How to Reduce False Positives in AI-Powered Email Security

Reduce false positives by finding out exactly why a legitimate message was quarantined or blocked, fixing the underlying authentication, reputation, content, or routing problem, and reporting the confirmed misclassification. In Microsoft 365, use message trace and Defender investigation views before changing a policy. Keep any allow entry narrow, temporary, documented, and monitored: a broad exception can turn a delivery problem into a phishing incident.

The workflow below is Microsoft-specific where it names Microsoft 365 controls. The diagnostic principles apply more broadly, but policy labels, investigation data, and exception behavior differ among email-security platforms.

What a false positive is—and why it matters

A false positive occurs when an email-security system classifies a legitimate message as spam, phishing, malware, impersonation, or another threat category. The result may be quarantine, rejection, junk-folder delivery, link rewriting, attachment stripping, or a warning shown to the recipient.

AI-assisted filtering combines several signals rather than relying on one rule. A valid newsletter, invoice, password-reset message, or business reply can therefore be caught because one signal is unusually risky: an authentication failure, a poor sending reputation, suspicious wording, a new sending infrastructure, or a connector that obscures the original sender.

Microsoft summarizes the operational tension this way: “Blocking legitimate email in any significant volume is unacceptable to users, but it’s almost inevitable that you’re going to get some false positives.” The goal is not to eliminate every detection. It is to restore legitimate mail without weakening the controls that stop spoofing, phishing, and malware.

Start with evidence, not an allowlist

Do not begin by adding the sender to a trusted list. First establish what happened and which control made the decision. The same visible symptom—“the message was blocked”—can come from an authentication check, a bulk-mail threshold, an impersonation policy, a custom transport rule, or a malware verdict. Each requires a different remedy.

1. Record the incident

Capture the sender address and domain, recipient, message subject, approximate delivery time, message ID if available, and the final disposition. Record whether the message was rejected, quarantined, placed in junk, or delivered with modifications. Preserve the sender’s original headers when the recipient can provide them.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

2. Run a message trace

In the Exchange admin center, open Mail flow and then Message trace. Search using the sender, recipient, and time window. The trace can show whether a mail-flow rule or filtering policy acted on the message and what happened afterward. Expand the event details rather than relying on the summary status.

3. Inspect threat details when licensed

In the Microsoft Defender portal, use Email & collaboration and Explorer (also called Threat Explorer in some deployments), or the real-time detections view. These views can add the detection technology, delivery action, and any override that influenced the result. Availability depends on the Defender capability assigned to the tenant.

4. Preserve the detection reason

Write down the actual reason before changing settings. “Spam” is not enough if the details identify a failed DKIM signature, high-confidence phishing, impersonation, a bulk complaint signal, or a rule match. This record becomes the baseline for testing whether a fix solved the original problem without creating a new gap.

Classify the likely cause

Most legitimate-message false positives fall into four overlapping areas: authentication, sender reputation, content, and mail flow. Use the evidence from trace, headers, and investigation views to decide which area to test first.

Likely cause Evidence to check First corrective action
Authentication SPF, DKIM, or DMARC failure; a result that does not align with the visible From domain Account for every sending system, correct DNS or signing configuration, and retest
Reputation Sender IP or domain reputation warnings, blocklist evidence, unusual volume, or a new sending host Have the sender address reputation, reverse DNS, list-hygiene, or volume issues
Content Detection tied to message wording, URLs, attachments, branding, or bulk-mail characteristics Review the message and links with the sender; remove risky or misleading elements
Routing and connectors Forwarding or a gateway changes the apparent source IP or authentication context Validate connector topology and consider Enhanced Filtering for Connectors where supported
Policy threshold Trace or Explorer identifies bulk, phishing, impersonation, or a custom-policy threshold Make a narrowly scoped policy adjustment and observe both legitimate and malicious test outcomes

Fix SPF, DKIM, and DMARC before weakening filtering

Authentication is the most controllable source of many false positives. Microsoft identifies failed SPF, DKIM, or DMARC checks as common reasons legitimate email is classified as spam. Before changing a recipient-side policy, make sure the sending organization has configured authentication for every service that sends mail for the domain.

Inventory all legitimate senders

List the organization’s Microsoft 365 mailboxes, marketing platform, help-desk system, CRM, website, accounting service, application servers, and any third-party gateway that sends as the domain. A domain can pass authentication for one service while failing for another that was forgotten during setup.

Validate SPF

Confirm that the SPF record authorizes every approved sending service and does not exceed the DNS-lookup limit. Remove obsolete mechanisms and coordinate changes with the domain owner. SPF authenticates the envelope sender, not necessarily the address a user sees in the From field, so a passing SPF result alone does not prove alignment.

