Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsPull-request reviews stop being a bottleneck when a team manages the entire path from request to merge—not just the time a reviewer spends reading code. Set a response-time service level, make ownership and queue status visible, keep changes coherent, automate repeatable checks, and measure where work waits or cycles back. The result is faster feedback without lowering the standard for correctness, security, or maintainability.
- Give every review request an explicit owner, backup path, and expected response.
- Separate queue time from active review time and rework time.
- Reduce the context a reviewer must hold by splitting unrelated changes.
- Move mechanical checks into automation before human approval.
- Improve the largest measured constraint rather than chasing a single speed number.
Find the actual bottleneck before changing the process
A pull request usually passes through five states: it is opened, waits for a reviewer, receives a first response, goes through one or more review-and-revision cycles, and is merged. Teams often call all of that “review time,” which hides the problem. A request that waits two days for its first look needs a different intervention from a request that receives immediate attention but requires four rounds of redesign.

| Stage | What to observe | Likely constraint |
|---|---|---|
| Request to first reviewer action | No comment, approval, or explicit handoff | Reviewer capacity, routing, or unclear ownership |
| First action to substantive decision | Time reviewers actively spend understanding and discussing the change | Large scope, missing context, or high technical risk |
| Requested changes to next revision | Author waits, revises, and returns to the queue | Unclear requirements, serial feedback, or weak local checks |
| Final approval to merge | Approved change remains unmerged | Release policy, required checks, or merge ownership |
Record timestamps for each transition for a representative period before redesigning the workflow. Include the repository, service, change type, author, reviewer area, and risk level so a team can see whether a problem is local to one ownership group or common everywhere.
Do not infer that a shorter review is automatically better. Google Engineering Practices describes the purpose of review as ensuring that the overall code health of the codebase improves over time. A process that produces quick approvals but increases defects, reversions, incidents, or maintenance cost has optimized the wrong stage.

Set a response-time service level that removes silent waiting
Google Engineering Practices states: “One business day is the maximum time it should take to respond to a code review request (i.e., first thing the next morning).” Treat that as a team operating rule for ordinary work: every request receives a substantive response within one business day.
A substantive response does not always mean a complete approval. It can be a review, an explicit place in the queue with an estimated time, or a handoff to another qualified reviewer. The important property is that the author is not guessing whether anyone has seen the request.
Define what counts as a response
- Review started: the reviewer has examined the change and left actionable comments, an approval, or a request for clarification.
- Queue acknowledgement: the reviewer records when they expect to start and identifies any dependency that could change that estimate.
- Handoff: the reviewer names another qualified person and transfers ownership instead of leaving the request unassigned.
Use visible labels or status text such as reviewing, waiting-for-author, waiting-for-checks, and ready-to-merge. A short comment or status update is enough; a private message does not make the queue observable to the rest of the team.

Fast response does not require interrupting a developer in the middle of a fragile implementation step. Google’s guidance recommends waiting for a natural break point and then responding promptly. Teams can reserve a review-duty block, use a rotating reviewer, or agree that a reviewer finishes a bounded task before switching context. The service level applies to the response, not to an arbitrary interruption at every moment of the day.
Handle missed responses explicitly
When the service level is missed, the author should not repeatedly broadcast a vague request. Use a defined escalation path: check the assigned owner, contact the backup, re-route through the repository’s ownership rules, and record the reason for the delay. Repeated misses in the same area are evidence of a capacity or routing problem, not an author communication failure.
Make the review queue visible and routable
Ownership rules should answer three questions before a pull request is opened: who is qualified to review this area, who is the backup when that person is unavailable, and who has authority to approve or request changes?
Use file and service ownership
CODEOWNERS or an equivalent ownership mechanism can map directories, services, and configuration areas to responsible reviewers. GitLab documents CODEOWNERS-based routing and lists approvers for files that require domain expertise. Keep ownership entries narrow enough to identify real expertise, and review them when teams or services change.

Assign more than one qualified reviewer for high-risk areas. A single specialist may be the right escalation point for an unusual migration, but should not become the permanent queue for every ordinary change in that subsystem.
Rotate review duty
Create a rotation that gives one person or a small group explicit responsibility for unassigned reviews during a defined period. Rotation prevents the same specialist from absorbing every request and makes review work visible in planning. The duty holder can perform the review, route it to the right expert, or state when the expert will respond.
Make approval authority unambiguous
Repository controls should distinguish people who may approve a change from people who may request changes or override a requirement. GitHub documents organization- and repository-level controls over who can approve or request changes. Configure these rules around risk and ownership rather than allowing an informal “whoever notices it first” system.
For regulated, security-sensitive, or production-critical code, require the appropriate domain approval while allowing a general reviewer to provide an earlier design or readability pass. This separates broad feedback from the approval that carries operational responsibility.

