How to Quit Smoking: A Practical Plan for Choosing a Method and Getting Started

The most useful first step is a plan, not a perfect promise. Choose a near-term quit date or arrange a conversation with a quit coach, clinician, or pharmacist; map the situations that trigger cigarettes; and decide what support and medication, if any, fit your health and preferences. For U.S. adults who smoke cigarettes, counseling and FDA-approved medicines are evidence-based options, and using both can more than double the chance of quitting compared with either alone. A lapse is information about a trigger—not proof that you cannot quit.

Start with one achievable next step

You do not have to settle every treatment decision before beginning. Pick one action you can complete in the next few days:

As an Amazon Associate I earn from qualifying purchases.

  • Set a quit date and tell a supportive person.
  • Call 1-800-QUIT-NOW in the United States to connect with a state quitline coach.
  • Book a visit with a healthcare professional or pharmacist to review medicines, health conditions, and other drugs you take.
  • Ask a counselor, text program, app, or web service to help you plan for cravings.

If you are unsure about medication, make the appointment itself your first move. A professional can explain choices without requiring you to start one immediately. You can also ask a pharmacist practical questions about access, labels, and what information the prescriber will need.

Take stock without judging yourself

For two or three days, note when you smoke and what is happening immediately beforehand. Record the time, place, people present, emotion, activity, and how strong the urge feels. Include cigarettes that seem automatic, such as the first one after waking or one with coffee. Look back at earlier attempts and write down what helped, what made urges harder, and why the attempt ended. This information gives a counselor or clinician something practical to work with; it is not a test of willpower.

Also note the tobacco products you use, how often you use each one, and whether you combine products. The medication instructions summarized in this guide are for U.S. adults quitting cigarettes, so a complete description helps a professional decide whether a different plan is needed. Bring a list of prescription medicines, over-the-counter products, supplements, and recreational drugs as well.

Identify high-risk routines

Common patterns include driving, alcohol, work breaks, arguments, social smoking, and being around other smokers. For each pattern, choose an alternative in advance: take a short walk, drink water, text a support person, change the route you drive, or leave a smoking area. The goal is to make the first few weeks less automatic while new routines form.

Write an if-then response for each major trigger: “If I want a cigarette while driving, then I will pull over safely, drink water, and call my support person.” Keep the list on your phone and where cigarettes used to be stored. Ask people who smoke around you not to offer you cigarettes and, when possible, to make breaks smoke-free.

Choose a quit approach

Set a specific quit date

A date turns an intention into a start line. Pick a day soon enough to keep momentum but far enough away to arrange support and medicine. Remove cigarettes, lighters, and ashtrays before that day, and tell people how they can help—for example, by not offering cigarettes or by joining you for a smoke-free break.

Use the days before the date to rehearse alternatives, save the quitline number, and check how you will obtain any medicine. If a pharmacy, insurance plan, or state program is involved, ask about requirements and current availability rather than waiting until the quit date.

Use a near-term preparation plan

If choosing a date feels overwhelming, start with a scheduled coaching or medical conversation. You can use that appointment to select a date, discuss medication, and make a cravings plan. A gradual reduction strategy may be discussed with a professional, but do not assume that reducing alone provides the same support as a complete quit plan. Set a review point so that preparation leads to a defined next step rather than an open-ended delay.

Behavioral support: what it does and where to find it

Counseling helps you prepare for stress, urges, withdrawal, and situations that previously led to smoking. It can be one-to-one, group-based, by telephone, by text, or online. CDC’s consumer guidance describes counseling and quitlines as practical ways to create a plan and handle cravings: How to Quit Smoking.

A counselor can help you rank triggers, practice responses, decide whom to contact during a difficult moment, and plan follow-up. You do not need to wait for a crisis to use support. Arrange a check-in around the quit date and another when you expect a difficult routine, such as a weekend, a social event, or a return to work.

Telephone quitlines

In the United States, call 1-800-QUIT-NOW. A coach can help identify triggers, choose coping strategies, arrange follow-up, and explain local access to medicines. Some state quitlines provide free or reduced-cost starter supplies, but eligibility and offers change by location; ask what is currently available rather than assuming every state provides the same package.

