For Microsoft Entra, that means maintaining at least two cloud-only emergency accounts, authenticating them with methods that do not share normal administrator dependencies, excluding them from sign-in policies that could block them, keeping their credentials under controlled but reachable custody, and alerting on every use.
This is a continuity control, not a shortcut around security. The accounts should be used only when ordinary administration is unavailable, tested at least every 90 days, and reviewed after each exercise or real incident. Entra-specific settings below must be adapted to the identity provider, policies, and recovery procedures your organization actually operates.

What privileged account lockout means in an outage
A privileged lockout occurs when nobody can reach an account with enough authority to repair identity, access, or policy controls. The outage may be in your identity provider, a federation service, an authentication factor, a network, or the approval workflow used to activate an administrative role. Applications and infrastructure can remain online while the identity control plane becomes unreachable.

- Federated administrators cannot sign in because the external identity provider is unavailable.
- An administrator cannot complete multifactor authentication because a phone, token, or supporting service is lost.
- A former administrator’s account is disabled or cannot be recovered through on-premises systems.
- A disaster takes mobile, internet, or other required networks offline.
- Every Global Administrator or Privileged Role Administrator assignment is eligible through Privileged Identity Management, but no one can approve activation.
- A Conditional Access or other access-policy change blocks the only remaining administrators.
Relying on one control, such as one MFA channel or one trusted network location, creates a single failure dependency. Emergency access removes that dependency without making routine administration less controlled.

Build the emergency-account foundation
Create at least two independent accounts
Microsoft’s current Entra guidance recommends at least two emergency access accounts. Two provides a second path if one credential, device, or account is unavailable; it also lets one responder use an account while another investigates the incident.
For Entra, make these accounts cloud-only and use the tenant’s .onmicrosoft.com domain. Do not federate or synchronize them from on-premises identity infrastructure. If the same directory, federation server, or synchronization process fails, an account sourced from it will fail with the normal administrator population.
Keep cloud emergency access separate from any on-premises emergency account. A cloud account should not depend on an on-premises system that could be affected by the same event. Other identity providers may use different terminology and controls, so follow their current recovery documentation rather than copying Entra settings literally.

Microsoft recommends assigning the Entra emergency accounts the Global Administrator role and keeping that assignment permanently active. An eligible assignment that requires Privileged Identity Management activation or a second person’s approval is not an escape path when the approval service or all approvers are unavailable.
Use dedicated accounts, not accounts belonging to named employees. Record an authorized responder list separately, so staff changes do not require changing the emergency account identity. Restrict the accounts to emergency administration; do not use them for daily work, email, software sign-ins, or automation.

| Design choice | Emergency-account approach | Reason |
|---|---|---|
| Account source | Cloud-only Entra account in the .onmicrosoft.com domain |
Avoids dependency on federation and synchronization that may be down. |
| Number | At least two | Preserves a second route if one account or credential is unavailable. |
| Role | Global Administrator | Provides authority to repair identity and access controls. |
| Role state | Permanently active, not only eligible through approval | Removes dependence on a working activation or approval path. |
| Ownership | Dedicated emergency identities, with an external authorized-user list | Prevents staff turnover from silently removing access. |
Choose authentication that fails differently
The emergency factor must not share every dependency with normal administrator sign-in. Microsoft recommends FIDO2 passkeys or certificate-based authentication for Entra emergency accounts, with a strong method different from the one used by ordinary administrators. Evaluate the identity provider’s support, registration process, device requirements, and recovery behavior before selecting one.
| Method | What to verify | Typical independence questions |
|---|---|---|
| FIDO2 passkey | Tenant support, registration, responder access to the authenticator, and compatibility during a network or device incident. | Does it avoid the phone, authenticator app, cellular carrier, or federation service used by normal administrators? Can two authorized responders retrieve and use the registered security key? |
| Certificate-based authentication | An operating public key infrastructure, certificate lifecycle, revocation behavior, compatible emergency devices, and protected private-key storage. | Will the issuing CA, certificate repository, device-management service, and network still be reachable during the outage? |
A physical security key can be appropriate when FIDO2 is selected, but compatibility and registration must be checked against the specific tenant. No manufacturer or model is universally endorsed by the guidance. Keep at least one registered factor available to authorized responders without tying it to an employee-owned phone.
For broader workforce resilience, Microsoft describes using multiple authentication methods and communication channels, such as an internet-based authenticator, an on-device OATH token, and SMS. Those are examples of resilience for users generally, not a blanket instruction to put every method on a highly privileged emergency account. Select the smallest set that gives a genuinely independent, testable path.
Prevent access policies from defeating the escape path
Review Conditional Access exclusions
In Entra, exclude the emergency accounts from Conditional Access policies that block or restrict sign-in. A policy requiring a compliant device, a particular location, a normal MFA route, or another unavailable signal can lock out the account during the incident it was designed to solve.
Report-only policies do not block access and therefore do not need an exclusion for this reason. Still, review their eventual enforcement plan so a future policy change does not accidentally remove the emergency route.

