When a chatbot gives a false or misleading answer, do three things: preserve the exact claim, verify it against an authoritative source, then use the chatbot’s current feedback or reporting control. If you already shared the answer, correct it wherever it appeared. A confident tone, fluent wording, or citation supplied by a chatbot is not proof that the information is true.
This guide covers the documented procedures for ChatGPT, Claude, and Gemini Apps. Interface labels can change, so use the current prompts shown in your account. Reporting can alert a platform to a quality, safety, or legal problem, but it is not a guaranteed correction of your conversation, a guaranteed reply, or a promise that the model will never repeat the error.

Why chatbot answers can be false even when they sound certain
Consumer chatbots generate responses from learned patterns rather than checking every sentence against a live, authoritative record. They can produce plausible-sounding claims, omit important context, misread a question, or invent quotations and citations. The wording may be polished and confident while the underlying statement is wrong.
OpenAI says ChatGPT is designed to provide useful responses based on patterns in data on which it was trained, and warns that it can provide incorrect facts or fabricated citations. Anthropic similarly advises users not to treat Claude as a singular source of truth and recommends checking original websites. These cautions apply to ordinary factual questions as well as summaries, translations, calculations, and explanations.

First decide what kind of problem you found:
- False: the answer states something contradicted by reliable evidence.
- Unsupported: the answer makes a precise claim but supplies no verifiable evidence.
- Misleading: individual details may be true, but the framing, omitted qualification, date, location, or definition creates a wrong impression.
- Incomplete or unhelpful: the answer does not address the question fully, without necessarily containing a factual error.
That distinction helps you choose ordinary quality feedback, a safety or legal report, or a correction to people who saw the answer.
Step 1: Preserve the exact claim before changing the chat
Save the original wording before asking follow-up questions or editing the conversation. Copy the sentence, number, quotation, or citation that appears wrong, and keep enough surrounding text to show how the chatbot reached it. A screenshot can preserve the interface, while copied text makes the disputed wording searchable.

- The country, state, language, product edition, or other scope specified in your question.
- Any source links, book titles, case numbers, study names, or quotations supplied by the chatbot.
- Whether the answer was produced in a normal chat, a specialized feature, or a shared conversation.
Do not paste passwords, payment details, medical records, private correspondence, or another person’s personal information into a feedback form. Redact unnecessary identifiers while retaining the facts needed to explain the error.
Step 2: Verify the disputed statement
Check the specific claim rather than trying to prove that the entire response is bad. Start with the original document or an authoritative primary source: an official record, government publication, court filing, regulator, original study, or organization directly responsible for the fact. A reputable secondary source can help explain context, but it should not replace the underlying record when the record is available.
Inspect every citation the chatbot gives you
Open the cited page yourself. Confirm that it exists, that it actually says what the chatbot claims, and that the relevant date, section, population, unit, and limitation match the answer. A real article can still be misquoted or applied to the wrong question. If a citation leads to a missing page, unrelated text, or a source that never made the quoted statement, treat it as a fabricated or unreliable citation.
Check dates, definitions, and scope
Many apparent hallucinations are stale or over-broad statements. A statistic may apply to one year, jurisdiction, age group, or survey sample. A legal rule may have changed. A product feature may exist only in one plan or region. Write down the qualification that makes your correction accurate instead of replacing one sweeping statement with another.

Use a second check for consequential claims
For medical, legal, financial, employment, safety, or security decisions, consult the relevant qualified professional or authority. OpenAI encourages critical assessment and verification of important information, and Anthropic specifically advises scrutiny of high-stakes advice. A chatbot’s answer should not be the sole basis for an irreversible decision.
Step 3: Report or give feedback inside the chatbot
Use the platform’s current in-product control whenever it is available. The documented routes are summarized below; labels and placement can change as interfaces are updated.

ChatGPT: choosing the right report path
For an inaccurate answer, begin with the thumbs-down control attached to that message. Select the issue category that best describes what happened. If the problem involves a safety or legal concern, choose “Safety or Legal concern” and complete the prompts. Use a separate content-reporting route when the issue concerns content or platform conduct covered by that route rather than ordinary answer quality.
OpenAI’s older “Chat model feedback” web form is marked deprecated. Do not rely on it as the general current route; use in-product feedback, the appropriate content-reporting option, or customer support as directed by the current help instructions.

Use the thumbs-down control on the particular response so the platform can associate your feedback with the answer. Anthropic’s help guidance, dated March 16, 2026, also lists [email protected] for thoughts and suggestions. Describe the exact sentence, why it is wrong, and the authoritative evidence. Do not assume that an email creates a personal case or guarantees a reply.
Gemini Apps: mark a hallucination or inaccurate answer
Click thumbs-down on the response and follow the feedback prompts. Identify the exact statement and explain what the supporting source says instead. Keep the report focused on the factual problem; include a source link only if it is safe and necessary to establish the correction.
What to include in a useful report
A concise, reproducible report is more useful than a general statement that the chatbot is “bad.” Include:

