Rapidly worsening hallucinations, sudden confusion, severe aggression, or thoughts of self-harm or harming someone else also require urgent help. While assistance is coming, remove immediate hazards, monitor breathing and responsiveness, avoid inducing vomiting or giving food and drink when poisoning is suspected, and follow the dispatcher’s instructions.
There is no universal “safe-use” formula. Psychedelic and dissociative effects vary with the substance, amount and potency, a person’s biology and health, mood, expectations, environment, and use of alcohol or other drugs. The precautions below can reduce some avoidable dangers, but they cannot guarantee safety or identify every contaminated product.

When to call emergency services
Use the emergency number for your location rather than waiting to see whether the experience settles. In the United States, the official guidance cited here uses 911; in the United Kingdom, NHS guidance refers to 999 or an emergency department. Elsewhere, use the local equivalent.

Breathing, consciousness and seizure emergencies
- Breathing is difficult, stopped, unusually slow, or otherwise abnormal.
- The person is unconscious, collapses, or cannot be awakened.
- The person has a seizure.
- The person is unresponsive or cannot follow simple requests.
These signs can indicate poisoning or another life-threatening problem even if the person says the experience is “just a trip.” Do not leave the person alone to sleep it off. If you are unsure whether breathing is normal, describe exactly what you observe to the dispatcher rather than trying to make your own diagnosis.

Dangerous temperature, behavior or confusion
- Very high body temperature.
- Severe agitation, violent behavior, or aggression that creates immediate danger.
- Confusion, delirium, or inability to respond normally.
- Rapidly worsening hallucinations or sudden, severe changes in behavior.
- Thoughts or actions suggesting the person may harm themselves or someone else.
SAMHSA identifies breathing trouble, loss of consciousness, seizures, severe agitation or violence, very high temperature, delirium or inability to respond, and suspected unknown or counterfeit substances as reasons to obtain emergency care. NHS mental-health guidance separately treats quickly worsening hallucinations, sudden confusion, severe agitation or aggression, and thoughts of harm as emergencies. A person does not need to display every sign before you call.
Do not wait for a particular number on a thermometer, a second seizure, or complete loss of consciousness. A fast deterioration, inability to keep the person safe, or a combination of concerning symptoms is enough to seek emergency help.
Unknown, counterfeit or mixed exposure
Tell the dispatcher if the substance is unknown, came from an unreliable source, may have been counterfeit, or was taken with alcohol, prescription medicine or another recreational drug. Products sold as hallucinogens may contain unexpected substances. NIH notes that fentanyl and related contaminants can be difficult to detect without chemical testing; appearance, smell, packaging or a consumer test cannot prove that a product is uncontaminated.
Do not postpone a call while trying to identify a tablet, blotter, powder or plant. The suspected name, approximate time, amount if known, and any co-use are useful, but treatment decisions should not depend on certainty about the contents.
What to do while help is coming
Emergency first aid depends on the person’s breathing and responsiveness. Follow the emergency dispatcher’s directions over any general checklist.
1. Make the immediate area safer
- Move away from traffic, heights, open water, fire, weapons, glass and machinery if you can do so without putting yourself at risk.
- Reduce crowding, loud noise and bright stimulation. Ask one calm person to speak in short, reassuring sentences.
- Do not physically restrain an agitated person unless emergency professionals instruct you and there is an immediate, unavoidable danger. Keep your own exit route clear.
Impaired perception and judgment can lead to falls, collisions and other injuries. A quieter, safer space helps reduce hazards while you wait; it does not treat poisoning or replace medical assessment. If the scene itself is unsafe, tell the dispatcher and wait for trained responders rather than becoming another casualty.
2. Check responsiveness and breathing
- Try to wake the person and ask whether they can answer simple questions.
- Observe whether the chest is moving and whether breathing appears normal.
- Tell the dispatcher exactly what you see and report any change promptly.
NHS poisoning guidance says to start CPR if someone is unresponsive and not breathing, use the recovery position if the person is unconscious but breathing, and continue checking breathing. Perform CPR only if you know how or the dispatcher coaches you. Do not assume that snoring, gasping or irregular noises are normal breathing; describe them to the dispatcher.

