How to Reduce the Risk of Ketamine Misuse and Dependence

Reducing or stopping ketamine use is the safest way to lower the risk of dependence and lasting injury. Frequent, high-dose use can lead to tolerance, escalating use, withdrawal when heavy use stops, and serious bladder, urinary-tract, kidney, abdominal, liver, or bile-duct problems. If someone continues to use, steps such as spacing sessions, avoiding depressants, and not using alone may reduce some risks—but no recreational dose or schedule is guaranteed safe.

Can ketamine cause dependence?

Yes. Repeated high-dose use can lead to tolerance, meaning a person may feel they need more to get an effect, and can develop dependence and withdrawal when heavy use stops. Dependence is not inevitable, and prescribed treatment is not equivalent to high-dose non-medical use: the Advisory Council on the Misuse of Drugs (ACMD) says chronic therapeutic ketamine at recommended doses is unlikely to cause significant addiction, while recognizing addiction potential among people regularly taking high doses.

The ACMD’s 2026 review estimates that 5–8% of responses to its 2025 call for evidence described more problematic use involving high doses taken daily. That is a finding about call-for-evidence responses, not a population prevalence estimate. A separate 2012 survey found that 218 of 1,285 surveyed ketamine users (17%) met DSM-IV dependence criteria; dependence was strongly associated with amount used per session and session frequency. That survey does not represent the general population. In England and Wales, 299,000 people aged 16–59 (0.9%) reported ketamine use in the previous year in 2022–23, according to figures cited in the ACMD review. These figures describe different populations and should not be compared as if they measured the same thing.

What is the safest way to lower the risk?

Reducing or stopping use is safer than continuing. If stopping feels difficult, or withdrawal or cravings are a concern, contact a healthcare professional or local specialist substance-use service for an individual plan rather than trying to manage it alone.

If use continues, reduce immediate risks

The ACMD’s risk-reduction advice is not a declaration that recreational use is safe. It recommends the following measures for people who continue to use:

  • Do not use alone. Avoid settings where intoxication could lead to injury, drowning, violence, or sexual exploitation.
  • Avoid combining ketamine with alcohol, opioids, or benzodiazepines. These depressants can increase adverse effects and overdose risk.
  • Avoid driving and other activities where impaired judgment or coordination could cause an accident.
  • Use smaller test amounts and pre-measured amounts rather than redosing unpredictably. Purity and adulteration can be uncertain, so a familiar amount may not have a predictable effect.
  • Space doses and sessions. The ACMD recommends longer gaps between episodes and says to aim for at least four weeks between sessions to help limit rapid tolerance and dependence. This is guidance, not a proven safe-use threshold.
  • Reduce frequency and amount, and seek support if use is escalating or hard to control.

If injecting or sharing equipment is a concern

Do not share injecting equipment or snorting paraphernalia such as straws; sharing can transmit infections. People who inject should contact a needle and syringe programme for sterile equipment and advice, use new sterile equipment each time, and discuss blood-borne virus testing with a healthcare provider. These steps address infection risk; they do not make ketamine use safe.

What are the signs of ketamine-related injury?

Frequent, high-dose or longer-term use is associated with serious harms affecting the bladder and upper urinary tract, kidneys, abdomen, liver, and bile ducts. Some effects may persist or resolve only partly after stopping, so urinary symptoms or persistent abdominal pain should not be ignored.

Bladder and urinary-tract symptoms

Ketamine-induced uropathy can involve bladder and upper urinary-tract injury. Seek prompt medical assessment for urinary symptoms, rather than waiting to see whether they settle. The ACMD reports that stopping ketamine is the most important component of treatment for ketamine-induced uropathy; medical or surgical care may also help, and some people need coordinated care for both urinary injury and ketamine use disorder.