Validate DKIM

Check that each sending service signs messages with DKIM, that the public key is published at the selector shown in the header, and that the signature survives the route to Microsoft 365. Forwarding, content rewriting, and gateway processing can invalidate a signature.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Check DMARC alignment

Configure SPF and DKIM before enforcing DMARC. Microsoft describes DMARC as passing when at least one aligned SPF or DKIM check succeeds. Alignment means the authenticated domain corresponds to the visible From domain according to the DMARC mode in use. A message can show “SPF pass” and still fail DMARC if the domains do not align.

Review DMARC aggregate reports for unexpected senders and recurring failures. Reporting vendors can make results easier to interpret, but a reporting dashboard does not replace correcting the DNS records or signing configuration.

Stage DMARC enforcement instead of making a sudden jump

DMARC enforcement is a change to delivery behavior, not merely a reporting switch. Move from observation toward quarantine and, eventually, reject only after you understand which systems send on the domain and why failures occur.

  1. Publish a DMARC record in monitoring mode and collect reports.
  2. Identify legitimate sources that fail SPF or DKIM alignment.
  3. Correct those sources and verify new reports.
  4. Introduce enforcement gradually, increasing the affected percentage in controlled increments.
  5. Review quarantine results and support tickets after each change.
  6. Move toward p=reject only when the remaining failures are understood and the organization accepts the delivery impact.

Microsoft’s staged examples are guidance, not a universal timetable. The appropriate interval and percentage depend on sender diversity, report volume, business criticality, and how quickly the organization can investigate failures.

Investigate sender reputation and content

Authentication can be correct while reputation or message content still triggers an AI classifier.

Reputation checks

Ask the sender’s mail administrator to examine the sending IP and domain history, reverse-DNS configuration, complaint rates, list hygiene, and recent volume changes. A newly provisioned host, a shared IP with another customer, or a sudden campaign can look risky even when the message itself is legitimate. The sender—not the recipient’s allowlist—is usually in the best position to correct those conditions.

Content checks

Review the complete message, including visible and hidden links, attachment type, display name, reply-to address, branding, and urgency language. A legitimate marketing or transactional message can resemble phishing when the From domain, link domain, and reply-to domain differ without explanation. The sender should simplify redirects, use consistent domains, remove unnecessary executable or macro-enabled attachments, and ensure that the displayed organization matches the authenticated sender.

Do not ask recipients to click through a warning merely to prove that a message is safe. Submit the message for analysis and use a controlled retest after the sender corrects the content.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Account for forwarding and connector topology

Forwarding and third-party gateways can change how Microsoft 365 evaluates source IP, SPF, and other authentication results. A message may originate from a trusted business system but arrive through a relay whose IP is the one Microsoft sees. Automatic forwarding can also break SPF, while message modification can invalidate DKIM.

Map the path: sender service, external gateway, connector, Microsoft 365 recipient, and any forwarding destination. Confirm which system terminates TLS, rewrites headers, modifies content, or signs the message. For supported Microsoft 365 designs in which another service precedes Microsoft 365, review Enhanced Filtering for Connectors. Microsoft documents it as a way to improve authentication interpretation and reduce false positives caused by connector routing. Verify that the feature matches the actual topology and current product guidance before enabling or changing it.

Report the confirmed false positive

After confirming that the message is legitimate, submit it through the reporting workflow available in the Microsoft 365 security portal or quarantine experience. Include the original message and headers when the interface requests them. Reporting helps Microsoft analyze the detection and can improve future classification; it is not a substitute for fixing a broken SPF record or DKIM deployment.

Record the submission date, message identifier, detection reason, and any response. If similar messages continue to be misclassified, search for them as a group rather than creating a separate exception for every recipient.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use allow entries only as controlled, temporary risk decisions

Microsoft recommends correcting the root cause and reporting the good message before resorting to a Tenant Allow/Block List entry. An allow entry may be appropriate while a sender repairs its authentication or reputation, but it should be the smallest exception that solves the operational problem.

Scope the exception narrowly

  • Prefer a specific sender address, message indicator, or narrowly defined domain over a broad trust of an entire mail-flow path.
  • Set an owner, purpose, creation date, review date, and removal condition.
  • Use the shortest practical duration while the sender investigates.
  • Check whether the exception applies to a URL, sender, domain, or policy category; these scopes have different consequences.
  • Review the entry after every sender-domain or infrastructure change.

Microsoft warns that allowlists create a high risk of malicious mail being delivered. A compromised account or spoofed address can benefit from an overbroad exception. Malware and high-confidence phishing protections may still apply regardless of an allow entry, so an allowlist is not a guarantee of delivery.

