How to Recognize and Respond to AI-Related Delusions in Yourself or Someone Else

If you or someone you know is developing an unshakeable belief that does not fit the available evidence—especially alongside hallucinations, severe sleep loss, confused speech, withdrawal or a sudden drop in functioning—treat it as a reason for prompt professional assessment, not as proof of a diagnosis. Stay calm, acknowledge the fear or distress without confirming the belief, and move toward qualified human care. If anyone may hurt themselves or another person, use emergency or crisis services immediately.

Interactions with a chatbot can be part of the context, but “AI-related psychosis” is an emerging area of concern rather than a formal diagnosis or established cause. A clinician must assess mental and physical health, medicines, substances, sleep, mood and day-to-day functioning.

What a delusion is—and what “AI-related” does and does not mean

A delusion is a strongly held belief that does not match reality. The person may be unable to consider evidence that conflicts with it, and may not recognize that the belief is unusual or untrue. Delusions can occur with hallucinations (hearing or seeing things others do not), confused thinking or speech, suspiciousness, unusual intense ideas, disrupted sleep, reduced self-care, social withdrawal and difficulty working, studying or managing ordinary tasks. These signs warrant assessment; they do not establish a diagnosis.

Psychosis is a group of symptoms, not a synonym for schizophrenia. The National Institute of Mental Health (NIMH) says, “There is no one cause of psychosis.” Possible contributors include a mental illness, a physical or neurological illness, severe sleep deprivation, prescription medicines, alcohol or other drugs, and combinations of these factors. An older adult with new symptoms, for example, needs medical evaluation rather than an assumption about a psychiatric disorder.

The phrase AI-related delusion is used informally for a situation in which chatbot conversations appear to reinforce, intensify or become woven into an unusual belief. It is not a recognized stand-alone diagnosis. The American Psychological Association (APA) has described reported cases and possible reinforcement or amplification, while emphasizing that the direction and causes are unsettled. A chatbot may be one influence among many; saying it caused psychosis would go beyond the evidence.

How common is psychosis?

NIMH gives a broad estimate of 15 to 100 people per 100,000 developing psychosis each year. That range describes population incidence, not an individual’s probability and not the likelihood that a chatbot is involved.

Warning signs to notice in yourself

One experience alone does not diagnose psychosis. Look for a cluster of changes, a marked departure from your usual functioning, or an experience that is becoming harder to question or control.

  • A fixed belief that people, institutions, technology or messages are targeting, monitoring or communicating with you despite contrary evidence.
  • Hearing voices or seeing things that other people do not, or interpreting ordinary events as uniquely directed at you.
  • Thoughts or speech that become difficult to follow, rapidly shift topics, or feel unusually intense and certain.
  • Difficulty separating a chatbot’s generated text from verified events, records or testimony.
  • Several nights of very little sleep, escalating energy or agitation, or a sharp change in mood.
  • Pulling away from people, neglecting food, hygiene, work, school, bills or other responsibilities.
  • A sudden decline in performance or an inability to carry out familiar tasks.

These are prompts to seek help, not a checklist for self-diagnosis. The NHS symptoms guidance notes that someone experiencing psychosis may not realize that perceptions or beliefs are not real. If you are unsure, ask a trusted person whether they have noticed changes and arrange an assessment with a doctor or mental-health professional.

When chatbot use is part of the story

Tell the clinician how your use changed: longer or overnight sessions, repeated requests for reassurance, treating generated replies as proof, or withdrawing from people in favor of the bot. Save relevant conversation excerpts if doing so feels safe, but do not keep using the chatbot to test or validate the belief. Generated text can sound certain while being wrong and cannot examine your personal context.

Warning signs in someone else

Friends and relatives often notice functional changes before the person seeks care. Concerning patterns include increasing suspicion, claims that a bot or online system is sending private instructions, intense fear, unusual certainty, sleeplessness, isolation, neglect of self-care, confused conversation or a rapid deterioration at work or school. Ask what they are experiencing and how it is affecting sleep, safety and daily life. Do not label them with schizophrenia, “AI psychosis” or any other diagnosis.

