A game-breaking bug report is useful when another person can follow it from a known starting state, trigger the same failure, and verify the impact. Give QA the exact game build, platform and hardware, account or save conditions, numbered reproduction steps, expected and actual results, reproduction rate, and evidence such as a short video and relevant logs. Submit it through the game’s official reporter, support form, or issue tracker—not a channel that never reaches the responsible team.
What makes a game-breaking bug report reproducible?
“Game-breaking” describes the player impact, not a special report format. The defect may stop a quest, prevent a save from loading, crash before play begins, make progression permanently unavailable, or prevent a match from finishing. QA still needs a precise, executable description of the defect.

A strong report answers five questions in order:
- What failed? Name the feature, location, quest, mode, or screen.
- Where and under what conditions? Identify the build, platform, hardware, operating system, account or character state, network, region, and relevant settings.
- How can it be triggered? Provide numbered actions from a defined starting point, including prerequisites.
- What should happen instead? State the expected result separately from the observed result.
- How badly does it affect play? Describe the blocked activity and provide your observed reproduction rate.
Keep the report short enough to scan, but never omit a condition needed to execute it. Bungie Help describes its reproduction field this way: “REPRO STEPS: Step-by-step instruction list on how to reproduce this bug, try to be as succinct as possible.” Concise means removing commentary, not removing setup information.

Start with a title that identifies the failure
A title is the triage team’s first filter. Include the broken feature or object, the location or mode, the trigger, and the player impact. A useful pattern is:

Feature or location fails after trigger; player impact
For example: Quest door stays locked after respawn in Chapter 3; progression blocked. “The game is broken” does not tell a tester what to search for, while the example distinguishes the object, timing, location, and consequence. If the issue is a crash, include the action and the crash point, such as Game crashes when opening the inventory after fast travel in version 1.4.2; save cannot be continued.
Record the exact build and environment
Version information is often the difference between a report that can be reproduced and one that cannot. Record the complete version or build number shown in the game, launcher, or console information screen. Identify the distribution branch or store when the game has separate releases, test branches, or cloud versions. Include the date and time of the session; a server-side defect may exist only during a particular deployment window.

For PC reports, a DxDiag export can provide the requested system information. Attach it only through an approved channel and remove personal data that the receiving team does not need.
Define the starting state before the steps
Reproduction begins before step one. State the save slot or character, quest or chapter progress, inventory items, party composition, game mode, difficulty, and any account entitlement required. Note whether the session started from a fresh launch, resumed from suspend, loaded a checkpoint, or reconnected to a server.
List modifications and unusual settings explicitly: mods, overlays, accessibility options, controller remapping, graphics settings, language, input method, cross-play status, and parental or account restrictions. If the bug occurs only with a particular item, class, party member, or difficulty, make that prerequisite unmistakable. If you tested without mods or with default settings, say so; that is useful negative information.
Example of a precise prerequisite
“Use save slot 2 with Chapter 3 completed through the flooded archive, the bronze key in inventory, two party members, Hard difficulty, English text, no mods, and a fresh launch.” This lets a tester create the same conditions instead of guessing which details are relevant.

Write executable reproduction steps
Use one action per numbered step and begin at a named menu, save, or location. Use the exact labels shown on screen and identify the input: “select Continue,” “press the right trigger,” or “enter the east doorway.” Avoid vague verbs such as “play normally,” “go around,” or “try again.” Include waits, dialogue choices, item use, deaths, respawns, reconnects, and repetitions when they affect the outcome.
- State the starting point and prerequisite, such as loading a named save or selecting a mode.
- Perform the first relevant input or movement.
- Continue with each interaction, choice, timing condition, or transition in the order used.
- End with the action that triggers the failure and describe what appears on screen.
Do not bury the trigger in a paragraph. If a step requires a timing window, give an observable cue (“press the button while the second warning icon is visible”) rather than an unmeasured instruction such as “do it quickly.” If the issue needs a sequence of failures, document each one.
Measure the reproduction rate
Run the sequence more than once when it is safe to do so and report your own count: 5/5 attempts, 2/10 attempts, intermittent after reconnecting, or only on the first launch after a patch. Do not present a guessed percentage or an industry benchmark. Explain what changed between successful and unsuccessful attempts if you observed a pattern.
Separate expected and actual behavior
Write two distinct fields. The expected result describes what the game should do after the final step if the defect is absent. The actual result records exactly what happened, including error text, frozen input, missing object, crash behavior, or a return to the title screen.

For example:
- Expected: After the character respawns, the archive door is unlocked and the player can enter to start the next objective.
- Actual: The door remains locked, the interaction prompt is absent, and the objective cannot advance. Reloading the same save reproduces the lock.
Copy error messages exactly, preserving capitalization and codes. If the screen contains several messages, identify which appeared first and attach a readable screenshot.
Describe impact and severity without guessing priority
Explain the consequence in player terms: a quest cannot continue, a save cannot load, a crash prevents play, progression is permanently blocked, purchased content cannot be entered, or a match cannot be completed. State whether another save, mode, character, or workaround remains usable.
Severity is the degree of harm to play. Priority is how soon the receiving team should address it, and the tracker may assign that separately. Do not label an issue “highest priority” merely because it is frustrating; provide evidence of the blocked path and let the team apply its own scale. Last Epoch’s support guidance asks for impact and severity so development can prioritize work, but no universal game-industry severity scale is established by the available guidance.