During the call, explain how much you smoke, prior attempts, any medicine side effects, and whether you have medical conditions or take other medicines. Ask how often coaching is available, what happens after a lapse, and where to obtain affordable products. Quitline services are not identical in every state, so confirm the current details for your location.

Digital and social support

Text programs, apps, websites, and online groups can provide reminders and urge-management prompts between appointments. A trusted friend, partner, or family member can be your “first call” when an urge peaks. Agree on exactly what you want from them—listening, a distraction, or help leaving a smoking situation. If one type of support feels uncomfortable, try another; the aim is dependable help, not a particular format.

Medication options for U.S. adults who smoke cigarettes

CDC lists seven FDA-approved cessation medicines for adults who smoke cigarettes: five forms of nicotine replacement therapy (NRT) and two prescription pills. FDA describes the categories and the need to discuss product-specific benefits, risks, and side effects with a healthcare professional at Smoking: Medicines to Help You Quit. Use the current package label and professional instructions; this guide does not provide personal dosing.

Category U.S. forms listed by CDC Access How readers commonly use the category
Long-acting NRT Nicotine patch Over the counter (OTC) Provides a steadier nicotine level through the day.
Short-acting NRT Nicotine gum; nicotine lozenge OTC Used for individual urges according to label directions.
Prescription NRT Nicotine oral inhaler; nicotine nasal spray Prescription Offers a clinician-guided alternative when appropriate.
Prescription non-NRT Varenicline; bupropion sustained release (SR) Prescription Pills that do not deliver nicotine; suitability and instructions require a prescriber.

The seven-medicine list and its adult-cigarette scope are summarized in CDC’s Quit Smoking Medicines guidance.

NRT replaces nicotine without cigarette smoke. A patch is the long-acting option in the list; gum and lozenges are short-acting options for individual urges. The prescription inhaler and nasal spray are also NRT, but require a prescription. Varenicline and bupropion SR are pills without nicotine. None is automatically right for everyone, and product labels and medical histories matter.

How to compare the choices

Counseling versus medicine

Counseling addresses triggers, routines, stress, and problem-solving. Medicine reduces withdrawal and cravings. Each can help; CDC says combining counseling and medication can more than double quit chances. If you can access only one at first, start with the one you can obtain now and add the other when possible. A quitline can be the fastest way to add coaching to an OTC medicine, while a clinician can coordinate a prescription and follow-up.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

OTC versus prescription access

Patch, gum, and lozenge are OTC in the United States, which can make a same-day start possible. The inhaler, nasal spray, varenicline, and bupropion SR require a prescription. Insurance rules, pharmacy availability, and state programs differ, so a pharmacist or quitline can help you locate an affordable route. Ask whether a local quitline has a current free or reduced-cost starter offer, what eligibility rules apply, and whether supplies can be mailed.

Price alone should not determine the choice. Include the cost of appointments, prescription copays, transportation, and missed work when comparing routes, and ask about lower-cost alternatives or coverage. Retail links should be treated as general product information, not a personal recommendation.

Steady versus rapid-response nicotine

A patch is designed for continuous delivery, while gum or a lozenge is a short-acting option for breakthrough urges. CDC consumer materials explain that combining a long-acting patch with short-acting gum or lozenge can increase the likelihood of quitting: Medicine Tips Just for You. Do not combine products casually; confirm the plan and label directions with a clinician or pharmacist, especially if you have medical conditions or take other medicines.

Ask how to use a short-acting product correctly, how to handle a missed use, and what symptoms should prompt a call. Do not add another nicotine product simply because an urge is strong. A professional can decide whether the plan needs adjustment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Nicotine-containing options versus pills

NRT supplies measured nicotine without cigarette smoke. Varenicline and bupropion SR are prescription pills that do not contain nicotine. The choice depends on your health history, other medicines, prior quit attempts, side-effect concerns, preference, cost, and access—not on a universal ranking. Tell the prescriber about previous reactions and any reason you may have difficulty following a schedule.

Match the method to your preferences

Some people prefer a visible, steady option; others want a product they can use when an urge appears, or a prescription conversation about a pill. Consider whether you want OTC access today, whether you can attend follow-up, how you feel about nicotine-containing treatment, and which routines are hardest. Write down two acceptable choices before the appointment so the discussion remains practical rather than all-or-nothing.