Reduce change size without creating artificial fragmentation
Reviewers understand a coherent change faster than a mixed bundle of feature work, formatting edits, dependency upgrades, and unrelated refactors. Ask authors to separate those concerns where the repository and release process allow it.
Split by intent
- Put a mechanical formatting or rename-only change in its own pull request.
- Separate a dependency upgrade from the feature that happens to use the new version.
- Move a broad refactor before or after behavior changes when both can be validated independently.
- Keep tests, documentation, and implementation for one behavior together so the reviewer can evaluate the complete intent.
A small change is not necessarily a low-risk change. A one-line permission change can be more consequential than a large isolated test update. Record the risk and required expertise separately from the number of changed lines.
Move clarification earlier for cross-cutting work
Some work cannot be divided cleanly: schema migrations, API contract changes, authentication updates, and coordinated infrastructure changes may touch many repositories or services. For these, use an early design discussion or pair programming before opening the final pull request. The 2023 Accelerate State of DevOps report discusses small batches, code-review bottlenecks, and pair programming as ways to reduce review time and feedback cycles.

Do not split merely to satisfy a quota
Artificially chained pull requests can increase queue time, make testing harder, and force reviewers to reconstruct the full change across several windows. Split when each part has a clear purpose, can be understood in context, and can be validated without hiding important dependencies. The metrics guidance from DORA also cautions that not all work splits cleanly.
Automate repeatable checks before requesting human attention
Human reviewers should spend their limited attention on design, correctness, maintainability, security context, and product risk. Tests, linters, formatters, dependency checks, and code scanning are better handled automatically and reported before approval is requested.
Build a predictable pre-review pipeline
- Run fast formatting and lint checks first so trivial failures return quickly.
- Run unit and targeted integration tests for the affected components.
- Run dependency review and security or code-scanning checks appropriate to the repository.
- Publish the results in the pull-request interface, with a direct link to the failing job and the relevant log section.
- Allow a human reviewer to begin when the change is safe to inspect, but block merge when a required check has failed or has not completed.
GitHub documents dependency and code-scanning information surfaced in pull requests. GitLab recommends checklists and thorough review practices. In either system, make failures understandable: identify whether the author should fix code, rerun a flaky job, or ask an owner to update the check.
Keep flaky automation from becoming a second bottleneck
Track recurring false failures and assign an owner to repair them. A required check that fails unpredictably teaches authors to rerun jobs until they pass and teaches reviewers to ignore warnings. Until a flaky check is fixed, mark its status clearly and define who may make a documented exception; do not silently bypass it.
Use a concise author checklist
- Describe the user or operational outcome.
- Identify the boundaries of the change and anything deliberately excluded.
- State how the change was tested and list known limitations.
- Call out migrations, feature flags, rollout steps, security implications, and compatibility concerns.
- Identify the reviewers or ownership areas that need specialist attention.
The checklist should prevent predictable clarification rounds, not force authors to write a long essay for a trivial change.

Standardize how reviewers work together
A review protocol reduces back-and-forth without turning judgment into a script. Agree on what reviewers must examine, how they label comments, and when they should move a discussion out of the pull request.
Use risk-based review depth
| Change profile | Minimum review focus | Useful routing |
|---|---|---|
| Low-risk, localized behavior or test update | Intent, correctness, tests, readability, and unintended side effects | One qualified reviewer; automated checks required |
| Moderate-risk service or data change | Interface compatibility, failure handling, observability, rollout, and tests | Service owner plus a reviewer familiar with the dependency |
| High-risk security, migration, or production-control change | Threat model, permissions, rollback, data integrity, operational ownership, and evidence of testing | Named domain approver and an available backup; early design review preferred |
These tiers are a team policy, not a substitute for expert judgment. A reviewer can request deeper examination when the apparent size of a change understates its impact.

