If sounds produce colors, another person’s touch seems to appear on your body, or you see, hear, smell, taste, or feel something without an obvious external source, the safest first step is to reduce stimulation and move to a calm, familiar place. Lifelong, stimulus-linked experiences can fit synesthesia or sensory-processing differences; new, distressing, persistent, or disabling perceptions can also result from migraine aura, epilepsy, medication or substance effects, delirium, sensory loss, or mental-health conditions. Record what happened and arrange a medical assessment. Call emergency services for dangerous commands, sudden confusion, a seizure, severe agitation, sudden severe headache, new weakness, or new speech or vision changes.
What “sensory crossover” or an unusual perception can mean
These phrases describe several different experiences. The same person can also have more than one cause at different times, so a label based on one episode is not a diagnosis.

Synesthesia
Synesthesia is a consistent crossover in which one input automatically produces an additional sensory experience. A sound may evoke a color, a number may have a particular visual character, or a word may trigger a taste. Mirror-touch synesthesia is a specific example: watching another person being touched can create a felt touch on the observer’s own body. Cleveland Clinic describes mirror-touch synesthesia as real, even though the sensation is generated by the observer’s nervous system rather than by contact with that body part.
Synesthetic experiences are often recognized as an internal response and may have been present for years. They are not automatically a disorder. They deserve clinical attention when they become overwhelming, interfere with work or school, disrupt relationships or travel, or make ordinary activities difficult.

Sensory-processing differences and overload
NHS Autism Central notes that some people have difficulty filtering incoming information. Sounds, smells, touch, or lights may feel unusually intense, and a busy room can become impossible to tolerate even when each individual stimulus seems ordinary. Intensity can change with sleep, emotional state, the environment, and other conditions. A person may know exactly what is causing the sensation but still be unable to dampen it.
Overload is different from perceiving an object or voice that has no external source. It is an excessive response to real input, although overload and other unusual perceptions can occur together.
Hallucinations
MedlinePlus and the NHS define hallucinations as seeing, hearing, smelling, tasting, or feeling something that seems real but has no external source. Possible causes include mental-health conditions, alcohol or other drugs, medication effects, infection or delirium, neurological disease, sensory loss, and severe medical illness. A hallucination can be frightening even when the person retains some doubt about it.

Migraine aura
NIH and NHS descriptions of migraine aura include flashing lights and other visual changes, tingling or other sensory changes, difficulty speaking, and occasionally a sensation of being touched. Aura generally lasts no longer than one hour. A migraine attack commonly lasts four hours to three days. Bright light, loud noise, strong smells, skipped meals, stress, and disrupted sleep can trigger or intensify migraine symptoms.

