Those signs can also stem from pain, hearing or vision loss, and other illness, so they are not enough to diagnose dementia at home. Arrange a veterinary visit for new or worsening changes; sudden balance problems need prompt attention rather than being assumed to be dementia.
What dementia symptoms can look like in dogs
Veterinarians often use the DISHAA pattern to organize changes associated with canine cognitive dysfunction. A dog may show one sign or several, and signs often develop gradually.

- Disorientation: Getting lost in familiar places, staring, or becoming stuck in a corner.
- Changed interactions: Becoming unusually clingy, avoidant, or irritable, or seeming not to recognize familiar people or animals.
- Sleep-wake changes: Wandering or restlessness at night and sleeping more during the day.
- House-soiling: Urinating or defecating indoors after being reliably house-trained.
- Activity and anxiety changes: Losing interest in play or walks, pacing or repeating an activity, or developing new fears, anxiety, irritability, or aggression.
Cornell notes that CDS may begin around age nine or older, but age is context—not a diagnosis. A senior dog’s new behavior should not automatically be written off as normal aging.
When to see a veterinarian
Book an evaluation if a senior dog develops persistent or progressive changes in behavior, sleep, toileting, anxiety, or interactions. Seek prompt veterinary advice for a sudden or marked change. A gradual pattern may fit CDS, but similar signs can arise from conditions that need different care.

Sudden balance signs are different
Sudden circling, inability to stand, a head tilt, nausea, or rapid repetitive eye movements can be signs of vestibular (balance-related) trouble. Cornell distinguishes this pattern from gradual CDS; have a veterinarian assess these signs instead of assuming they are dementia.
Keep a dated record
Before the appointment, note what changed, when it happens, and how often. Include appetite, mobility, toileting, sleep, and medications. This record can help the veterinarian understand the pattern; a symptom checklist can organize observations but cannot confirm CDS.

How a veterinarian assesses possible CDS
CDS is assessed from clinical signs after other possible causes have been considered. Cornell describes a thorough physical examination and possible blood and urine tests to investigate look-alikes such as pain, arthritis, seizures, systemic illness, and vision or hearing loss. In selected cases, MRI may be used to rule out a brain tumor or another condition.

The assessment considers the dog as a whole and whether another health problem better explains the changes.

What care and management may involve
There is no single cure. Management is individualized and may combine care for concurrent health problems with changes at home, enrichment, veterinarian-guided nutrition, and medication when appropriate.
- Make the home safer and more predictable: Keep routines consistent and block access to unsafe stairs or spaces if wandering or disorientation puts the dog at risk.
- Offer enrichment: Mental and physical activities suited to the dog can provide stimulation. Interactive toys are one optional activity, not a treatment by themselves.
- Discuss nutrition with the veterinarian: Cornell identifies prescription diets as one possible part of care. VCA says that when a therapeutic diet with sufficiently high antioxidant levels is used, behavioral improvements are typically seen within 6–12 weeks of starting it. VCA also says that if there is no improvement within 12 weeks, nutrition alone is unlikely to make a difference. This is guidance, not a guaranteed response or a reason to delay veterinary follow-up.
- Ask about medication: Cornell identifies selegiline as a medication approved for CDS treatment in North America. Whether it is appropriate depends on the individual dog; discuss medication and supplements with the veterinarian.
Cornell University College of Veterinary Medicine states: “There is no one treatment or cure for CDS, and research is ongoing in this area.”

How common are dementia-like signs?
Merck Veterinary Manual reports two estimates based on different studies and designs. They should not be combined into a single prevalence figure, and signs consistent with CDS are not the same as a confirmed clinical diagnosis.
