Low testosterone (male hypogonadism) cannot be diagnosed from a symptom checklist or a single home-test result. Persistent symptoms such as reduced sex drive, erectile problems, unexplained fatigue, depressed mood, infertility, loss of muscle or body hair, breast enlargement, bone concerns, or concentration difficulty justify a medical appointment. A clinician confirms the diagnosis only when compatible symptoms or signs occur with consistently low blood testosterone, measured accurately in the morning and usually repeated on a separate day.
The American Urological Association (AUA) uses a total testosterone below 300 ng/dL as a reasonable cut-off that supports diagnosis, but that number is not a stand-alone verdict. Your symptoms, timing of the blood draw, test method, medications, health conditions, fertility plans, and follow-up results all matter.

What “low testosterone” actually means
Testosterone is produced mainly by the testes under the control of the hypothalamus and pituitary gland. It contributes to sexual desire and erections, sperm production, muscle and bone maintenance, red-blood-cell production, mood, energy, and other body functions. A persistently inadequate level together with related symptoms is called testosterone deficiency or male hypogonadism.

The Endocrine Society’s 2026 statement is explicit: “Symptoms alone are not diagnostic of hypogonadism.” Its clinical guideline recommends diagnosing hypogonadism only in men who have symptoms or signs of testosterone deficiency and “unequivocally and consistently low serum total testosterone and/or free testosterone concentrations (when indicated).”

This distinction prevents two common errors: attributing ordinary tiredness to testosterone without checking other causes, and starting testosterone when a temporary or laboratory-related low result has not been confirmed.
Symptoms that can occur in adults
Symptoms vary with the degree and duration of testosterone deficiency, the person’s age and health, and whether the underlying problem is in the testes or in the hypothalamus and pituitary. No single symptom proves the cause. Use the list below to decide whether to arrange an evaluation, not to diagnose yourself.
| Symptom or sign | What it may look like | Why a clinician still needs to investigate |
|---|---|---|
| Reduced sexual desire | Less interest in sex than usual or a sustained change from your baseline. | Low mood, relationship stress, medication effects, sleep problems, and other illnesses can produce the same change. |
| Erectile dysfunction | Difficulty getting or keeping an erection adequate for sex. | Vascular disease, diabetes, medication effects, anxiety, and other conditions are common alternatives. |
| Infertility or reduced fertility | Difficulty conceiving or a concern about sperm production. | Sperm production depends on several hormones and testicular factors; testosterone treatment itself can reduce sperm production. |
| Low energy or fatigue | Persistent lack of energy, reduced stamina, or unusual tiredness. | Anemia, thyroid disease, depression, poor sleep, infection, medication effects, and metabolic conditions can feel similar. |
| Depressed or irritable mood | Low mood, loss of motivation, irritability, or reduced sense of well-being. | Mood disorders and major life stressors need their own assessment and treatment. |
| Difficulty concentrating | Problems focusing, remembering, or completing familiar tasks. | Sleep disorders, depression, medication effects, and many medical conditions can impair concentration. |
| Reduced muscle mass or strength | Noticeable loss of muscle or strength without an intentional training change. | Nutrition, inactivity, chronic illness, and aging can contribute. |
| Reduced facial or body hair | Less beard or body-hair growth than previously expected. | Hair growth also reflects genetics, age, skin conditions, and other hormones. |
| Breast enlargement (gynecomastia) | New enlargement or tenderness of breast tissue. | Medication effects, liver or kidney disease, and other hormone changes are possible. |
| Bone loss or fractures | Known low bone density, unexplained fractures, or bone concerns. | Bone health has multiple causes and may need separate testing. |
| Hot flashes | Sudden episodes of heat or sweating. | They can occur with hormone changes but are not specific to testosterone deficiency. |
Symptoms that are sudden, severe, or rapidly worsening deserve prompt medical attention because they may signal a condition unrelated to testosterone. Chest pain, severe shortness of breath, fainting, or acute neurologic symptoms require emergency care; these are general emergency warnings, not diagnostic signs of low testosterone.
When to schedule a doctor’s appointment
Mayo Clinic patient guidance says to make a medical appointment when symptoms of male hypogonadism are present. Arrange a routine visit when one or more symptoms persist, interfere with daily life, or represent a clear change from your normal state. A visit is especially appropriate for:
- Persistent loss of libido or erectile problems.
- Unexplained fatigue, low mood, irritability, or concentration changes.
- Infertility or concern about sperm production.
- Loss of muscle or body hair without an obvious explanation.
- Breast enlargement or tenderness.
- Known bone loss, an unexplained fracture, or other bone-health concern.
- A prior low testosterone result that was taken at an unusual time, during an illness, or without a confirmatory test.
You do not need to wait until several symptoms appear. Conversely, having one symptom does not mean testosterone is the cause. Primary care clinicians can begin the evaluation and refer you to an endocrinologist or urologist when the results or history require specialist input.

