Reducing false negatives—fraudulent payments that your system approves—requires more than choosing a more complex model. The reliable approach is an operating loop: define what a miss is, map every payment flow and fraud scenario, make sure the right data arrives at decision time, investigate confirmed misses and low-risk outcomes, tune actions against both fraud loss and customer impact, and retest controls as behavior changes.

No single algorithm, threshold, or vendor feature is proven to work best for every payment system. Your target should be measurable improvement in confirmed fraud caught and loss avoided without creating unacceptable false declines, customer friction, investigation queues, privacy risk, or operational cost.

What a false negative is—and why the definition matters

A false negative occurs when a payment is fraudulent but the detection process treats it as legitimate, usually by approving it or allowing it to complete. An attempted payment that is blocked is not a false negative, even if it would later have proved fraudulent. Keeping those outcomes separate prevents teams from overstating model performance.

Actual outcome System action Operational meaning
Legitimate payment Approved Correct approval
Legitimate payment Declined or challenged False positive, false decline, or unnecessary friction
Fraudulent payment Declined, challenged, held, or detected in time Fraud caught
Fraudulent payment Approved and later confirmed False negative (a missed fraud event)

Agree on the fraud outcome and an observation window with fraud operations, data science, finance, customer support, and dispute teams. Document how chargebacks, customer reports, reversals, investigations, law-enforcement information, and other post-transaction evidence become labels. There is no universal label window or mandatory delayed-label method in the guidance cited here; choose a window that reflects your rail, dispute cycle, and loss accounting, then apply it consistently.

Record the information available when the decision was made. A later device change, account takeover report, or chargeback can explain a miss, but it must not be treated as a signal the model could have used at authorization time.

Build a coverage map before changing the model

A model cannot detect a pattern that is outside its data or monitoring scope. Create a coverage inventory that connects payment flows to fraud scenarios and available signals.

Segment the payment estate

  • Channel: web checkout, mobile app, in-store, call center, recurring billing, refunds, payouts, and account-to-account transfers.
  • Instrument and transaction type: cards, bank debits and credits, wallets, real-time payments, cash-out, and merchant-initiated transactions.
  • Lifecycle state: new account, established account, dormant account, recently recovered account, changed credentials, and changed payout destination.
  • Customer and counterparty segments: consumer, business, marketplace seller, beneficiary, and agent, subject to lawful use of geography and personal data.
  • Known scenarios: account takeover, payment-card testing, stolen credentials, authorized push payment scams, synthetic identities, mule activity, refund abuse, and internal or third-party compromise.

For every segment, write down which controls run before authorization, during payment processing, immediately afterward, and only during investigation. Compare that map with the events you actually retain. Saudi Central Bank (SAMA) standards identify customer records, transaction and payment systems, identity and access-management systems, and external databases as possible sources for fraud detection. Use only data that is necessary, accurate, access-controlled, and permitted by applicable law.

Find flows that are invisible to the detector

Reconcile the payment ledger with the events arriving in the fraud platform. Look for channels, processors, currencies, recurring transactions, reversals, or partner-originated payments that bypass scoring. A high model score on one channel does not compensate for an unmonitored channel where the majority of losses occur.

Audit input data and signal quality

Many misses are data failures rather than modeling failures. For each signal, check whether it is present, timely, correctly joined, and stable enough to support a decision.

Checks to perform

  • Completeness: measure missingness by payment flow, customer segment, processor, and time period. A field that is usually present overall may be absent for a high-risk partner.
  • Freshness: record event timestamps and ingestion delay. A device, credential, or account-change event that arrives after authorization cannot protect that transaction.
  • Identity resolution: test links among accounts, devices, cards, bank accounts, IP addresses, beneficiaries, and counterparties. Incorrect joins can hide networks or create misleading risk.
  • Consistency: compare formats, enumerations, currency handling, time zones, and merchant-category mappings across processors and regions.
  • Drift: monitor changes in amount, velocity, location, device, authentication, and approval distributions. A stable pipeline can still become less informative as customer behavior changes.
  • Access and governance: document provenance, retention, permissions, lawful purpose, and deletion processes for personal and external data.