Identify Diagnostics 12 Panel At Home Drug Test Kit, Urine Cup - 5 Pack
  • 12-panel urine drug test kit: AMP, BAR, BUP, BZO, COC, MDMA, MET, MOP, MTD, OXY, PCP, and THC. These abbreviations identify all drugs covered, including marijuana, ecstasy, cocaine and morphine. Find the full list of drug names featured in the image gallery for this multi-drug screening cup.
  • At-home, workplace, and pre-employment drug testing: This all-in-one format screens a sample with built-in strips, with no separate testing device to prepare. Clear markings show where to look, making personal checks and employee programs easier to set up.
  • Rapid 5-minute results: Follow the included instructions for starting and interpreting the test, then review each panel strip for its listed drug class. Simple guidance makes it easier for a first-time user to know when to read and what the lines mean with confidence.
  • Temperature strip and security seal sticker: The built-in temperature strip helps ensure the urine sample has been freshly collected. Apply the security seal sticker over the top and down both sides of the cup for specimen validity after collection.
  • 5-pack convenience: Includes 5 individually wrapped multi-drug cup tests for occasional personal screening. A compact amount takes little storage space; sealed pouches protect contents until needed. At least 11 months of shelf life is available upon receipt, leaving time to plan a future check.

Abdominal pain and liver or bile-duct injury

Chronic abdominal pain, sometimes accompanied by vomiting, is often called “K-cramps.” The ACMD review reports that K-cramps affect more than a quarter of regular users; it also cites estimates that cholestatic liver injury and chronic bile-duct dilatation occur in about 10% of regular users. These are review-reported estimates for regular users, not predictions for any individual. Persistent abdominal pain, especially with vomiting, warrants prompt medical assessment.

What treatment and support are available?

Care for ketamine use disorder centers on psychosocial support for behavior change, including support toward abstinence or reduced use, and help with co-occurring psychological, social, or physical problems. The ACMD says there are no specific drug treatments for ketamine dependence. Medication approaches described in the evidence base have not been evaluated in high-quality studies, and reported benefits are anecdotal; do not self-medicate or treat ketamine-related pain with opioids or more ketamine.

Evidence on managing withdrawal and dependence, and on ketamine-specific harm-reduction interventions, remains limited. A clinical or specialist assessment can help tailor a plan and coordinate care. When choosing a service, consider whether it can address substance use, urinary or urological problems, and mental health, and whether it can coordinate those areas of care. The right route depends on where you live; community-based support and outreach can improve access.

The ACMD’s Recommendation 12 calls for “Integrated harm reduction approaches” combining education, professional training, access to drug checking, and safer-use practices. Drug checking can inform decisions where available, but it cannot guarantee that a substance is safe.

  • CREDIBLE: This test is CLIA Waived for home or over the counter (OTC) use, utilizes the same technology as lab screening tests, and its results are accepted in most professional and clinical settings.
  • COMPREHENSIVE: This lab-grade test can detect 14 drugs simultaneously: Buprenorphine (BUP), Tetrahydrocannabinol (THC), Opiates (OPI - 2000 ng/mL cut-off level), Amphetamine (AMP), Barbiturates (BAR), Benzodiazepines (BZO), Cocaine (COC), Methamphetamine (MET), Methylenedioxymethamphetamine (MDMA), Methadone (MTD), Oxycodone (OXY), Phencyclidine (PCP), Propoxyphene (PPX), and Tricyclic Antidepressants (TCA).
  • ACCURATE: More than 99% accurate, it uses lab-recommended cutoff levels for each drug and includes a temperature strip to verify the temperature of the fresh sample for Specimen Validity Testing.
  • EASY TO USE: To use, simply remove the cap, collect urine in the test cup, wait a FULL 5 minutes, and then read the result; disposable plastic gloves are included, a negative result shows two lines, a positive result shows only the control line, a faint test line still means negative, and results should not be read after 5 minutes.

Does prescribed ketamine carry the same risk?

No. Ketamine has legitimate human and veterinary uses, and esketamine is licensed in the UK and elsewhere for treatment-resistant depression. The ACMD says chronic therapeutic ketamine at recommended doses is unlikely to produce significant addiction. This does not remove the need to follow the prescribing clinician’s instructions or raise concerns about side effects and use with that clinician. Prescribed care should not be conflated with frequent, high-dose non-medical use.