How doctors confirm or rule out testosterone deficiency
Evaluation is a process rather than a single test. The usual sequence is below.
1. Review symptoms, timing, and health history
The clinician asks when symptoms began, whether they are constant, and how they affect sexual function, energy, mood, strength, fertility, sleep, and concentration. Bring a complete medication and supplement list, including prescribed opioids, glucocorticoids, or other drugs that may affect hormones, even if you think they are unrelated. Discuss alcohol or recreational-drug use honestly.
Tell the clinician about sleep quality, possible sleep-disordered breathing, weight or metabolic changes, recent severe illness, head injury, testicular injury, cancer treatment, chronic diseases, and previous hormone results. These details help distinguish a temporary change from a persistent disorder and identify reversible contributors.
2. Have an accurate morning total-testosterone test
Testosterone follows a daily rhythm and is generally highest in the morning. Mayo Clinic says blood testing is usually performed between 8 and 10 a.m. Follow the laboratory’s instructions about fasting, medications, and appointment timing rather than assuming that an afternoon sample is equivalent to a morning sample.
The test should use an accurate assay. Tell the clinician if you were acutely ill, severely sleep-deprived, or had a major change in medicines around the time of testing, because those circumstances can affect interpretation. Do not stop a prescribed medicine on your own to “improve” the result.

3. Repeat a low or borderline result on a separate morning
Testosterone can vary from day to day. Mayo Clinic notes that testing may be repeated on more than one day. The AUA requires two early-morning total-testosterone measurements on separate occasions, together with compatible symptoms or signs, before diagnosing testosterone deficiency. A single low number should therefore trigger confirmation, not immediate treatment.

Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →4. Consider free testosterone when the clinician indicates it
Total testosterone measures testosterone in the blood overall. In some situations, the amount available to tissues (free testosterone) helps clarify a result, especially when total testosterone is near the lower range or when factors affecting binding proteins are suspected. The Endocrine Society includes free testosterone “when indicated”; your clinician decides whether it adds useful information and which method is appropriate.
5. Investigate the cause if deficiency is confirmed
Once consistently low levels and compatible symptoms are established, the next question is why. Luteinizing hormone (LH) and follicle-stimulating hormone (FSH) help distinguish:
- Primary hypogonadism: a problem centered in the testes, where the glands cannot produce adequate testosterone despite pituitary signals.
- Secondary hypogonadism: a problem involving the hypothalamus or pituitary, where signaling to the testes is inadequate.
Depending on the history and initial results, the clinician may order prolactin, a semen analysis, pituitary imaging, or other blood tests. A semen analysis is particularly relevant when fertility is a current or future goal. Imaging is not automatic; it is selected when the examination and hormone pattern make a pituitary cause plausible.
How to interpret a testosterone number
Reference ranges differ by laboratory, assay, age group, and the population used to establish them. Always read the result with the laboratory’s units and reference interval. The AUA’s 2018 guideline identifies total testosterone below 300 ng/dL as a reasonable cut-off supporting the diagnosis, but it also requires two early-morning tests on separate occasions and symptoms or signs. It is not a universal treatment threshold or a substitute for clinical judgment.

| Result pattern | What it generally means | Typical next step |
|---|---|---|
| One low result | Possible deficiency, temporary variation, illness-related change, or testing issue. | Review timing and circumstances, then repeat an early-morning test. |
| Two low early-morning results plus compatible symptoms | Supports a diagnosis of hypogonadism when the clinician confirms the assay and clinical context. | Evaluate LH, FSH, and other tests to identify the cause and discuss options. |
| Borderline total testosterone | May not clearly explain symptoms; binding-protein effects or assay differences can matter. | Consider repeat testing and free testosterone when indicated. |
| Normal testosterone with persistent symptoms | Testosterone deficiency is less likely to be the explanation, although the symptoms remain real. | Look for other medical, sleep, mental-health, medication, or lifestyle causes rather than self-treating. |
Do not compare your number with a friend’s result or an online calculator without checking units, collection time, assay, and the laboratory’s range. A result reported in ng/dL is not numerically interchangeable with one reported in another unit without conversion by a qualified professional.
What can mimic low testosterone symptoms
The symptoms listed above overlap with many common conditions. A clinician may consider depression or anxiety, inadequate or disrupted sleep, metabolic disease, thyroid or other endocrine disorders, anemia, chronic infection or inflammation, medication effects, excess alcohol, nutritional problems, and ordinary changes associated with aging. Erectile dysfunction may also reflect vascular disease, and infertility may have causes unrelated to testosterone levels.
This overlap is why an over-the-counter “testosterone booster” or an online prescription obtained without evaluation can be harmful: it may delay diagnosis of the actual problem, expose you to side effects, and complicate fertility or prostate monitoring.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. What to bring to the appointment
A short, organized record makes the visit more productive. Prepare:
- A symptom timeline, including the first noticeable change and whether it is worsening, stable, or intermittent.
- All medicines, injections, supplements, and performance-enhancing products, with doses if known.
- Past testosterone results, the collection times, laboratory ranges, and whether you were ill when tested.
- Information about sleep, snoring or witnessed breathing pauses, exercise, weight change, alcohol, and other relevant health factors.
- Your fertility plans. State clearly if you are trying to conceive now or may want biological children later.
- Questions about which assay will be used, whether you need to repeat testing, and what follow-up is planned.
Testosterone therapy: why confirmation and safety review come first
Testosterone therapy is a medical treatment, not a general energy supplement. It may be considered only after the clinician confirms a consistent deficiency, discusses the cause and alternatives, and reviews goals and risks. Treatment choice and monitoring are individualized; the appropriate decision for one patient may be unsafe for another.
Best Value
Fertility requires special attention. External testosterone can suppress the hormonal signals that drive sperm production, so someone who is trying to conceive should discuss that goal before any treatment is started. Do not begin, stop, or change testosterone without the prescribing clinician.
The Endocrine Society lists circumstances in which therapy should be avoided or deferred, including:
- Plans for fertility in the near term.
- Prostate or breast cancer, or a concerning evaluation that has not been resolved.
- Elevated hematocrit.
- Untreated severe obstructive sleep apnea.
- Severe lower-urinary-tract symptoms.
- Uncontrolled heart failure.
- A recent heart attack or stroke.
- Thrombophilia.
These are not a complete personal risk assessment. Your clinician considers age, examination findings, cardiovascular and urinary history, the cause of low testosterone, and the monitoring that can be provided. If treatment is prescribed, keep every follow-up appointment and report troublesome or rapidly changing symptoms.