The UK government’s Public Sector Fraud Authority practitioner standard describes monitoring suspicious bank-account changes, bulk data patterns, device or voice detection, data matching, and anomaly monitoring. It also cautions that matching effectiveness depends on data quality and that analytical results should be tested for validity. Treat those as engineering tests, not assumptions.

Use relationships and sequence, not only single-transaction fields

Fraud often appears as a sequence: a credential change followed by a new device, a beneficiary addition followed by a high-value transfer, or many similar credits into a new or dormant account. Preserve event order and time intervals so your controls can evaluate these relationships. Where lawful and proportionate, graph or network features can expose shared devices, instruments, addresses, or counterparties that a single-payment view misses. Validate such links to avoid penalizing legitimate households, businesses, or shared infrastructure.

Investigate confirmed misses and sample low-risk outcomes

Every confirmed miss should produce a reproducible case record. Reconstruct the decision as it existed at the time, rather than scoring the transaction again with today’s data.

Miss review template

  1. Capture the transaction, account state, channel, instrument, processor, and decision timestamp.
  2. Snapshot every input available at that timestamp, including missing or delayed fields.
  3. Record the model version, rules fired, score, action, authentication result, and any manual override.
  4. Identify the first reliable fraud indicator discovered later and determine whether it was available before authorization.
  5. Classify the primary cause as coverage gap, data quality or latency, identity-link failure, model limitation, threshold or action error, workflow failure, or label error.
  6. Assign an owner, corrective action, due date, and validation test. Retain evidence for audit and future training.

Inspect transactions that looked safe

Do not review only high-risk alerts. Sample approved and low-risk transactions by channel, segment, amount, geography where lawful, and time period. Exception reports and random sampling can reveal novel patterns that are not represented in confirmed-fraud labels. The PSFA standard recommends periodic review of low-risk data-matching results for unknown or emerging fraud and describes random sampling as a way to identify suspicious patterns.

Use a sampling design that records the selection probability. Otherwise, a sample enriched for unusual transactions can be useful for discovery but cannot be used to estimate the overall miss rate without adjustment.

Tune actions against both sides of the cost curve

A score is not an outcome. Define what happens at each risk band: approve, request stronger authentication, hold for review, or decline. Then compare results by payment flow rather than assuming one global threshold is optimal.

Action Potential benefit Potential cost or failure mode Questions to monitor
Approve Low friction and high conversion More fraudulent payments can pass Confirmed fraud rate, loss, recovery, and downstream complaints
Challenge or authenticate Can stop or verify borderline events without an immediate decline Authentication failure, abandonment, accessibility problems, and attack on the challenge itself Challenge completion, fraud after successful challenge, abandonment, and customer contacts
Hold for review Allows evidence gathering for uncertain cases Latency, queue growth, missed service levels, and inconsistent analyst decisions Review time, queue age, overturn rate, and loss while pending
Decline Immediate prevention for high-risk events False declines, lost approvals, and customer attrition Confirmed fraud prevented, legitimate decline rate, appeals, and recovery

Lowering a threshold may catch more fraud, but it can also increase false positives, reviews, authentication prompts, and lost approvals. Measure the trade-off by segment and rail. Include alert workload, investigation effort, customer friction, recovery, and net loss—not just a classification metric.

SAMA describes proportionate controls and permits more stringent criteria where inherent risk is higher. That supports differentiated thresholds, not an assumption that the strictest setting should apply everywhere.

Use layered controls and an investigation path

Risk scoring works best as one layer in a system that can act quickly and explain why. Combine controls whose failure modes differ.

  • Exception reporting: targeted reports for new, dormant, high-value, repeated, or network-linked activity.
  • Post-transaction monitoring: rapid review of reversals, disputes, customer reports, and linked events to stop repeat attempts.
  • Human investigation: evidence display, documented playbooks, escalation routes, and authority to hold, release, or close a case.