Take a new or rapidly worsening change seriously even when the person appears articulate. Psychosis can be associated with a medical condition, a medication effect or substance use, and delays can make assessment harder. If the person is pregnant or recently gave birth, urgent evaluation is especially important: the NHS estimates postpartum psychosis affects about 1 in 1,000 women who give birth and describes it as a medical emergency.

Why symptoms can appear

A proper assessment considers several possible explanations at once. Relevant questions include:

  • Recent or prolonged sleep loss.
  • Prescription or non-prescription medicines, dose changes and interactions.
  • Alcohol, cannabis, stimulants, psychedelics or other drugs, including withdrawal.
  • Mood symptoms such as severe depression, mania or agitation.
  • Physical or neurological illness, particularly when symptoms begin later in life.
  • Family history, previous episodes and changes in functioning.
  • The content, timing and intensity of unusual beliefs or perceptions.

Do not stop a prescribed medicine abruptly or change a dose to test a theory. The NHS treatment guidance says medication decisions and side effects should be discussed with the responsible professional.

What to do if this is happening to you

  1. Tell a trusted person today. Choose someone who can stay in contact, help you sleep and eat, and notice if safety worsens. Say plainly what you are experiencing, including voices, fears or beliefs that feel certain.
  2. Arrange prompt clinical assessment. Contact a primary-care doctor, psychiatrist, psychologist or other qualified service. Explain when the change began, how much you sleep, all medicines and substances, recent illness, and how chatbot use has changed.
  3. Stop treating AI output as evidence. A model predicts text; it does not verify events, read minds or provide a clinician’s judgment. Take a break from conversations that increase certainty or fear, and use human support for decisions.
  4. Reduce immediate strain while waiting. Stay with supportive people if possible, avoid intoxicants, keep a regular sleep and meal routine, and postpone major financial, legal or relationship decisions until you have been assessed.
  5. Escalate for danger. If you might harm yourself or someone else, cannot care for basic needs, or feel commanded by voices, move to live crisis or emergency help rather than continuing an online conversation.

Be candid even if the belief feels embarrassing. Clinicians need the unedited account to distinguish possible causes and choose safe care.

How to respond to a friend or family member

Use a calm, reality-based approach

Speak privately, slowly and without ridicule. You can say, “I can see this is frightening,” or “I believe that you are experiencing this, and I want to help you feel safe.” That acknowledges emotion without agreeing that the belief is true. Ask open questions: “What has been happening?” “How much have you slept?” “What would help you feel safer right now?”

Offer concrete help: sit with the person, call a doctor together, arrange transport, or contact an existing care or crisis team. Keep the focus on safety and distress rather than winning an argument. A short, respectful statement that you do not see the situation the same way is usually safer than debating every detail.

What to avoid

  • Do not confirm the questionable belief or add your own theories about hidden messages.
  • Do not argue point by point, mock, threaten, corner or physically restrain the person unless trained responders direct you.
  • Do not promise secrecy when someone may be in danger.
  • Do not diagnose, recommend someone else’s medication, or tell them to stop treatment.
  • Do not make the chatbot the mediator. Its confident wording is not an independent assessment.

If the person refuses a routine appointment, continue offering practical support and contact a known care team for advice. The NHS diagnosis guidance notes that someone may not recognize symptoms and may resist seeing a general practitioner; in the United Kingdom, an existing crisis team or NHS 111 can advise when no team is available.

When and where to seek help

Choose the least delayed route that matches the risk. Availability and names differ by country.

