The reliable way to stop an AI system from exposing confidential information is to control the entire data path, not just the model’s wording. Minimize what the system can access, enforce user- and task-level permissions, treat retrieved content as untrusted, inspect prompts and context, govern memory and logs, scan outputs before release, and require approval for consequential actions. A system prompt can state “do not reveal secrets,” but it is not a security boundary: prompt injection and other failures can bypass it.

Where sensitive information can enter an AI response

A leak does not require the model to have memorized a secret during training. Confidential material can be supplied at run time or exposed by surrounding software. Microsoft’s guidance on sensitive-information disclosure identifies several paths that must be considered together:

As an Amazon Associate I earn from qualifying purchases.

Exposure path How a leak occurs Controls to apply
User prompt An employee pastes a customer record, credential, contract, or source code into a public or unapproved service. Classify data before submission; prohibit or de-identify restricted data; provide an approved environment.
Retrieval and vector stores A search or retrieval step returns documents the requesting user is not authorized to see, or returns more text than the task needs. Enforce authorization at query time, isolate tenants and projects, minimize retrieved fields, and record retrieval decisions.
Conversation history and caches A later turn, shared cache, or incorrectly keyed session exposes an earlier user’s context. Separate users and sessions, set retention and deletion rules, and test cache keys and access checks.
Persistent memory and agent state An assistant stores a secret or poisoned instruction, then recalls it in another task or for another user. Define what may be written, who may read or delete it, how long it persists, and how stale or malicious entries are corrected.
Tools and connectors An agent can query a database, mail system, file share, or API with a broad service identity and places the result in model-visible context. Use short-lived, scoped identities; grant only task-required permissions; filter tool results before they reach the model.
Logs and telemetry Prompts, retrieved passages, tool results, or generated text are copied into application logs that have wider access or longer retention. Minimize and redact logs, encrypt them, restrict access, and set a documented retention period.
Downstream handling Generated text is rendered as HTML, stored in a database, sent externally, or executed as a command without validation. Use destination-specific encoding, an approved schema, allow-lists, and human confirmation for high-impact actions.

Think of the model as one component in a chain. A clean prompt-only test says little if retrieval, memory, tools, or downstream code can still expose data.

1. Set a data policy before anyone uses an AI workflow

Classify information by permitted use

Give employees and developers an unambiguous classification such as public, internal, confidential, and highly restricted. For each class, state whether it may be sent to an AI service, only to an approved private deployment, or only after redaction or de-identification. Include examples: account numbers, authentication tokens, health details, legal advice, unpublished financial results, and personal identifiers should not be pasted into an unapproved chatbot.

Tell users that a prompt is a disclosure to the service receiving it. “It is only a question” does not change the handling of the data included in that question. Provide a sanctioned tool and a clear escalation route for legitimate sensitive work; a prohibition without a workable alternative encourages shadow use.

Keep secrets outside model context

Do not put passwords, API keys, private certificates, or long-lived access tokens in prompts, system instructions, retrieval documents, or memory. Store them in a secrets manager and let a narrowly scoped backend operation use them without exposing their value to the model. Replace personal or commercial identifiers with stable pseudonyms when the task does not require the real values, and restore identities only in a controlled post-processing step.

Minimize every request

Send the smallest time range, record set, and field set that can answer the question. Limit context length and retrieval count where possible. A model cannot reveal data it never receives, and reducing context also reduces the impact of an injection hidden in an irrelevant document.

2. Enforce identity-aware access for retrieval, memory, and tools

Authorize the requesting user, not just the application

A common failure is allowing an application’s broad service account to retrieve documents on behalf of every user. The retrieval layer must evaluate the requesting user’s permissions, tenant, project, and current task. Carry those attributes into vector-store filters, document APIs, memory reads, and tool calls. If a user loses access, subsequent retrievals and memory reads must stop immediately rather than relying on an old index or cached result.

Separate users, sessions, and tasks

  • Use unguessable, independently scoped session and conversation identifiers.
  • Prevent one tenant, workspace, or browser session from reading another’s history or cache entries.
  • Pass an explicit task purpose to retrieval and tools, then reject requests outside that purpose.
  • Do not let a model choose its own authorization scope or elevate a permission through natural-language instructions.

Use least privilege for agent tools

Give each tool a separate identity with only the operations it needs. Prefer short-lived credentials and read-only access for analysis. Separate “draft an email” from “send an email,” and “propose a database change” from “apply the change.” The model may request an operation, but application code should verify identity, parameters, and policy before execution.