A flagged payment helps only if the control can act in time and the alert can be investigated. Confirm routing, service-level targets, staffing, training, analyst access to evidence, and escalation to payments, security, customer support, and law-enforcement teams. SAMA specifically calls for adequate resources, skills, training, and workflow to allocate alerts; its examples include secondary checking and real-time blocking.

Measure false negatives as an operating outcome

Create a scorecard with definitions, time windows, ownership, and segment breakdowns. A single aggregate “accuracy” number can improve while losses worsen if the payment mix changes.

Measure Definition to document Why it matters
Confirmed fraud passed Fraudulent transactions approved or otherwise completed during the chosen observation window Direct measure of missed events
Fraud loss and recovery Gross loss, prevented loss, recovered amount, fees, and write-offs under a stated accounting period Connects detection to financial impact
False-positive or false-decline impact Legitimate payments challenged, held, or declined and subsequently validated as genuine Shows customer and approval cost
Approval and completion rate Approved or completed legitimate payments by channel and segment Detects commercial harm hidden by stricter controls
Alert workload Alerts created, investigated, escalated, closed, and aged by queue Reveals whether operations can keep up
Investigation quality Evidence completeness, overturn rate, repeat-fraud rate, and quality-review findings Tests whether human controls add value
Label quality Confirmed, pending, disputed, and unknown outcomes with source and date Prevents training and reporting on unreliable ground truth

Review effectiveness periodically. Saudi Central Bank’s Fraud Detection Standards state: “The effectiveness of fraud detection standards and related controls should be measured and periodically evaluated.” Use that principle to set a review calendar and trigger reviews after major incidents, feed outages, model changes, authentication changes, or material shifts in fraud tactics.

Build a feedback loop without contaminating labels

Feedback should flow from disputes, customer reports, investigator decisions, recoveries, and external intelligence into detection design. Separate confirmed fraud, suspected fraud, legitimate customer disputes, and unresolved cases. Keep the evidence source and confirmation date with each label.

When a feature is created from a future event—such as a chargeback or account closure—prevent it from entering the training or evaluation data for the earlier authorization decision. Use time-based evaluation that mirrors deployment and test performance separately on new customers, new devices, new merchants, and new fraud patterns.

When labels are delayed or incomplete, report the uncertainty instead of presenting a precise false-negative rate that the evidence cannot support. Revisit provisional outcomes as cases mature and version the metric definition when the observation window changes.

Reassess after changes and emerging threats

Retest controls whenever payment behavior, customer mix, fraud tactics, data feeds, model versions, rules, processors, or authentication steps change. Maintain a control register with an owner, purpose, input fields, decision point, fallback action, review date, and evidence of testing.

Minimum change checklist

  • Compare input completeness, latency, and distributions before and after the change.
  • Run replay or shadow evaluation using transactions representative of each affected flow.
  • Check fraud caught, fraud missed, false declines, approval impact, alert volume, and analyst capacity.
  • Verify explainability, access controls, audit logs, rollback steps, and customer communications.
  • Set a post-release review date and define the trigger for emergency rollback.

For participants covered by Nacha’s US ACH Network Phase 2 material, the new fraud-monitoring requirements took effect June 19, 2026; the practical implementation date was June 22 because June 19 was a federal holiday. The guidance describes risk-based procedures intended to identify unauthorized entries or entries authorized under false pretenses, with review at least annually and updates for evolving risk. It does not require every ACH entry to be screened individually or require monitoring before processing. Applicability depends on the participant’s role and the relevant phase.

Nacha gives examples of suspicious credits such as an atypically high-dollar entry, repeated similar credits in a short period, or that activity involving a new, dormant, or possible mule account. Treat these as examples for risk-based procedures, not a universal rule that every such payment is fraudulent.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

In the European Union, Commission Delegated Regulation (EU) 2018/389 requires in-scope payment service providers to maintain transaction-monitoring mechanisms for unauthorized or fraudulent transactions, based on normal user behavior and specified risk factors. It also calls for security measures to be documented, periodically tested, evaluated, and audited. The regulation’s scope depends on the provider and service involved.

