Revenue growth or a lower MCR is not, by itself, proof that profitability or cash generation improved.
1. Confirm which UnitedHealth report you are reading
Start by writing down three items shown on the document: the reporting period, the release date and the document type. UnitedHealth’s filing index contains earnings releases, quarterly Form 10-Q reports, annual Form 10-K reports, proxy statements and other SEC filings. An earnings release is a management summary; the 10-Q or 10-K supplies the fuller financial statements, footnotes and definitions.

The latest results covered by the figures in this guide are the second quarter of 2026, released July 16, 2026. UnitedHealth announced that third-quarter 2026 results were scheduled before market open on October 13, 2026, followed by an 8 a.m. Eastern Time call. No third-quarter release is included in the available results, so do not assume any Q3 revenue, MCR, EPS or guidance figure. Before relying on this article for a current decision, open the investor-relations page and replace the Q2 references if a later release has been posted.

Keep the period attached to every number. “EPS of $6.04” is incomplete unless you say it is reported diluted EPS for Q2 2026. Likewise, a full-year 2025 MCR cannot be compared with a single quarter without labeling the different time bases.

2. Read the consolidated income statement first
The income statement shows how much the group brought in, what it spent to deliver services, and what remained for shareholders. Read down the statement rather than starting with the headline supplied in a news alert.
| Line or measure | What it tells you | What to ask |
|---|---|---|
| Revenue | Total reported inflow from premiums, products and services | Which segment and revenue type produced the change? |
| Medical costs | Claims and other medical benefits associated mainly with insurance premiums | Did medical costs grow faster or slower than premium revenue? |
| Other operating costs | Administrative, technology, selling, service and other operating expenses | Are investments or one-time charges changing the cost base? |
| Earnings from operations | Profit from operating activities before interest, other non-operating items and taxes | What is the operating margin and how did it move? |
| Interest and other non-operating items | Financing costs, investment results and other items outside operations | Is a change in net income coming from operations or below the operating line? |
| Net earnings attributable to common shareholders | Profit available to common shareholders after all listed expenses | How does it compare with the prior period and diluted share count? |
| Diluted EPS | Attributable earnings divided by diluted shares | Is the figure reported (GAAP) or adjusted (non-GAAP)? |
For fiscal 2025, UnitedHealth reported total revenue of $447.567 billion, up 12% from 2024. Operating earnings were $18.964 billion, net earnings attributable to common shareholders were $12.056 billion and reported diluted EPS was $13.23. Because operating earnings and EPS fell even as revenue increased, “revenue grew” cannot be used as a synonym for “profit grew.” The 2025 Form 10-K describes substantial medical-cost growth and other items affecting the result.
A useful first calculation is the growth rate for each line, not just revenue: (current period minus year-ago period) divided by year-ago period. Then inspect whether costs rose faster than revenue and whether the share count changed. For a sequential view, compare Q2 with Q1 2026 only after checking seasonality and management’s explanation; insurance utilization and government-program timing can make one quarter unlike the next.
3. Understand what UnitedHealth’s medical care ratio means
UnitedHealth defines the medical care ratio as:
MCR = medical costs ÷ premium revenue
The ratio measures how much of premium revenue was consumed by medical costs. A higher MCR generally leaves less premium revenue to cover administrative costs and profit, all else equal. It is an insurance-cost ratio, not the group’s consolidated profit margin.
The denominator matters. MCR is not medical costs divided by all UnitedHealth revenue. UnitedHealth also reports Optum businesses and other revenue streams that do not belong in this insurance ratio. Therefore, subtracting MCR from 100% does not produce the company’s net margin.
2025 MCR and the drivers management identified
Reported MCR was 89.1% in 2025, compared with 85.5% in 2024. The 2025 results release also presented an adjusted MCR of 88.9%; the reported figure included a 20-basis-point negative impact from loss contracts. The annual filing discusses several contributors to the year-over-year change: Medicare funding reductions, elevated medical-cost trend, member mix, losses on value-based-care contracts, lower favorable development, Medicare Part D and market morbidity. It also identifies offsets, so the ratio should be read with that full explanation rather than attributed to one cause.