Situation What to do What the service provides
New or persistent symptoms, but no immediate danger Arrange a prompt appointment with a primary-care clinician or mental-health professional. History, physical and mental-health assessment, medication and substance review, and referral or treatment planning.
Symptoms are intensifying, sleep is collapsing, or daily functioning is rapidly deteriorating Contact an urgent mental-health service or crisis team the same day; ask a trusted person to stay involved. Rapid risk assessment and connection to appropriate care.
Immediate threat, suicidal intent, violent intent, severe confusion, or inability to meet basic needs Call local emergency services or go to an emergency department. Do not leave the person alone if doing so is unsafe. Emergency safety response and medical assessment.

United States

NIMH lists 988 for crisis support and 911 for life-threatening emergencies. Tell the responder that psychosis-like symptoms, suicidal thoughts, weapons, substances or medical conditions may be involved, and follow instructions about staying safe.

What a professional assessment involves

A qualified psychologist, psychiatrist, social worker or medical doctor will ask about the experiences themselves and their timing, sleep, mood, functioning, medicines, alcohol and other drugs, physical symptoms, family history and previous episodes. They may perform a physical examination or arrange tests when a medical cause is possible. Bring a medication list, names of substances, dates of major changes and a trusted observer’s account if the person agrees.

The goal is not merely to decide whether a belief is “real.” It is to identify risks and causes and to create a treatment plan. Care can include antipsychotic medication, talking therapies, family intervention, peer or self-help groups, and rehabilitation or practical support, as described by the NHS treatment guidance. Medication choice, monitoring and side effects require clinician discussion; never start, stop or alter medication independently.

What is known—and not known—about chatbots and psychosis

The APA’s guide to using AI-generated advice says, “Do not rely on AI in a crisis.” Its warnings are practical: a chatbot may affirm a user’s framing, produce confident errors, lack personal context and have no professional duty of care. The APA’s health advisory says generative-AI chatbots and wellness applications should not replace a qualified mental-health provider because validation, oversight and safety protocols may be limited.

An article in APA Monitor dated September 1, 2026 describes reported cases of delusional thinking after extended chatbot interactions and experts’ concern that conversations might reinforce or amplify beliefs. It also stresses that evidence is still developing and that causation and direction are not settled.

The same article reports an OpenAI estimate that about 0.07% of weekly ChatGPT users—approximately 630,000 people—had conversations showing signs of mania or psychosis in 2025. That figure refers to conversations flagged as showing signs, not clinically diagnosed people, and it does not show that the chatbot caused symptoms. It should not be used to calculate your personal risk or to dismiss symptoms that need care.

Safety rules for using AI when someone is distressed

  • Use AI, at most, for neutral administrative help such as drafting questions for a clinician—not for diagnosis, reassurance loops or crisis decisions.
  • Verify factual claims with qualified professionals and reliable health services.
  • Do not paste private clinical information into a service unless you understand its privacy terms and have a clear reason.
  • Set a human check-in before using any tool when sleep loss, paranoia, hallucinations or suicidal thoughts are present.
  • If a conversation increases fear, certainty or isolation, end it and contact a person or crisis service.

No supplement, device, journal or consumer app can diagnose, prevent or treat psychosis. The appropriate next step is human assessment and support.

Questions people commonly ask

Could a chatbot be the sole cause?

Current evidence does not establish that. Chatbot interactions may reinforce or amplify an existing vulnerability in some reported cases, but sleep, illness, medicines, substances, mood and other factors also matter. Only a clinical assessment can evaluate an individual situation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should I challenge the belief with facts?

Aggressive fact-checking can increase mistrust. Acknowledge the distress, state your own view briefly without ridicule, ask about safety and offer professional help.

What if the person says nothing is wrong?

That lack of insight can occur with psychosis. Keep contact calm, involve a trusted clinician or crisis team, and seek emergency help if danger or inability to care for basic needs develops.

Is hearing a voice always psychosis?

No. Many experiences have different explanations, and a single symptom is not a diagnosis. New, distressing or impairing voices should still be discussed promptly with a qualified professional.