Prepare a contingency policy
Maintain a documented contingency configuration for the policies most likely to fail or require urgent change. Define which authorized responder may enable it, what evidence qualifies as an emergency, which protections are temporarily relaxed, and how the standard configuration is restored. Test the planned configuration before relying on it.
Microsoft’s resilient-control guidance says contingency policies should be assessed against business continuity, operational and financial cost, and security risk. Treat an exclusion as a narrowly governed exception: keep it limited to the emergency identities, record every change, and remove temporary changes after recovery.
Store credentials so they are secure and reachable
Emergency credentials have exceptional power, so access should be restricted to authorized people. At the same time, a credential locked inside the failed system, on one person’s phone, or in one office is not an emergency control.
- Keep the authorized-user list and retrieval procedure outside the identity system that may be unavailable.
- Use secure, separate storage locations; Microsoft gives fireproof safes in separate sites as an example.
- Do not tie the only factor to an employee-supplied phone or personal account.
- Store physical authenticators and recovery information with tamper-evident handling appropriate to their risk.
- Document who may retrieve a credential, how identity is verified, how the event is logged, and when the item must be returned or rotated.
- Ensure multiple members of the administration team can retrieve the material without requiring one absent person.
The UK National Cyber Security Centre advises discussing cloud-provider account-recovery options before they are needed. If normal security-enabling systems are unavailable, responders may have to use an unmanaged device or provide fewer usual zero-trust signals; the compensating control is rapid detection and investigation, not pretending the normal posture still exists.
Monitor every use and make routine use uncomfortable
Configure alerts for every emergency-account sign-in and relevant audit activity. Alerts must go to administrators or operations personnel other than the person using the emergency account. A sign-in should create an incident record, identify the responder, and prompt verification that the event is authorized.

James L, a senior security researcher at the UK National Cyber Security Centre, wrote in Protecting how you administer cloud services on 21 June 2023: “When any emergency access is triggered (such as signing in to an emergency administrator account), it should send immediate alarms to your operations personnel so that they can investigate.” Immediate notification both limits dwell time if the account is compromised and discourages casual use.
Review sign-in and audit logs after each use. Confirm the responder performed only the actions needed to restore administration, rotate or replace exposed credentials when appropriate, and close any temporary policy exception. Keep an evidence trail even when the use was part of a planned exercise.
Test the complete recovery path every 90 days
Microsoft specifies validating emergency-account functionality at least every 90 days. A password or key inventory check is not enough; the exercise must prove that an authorized person can retrieve the credential, authenticate, receive the alert, and perform a harmless administrative task.
- Announce the exercise to the small group responsible for monitoring, while preserving the same approval and logging discipline used for a real event.
- Verify that the account remains enabled, has the intended permanently active role, and is still excluded from blocking Conditional Access policies.
- Have an authorized responder retrieve the credential and authenticator from the documented storage location without using the ordinary administrator’s phone or account.
- Sign in through the emergency authentication method and confirm that the sign-in alert reaches a separate operations channel.
- Perform a harmless, reversible administrative action, such as viewing configuration or making a controlled test change, then restore the original state.
- Review sign-in and audit records, record the elapsed time and any dependency discovered, and close the exercise as an incident or test according to your process.
- Update the authorized-user list, storage instructions, contact details, and contingency policy notes when anything has changed.
Re-run the test after changes to identity policies, authentication methods, federation, staff, device management, network design, or provider configuration; do not wait for the next 90-day date when a material dependency has changed.