Attach evidence that shows setup and failure
Evidence should let a tester verify both the conditions and the result without opening an unsafe or inaccessible file. A short screen recording is often the clearest option: begin with the version or settings screen when practical, show the relevant save or location, perform the steps at normal speed, and keep the failure visible for several seconds. Include the system clock or an in-game timestamp when server correlation matters.

Gaijin’s instructions ask reporters to include the required client or launcher logs, screenshots, or video for the selected platform and game version. Last Epoch asks users to use its in-game reporter, select Attach Player Data, and select Add Screenshot. These labels illustrate why you should follow the receiving form’s own attachment controls rather than assuming every channel accepts the same files.
Redact private information
Inspect logs, screenshots, video, and save files for email addresses, account IDs, authentication tokens, chat messages, payment information, API keys, or unrelated users’ names. Redact only information that is not needed for diagnosis, and do not edit out the error, timestamp, build, or relevant settings. Keep an unredacted original locally in case official support requests it through a secure channel.
Submit through the channel that reaches QA
Use the game’s official in-game reporter, support form, or issue tracker. A forum post, chat message, or social-media thread may be visible to other players but does not necessarily enter the development queue. Read the form’s categories and attachment limits before writing; choosing the closest accurate category helps routing.

Gaijin notes that its dedicated issue service is reviewed by personnel who gather information and forward it to the development team. Bungie’s official template includes platform, issue, concise reproduction steps, actual result, expected result, links, and notes, with additional PC fields for CPU, GPU, RAM, driver, operating system, connection details, and DxDiag. Treat these as examples of the level of detail a channel may require, not as a universal form.

If the issue is still present after a patch, update the existing report with the new build and reproduction result instead of creating several indistinguishable reports, unless the tracker explicitly asks for a new ticket.

5. Return to the east archive door and approach the interaction point.
Reproduction rate: 5/5 attempts.
Expected result: The door is unlocked after respawn and the player can enter to continue Chapter 3.
Actual result: The door remains locked and has no interaction prompt. Reloading save slot 2 does not restore access.
Impact and severity: Chapter 3 cannot progress from this save; a new character is the only tested alternative.
Evidence: 42-second screen recording showing the version screen, steps 1–5, and the locked door; screenshot of the missing prompt; client log from the 19:40 UTC session.
Workaround and regression notes: No workaround found. Issue first observed after updating from version 1.4.1.Replace the example’s values with your own observed facts. Do not claim a regression unless you tested an earlier build or have a reliable record that the behavior changed.

“It happens randomly”
Randomness is often an unrecorded condition. Compare successful and failed attempts: launch state, server region, party, input device, timing, graphics settings, reconnect status, and save slot. Report the exact count and the variables you noticed rather than assigning a cause you cannot verify.
Missing build or platform
A screenshot of the bug without a version number cannot tell QA which code to install. Add the complete build, platform, store or branch, operating system, and relevant driver or console model.
Steps that start in the wrong place
“Go to the boss and fight it” leaves the tester to invent the path. Name the save, menu, checkpoint, quest state, and required items. If travel or a dialogue choice matters, include it.
Expected and actual results mixed together
Separate the two fields and copy the exact error text. This prevents a description such as “the door should be open but is locked” from hiding whether the prompt is missing, the animation fails, or the game crashes.
Evidence that cannot be opened
Test every video or log link in a private browser window or with the same permissions a support agent will have. Observe attachment limits, use the requested file format, and provide a timestamp that identifies the relevant segment.
Publicly exposing private data
Never post tokens, email addresses, account identifiers, or unredacted diagnostics in a public tracker. Use the official secure upload path and retain the original privately.
Duplicate reports after every patch
Update the existing ticket with the new build, whether the issue persists, and any changed reproduction rate. Open a new report only when the channel’s rules say the defect is distinct or the old ticket is closed and cannot be updated.
Special cases that need extra detail
Crashes and hard locks
State whether the game closes, freezes, returns to the dashboard, or forces a reboot. Note whether audio continues, whether input responds, and whether the crash creates a dump. Include the action immediately before failure, the number of attempts, and the corresponding log or dump timestamp.
Online, server, and region-specific failures
Record connection type, region, platform network, party or matchmaking state, reconnect sequence, and approximate UTC time. Say whether the problem affects one account, every account tested, or only a particular character. Do not include passwords or session tokens.

Keep a small attempt log: date and time, build, conditions, outcome, and any changed variable. Submit the pattern and the full sequence for a failed attempt. “2/10 after reconnecting” is more actionable than “sometimes.”
Mods and overlays
List every mod, injector, overlay, reshader, controller mapper, and accessibility tool active during the failure. If possible, repeat with all third-party components disabled and report both results. Do not assume a clean test proves the mod is responsible; it only establishes whether the symptom remains under that condition.
Regression checks
If an older build worked, name the last known good version and the first version where you observed the failure, along with how you tested each. If you did not test an earlier build, describe the timing as an observation rather than a confirmed regression.

Two studies help explain why structured prompts matter. A 2022 study known as the Burt study evaluated guided end-user bug reporting in an Android experiment and reported higher-quality information than a template-based system. A 2019 study, Assessing the Quality of the Steps to Reproduce in Bug Reports, described poor reproduction steps as a major problem and proposed feedback to improve them. These findings support asking focused questions and testing whether another person can execute the sequence; they do not establish a universal severity scale for games.
Before submitting, read only the numbered steps and prerequisites as if you were a tester who has never seen the problem. If you cannot tell where to start, which input triggers the failure, or what success should look like, revise the report. Then verify that the evidence, build, privacy treatment, and submission channel meet the receiving team’s requirements.