Q2 2026 MCR
For Q2 2026, MCR was 86.7%, versus 89.4% in Q2 2025. UnitedHealth attributed the decrease to benefit design and pricing discipline, member mix and medical-cost management initiatives. The quarter also included $860 million of net favorable prior-period medical development, mostly related to 2026 dates of service. Favorable development means earlier claims estimates proved higher than the claims ultimately paid; it can lower the reported ratio in that period. Do not turn the $860 million into an “adjusted MCR” by dividing it by consolidated revenue, because that would use the wrong denominator and mix different measures.
When comparing MCRs, record whether each number is reported or adjusted, the quarter or year, the business covered and any reserve development or contract item disclosed. A lower ratio is encouraging only in context: premium pricing, benefit design, membership mix, utilization, coding, reserve releases and government-rate changes can all affect it.
4. Separate operating costs, margins and earnings
UnitedHealth reports several ratios that answer different questions. The 2025 Form 10-K reported an operating cost ratio of 13.3%, an operating margin of 4.2% and a net earnings margin of 2.7%.
| Ratio | General calculation | 2025 reported value | Interpretation |
|---|---|---|---|
| Medical care ratio | Medical costs ÷ premium revenue | 89.1% | Insurance medical-cost burden; not a group profit margin |
| Operating cost ratio | Operating costs ÷ the applicable revenue base reported by the company | 13.3% | Non-medical operating expense burden |
| Operating margin | Operating earnings ÷ revenue | 4.2% | Profit from operations before non-operating items and tax |
| Net earnings margin | Net earnings attributable to common shareholders ÷ revenue | 2.7% | Bottom-line profit after all listed items |
In Q2 2026, operating earnings were $8.0 billion and net margin was 4.9%. The operating cost ratio was 12.7%, compared with 12.3% in the year-ago quarter. Management said the increase reflected targeted investments in technology, operations and community support. That explanation matters: a higher operating-cost ratio can be deliberate spending intended to support future capacity, not automatically deterioration.
Use the ratios together. If MCR falls but operating costs rise sharply, the benefit may not reach operating earnings. If operating margin improves while net margin does not, inspect interest, investment results, taxes and other non-operating lines.
5. Reconcile reported EPS with adjusted EPS
Reported diluted EPS is the GAAP result attributable to common shareholders. Adjusted EPS is a company-defined, non-GAAP measure that removes specified items. The reported number belongs in the headline comparison; adjusted EPS can help isolate management’s view of ongoing performance only when you read the reconciliation.

| Period | Reported diluted EPS | Adjusted EPS | How to use the pair |
|---|---|---|---|
| Full-year 2025 | $13.23 | $16.35 | Read the reconciliation and named exclusions before judging the gap |
| Q2 2026 | $6.04 | $6.38 | Use the detailed release’s reconciliation to identify the quarter’s adjustments |
For 2025, the release disclosed a $1.78 diluted-EPS impact from specified portfolio divestiture, restructuring and other items. Those exclusions may be economically important, may recur or may affect cash even when excluded from adjusted EPS. “Adjusted” does not mean “more real”; it means the company applied its stated definition. Compare the same definition across periods and check whether the list of adjustments changed.
When an earnings headline cites only adjusted EPS, locate the reported EPS, the reconciliation table and the tax effect of each adjustment. A large difference can signal meaningful restructuring or portfolio activity rather than a trivial accounting footnote.
6. Check operating cash flow and the balance sheet
Accounting earnings and cash generated are related but distinct. Accrual accounting recognizes revenue and expenses when earned or incurred, while operating cash flow tracks cash received and paid through operations, including changes in working capital and claim reserves.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minute- Full-year 2025: operating cash flow was $19.7 billion, or 1.5 times net income.
- Q2 2026: operating cash flow was $11.1 billion, or 1.9 times net income.
- June 30, 2026: debt-to-capital ratio was 41.2%.
The 1.9-times figure is a quarterly relationship, not a forecast of the full-year conversion rate. Working-capital timing, premium collections, claim payments, taxes and reserve movements can make cash flow volatile. Read the cash-flow statement and balance-sheet changes for the same period before concluding that earnings quality improved or weakened.
Debt-to-capital adds financing context. A company can report strong EPS while carrying a rising debt burden, or produce strong cash while margins are pressured. Look for changes in debt, cash, receivables, medical claims payable and shareholders’ equity, then read management’s explanation for material movements.
7. Use UnitedHealthcare and Optum to explain the consolidated result
UnitedHealth reports through UnitedHealthcare and Optum, with Optum containing distinct businesses. Segment revenue, operating earnings and margin show where the consolidated change originated. A group-level MCR mainly describes the insurance business; it does not describe every Optum activity.