A cravings and routines toolkit

Prepare several responses because no single tactic works in every situation. Drink water, chew sugar-free gum, take a short walk, practice slow breathing, text a support person, or leave the place where others are smoking. Change a coffee or driving routine temporarily if it reliably cues a cigarette. Keep alternatives within reach and rehearse them before the quit date.

When an urge rises, pause long enough to name the trigger and choose one response. Contact a coach or friend early rather than waiting until the urge feels unmanageable. Record which strategies helped; that record gives your counselor useful information for the next adjustment.

A practical first-week plan

  1. Today: Write down your smoking patterns and choose a support contact. If you are in the United States, save 1-800-QUIT-NOW.
  2. Within several days: Set a quit date or schedule a clinician/pharmacist or quit-coach conversation. Ask which medicines are suitable and how to use them.
  3. Before the quit date: Remove tobacco supplies, list your top triggers, and prepare alternatives such as water, sugar-free gum, a walk, breathing exercises, or a brief phone call. Confirm where any medicine will come from and read the current label.
  4. On the quit date: Follow the agreed medicine and support plan. Avoid “just one” cigarettes, which can reactivate a routine, and leave situations where others are smoking if you need to.
  5. During the first weeks: Track cravings, sleep, mood, and side effects. Contact your support person or coach when an urge rises instead of waiting until it feels unmanageable. Keep scheduled follow-ups even if the attempt is going well.
  6. After a lapse: Stop the episode, discard remaining cigarettes, identify the trigger, and resume the plan. Tell your counselor or clinician so the strategy can be adjusted; do not treat a lapse as a character verdict.

If you are not ready for a quit date after the first conversation, schedule another date to decide. A plan can be revised; abandoning support because one day went badly is not required.

When to get individualized medical guidance first

The adult cigarette instructions above should not be applied automatically if any of these situations describes you:

  • Pregnancy: CDC encourages behavioral counseling first. Medication requires a careful, case-by-case discussion because the evidence and benefit–harm balance are uncertain.
  • Breastfeeding: Ask a doctor before using cessation medicines.
  • Under 18: Do not use these medicines without a doctor’s advice.
  • Other tobacco products: CDC’s medicine guidance is framed for cigarettes. If you use cigars, chew, snuff, hookah, heated tobacco, e-cigarettes, roll-your-own tobacco, or more than one product, seek tailored help rather than transferring cigarette directions.
  • Complex health or medication history: A clinician or pharmacist should check interactions, contraindications, and product-specific warnings.

FDA advises discussing benefits, risks, and side effects for the specific product you are considering. Seek urgent medical help for severe or alarming symptoms, and report troublesome effects promptly rather than silently stopping or adding medicines. Keep the package and medication list available when you call so the professional can identify the exact product.

What global guidance adds

The World Health Organization’s July 2, 2024 guideline announcement applies to adults trying to quit cigarettes, waterpipes, smokeless tobacco, cigars, roll-your-own tobacco, and heated tobacco products. WHO recommends behavioral interventions and identifies varenicline, NRT, bupropion, and cytisine as effective treatments. Approval, labeling, and availability differ by country, so U.S. OTC and prescription categories cannot be transferred automatically to another health system. See WHO’s 2024 guideline announcement.

WHO reported in 2024 that more than 60% of the world’s 1.25 billion tobacco users—more than 750 million people—want to quit, while 70% lack access to effective cessation services. Those figures describe the global population, not an individual’s chance of success. WHO Director of Health Promotion Dr Rüdiger Krech said: “The immense struggle that people face when trying to quit smoking cannot be overstated.”

If you live outside the United States, ask a local clinician, pharmacist, or tobacco-cessation service which products are authorized and how support is delivered in your country. Cytisine, for example, appears in WHO’s recommendations but is not part of the seven-medicine U.S. CDC list summarized here.

How to decide what to discuss with a professional

Bring a short list to your appointment or quitline call:

  • How many cigarettes you use and when the first one usually occurs.
  • Previous quit attempts, including medicines tried and any side effects.
  • Pregnancy, breastfeeding, age, medical conditions, allergies, and all prescription, OTC, and recreational medicines.
  • Whether you prefer a patch, a short-acting product, or a pill, and whether you can obtain prescriptions.
  • How much counseling, telephone coaching, text support, or follow-up you want.
  • Your insurance, pharmacy, or local-program constraints.

