a security program merely because the output was alarming. Conversely, do not publish a broadly reusable exploit in an ordinary feedback form if the provider provides a responsible-disclosure route.

What happens after a report?

Providers may combine automated screening, specialist review, policy analysis, engineering investigation, and model or product changes. OpenAI describes automated detection and human review, while also noting that outcomes depend on legal requirements, severity, and repeat-violation history. Documentation from the other providers similarly describes collection of details for review rather than a guaranteed response time.

You may receive no individualized explanation. That does not establish that the report was ignored, and a confirmation message does not establish that a particular fix was deployed. Keep your own record if the behavior affects a workplace, customer, safety incident, or regulated process. If the same failure remains reproducible, submit a focused follow-up that references the earlier report and supplies new evidence rather than sending many identical complaints.

Practical ways users and teams can improve chatbot safety

Verify high-consequence answers independently

Do not let a chatbot’s confident tone substitute for a qualified professional, emergency service, official documentation, or primary record. For medical, legal, financial, security, and physical-safety decisions, treat the model as an assistive drafting tool until the relevant authority verifies the result.

Reduce sensitive-data exposure

Use the least amount of personal, confidential, or proprietary information needed for the task. Before connecting an app or uploading a document, determine what data a future feedback report could include and whether your organization permits that transfer.

Keep an internal incident log

For a team using chatbots, record the feature, date, prompt pattern, output, impact assessment, and provider ticket or report identifier. Grouping repeat failures helps distinguish an isolated hallucination from a systemic safety problem and gives the provider reproducible evidence.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Test safeguards without escalating harm

Use controlled, non-operational examples when checking whether a safeguard fails. Do not generate, store, or circulate real harmful instructions solely for testing. If the weakness appears universal or security-relevant, stop testing once you have enough evidence to reproduce it safely and use the appropriate disclosure channel.

Share patterns responsibly

When warning colleagues or users, summarize the risk and mitigation rather than reposting dangerous output verbatim. Redact personal data and access details. Public discussion can help others avoid harm, but broad distribution of an exploit can increase the harm the original report was meant to reduce.

When the report button is missing

  • Check whether you are viewing a shared item, an embedded assistant, a Workspace app, or a mobile surface; each can expose a different control.
  • Look under a three-dot or More options menu and inspect the provider’s current Help Center instructions.
  • Use the provider’s general feedback or content-reporting form when the answer-level control is unavailable.
  • Capture the exact interface, account type, locale, and date in your notes so the provider can understand why the expected control was absent.
  • If there is an immediate physical threat, criminal emergency, or active account compromise, contact the appropriate emergency, legal, or security response channel rather than waiting for chatbot feedback.

How to write a concise report

A useful report can be short if it answers five questions: what product and feature were involved, what prompt or event preceded the answer, what the model produced, why that creates a safety or legal risk, and what steps reproduce it. This format keeps the report readable while preserving the evidence investigators need.

Product and surface: [name of app or feature]
Date and approximate time: [date and time]
Preceding prompt or event: [minimal relevant context]
Problematic response: [short exact excerpt]
Risk or policy concern: [what makes it unsafe]
Reproduction steps: [numbered steps, if repeatable]
Attached data checked: [files, images, documents, or connected data included or removed]

Keep variable text such as the fields above inside your private notes or the provider’s form; do not post sensitive examples publicly.

What the available evidence does—and does not—show

Google reported that in 2025 it awarded over $17 million to more than 700 researchers who contributed to the safety and security of its products. That is a bug-bounty program figure, not a measurement of harmful chatbot-response rates, report volume, or the effectiveness of user feedback.

There is no established cross-provider statistic in the available provider guidance for how often unsafe outputs occur or how many reports produce a specific mitigation. The defensible practice is therefore to report accurately, preserve reproducible evidence, protect attached data, and use the route matched to the problem.

Bottom line

Report the exact unsafe answer through the provider’s own control, select the correct safety, legal, abuse, or security category, and include enough context for reproduction without oversharing private data. ChatGPT, Claude, Gemini Apps, and Gemini for Workspace each separate ordinary response feedback from specialized routes, and each may attach more conversation or connected content than the reporter expects. A careful, well-scoped report gives safety teams useful evidence while reducing the risk that the reporting process creates a second privacy or security problem.