SAMA’s standards are framed for SAMA member organizations and call for risk-aligned controls, updates when the risk landscape changes, measurement of effectiveness, and proportionality. The UK PSFA document is a public-sector practitioner standard rather than universal payments regulation; its guidance on exception reports, sampling, validation, data matching, and human oversight can still inform operating design.

How to compare fraud-detection controls or platforms

Compare capabilities using the same data, definitions, observation windows, and segments. A polished demonstration or a single vendor statistic is not a substitute for a controlled evaluation.

Axis Questions to answer
Coverage Which rails, channels, account-takeover scenarios, payment-fraud types, and post-authorization events are monitored?
Signals Can it use transaction history, account and identity/access data, device or location signals, external data, and investigator feedback? What are the freshness and data-quality requirements?
Decision timing Does it operate pre-authorization, in real time, post-transaction, or only during investigation? What latency and data-sharing constraints apply?
Outcomes Are confirmed fraud caught, fraud missed, false-positive impact, approval rate, loss, recovery, and investigation effort reported with definitions and segment detail?
Operations Can analysts see evidence, route alerts, escalate cases, document decisions, and meet service levels? What staffing and training are required?
Governance Are provenance, quality checks, lawful use, access controls, audit logs, documentation, periodic evaluation, and jurisdictional obligations supported?

Visa describes a UK national account-to-account fraud-detection pilot in which it reported a 40% average uplift in fraud detection at a 5:1 false-positive rate across participating partners and identified 54% of fraudulent transactions that had already passed through sophisticated bank and payment-service-provider systems. Those are Visa-reported pilot results, with no publication year stated on the accessed page; they are not an independent benchmark or an expected result for a particular institution. The same page says Visa Advanced Authorization evaluates over 500 risk attributes per transaction. That product claim does not establish that adding more attributes will reduce false negatives in your environment.

Common approaches that fail—and the correction

“Add more data” without validating it

More fields can add noise, leakage, privacy exposure, and processing delay. Profile missingness, timing, join accuracy, and incremental value before deployment. Remove or quarantine signals that cannot be used lawfully or consistently.

“Lower the threshold everywhere”

This can catch more fraud while overwhelming investigators and declining genuine customers. Test threshold changes by segment and action, with queue capacity and approval impact included in the decision.

“Optimize a model metric only”

Precision, recall, or area-under-curve values do not show net loss, recovery, friction, or operational feasibility. Tie model evaluation to confirmed outcomes and business costs over a defined time window.

“Train on every alert as fraud”

An alert is a suspicion, not ground truth. Separate confirmed, disproved, pending, and unknown cases, and retain the evidence source.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

“Review only high-risk alerts”

Novel fraud can receive low scores because it does not resemble known cases. Sample low-risk approvals and exception-report results to discover patterns outside the current model.

“Assume compliance is a one-time project”

Rules, risks, feeds, and customer behavior change. Assign owners, document testing, review at least as often as the applicable requirement, and trigger an earlier review after material change.

A practical operating cadence

Cadence Activities
Daily or near real time Watch feed health, latency, rule failures, alert queues, high-severity incidents, and payment-flow outages.
Weekly Review confirmed misses, significant false declines, analyst overrides, emerging clusters, and aged investigations.
Monthly Break down fraud passed, fraud caught, loss, recovery, approvals, friction, and workload by rail, channel, segment, and model or rule version.
After a material change Run replay or shadow tests, verify data and controls, set rollback criteria, and conduct a post-release review.
At least annually where required Reassess risk, review procedures and effectiveness, update controls for evolving threats, and preserve audit evidence.

The result is an auditable improvement loop: discover what the system cannot see, restore missing or late signals, investigate why approved fraud passed, choose proportionate actions, and verify that the change improves real outcomes without simply moving harm to genuine customers.
