A breach notice does not prove that someone has stolen your identity. First find out what information was exposed and secure the affected accounts. If you find unauthorized accounts, charges, or other misuse, report identity theft at IdentityTheft.gov, follow its recovery plan, and keep a dated record of every document and contact.
Start by identifying what was exposed
Read the notice carefully and record the organization, the incident and notification dates (if given), the affected account or service, the types of information involved, and any response deadline or complimentary service offered. A password exposure calls for different steps from exposure of a Social Security number or bank information. The FTC’s breach guidance helps consumers choose actions based on the information involved.

A notice is a reason to act, but it is not evidence by itself that identity theft occurred. Use the notice to identify the relevant risks and protect the affected information.

If a password was exposed
Change the password on the affected account promptly, along with any other accounts where you reused it. Turn on multifactor authentication wherever it is available. The FTC also suggests that a password manager can help generate and manage unique passwords.

If financial or account access may be compromised
Contact the bank, service, or other company involved using contact details you verify independently from its official website or a statement—not simply a link or phone number in an unexpected message. Note the date, department, representative if available, what you asked for, and the outcome.
Check whether misuse has occurred
If exposed information could be used to open credit accounts or otherwise affect your identity, check your credit reports for unfamiliar accounts or activity. IdentityTheft.gov directs consumers to obtain reports from Equifax, Experian, and TransUnion and note items they do not recognize. Review relevant account statements as well, especially if payment or banking information was exposed.

You can place a free initial fraud alert through one of the three nationwide credit bureaus; that bureau must notify the other two. IdentityTheft.gov says this initial alert lasts one year. A fraud alert asks creditors to take steps to verify identity before opening new credit in your name; it is a protective measure, not a substitute for disputing activity you do not recognize.

Report identity theft when you find actual misuse
If someone has used your information—for example, to open an account or make a purchase—report it through IdentityTheft.gov. The FTC describes the site as “the federal government’s one-stop resource for identity theft victims.” Reporting there provides an Identity Theft Report and a recovery plan tailored to your situation.

Follow the plan’s steps for contacting businesses where fraud occurred, placing a fraud alert, getting credit reports, and reporting the theft. Keep copies of disputes and correspondence. You may also choose to file a local police report. IdentityTheft.gov advises bringing photo identification, proof of address, your FTC Identity Theft Report, and other evidence; ask for a copy of the police report for your records.
Preserve evidence and keep a contact log
Save the original breach notice and retain copies of later messages, relevant statements, credit-report entries, dispute letters, FTC report confirmations, any police report, and company responses. Keep a dated log of calls and other contacts, including whom you reached, what you requested, and any promised follow-up. IdentityTheft.gov specifically advises victims to record who they contacted and when, and to keep copies of letters.

Store sensitive documents securely. Send only the information the relevant company or agency requests, and use its secure submission method. The FTC’s separate Health Breach Notification Rule form is for covered organizations reporting certain health-information breaches; it is not the consumer route for reporting identity theft.

If fraudulent activity took place at a company, you can send that company a written request for records related to the identity theft. The FTC’s guidance says to include proof of identity, a completed FTC Identity Theft Report, and a police report. Under the FCRA provision described by the FTC, the company has 30 days to provide the requested records free of charge after receiving the specified materials. Records may include transaction records or applications. Ask for records relevant to the incident, and retain a copy of your request and proof of delivery.

Keep the deadlines in context
The one-year period for an initial fraud alert and the FTC’s 30-day period for a company to provide qualifying identity-theft records apply to those specific procedures. They do not measure how likely a breach is to cause identity theft or how quickly misuse will appear.
For covered health-information breaches involving 500 or more people, the FTC says an organization must report as soon as possible and no later than 60 days after discovering the breach. For fewer than 500 people, the FTC page gives an annual reporting deadline. These are organization obligations under a specific rule, not deadlines for consumers reporting identity theft.
This is a general U.S. consumer workflow based on federal guidance. State or non-U.S. rules and situations such as tax, medical, child, or employment identity theft may require additional steps.