Make comments actionable
Distinguish a required correction from a suggestion, question, or preference. Explain the failure mode or maintenance concern, point to the relevant code, and state what evidence would resolve the issue. If a disagreement is architectural or affects several files, schedule a short discussion and record the decision in the pull request so later reviewers do not reopen the same question.
Review in parallel when the risk warrants it
Parallel specialist reviews can shorten a cross-cutting change, but only when each reviewer has a distinct responsibility. For example, one person can inspect data migration safety while another checks an externally visible API contract. Serially asking five people to repeat the same general review creates queue time without adding equivalent confidence.
Track metrics that separate waiting, work, and rework
GitLab identifies four useful review metrics. Add review rounds and reviewer load so the team can explain why a number changed rather than merely celebrate a faster average.
| Metric | Definition | Question it answers |
|---|---|---|
| Reviewer First Engagement Time | Time from review request to the first reviewer action | How long does a request wait before anyone responds? |
| Review Time to Merge | Elapsed review period from request through merge | How long does the complete review path take? |
| Mean Time to Merge | Average elapsed time from opening to merge over a defined period | Is overall throughput changing? |
| Merge-request rate | Number of merge requests completed in a defined period | How much review flow is the team processing? |
| Review rounds | Number of substantive request-and-revision cycles | Are changes returning repeatedly because of unclear intent or missing checks? |
| Reviewer load by area | Requests and active review time grouped by reviewer and ownership area | Is one person or subsystem carrying disproportionate demand? |
Use distributions, not only averages. The oldest open reviews and the upper end of waiting time expose queue failures that a mean can hide. Segment the data by repository, change type, risk, author experience, and reviewer area. A high average caused by a few migrations calls for a different response from a broad increase across ordinary changes.
Compare review metrics with delivery lead time and deployment outcomes. DORA recommends improving the most significant constraint rather than optimizing one local measure. A higher merge-request rate is not an improvement if it comes from merging smaller fragments that increase downstream failures or operational work.
Match the pattern to the intervention
| Observed pattern | Most plausible cause | First intervention to try |
|---|---|---|
| First engagement is slow, but active reviews are short | Queue capacity or unclear assignment | Add a backup, rotate review duty, and enforce ownership routing |
| First engagement is prompt, but merge time is long | Large scope, difficult design, or missing context | Split the change, add an early design review, and improve the author checklist |
| Many revisions address the same type of mechanical issue | Missing automation or inconsistent standards | Add a formatter, linter, test, or checklist item before human review |
| One reviewer or team owns most waiting time | Specialist dependency or outdated ownership map | Train a backup, narrow approval requirements, or redistribute ownership |
| Reviews are fast but defects or reversions rise | Quality has been traded for local speed | Restore risk-based review depth and inspect downstream quality signals |
| Several pull requests remain approved but unmerged | Required checks, release timing, or merge authority | Make the final merge owner and blocking condition visible |
Run a short, recurring improvement loop
A review process should be tuned from real queue evidence rather than redesigned in a large one-time initiative.
- Inspect the oldest open reviews. Note the current owner, status, risk, dependency, and last meaningful action.
- Group the delay. Decide whether the time is waiting for a reviewer, active technical discussion, author revision, automation, or merge authority.
- Choose one intervention. Examples include adding a backup, changing an ownership rule, splitting a recurring change type, repairing a flaky check, or reserving review-duty time.
- Assign an owner and a review date. The person who changes the workflow should not be the only person evaluating it.
- Check speed and quality together. Compare first engagement, merge time, review rounds, load distribution, defects, reverts, and delivery outcomes.
- Keep, adjust, or revert the change. Write the decision in the team’s normal engineering documentation so the same experiment is not repeated without context.
Google Engineering Practices warns against sacrificing review standards for imagined velocity gains. If an intervention improves a local queue number while making reviewers less willing to approve risky work, it is a regression even when the dashboard looks faster.

Implementation sequence for a team starting from scratch
First, establish visibility
For the next review period, require an owner, status, risk note, and requested reviewers on every pull request. Capture the timestamps needed for first engagement, revision cycles, and merge. Do not set individual reviewer quotas; the purpose is to expose system constraints, not rank people.
Next, fix routing and response behavior
Publish the one-business-day response rule, define the acknowledgement and handoff formats, add CODEOWNERS or an equivalent map, and name backups for high-risk areas. Configure repository approval controls so authority matches the ownership model.
Then, reduce avoidable work
Review the most common repeated comments and turn mechanical ones into automated checks or checklist items. Ask authors to separate unrelated edits and to provide test, rollout, and risk context before a reviewer begins.

After the team has enough observations to identify a dominant constraint, make one targeted change and compare the same metrics and quality signals afterward. Keep exceptions visible: emergency fixes, large migrations, and externally imposed release windows should be tagged rather than mixed into ordinary-flow comparisons.
Use a clear runbook for stalled reviews
- Open the pull request and read its current status, assigned owner, required checks, and latest activity.
- If no qualified reviewer has acknowledged it within the service level, contact the owner once through the visible workflow.
- If the owner is unavailable, invoke the documented backup or rotation; do not wait indefinitely for a private reply.
- If the change is too broad to review safely, agree with the author on a coherent split or schedule an early design discussion.
- If reviewers disagree on a design point, identify the decision owner, hold a focused discussion, and record the outcome.
- If automation is blocking progress, classify the failure as a real defect, a test-environment problem, or a flaky check and route it to the responsible owner.
- For an urgent production fix, use the emergency path and document the follow-up review needed after stabilization.
What a healthy review system looks like
A healthy system makes waiting visible, gives every request a credible next action, and reserves human attention for decisions automation cannot make. Authors understand what information to provide; reviewers know when to respond and when to hand off; ownership rules prevent a single specialist from becoming a permanent queue; and metrics show whether a delay comes from capacity, complexity, rework, or merge policy.
The practical test is not whether every pull request is approved immediately. It is whether the team can explain the delay, address the largest constraint, and preserve or improve code health as delivery continues.