A first aura, an aura with an unusual pattern, or symptoms that do not resolve as expected need medical assessment because other neurological problems can look similar.
Compare the pattern before deciding what to do
The following features help a clinician judge urgency. They can organize your observations, but they cannot establish a diagnosis on their own.
| Feature | More consistent with a stable crossover or sensory difference | Features that warrant evaluation for another cause |
|---|---|---|
| Onset | Present for as long as you remember or unchanged over a long period. | New onset, a sudden change, or a rapidly escalating pattern. |
| Trigger | A predictable, specific input such as a particular sound, sight, word, texture, or another person being touched. | Spontaneous episodes, no external stimulus, or episodes after a new medicine, dose change, alcohol, another substance, illness, or head injury. |
| Insight | You recognize the extra color, touch, or intensity as an internal response, even if it is vivid. | The perception seems unquestionably external, or you cannot distinguish it from events around you. |
| Duration and pattern | Brief or repeatable in the same way whenever the trigger occurs. | Episodes that are new, rapidly worsening, prolonged, or recurrent in a fixed pattern lasting seconds to minutes. |
| Associated signs | No new neurological or medical symptoms. | Headache, flashing lights, tingling, speech difficulty, confusion, fever, seizure, weakness, or vision loss. |
| Function | Manageable with ordinary adjustments. | Interferes with work, relationships, driving, travel, sleep, or basic daily activities. |
NICE recommends neurological assessment for recurrent brief, fixed-pattern sensory disturbances and for repetitive smell or taste hallucinations, which can be signs of epilepsy. That recommendation does not mean every brief sensation is a seizure; it means a repeatable neurological pattern should not be dismissed.
What to do during an overwhelming episode
- Check immediate safety. If you are confused, having new vision or speech changes, weak on one side, having a seizure, or unable to tell what is real, ask someone you trust to stay with you and seek urgent help. Do not attempt to push through a dangerous situation.
- Reduce the sensory load. When it is safe, leave bright, flashing, loud, crowded, or strongly scented surroundings. Move to a quiet, familiar place with fewer competing inputs. Environmental load can intensify sensory-processing differences and migraine symptoms.
- Remove avoidable triggers. Turn down sound, step away from flashing screens, and avoid strong smells. Make one change at a time so you can tell which adjustment helps.
- Record the start time. Note what you noticed first, which senses were involved, and whether the experience followed a particular stimulus. Timing helps distinguish a brief aura or fixed-pattern event from a longer episode.
- Do not change prescribed treatment on your own. Do not stop, skip, or double a medicine because of an unusual perception. Contact the prescriber or an urgent service for advice, especially after a new medicine or dose change. Also record alcohol or other substances rather than assuming they explain everything.
- Arrange follow-up. New, distressing, persistent, or function-limiting experiences need medical assessment even if they settle in a quiet room.
Keep an episode log that a clinician can use
A short, factual log is more useful than trying to decide the diagnosis while an episode is happening. Use a phone note or paper page and bring it to the appointment.
| Record | What to write |
|---|---|
| Date and time | When it began, when it ended, and whether it stopped suddenly or faded. |
| Sensory modality | Visual, auditory, smell, taste, touch, or several at once; describe the perception in plain words. |
| External trigger | The sound, image, person, texture, smell, crowd, or light present immediately beforehand—or state that no trigger was apparent. |
| Headache and neurological symptoms | Headache, flashing lights, tingling, weakness, speech difficulty, vision change, loss of awareness, or unusual movements. |
| Sleep, food, and stress | Sleep quality, skipped meals, emotional stress, and major changes in routine. |
| Medicines and substances | Prescription and over-the-counter medicines, recent dose changes, alcohol, and other substances, with approximate timing. |
| Illness or injury | Fever, infection symptoms, recent head injury, or a new medical problem. |
| Impact and response | What you could not do, which environmental change helped, and whether another person observed confusion or unusual behavior. |
Bring the complete medicine and supplement list to the visit. A clinician may also ask about hearing or vision loss, sleep, mental-health symptoms, seizures, migraine history, and family history.

Lower the background load between episodes
Shape the environment
Use the least stimulating setting that still lets you function. Reduce unnecessary background audio, avoid rapidly flashing content, and choose softer or steadier lighting when bright light is a trigger. Plan a quiet route or recovery space for crowded places. These are environmental aids, not treatments for the underlying cause.
Protect sleep and regular meals
Sleep disruption and skipped meals are reported migraine triggers, and sleep and emotional state can change the intensity of sensory-processing differences. Keeping a regular sleep and meal routine can make patterns easier to see. If episodes continue despite these adjustments, that is a reason for assessment, not evidence that you have failed to manage them.
Plan an exit signal
Tell a trusted person what overload looks like for you and agree on a simple way to leave a noisy or crowded setting. A written card or phone note can explain that you need a quiet place and medical help if you become confused or develop neurological warning signs. Do not ask another person to diagnose the episode; ask them to help with safety and timing.
When a stable crossover still needs treatment
A lifelong experience can still be disabling. Mirror-touch or another synesthetic response may make watching medical procedures, contact sports, public transport, work interactions, or close relationships difficult. If it interferes with work, relationships, travel, or daily activities, discuss it with a healthcare provider. Cleveland Clinic notes that therapy may help when mirror-touch synesthesia is bothersome. The goal is not to prove the experience is imaginary; it is to reduce distress and improve functioning while checking for other causes.
There is no single product, supplement, or setting that treats every sensory crossover, migraine aura, and hallucination. Ear protection, screen filters, sunglasses, or a quieter room may reduce environmental input for some people, but they do not treat epilepsy, infection, medication reactions, or a mental-health condition. Stop using any aid that makes you less aware of hazards or worsens symptoms.