In Q2 2026, UnitedHealthcare revenue was $86.0 billion, operating earnings were $3.9 billion and operating margin was 4.6%. Compare those figures with the segment’s year-ago values in the release, then examine Optum’s revenue, earnings and margin separately. Ask whether growth came from membership, premium rates, acquisitions, care-delivery volume, pharmacy services or another disclosed source.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.8. Treat guidance as a dated outlook
Guidance is management’s expectation at a specific date, not a realized result. In the July 16, 2026 Q2 release, UnitedHealth’s full-year 2026 outlook was reported EPS of $18.45–$18.95, adjusted EPS of $19.50–$20.00 and operating cash flow of approximately $24 billion.
Record the release date beside each range. Then compare the next actual quarter with the implied path to the range and note whether management raised, narrowed, lowered or reaffirmed it. Do not treat the midpoint as a promise, and do not carry the Q2 outlook forward after a newer release without checking for an update. The scheduled Q3 date was October 13, 2026; because the Q3 results are not present in the available materials, no Q3 outlook change can be stated here.
9. A repeatable workflow for every earnings release
- Identify the document: write down the period, release date and whether you are reading the release, 10-Q or 10-K.
- Capture consolidated figures: record revenue, medical costs, operating costs, operating earnings, net earnings and diluted EPS for the current and year-ago periods.
- Calculate growth: compute year-over-year changes for each line; add a sequential comparison only when seasonality and the release make it useful.
- Read MCR correctly: confirm premium revenue is the denominator, label reported versus adjusted, and copy management’s stated medical-cost drivers.
- Review margins: place MCR, operating cost ratio, operating margin and net margin in separate rows; never substitute one for another.
- Open the EPS reconciliation: list every named exclusion, its diluted-EPS effect and whether it appears recurring, non-cash or cash-related.
- Test cash and leverage: record operating cash flow, its relationship to net income and the debt-to-capital ratio for the stated period.
- Trace the segments: compare UnitedHealthcare and Optum revenue, operating earnings and margins with the consolidated movement.
- Mark the outlook date: copy guidance ranges exactly, then replace them when a newer release is available.
- Write the conclusion last: describe what improved, what deteriorated, what was affected by one-time or reserve items and what remains uncertain.
10. Common reading mistakes and how to avoid them
Calling revenue growth profit growth
Revenue can rise while medical costs, operating costs, interest or taxes rise faster. Pair the top line with operating earnings, net earnings and margins.

Using 100% minus MCR as net margin
MCR uses medical costs and premium revenue. Net margin uses consolidated net earnings and revenue. Their scopes and denominators differ.
Best Value
Treating adjusted EPS as a replacement for GAAP EPS
Show reported EPS first, then explain the company’s adjusted measure and reconciliation. Never hide the exclusions.
Annualizing one quarter’s cash flow
Q2 operating cash flow of $11.1 billion and a 1.9-times conversion ratio describe that quarter. They are not automatically a full-year run rate.
Ignoring reserve development
The $860 million of favorable prior-period medical development in Q2 2026 helped the reported MCR. Identify such items before comparing underlying medical-cost trends.
Recommended Free Tools
Reading a segment percentage without its denominator
State whether a margin applies to UnitedHealthcare, Optum or the consolidated group, and use the corresponding segment revenue.
Using stale guidance
Attach the outlook to its release date and refresh it after the next earnings release. A range is management’s dated expectation, not an achieved result.
11. Management commentary in context
Numbers should lead, but management’s explanation helps identify the operational mechanism behind them. In the January 27, 2026 results release, Chief Executive Officer Stephen Hemsley said: “We confronted challenges directly and finished 2025 as a much stronger company, giving us the momentum to better serve those who count on us and continue to improve our core performance.” Treat that statement as management’s perspective and test it against the reported MCR, margins, cash flow, segment results and guidance rather than treating it as independent evidence.
What a careful conclusion looks like
A sound reading might say that Q2 2026 revenue reached $112.0 billion, operating earnings were $8.0 billion, reported EPS was $6.04 and MCR improved to 86.7% from 89.4% a year earlier. It would also note the $860 million favorable prior-period development, the 12.7% operating cost ratio versus 12.3%, $11.1 billion of quarterly operating cash flow and the 41.2% debt-to-capital ratio at June 30. That combination tells a fuller story than any single headline: insurance-cost performance improved on the reported measure, operating investments raised the cost ratio, and cash and leverage provide separate tests of financial strength.