3. Do not give substances by mouth
Do not make the person vomit. NHS poisoning advice also says not to give food or drink when poisoning is suspected. Do not give alcohol, another drug, a “counteracting” substance or an unprescribed sedative. Wait for professional instructions, particularly if the person is drowsy, confused, vomiting or unable to swallow normally.
4. Gather information for clinicians
- Keep the package, label, remaining material or prescription container if it can be collected safely.
- Note what was reportedly taken, approximately when, and whether alcohol or other drugs were involved.
- Record symptoms, changes in consciousness, seizures, vomiting, injuries and the person’s known medical conditions or medications.
- Give responders the truth even if you are worried about judgment or legal consequences. Accurate information helps them choose safer treatment.
Do not delay the emergency call while searching for a package or trying to identify a pill. If collecting it would expose you to danger, leave it where it is and tell responders what you know.
Choosing the right level of help
Use the fastest appropriate pathway. When in doubt about breathing, consciousness, seizure, temperature, severe behavior or immediate safety, choose emergency services.
| Situation | Action | Information to provide |
|---|---|---|
| Life-threatening or rapidly worsening signs | Call the local emergency number now; follow dispatcher instructions. | Breathing and responsiveness, seizure or temperature concerns, behavior, substance and timing, co-use, and location. |
| Concerning exposure without an immediate emergency sign | Contact a poison center or healthcare professional promptly for case-specific advice. | Exact or suspected product, amount if known, time, age and weight where relevant, symptoms, medicines and other substances. |
| Persistent or distressing symptoms after the acute effects | Arrange clinical assessment; use emergency care if severe warning signs develop. | How long symptoms have lasted, sleep and functioning changes, perceptual symptoms, mood, safety concerns and previous mental-health history. |
Poison-center options
Service numbers depend on country. In the United States, Poison Control lists 1-800-222-1222 and the webPOISONCONTROL service. In the United Kingdom, NHS poisoning guidance directs uncertain cases to NHS 111. These services can help determine whether home observation, urgent assessment or emergency care is appropriate, but they are not a substitute for calling emergency services when red flags are present.
Have the person nearby when you call if possible. Poison specialists may ask about age, weight, health conditions, medicines, the suspected product, timing and symptoms. If symptoms worsen while you are waiting for advice, switch to emergency services rather than continuing a non-emergency consultation.
Why reactions are hard to predict
NIH explains that psychedelic and dissociative outcomes depend on multiple interacting factors:
- Substance, amount and potency: Labels may be inaccurate, and products sold under one name can differ in strength or contents.
- Individual biology and health: Medical conditions, prescribed medicines, sleep deprivation and sensitivity to a substance can change the response.
- Mood and expectations: Fear, panic or an unsafe emotional state can intensify distress and risky behavior.
- Environment: Traffic, heights, water, crowds, heat and unfamiliar surroundings increase injury opportunities when perception and judgment are impaired.
- Alcohol or other drugs: SAMHSA identifies co-use as a factor that can alter effects and increase danger.
Because these variables interact, a previous uneventful experience does not predict the next one. A familiar appearance or dose estimate also cannot establish what a product contains. A person may also become unable to describe what happened accurately, so a bystander’s observations and any packaging can be important.

Precautions that reduce avoidable hazards
The official guidance supports broad precautions rather than a recreational-use protocol. They lower some risks but do not make psychedelic use safe.
Avoid unknown or counterfeit products
Do not assume that a product sold as a particular hallucinogen contains only that substance. If the contents are uncertain, treat an unusual or severe reaction as a possible poisoning and tell clinicians that the product may be counterfeit or contaminated. Visual inspection and a consumer test cannot rule out fentanyl or every other unexpected compound.
Do not combine hallucinogens with alcohol or other drugs
SAMHSA advises avoiding combinations with alcohol and other drugs. Interactions can make effects less predictable and can complicate recognition of which substance is causing breathing problems, agitation, loss of consciousness or other symptoms. Do not attempt to “balance” an experience by adding another substance.
Do not drive or operate machinery
Hallucinogens can impair thought, perception and judgment. Do not drive, cycle in traffic, swim, climb, use power tools or operate machinery after use. Arrange a sober, trusted person and a hazard-free environment rather than relying on the person to judge their own impairment.
Do not treat precautions as guarantees
No setting, companion or test can eliminate poisoning, panic, injury or a psychiatric emergency. The purpose of these precautions is to remove avoidable hazards and speed recognition of trouble, not to certify that use is safe. Official guidance does not provide a dose, timing plan or home “antidote” that guarantees a safe experience.

After the acute experience: symptoms that need follow-up
Persistent perceptual changes
NIH describes flashbacks and hallucinogen-persisting perception disorder, in which perceptual symptoms can recur or continue after the acute effects have ended. Seek medical assessment for recurring visual disturbances, derealization, frightening perceptual changes or symptoms that interfere with work, sleep, relationships or safety. Do not self-diagnose; other medical or psychiatric conditions can cause similar experiences.
Mental-health changes
NIH notes that psychedelics may trigger schizophrenia-like illness in people with predisposing factors or worsen symptoms in people who have schizophrenia. New paranoia, severe insomnia, disorganized behavior, inability to care for basic needs, or thoughts of self-harm warrant prompt professional help. If there is immediate danger, inability to respond, severe agitation or rapidly worsening hallucinations, use emergency services rather than waiting for a routine appointment.