Govern persistent memory

Define an allow-list of information that may be remembered. Set retention, deletion, correction, and export rules; identify who can administer those functions; and maintain lineage showing when an entry was written and which source created it. Monitor reads as well as writes. A poisoned or stale memory item can cause repeated disclosure even after the original conversation is gone.

3. Treat external content as untrusted data

Understand direct and indirect prompt injection

Direct injection is an instruction in the user’s message that attempts to override policy. Indirect injection is hidden in a web page, email, document, image description, or retrieved record that the assistant is asked to analyze. The content can tell the model to ignore its rules, reveal hidden context, call a tool, or send data elsewhere. Microsoft’s prompt-injection guidance recommends layered controls because filtering alone can miss an attack.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Separate instructions from content

Mark retrieved material as untrusted data in the application’s message structure and tell the model to summarize or extract from it rather than obey it. This is a helpful constraint, not a guarantee. Never place secrets in the same context merely because an external document asks for them, and do not allow document text to redefine the user, tenant, or tool permissions.

Reduce what an injected instruction can do

  • Retrieve only documents the user is already allowed to access.
  • Strip unnecessary metadata, hidden fields, and embedded instructions before retrieval results reach the model.
  • Disable network, file-write, message-send, and code-execution tools for read-only tasks.
  • Require an independent policy check and, for sensitive actions, a human confirmation before execution.
  • Rate-limit repeated extraction attempts and alert on unusual cross-user or cross-tenant queries.

Assume an input filter can fail. Security comes from limiting privileges and requiring validation, not from claiming that every malicious string will be detected.

4. Inspect the complete data path

Build checks at each boundary. Microsoft’s sensitive-information and DLP guidance describes controls for prompts, context, memory, tool results, outputs, and audit events; applying a check only at the final screen leaves earlier copies unprotected.

  1. Before submission: classify the user’s text and attachments. Block prohibited categories, warn on risky content, or route the request to an approved environment. Remove secrets and unnecessary identifiers.
  2. Before retrieval: enforce the user’s authorization, tenant and purpose filters. Return only the minimum passages and fields needed.
  3. Before memory writes: scan proposed entries, reject credentials and restricted personal data, attach an owner and retention period, and record provenance.
  4. Before memory reads: re-check authorization and task scope. Do not assume that an item was safe to store merely because it is already present.
  5. Before tool calls: validate the tool, identity, arguments, destination, and expected side effects. Use a separate approval path for external transmission, access changes, code execution, or high-impact decisions.
  6. Before release: scan the complete generated response and any tool-produced text for secrets, personal information, regulated data, and organization-defined confidential markers. Block, redact, or send for review according to policy.

Use layered outcomes rather than a single pass/fail decision: allow low-risk content, redact a recoverable identifier, warn and ask the user to revise, or block and create an incident record.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

5. Validate and safely render every output

Match validation to the destination

Text that is safe in a plain-text chat may be dangerous in a browser, database, shell, or email template. Encode or sanitize output for its exact destination. Escape HTML before inserting model text into a page; use parameterized queries for databases; and never pass free-form model text directly to a shell, interpreter, access-control expression, or policy engine.

Constrain machine-readable responses

For workflows consumed by software, require a narrow, versioned schema with explicit types, required fields, length limits, and allowed values. Reject unknown fields, malformed data, and values outside policy ranges. Parse and validate independently of the model; a statement that the output is “JSON” is not validation.

Keep consequential actions behind confirmation

Sending information outside the organization, changing permissions, deleting records, executing code, or making a high-impact decision should require an explicit confirmation step. Show the user the destination, payload, and relevant provenance before approval. Preserve enough of the request, retrieved sources, policy decisions, and final action to reconstruct what happened without retaining unnecessary sensitive content.

These output-safety practices are also described in Microsoft’s “Output Safety and Downstream Handling” guidance, published August 1, 2026. Features, retention settings, and regional availability of specific services change over time, so verify current contractual and technical behavior before deployment.

6. Log and monitor without creating a second leak

Record decisions, not indiscriminate copies

Useful audit events include the actor, tenant, session, retrieval and memory events, policy decision, scan result, approval, destination, and final action. Store hashes, category labels, or short redacted excerpts where full content is not required. If investigators need the original payload, place it in a separately protected store with a justified retention period.

Protect the audit trail

  • Restrict log access by role and separate application operators from investigators who can view sensitive evidence.
  • Encrypt logs in transit and at rest, and document deletion dates.
  • Redact credentials, personal identifiers, and regulated data before they enter general telemetry.
  • Monitor who searches or exports logs; audit data is itself a sensitive data store.