- The disputed output: quote the exact sentence or attach a readable screenshot.
- Your prompt and relevant context: include the wording needed to reproduce the answer, removing private details.
- The error type: false fact, fabricated citation, misleading omission, unsafe advice, or another specific problem.
- The correction: state the accurate claim with its date, jurisdiction, unit, or other necessary qualification.
- The evidence: name and link to the original or authoritative source when appropriate.
- The potential impact: explain briefly if the answer could cause physical harm, legal exposure, financial loss, discrimination, or privacy damage.
Avoid sending a long unrelated transcript. If several statements are wrong, list them separately so each can be checked.
Step 4: Correct the information wherever you repeated it
Reporting the chatbot does not repair a post, message, document, or conversation that other people have already seen. Correct the record in the same place whenever practical.
For a public post or article
Edit the original text if the service allows it. State what was wrong, give the corrected wording, and cite the evidence. If an edit history is visible or the original has been widely shared, add a clearly dated correction rather than silently changing a consequential claim.
For a private message or email
Send a follow-up to the same recipients. Identify the earlier statement, explain that it was inaccurate, provide the corrected fact, and include the source. Ask recipients to disregard or replace the earlier version if they may have forwarded it.
For a work, school, or project document
Replace the erroneous passage and preserve the source used for the correction. Tell collaborators which version is authoritative. If a decision was made using the false answer, notify the responsible person promptly and describe any action that should be reviewed.

If reliable sources disagree or the situation is still developing, say so. Distinguish an established fact from an estimate, allegation, preliminary finding, or unresolved interpretation. Do not correct a chatbot’s overstatement by making a new unsupported absolute claim.
Useful correction wording
A correction should be direct enough that readers do not have to compare two versions themselves. For example: “Correction: I previously made an inaccurate statement. The official record or original study shows the corrected claim. The relevant qualification is the applicable scope, jurisdiction, or uncertainty.”

For a private message, a shorter version works: “Please disregard my earlier message. I checked the source, and the correct information is stated there.” Do not describe a claim as proven merely because one page repeats it; explain why the source is authoritative and identify any remaining uncertainty.
When the answer creates a high-stakes risk
Stop relying on the chatbot and contact an appropriate professional or authority when the answer concerns diagnosis or treatment, legal rights or deadlines, taxes or investments, physical safety, emergency instructions, identity, employment, housing, or access to essential services. Preserve the answer and your verification notes, but do not wait for a platform to respond before taking a safe next step.
- Medical: contact a licensed clinician, pharmacist, poison center, or emergency service as appropriate.
- Legal: check the controlling law or court/agency guidance and speak with a qualified lawyer for advice about your situation.
- Financial: verify figures, fees, eligibility, and deadlines with the relevant institution or regulated professional.
- Safety or security: follow official emergency, manufacturer, or incident-response instructions rather than an unverified chatbot procedure.
When reporting a high-stakes answer, explain the likely harm and whether anyone acted on it. Remove personal data that is not needed for the platform to assess the issue.

Official guidance documents mechanisms for submitting feedback and reports. OpenAI says reported domains and other content may be reviewed by its Model Quality team, which may apply filters or other mitigations. That describes a possible action, not a promise that every report changes a model or filter.
The reviewed guidance does not promise an individual correction, a response time, or prevention of future errors. A thumbs-down submission may help a platform evaluate quality, but it does not edit the text already shown to you, retract a message you sent, or guarantee that a similar prompt will produce a different answer later.
Keep your own corrected source and update copies under your control. If the false answer remains visible, label it clearly as inaccurate when you share the conversation for educational or investigative purposes.

Troubleshooting when the reporting control is missing
The thumbs-down icon is not visible
Check that you are viewing the individual response, not only a conversation list or shared preview. Interface experiments, mobile layouts, account types, and feature changes can move or remove controls. Open the platform’s current help page from within the service and follow the route it currently documents.
The report menu does not match these labels
Use the closest current category and describe the discrepancy in your written explanation. Do not force a quality complaint into a legal or safety category unless the content genuinely presents that concern. Menus can change after the instructions summarized here were published.
The chatbot supplied no usable citation
Verify the claim independently using the original or authoritative source. A missing citation is not proof that the claim is false, but it is a reason to withhold trust until the statement is supported.
You submitted feedback but the answer remains wrong
That is not evidence that the report failed or that the platform accepted the claim. Feedback review and model changes are separate from the conversation you already have. Correct your own copies and use an authoritative source for future decisions.
Common mistakes to avoid
- Arguing with the chatbot instead of checking the source: a revised answer is not proof of accuracy.
- Reporting only a vague impression: identify the exact sentence and explain the measurable problem.
- Sharing sensitive material to prove the point: redact private information and provide only what is necessary.
- Using the wrong report category: reserve safety or legal reporting for a genuine safety or legal concern.
- Silently editing a widely shared claim: make the correction visible and dated when the original may still circulate.
- Expecting a platform to notify everyone: you remain responsible for correcting recipients of information you repeated.
A repeatable checklist
- Copy the exact false, unsupported, or misleading statement.
- Save the surrounding context, date, scope, and any supplied citations.
- Open the citations and compare them with the actual claim.
- Verify the point against an original or authoritative source.
- Remove unnecessary personal or confidential information.
- Submit the platform’s current thumbs-down feedback or applicable content report.
- Describe the error, correction, evidence, and potential impact concisely.
- Correct every post, message, document, or decision record where you repeated the answer.
- Use a qualified professional or authority for high-stakes matters.
- Keep the corrected source and date for your records.