Pregnancy, medical conditions and interactions
Effects during pregnancy are not well understood. Anyone who is pregnant, planning pregnancy, has a significant medical or psychiatric condition, or takes prescription medicines should ask a clinician about potential risks and interactions. A clinician can review the person’s history and symptoms without assuming that every reaction has the same cause.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.A practical bystander checklist
Save this sequence for a situation in which someone may have taken a psychedelic or another hallucinogen:
- Look for danger: breathing trouble, unresponsiveness, seizure, very high temperature, delirium, severe agitation, rapidly worsening hallucinations or risk of harm.
- Call the local emergency number immediately if any danger sign is present. State the location first if the dispatcher requests it.
- Protect the scene: move hazards away, reduce stimulation and keep traffic, water, heights and weapons out of reach.
- Monitor: continue checking responsiveness and breathing. Use the recovery position if unconscious but breathing; begin CPR if unresponsive and not breathing, following dispatcher coaching.
- Do not give food or drink and do not induce vomiting when poisoning is suspected unless a dispatcher or clinician specifically instructs otherwise.
- Preserve information: packaging, suspected substance, time, co-use, medicines, symptoms and changes in condition.
- Stay with the person when safe, and hand all information to responders.
If there is no immediate emergency sign but the exposure is concerning, contact a poison center or healthcare professional promptly. Keep observing for deterioration while waiting for advice.
What the available evidence can—and cannot—tell you
A 2024 study, Clinical Effects of Psychedelic Substances Reported to United States Poison Centers: 2012 to 2022, found that more than half of reported psychedelic exposures to US poison centers had symptoms requiring treatment, severe residual or prolonged symptoms, or death. This is a statistic about reports received by poison centers, not the percentage of all people who use psychedelics and not an individual’s predicted outcome. It does, however, show why severe or unusual symptoms should not be dismissed.
Official sources do not establish a universal psychedelic-specific triage score, a dose that guarantees safety, or a home treatment that replaces professional assessment. The safest decision rule is symptom-based: emergency signs go to emergency services; concerning but non-emergency exposures go promptly to poison or clinical advice; persistent or distressing after-effects need follow-up care.
Frequently Asked Questions
What are the signs of an overdose or dangerous reaction to a psychedelic?
Trouble breathing, unconsciousness or inability to wake, a seizure, very high temperature, severe agitation or violence, delirium, inability to respond, rapidly worsening hallucinations, sudden confusion, or thoughts of harming self or others are emergency signs. An unknown or counterfeit product also warrants urgent caution.

Best Value
What should I do while waiting for medical help?
Make the area safer, reduce stimulation, monitor responsiveness and breathing, and follow dispatcher instructions. Use the recovery position if the person is unconscious but breathing; begin CPR only if instructed or trained. Do not induce vomiting or give food or drink when poisoning is suspected.
Should I give another drug to counteract the experience?
No. Do not give alcohol, another recreational drug, an unprescribed sedative or a supposed counteracting substance. Combinations make effects less predictable and can worsen breathing or consciousness problems.
Can I identify contamination by looking at the product or using a consumer test?
No method described here can prove that a product is uncontaminated. Products sold as hallucinogens may contain unexpected substances, and NIH notes that fentanyl-related contamination can be difficult to detect without chemical testing.

For a concerning exposure without immediate emergency signs, contact a poison center or healthcare professional promptly for case-specific advice. In the US, Poison Control is 1-800-222-1222 or webPOISONCONTROL; in the UK, NHS guidance directs uncertain cases to NHS 111. Worsening symptoms require emergency services.
Can symptoms return after the trip is over?
Yes. NIH describes flashbacks and hallucinogen-persisting perception disorder. Recurring or distressing perceptual changes, mood changes or problems with sleep, work, relationships or safety deserve clinical assessment.
Are psychedelics understood to be safe during pregnancy or with mental-health conditions?
Effects during pregnancy are not well understood. NIH also notes possible schizophrenia-like illness in people with predisposing factors and worsening symptoms in people with schizophrenia. Discuss pregnancy, medical conditions and prescription medicines with a clinician.
The Bottom Line
Use a symptom-based decision rule: call emergency services for breathing trouble, unresponsiveness, seizure, very high temperature, severe agitation or confusion, rapidly worsening hallucinations, or immediate risk of harm. For a concerning but non-emergency exposure, contact a poison center or clinician promptly. Persistent or distressing after-effects need follow-up care. Avoid unknown products, mixing substances and driving, but remember that no precaution guarantees safety.