Why new unusual perceptions require medical assessment
Medication and substance effects
Prescription medicines, over-the-counter products, alcohol, and other drugs can alter perception. The timing of an episode relative to starting, stopping, or changing a medicine is important. Do not discontinue a prescribed medicine without advice; the prescriber can decide whether a dose change, substitution, or examination is needed.
Infection, delirium, and severe illness
Hallucinations with fever, sudden confusion, fluctuating alertness, or a serious medical illness may indicate delirium or another urgent condition. Confusion that develops quickly is not explained safely by assuming sensory overload.
Neurological conditions and sensory loss
Epilepsy, migraine, head injury, and other neurological conditions can produce visual, auditory, smell, taste, touch, or language symptoms. Reduced hearing or vision can also contribute to hallucinations. Repetitive smell or taste hallucinations and recurrent brief fixed-pattern disturbances deserve neurological review under NICE guidance.
Mental-health conditions
Mental-health conditions can cause hallucinations, but a hallucination is not proof of one. Medical, neurological, medication-related, substance-related, and psychological causes can overlap. A proper assessment keeps those possibilities open and chooses treatment based on findings.
Urgent and emergency warning signs
Seek urgent medical advice for any new hallucination or unexplained perception, especially if it keeps returning, is distressing, or affects daily function. Call emergency services immediately if any of the following occurs:
Best Value
- The perception gives commands or creates an intention to harm yourself or someone else.
- Hallucinations are rapidly worsening, or there is severe agitation, sudden confusion, or incoherent speech.
- A seizure occurs, including loss of awareness or uncontrolled movements.
- There is a sudden severe headache, new weakness, or a sudden change in speech or vision.
- The episode follows a recent head injury.
If you are unsure whether a symptom meets an emergency threshold, describe the exact onset, duration, associated signs, medicines, substances, and injury history to an emergency or urgent-care service. Do not drive yourself when vision, speech, awareness, or strength is impaired.
How clinicians work out the cause
Assessment usually begins with a detailed history rather than a test chosen from the symptom alone. Explain whether the experience is lifelong or new, stimulus-linked or spontaneous, recognized as internal or experienced as external, brief or prolonged, and stable or changing. Show the episode log and medicine list.
The clinician may examine neurological function, review medicines and substances, ask about migraine and seizures, and check for infection, delirium, hearing or vision loss, sleep problems, and mental-health symptoms. Further testing or referral depends on those findings. A normal conversation at one appointment does not erase an episode that was real and impairing; the log helps document events that are not happening during the visit.

- Stable and stimulus-linked: reduce the trigger, document the pattern, and seek help if it becomes burdensome or limits daily life.
- Overload from real input: lower brightness, noise, crowding, and smells; review sleep, stress, and meal patterns; arrange assessment if control remains difficult.
- New or source-free perception: obtain medical advice promptly rather than labeling it synesthesia.
- With headache, tingling, flashing lights, or speech symptoms: record timing and seek assessment, particularly for a first or atypical episode.
- With confusion, seizure, severe agitation, sudden neurological deficit, dangerous commands, or head injury: use emergency services.
These categories guide the next safe step; they do not replace a diagnosis. The most useful information is a precise description of what happened, when it happened, what preceded it, and how it affected your ability to function.