Home kits and direct-to-consumer services may appear convenient, but convenience does not replace diagnostic context. The clinically supported pathway is to confirm the result and investigate its cause before treatment.
| Question | Clinician evaluation with confirmation | Self-testing or self-treatment without confirmation |
|---|---|---|
| Diagnostic reliability | Uses a correctly timed morning sample, repeats an abnormal result, and interprets it with symptoms and laboratory context. | May rely on one sample, an unknown collection time, or a method that is difficult to interpret alone. |
| Finding reversible causes | Reviews medicines, sleep, metabolic factors, illness, and other conditions; orders targeted follow-up tests. | Usually cannot distinguish testicular disease from hypothalamic-pituitary disease or another diagnosis. |
| Fertility protection | Discusses current and future fertility before treatment and can arrange semen or hormone evaluation. | May expose someone trying to conceive to a treatment that suppresses sperm production. |
| Safety screening | Checks contraindications and establishes an individualized monitoring plan. | Can miss elevated hematocrit, sleep apnea, prostate or breast concerns, heart failure, or other reasons to defer therapy. |
| Follow-up | Provides a plan for repeat measurements, symptom response, adverse effects, and adjustment. | Often leaves the person to interpret changing symptoms or numbers without medical oversight. |
If a direct-to-consumer test was the event that raised your concern, bring the complete report to a licensed clinician rather than treating the number as a diagnosis.
A practical decision path
- Notice a persistent change. Record the symptom, when it began, and whether it affects sex, fertility, energy, mood, strength, sleep, or concentration.
- Book a routine appointment. Primary care is an appropriate starting point; referral can follow if the findings are complex.
- Share the full context. Include medicines, supplements, health conditions, sleep concerns, recent illness, and fertility plans.
- Complete a correctly timed morning test. Follow the laboratory’s instructions and ask what to do about regular medicines.
- Repeat the test if it is low or borderline. A second early-morning sample on a separate occasion is central to confirmation.
- Investigate the cause if deficiency is confirmed. LH, FSH, and selected additional tests guide the next step.
- Discuss treatment only after the review. Weigh expected benefits, fertility implications, contraindications, and monitoring rather than relying on a product claim.
Questions worth asking your doctor
- Were both blood samples collected early in the morning, and should the result be repeated?
- Does my symptom pattern fit testosterone deficiency, or should we evaluate another cause first?
- Would free testosterone add information in my case?
- Do LH and FSH suggest a testicular problem or a hypothalamic-pituitary problem?
- Do I need prolactin, a semen analysis, pituitary imaging, or another targeted test?
- Could any of my medicines, sleep problems, or recent illness be affecting the result?
- How would treatment affect fertility, and what monitoring would be required?
Bottom line
Persistent symptoms are a reason to see a doctor, not proof that testosterone is low. A reliable diagnosis combines compatible symptoms or signs with consistently low blood testosterone, normally confirmed with two early-morning tests on separate occasions. The AUA’s 300 ng/dL cut-off can support that diagnosis, while the Endocrine Society emphasizes the whole clinical picture. Proper evaluation also identifies reversible causes, protects fertility, and screens for situations in which testosterone therapy should be deferred or avoided.