Use a written break-glass runbook during an incident
A concise runbook prevents responders from improvising under pressure. Keep a printed or otherwise independently reachable copy with the credential-retrieval instructions. It should contain:
Best Value
- FIDO2 & Passkey Ready: Business-ready and FIDO2 L1 certified. This key is supported by major management suites and is ideal for both individual and enterprise deployment. Works seamlessly with Gmail, Facebook, GitHub, Dropbox, Coinbase, and more.
- Universal Connectivity (USB-A ): Features a built-in USB-A connector—simply unfold the key and plug it into your compatible PC or laptop for seamless authentication on the go.
- Dedicated Manager App: Use the Thetis Manager App for the initial hardware PIN setup. Setting the PIN on the device first ensures a smooth registration process. Once the PIN is configured, you can begin registering the key across your favorite FIDO2-compatible online services.
- Ultra-Durable & Portable: Featuring a rotating metal cover, this key is water, crush, and tamper-resistant. It fits easily on a keychain and requires no batteries or network connectivity.
- Check FIDO2 compatibility before purchase - Known limitations: ID Austria is not supported (requires FIDO2 Level 2). Windows Hello login only works with Windows Enterprise editions that support Entra ID, and NFC is NOT supported.
- The conditions that qualify as an emergency, such as an identity-provider outage, unavailable MFA service, lost network, restrictive policy error, or failed role-approval path.
- The names or identifiers of the emergency accounts and the role each should hold.
- The authorized responder list and the independent method for verifying a responder’s identity.
- The storage locations, retrieval sequence, and dual-control or sign-out requirements your organization uses.
- The authentication method, required physical items, supported device type, and fallback recovery contact.
- The alert destination, incident number format, and person responsible for monitoring.
- The exact temporary policy or role changes that may be made and the restoration steps.
- The credential-rotation, evidence-preservation, and post-incident review requirements.
The runbook should tell responders to stop using the emergency account once ordinary administration is restored. It should also state who decides that recovery is complete, so an emergency exception does not become a permanent alternate operating mode.
Match the response to the failure
| Observed failure | Why ordinary administrators are locked out | Emergency design that addresses it |
|---|---|---|
| Federation outage | Federated accounts cannot obtain a token from the unavailable identity provider. | Cloud-only emergency accounts that do not depend on federation. |
| MFA device or service outage | The required phone, authenticator, or supporting service cannot complete the challenge. | A separately registered FIDO2 or certificate method with independent custody and tested devices. |
| Lost administrator or disabled account | Recovery depends on an absent person or an on-premises directory that cannot be reached. | Dedicated emergency identities and a current authorized-user list held outside that directory. |
| Network or disaster disruption | Normal mobile, office, or trusted-network signals are unavailable. | Credentials and authenticators stored in separate locations, with a provider-supported recovery path that can operate under degraded conditions. |
| Approval workflow failure | Every privileged assignment requires an approver or activation service that is unavailable. | Permanently active emergency roles rather than assignments that are only eligible through approval. |
| Restrictive policy change | Conditional Access or another control blocks all normal administrator sign-ins. | A narrowly governed emergency-account exclusion and a preplanned contingency policy. |
Balance availability against the risk of a superuser account
Emergency accounts are attractive targets because a stolen credential can alter identity, access, and audit controls. Microsoft security guidance highlights insecure credential custody, misuse, inadequate auditability, and untested accounts as material risks.
Use layered safeguards rather than removing the accounts:
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problems- Use a strong authentication method that is independent of normal administrator access.
- Limit knowledge and physical custody to authorized responders.
- Alert immediately and send alerts to a separate operations group.
- Review every sign-in and administrative action after use.
- Make ordinary use inconvenient enough that staff choose normal named accounts for routine work.
- Exercise the process on schedule and after material configuration changes.
- Rotate or replace credentials when exposure is suspected or custody changes.
The NCSC’s point that emergency access should be “irritating enough” to prevent habitual use is practical: a controlled retrieval and alert process preserves availability without turning a superuser identity into a convenient shared login.
Adapt the pattern to other identity providers
The principles are portable, but Entra’s exact settings are not universal. For another cloud identity provider, confirm all of the following in its current documentation:
- Whether an account can be created outside federation, synchronization, or a single sign-on dependency.
- Which role provides sufficient recovery authority and whether it can remain permanently active.
- Which independent authentication methods are supported and how they are registered, revoked, and recovered.
- How policy exceptions, trusted locations, device requirements, and MFA rules apply to emergency identities.
- How provider support performs account recovery when normal administrators and factors are unavailable.
- Which audit events can be exported and which alerting channels remain reachable during an outage.
Do not assume that an Entra cloud-only account, Global Administrator role, or Conditional Access exclusion has an equivalent name or behavior elsewhere. Map the failure dependencies, document the provider-specific implementation, and test the resulting path with authorized responders.
Quick Recap
Implementation checklist
- At least two emergency accounts exist before an outage.
- Each cloud emergency account is independent of federation and synchronization where the provider supports that model.
- The emergency role is permanently active and sufficient to repair identity and access configuration.
- Authentication uses a strong method with failure dependencies different from ordinary administrator sign-in.
- Factors, devices, and recovery information are compatible with the tenant and stored in secure, separate locations.
- Blocking or restrictive policies cannot accidentally deny the emergency accounts.
- A contingency policy and restoration procedure are documented and tested.
- Authorized users, retrieval steps, incident contacts, and provider recovery options are current.
- Every sign-in generates an immediate alert to a separate operations group.
- The complete retrieval, sign-in, harmless-action, alert, logging, and restoration path is validated at least every 90 days.
- Each exercise or real use produces a review, and credentials are rotated when exposure or custody changes warrant it.