Alert on meaningful signals

Alerts should cover repeated attempts to extract restricted terms, cross-user retrieval, anomalous memory reads, sudden increases in blocked outputs, unexpected destinations, and tools invoked outside their normal purpose. Track false positives and policy overrides so controls can be tuned without silently weakening them.

7. Test attacks and leakage scenarios end to end

Include both direct and indirect attacks

Create test cases in which a user asks for hidden instructions, a retrieved document tells the assistant to disclose context, an email contains an exfiltration request, and a memory entry contains a poisoned rule. Test attempts to cross tenants, read another session, retrieve deleted content, and induce a tool to send data externally.

Verify failure behavior

  • Unauthorized retrieval returns no data rather than a partially redacted document that reveals its existence.
  • A blocked prompt is not copied into an unrestricted log or analytics stream.
  • Output scanners catch secrets in normal prose, code blocks, attachments, and tool results.
  • Schema violations, unknown fields, and destination mismatches stop downstream processing.
  • Human approval cannot be bypassed by a second tool call or a retry with altered wording.

Exercise incident response

Define who can disable a connector, revoke a tool identity, purge memory, preserve evidence, notify affected parties, and restore service. Test those steps periodically. OWASP’s 2025 “LLM02 Sensitive Information Disclosure” lists disclosure as a major risk and notes that system-prompt restrictions may be bypassed through prompt injection or other methods; a clean prompt test therefore does not establish safety.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Controls by deployment type

Scenario Minimum practical controls Controls that become essential at larger scale
Small, non-sensitive personal task Do not submit secrets or restricted personal data; minimize and de-identify input; use a service you are authorized to use. Documented retention and deletion settings, local redaction, and a separate approved environment for sensitive work.
Enterprise assistant using internal data Identity-aware retrieval, tenant and session isolation, approved service configuration, DLP, retention rules, output scanning, and audit access controls. Memory lineage, connector-specific identities, approval workflows, incident response, continuous attack testing, and monitored policy overrides.
Custom agent or AI application Minimal context, scoped tools, untrusted-content handling, schema validation, destination-safe rendering, and human confirmation for consequential actions. Independent policy services, secrets isolation, full-path telemetry, red-team tests, rollback controls, and formal change management.

How to choose a guardrail or DLP implementation

Whether controls are built in-house or supplied by a platform, compare capabilities rather than marketing labels. Microsoft names Purview DLP and Information Protection as examples for classification and policy enforcement, and describes Azure AI or other approved enterprise deployments for governed sensitive workflows; these are examples, not universal requirements.

Decision axis Questions to answer
Coverage Does it inspect prompts, attachments, retrieval, memory, tool output, generated output, and logs, or only one channel?
Enforcement Can it block, redact, warn, require approval, and audit separately?
Timing Do checks run before prompt submission and before output release, with controls around retrieval and tools?
Identity and tenancy Can policies follow user, tenant, role, device, project, and task instead of trusting one broad application identity?
Retention and access Who can read, export, correct, and delete prompts, memory, and logs, and for how long are they retained?
Downstream integration Can it enforce schemas, safe rendering, destination allow-lists, and approval for external actions?
Transparency Can operators inspect why a decision was made, measure false positives, and test changes safely?

No cross-vendor performance ranking is established here. Confirm current features, plan limits, geography, retention behavior, and contractual terms directly with the provider before relying on them.

A practical rollout sequence

  1. Inventory every AI entry point, connector, memory store, cache, log, and downstream action.
  2. Classify the data each path can handle and prohibit unapproved sensitive inputs.
  3. Remove secrets from prompts and context; implement redaction and de-identification.
  4. Enforce user-, tenant-, task-, and session-scoped retrieval and tool permissions.
  5. Define memory write/read, retention, deletion, and correction policies.
  6. Add untrusted-content handling and reduce tool permissions for retrieval-based workflows.
  7. Deploy prompt, context, memory, tool, output, and log scanning with documented block, redact, warn, and approval outcomes.
  8. Validate schemas and destination-specific encoding; place human confirmation before consequential actions.
  9. Protect and minimize audit data, then alert on extraction and cross-boundary anomalies.
  10. Run direct and indirect injection tests, verify incident playbooks, and reassess whenever models, connectors, or retention settings change.

What a secure failure looks like

A well-designed assistant does not merely say “I can’t help with that.” It prevents unauthorized data from entering context, refuses or redacts a risky request, records a minimized policy event, and leaves no alternate route through memory, logs, tools, or downstream rendering. If a user is authorized and the task is legitimate, the same controls should provide a clear approval path rather than forcing unsafe workarounds. That combination—least privilege, untrusted-input handling, full-path inspection, safe output processing, and accountable monitoring—is what prevents sensitive information from appearing in AI outputs.
