Reduce false alarms without accepting more missed deterioration by validating the prediction system in the hospital where it will be used, choosing a locally appropriate trigger threshold, and designing each alert around a response the care team can actually take. There is no universal threshold or alert-rate target established for every hospital patient-risk prediction system.

What counts as a false alarm?

Define the predicted event, prediction horizon, eligible patient population, and intended response before assessing whether an alert is “false.” A prediction can be statistically false—the predicted event did not occur within the specified horizon—yet still have prompted a useful assessment or preventive action. Conversely, an alert may be operationally non-actionable if it reaches no responsible team, arrives too late, or does not lead to a meaningful response.

These are different problems. Model performance describes how well predictions correspond to outcomes; operational usefulness describes whether alerts support timely care. State which meaning of “false alarm” is being measured, and use the same definitions when comparing model versions or changing thresholds.

Validate the system in its intended setting

Test the system on the hospital’s own patients and in the care setting and workflow where it will run. AHRQ’s guidance on AI and patient safety recommends organization-specific validation and ongoing quality assurance, including evaluation for bias. Validation should reflect the intended population, prediction horizon, data available at the time of prediction, and clinical workflow—not just an overall score from a development dataset.

Review discrimination and calibration, then examine operating characteristics at plausible trigger thresholds. The following measures answer different questions and should be considered together:

Measure What it helps answer Why it matters locally
Sensitivity Among patients who experience the event, how many were flagged? Lower sensitivity can mean more missed events; weigh that risk against the burden of additional alerts.
Specificity Among patients who do not experience the event, how many were not flagged? Lower specificity can increase false-positive predictions and unnecessary evaluations.
Positive predictive value Among those flagged, how many experience the event within the defined horizon? It helps describe how often an alert corresponds to the outcome in the local population.
Calibration Do predicted probabilities correspond to observed event rates? A score’s numeric risk estimates may not mean the same thing in a new hospital or population.
Alert volume How many alerts occur per patient, unit, or period? Even an acceptable model metric can create an unsustainable workload when applied at scale.

Compare threshold choices against the consequences of missed deterioration and the costs of unnecessary assessment, escalation, or interruption. Look at alert rates over meaningful periods and across units, not only at aggregate model statistics. If the event is uncommon, positive predictive value and alert volume are particularly important to understand alongside sensitivity.

Set a local threshold and govern changes

Do not adopt a threshold solely because it was used in another hospital or reported for a different model. NICE guidance for recognizing and responding to deterioration in acutely ill adults in hospital recommends local trigger thresholds and says: “The threshold should be reviewed regularly to optimise sensitivity and specificity.” That guidance offers useful threshold-governance principles, but it is not a validation protocol for every machine-learning prediction system.

For each candidate threshold, document the local trade-off: how many additional patients would be flagged, how many events might be detected or missed, what response is expected, and whether staffing and workflow can support that response. Set a review schedule and revisit the threshold after meaningful changes in patient mix, workflow, model version, or data sources. A threshold change is not automatically an improvement: verify its effects on both missed cases and alert burden.

Make every alert clear, timely, and actionable

Reducing interruptions is not just a modeling task. AHRQ guidance emphasizes clear, concise, timely, user-centered AI outputs and balancing true and false positives. Present the risk in context, identify the relevant time horizon, and state what action the recipient is expected to consider. Route alerts to a team able to respond; avoid repeated notifications to several staff about the same unresolved prediction unless escalation is intentional.

There is no single best routing configuration or alert cap established for all hospitals. Design and assess routing with the clinical teams who receive alerts, including who owns follow-up, what happens when there is no response, and how duplicate or continuing alerts are handled. If no feasible response exists, changing the display alone will not make the alert useful.

Monitor safety and workload after deployment

Continue quality assurance after launch and after changes to the threshold, model, data, or workflow. Review performance and its effects in the actual deployment setting, including whether results differ across relevant patient groups. The ONC SAFER Guides, whose page was updated February 27, 2026, include organizational responsibilities addressing AI-enabled systems and system-management practices for EHR configuration, validation, and maintenance.

A practical monitoring set should include:

  • Alert counts and rates by unit, patient, and time period.
  • Response time and whether the intended clinical action occurred.
  • Events that were missed or recognized only after deterioration.
  • Alerts judged non-actionable, ignored, or escalated, with reasons where they can be established.
  • Staff burden and workflow effects, alongside patient outcomes.
  • Performance, calibration, and data quality across relevant patient groups.

Use case review and frontline feedback to find whether problems arise from the prediction, threshold, presentation, routing, or an unavailable response. Evidence is still needed from prospective studies to establish reliability, validity, and effects on important patient outcomes; favorable retrospective metrics alone do not establish safety in practice.

Mount-It! MedHub Medical Cart with Tablet Mount, Height Adjustable Mobile Workstation on Wheels, Fits 10–15" Tablets & Monitors up to 20", Scanner Holder, for Hospital, Clinic & Dental Office
  • Bring the workstation to the patient, not the other way around – Designed for busy hospitals, clinics, laboratories, and dental offices, the MedHub rolling medical cart lets nurses, physicians, and dental staff access EHR software, run patient intake, and manage point-of-care documentation wherever care happens — no more walking back to a fixed station between every room.
  • Fits the tablets and monitors your team already uses – The secure adjustable tablet mount fits most 10"–15" tablets including iPad Pro, iPad Air, and Android medical tablets. Need a monitor instead? The VESA-compatible column (75x75 and 100x100mm) supports screens up to 20" and 8.8 lbs. Note: tablet mount and monitor mount are interchangeable — one device mounted at a time.
  • Adapts to every clinician, every shift – Pneumatic height adjustment via hands-free foot pedal raises or lowers the worksurface from 31.5" to 47.2" without stopping or bending, keeping staff comfortable through long shifts. The screen arm positions the center of your tablet or monitor 7.4" above the worksurface for a natural, neck-friendly viewing angle whether seated or standing.
  • Everything your workflow needs, organized in one place – Built-in scanner holder keeps your barcode or document scanner within reach for medication administration, specimen tracking, or dental charting. The wire storage basket holds supplies and accessories, the power strip mount routes and conceals cables, and the cup holder keeps a drink close during long rounds — so your team stays focused, not searching.
  • Moves effortlessly, stays put when it needs to – Four smooth-rolling locking casters glide across hard floors and low-pile carpet between exam rooms, patient bays, and nursing stations. Lock all four wheels instantly for a stable workstation during procedures or documentation. The wider front base (20.2") and tapered rear (15.4") provide a low center of gravity that resists tipping even when the cart is fully loaded.

Keep monitor-alarm statistics in context

Physiologic-monitor alarm figures are not false-alarm rates for patient-risk prediction systems. AHRQ PSNet reported a 2014 study from an academic hospital’s 66 adult ICU beds that recorded more than 2 million physiologic-monitor alerts in one month, or 187 warnings per patient per day. AHRQ PSNet also cited prior research in 2016 estimating that 80%–99% of ECG monitor alarms were false or clinically insignificant. Those figures illustrate alarm burden in monitor settings; they should not be presented as the performance of a risk-prediction model.

Check regulatory scope for the specific software

Include regulatory review in system governance. FDA guidance identifies patient-specific risk scores and time-critical alerts as software functions relevant to clinical decision-support oversight, but whether a particular function is subject to oversight depends on its intended use and the applicable criteria. Do not assume that all hospital prediction systems have the same regulatory status.