Tune anti-phishing policies in small steps

Microsoft 365 anti-phishing policies provide trusted sender and domain settings and configurable phishing thresholds. Use them only when the investigation demonstrates that a threshold or impersonation setting—not a broken sender configuration—is causing the false positive.

Change one control at a time

Document the policy, setting, scope, administrator, reason, and expected result. If you change a threshold and a trusted-domain setting simultaneously, you cannot tell which change affected the outcome. Start with the narrowest policy that covers the affected users or message pattern.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Test both sides of the decision

After the change, send a controlled legitimate message that previously failed and inspect its trace and detection details. Also test a known suspicious pattern or use security-approved simulation mail to confirm that phishing protections still trigger. A delivery improvement is not successful if it lets a malicious message through.

Review impersonation settings separately

Executive names, domains, and recently registered look-alike domains can trigger impersonation defenses. If a legitimate partner repeatedly resembles a protected identity, adjust the specific protection or educate the sender about consistent branding rather than trusting every message from a broad domain.

Monitor for recurrence and false negatives

Every remediation needs an observation period. Search message trace and Defender investigation views for later messages from the same sender, domain, IP, or application. Track both legitimate mail that is still quarantined and suspicious mail that is now delivered.

  • Review new quarantine events for the original detection reason.
  • Check whether the sender’s SPF, DKIM, and DMARC alignment remains stable.
  • Inspect DMARC reports for new unauthorized sources or regressions.
  • Look for changes in volume, infrastructure, URLs, or attachment types.
  • Remove temporary allow entries when the root cause is corrected.
  • Escalate repeated failures to the sender’s security or messaging team with headers and timestamps.

Keep a change log linking each policy adjustment to the evidence that justified it. This makes rollback possible and prevents a temporary exception from becoming invisible permanent trust.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A practical decision framework

When several remedies appear possible, compare them on five dimensions before choosing one:

  1. Cause addressed: Does the action fix authentication, reputation, content, routing, or a demonstrated policy threshold?
  2. Scope: Does it affect one message or sender, or does it trust a whole domain or bypass a filter?
  3. Observability: Can you inspect detection technology, delivery action, headers, and subsequent DMARC results?
  4. Security trade-off: Could a compromised account, spoofed sender, malicious URL, or weaponized attachment benefit from the change?
  5. Reversibility: Can you withdraw the exception or restore the previous policy quickly?

In most cases, repairing authentication or sender infrastructure scores better than a permanent allowlist because it addresses the cause for every recipient while preserving independent security checks. A temporary, narrow exception can be justified when business mail is time-critical, but it should remain a monitored bridge to the real fix.

Troubleshooting common scenarios

A vendor’s invoices are quarantined after a domain change

Trace one invoice, inspect SPF and DKIM alignment, and compare the authenticated domains with the visible From address. Ask the vendor whether its invoice platform changed sending IPs or selectors. Correct the vendor’s DNS or signing setup, submit a confirmed false positive, and use a narrowly scoped temporary exception only if the business process cannot wait.

A forwarded message fails authentication

Identify the forwarding hop and the connector handling it. Check whether forwarding altered the envelope sender or message body. Review Enhanced Filtering for Connectors if the topology is supported, then retest with a new message rather than relying on the original headers.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A legitimate campaign is caught as bulk or phishing

Inspect the detection technology and campaign content. Verify list consent, sending volume, reputation, links, reply-to behavior, and attachment types. Ask the sender to correct the campaign characteristics and report the message. Do not broadly trust the campaign domain if it also sends password resets, account notices, or other high-value mail.

An allow entry restores delivery but suspicious mail appears

Remove or narrow the entry immediately, review the messages that bypassed filtering, and determine whether the exception scope covered a domain, URL, or mail-flow path. Re-run the original investigation and fix the sender or policy cause instead of recreating the broad exception.

How to prevent future false positives

  • Maintain an inventory of every approved sending service and its owner.
  • Keep SPF records current and remove retired services.
  • Rotate and monitor DKIM selectors when providers change.
  • Use DMARC reporting to discover unauthorized or forgotten senders.
  • Document connector and forwarding paths, including external gateways.
  • Review anti-phishing policies after organizational, domain, or vendor changes.
  • Train help-desk staff to collect headers, timestamps, recipients, and trace results before requesting an allowlist.
  • Set expiry and review dates for every exception.

A mature process treats a false positive as an incident to diagnose, not a request to permanently trust a sender. The evidence should determine the correction, and monitoring should determine whether the correction worked.