Ask what to do if cravings remain strong, a dose is missed, side effects occur, or you smoke after starting. Make sure you know who to contact and when to review the plan. Ask whether combining counseling with medicine, or a patch with a short-acting NRT product, is appropriate for you rather than assuming it is.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Review and adjust instead of abandoning the attempt

Tobacco dependence can relapse, so ongoing support matters. CDC clinical guidance emphasizes repeated intervention and long-term support. If the method is not controlling cravings, routines are defeating you, or side effects are difficult, contact the clinician, pharmacist, or coach who helped you choose it. They may recommend a different approach, add behavioral support, or revise the follow-up schedule. Do not add, stop, or combine medicines on your own.

A review is also useful when things are going well. Identify which triggers have changed, which supports you actually use, and what situations still feel risky. Keep the plan available for travel, stressful events, or changes in work and home routines. A lapse can become a prompt to strengthen support, not a reason to discard the entire attempt.

Your next concrete move can be small: write down one trigger, call a quitline, or book a medication conversation. That action turns quitting from a general hope into a supported experiment that can be improved over time.

Frequently Asked Questions

What is the best way to start quitting smoking?

Choose one near-term action: set a quit date, call 1-800-QUIT-NOW in the United States, or book a clinician or pharmacist visit. Map your triggers and arrange support; you do not have to decide on medicine before asking questions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is counseling really necessary if I use medicine?

Medicine and counseling each help, and CDC says combining them can more than double quit chances. Counseling adds planning for triggers, stress, cravings, and lapses, so add a quitline, counselor, text program, app, or group when possible.

Can I use a nicotine patch with gum or a lozenge?

CDC materials describe a long-acting patch combined with short-acting gum or lozenge as an approach that can increase quitting chances. Confirm the exact plan and label directions with a clinician or pharmacist, especially if you have health conditions or take other medicines.

Which quit-smoking medicines are available without a prescription in the United States?

The nicotine patch, nicotine gum, and nicotine lozenge are OTC. The nicotine oral inhaler, nasal spray, varenicline, and bupropion SR require prescriptions. Availability, insurance coverage, and local assistance vary.

What if I smoke cigars, use chew, hookah, heated tobacco, or e-cigarettes?

Do not automatically transfer cigarette-medication directions. CDC’s list is framed for adults quitting cigarettes. Tell a clinician or quitline exactly which products you use so the plan can be tailored; WHO’s 2024 guidance addresses several tobacco products globally.

Can I use these medicines during pregnancy or breastfeeding?

Ask a doctor first. CDC encourages behavioral counseling during pregnancy, while medication requires case-by-case consideration because the evidence and benefit–harm balance are uncertain. Breastfeeding also warrants medical guidance before medication.

What should I do after smoking one cigarette during a quit attempt?

Stop the episode, discard remaining cigarettes, identify the trigger, and resume your plan. Tell your counselor or clinician so support can be adjusted. A lapse is information, not proof that the attempt has failed.

How can I make quitting more affordable?

Ask your pharmacist, insurer, and state quitline about current coverage, prices, and free or reduced-cost starter supplies. Offers and eligibility change by location, so verify what is available now. OTC and prescription access differ.

What should I ask before starting a medicine?

Ask whether it fits your health history and other medicines, how to follow the current label, what side effects require a call, what to do after a missed use or lapse, and when follow-up will occur. Do not change or combine medicines without professional advice.

What if cravings remain strong or side effects are difficult?

Contact the clinician, pharmacist, or coach who helped you choose the method. The plan may need different support, a different medicine, or closer follow-up. Do not silently stop, add, or combine products.

The Bottom Line

Start with a concrete step you can take this week: map your triggers, choose a quit date or schedule a quitline or medication conversation, and line up support. For U.S. adults who smoke cigarettes, counseling and FDA-approved medicines are evidence-based, and a professional can help match OTC or prescription options to your health, preferences, and access. Seek individualized advice for pregnancy, breastfeeding, being under 18, other tobacco products, or complex medical histories. If a lapse occurs, learn from the trigger, reconnect with support, and continue.
