Most people do not need to fast for a PSMA PET/CT and can usually take their regular medicines. Drink water as your imaging center directs, arrive with a list of medicines and relevant medical history, and expect an intravenous radiotracer injection followed by an uptake wait of about an hour and scanning of roughly 30 minutes. Check-in, preparation and scheduling can make the full visit considerably longer.
Your center’s appointment letter is the final authority. Local protocols, the tracer being used and your individual circumstances can change the general instructions below.

What a PSMA PET/CT examination is
A PSMA PET/CT combines two types of imaging in one appointment. A small amount of radiotracer is injected into a vein. The tracer circulates and binds to prostate-specific membrane antigen (PSMA) proteins. The PET portion detects where the tracer accumulates, while the CT portion supplies anatomical detail that helps the specialist locate that signal in the body.
The examination may be ordered to help evaluate prostate cancer or to answer a specific question after diagnosis or treatment. It is an imaging study, not a stand-alone diagnosis. Tracer uptake can occur in normal organs, and uptake by itself does not prove that a finding is cancer. Very small tumors may also be below the scan’s ability to show them. A nuclear medicine physician or radiologist interprets the images and prepares a report; your treating clinician considers that report alongside your symptoms, PSA and other clinical information.

Start with the imaging center’s instructions
Preparation advice differs slightly between facilities because centers may use different tracers, appointment workflows and patient-screening procedures. Read the appointment letter when you receive it and call the imaging service if any instruction is unclear or conflicts with advice you have received elsewhere.
| Question | General guidance | What takes priority |
|---|---|---|
| Do I need to fast? | Patients generally do not need to fast. | A written or verbal instruction from your imaging center, including any direction based on your medical circumstances. |
| Can I take medicines? | General guidance says usual medicines may be taken. | Specific instructions from the center or the clinician who manages a time-sensitive medicine. |
| How much should I drink? | Adequate hydration is encouraged. | The center’s directions about how much and when to drink; do not assume one universal volume or schedule. |
| What should I wear? | Comfortable clothing is sensible. | Any clothing or changing instructions supplied by the facility. |
The 2023 joint EANM/SNMMI guideline states: “Patients do not need to fast and may take all their medications.” That sentence describes general preparation, not a guarantee that every appointment will use identical instructions. If your center tells you to do something different, follow the center’s protocol and ask why if you need clarification.
How you prepare before the appointment
Review the appointment details
Confirm the date, arrival time and location. Some medicines are time-sensitive, and a particular injection time may be reserved for you. If you take a medicine that must be given at a set time, contact the imaging service before the appointment rather than changing the timing yourself.

Hydration is commonly encouraged for PSMA PET/CT preparation. The professional guideline recommends adequate hydration, but individual instructions can differ. Follow the center’s stated timing and amount. If you have a condition that affects how much fluid you can drink, ask the ordering or imaging team for patient-specific advice instead of applying a generic drinking plan.
Continue or clarify medicines
Do not stop a prescription, nonprescription medicine, vitamin or supplement simply because you are having a scan. General guidance allows usual medicines, but the imaging team needs to know what you take and whether any medicine requires a fixed schedule. Bring a current list, including doses when available, or have the list accessible on your phone.

Tell the team about allergies and previous reactions
Before the appointment, report allergies, previous reactions to injections or imaging procedures, and any concern you have about the planned radiotracer or IV. This information lets staff review the protocol and decide whether additional preparation or monitoring is needed.
Report implants, metal and other practical issues
Tell the center about implanted medical devices or metal in your body. Also mention claustrophobia or difficulty remaining in one position. The team can explain the scanner, positioning and available support before you begin. If a medicine to help with anxiety is discussed, ask whether you will need someone else to drive you; do not assume you can drive after taking a sedating medicine.
Make sure your history is available
The referring team should provide information needed to interpret the examination. Relevant details can include your prostate-cancer diagnosis and risk information, PSA history, previous prostate-cancer treatments, current symptoms, earlier imaging and important coexisting conditions. If you are unsure whether a report or treatment date was sent, ask the referring office to confirm.
Choose practical clothing and arrival plans
Mayo Clinic’s patient guidance advises comfortable clothing. Wear something that allows you to lie still without discomfort and be prepared to follow local instructions about changing or removing items. Allow time for registration, screening and the possibility that the appointment runs longer than the scan itself. If you need an interpreter, mobility assistance or another accommodation, arrange it with the center in advance.
What happens during the appointment
| Stage | What usually happens | Approximate timing or variation |
|---|---|---|
| Check-in and screening | Staff confirm your identity, referral and relevant health information, then review the local preparation instructions. | Timing varies by facility and by the information that must be confirmed. |
| IV placement | A technologist places an intravenous line. | Usually a short preparation step; difficult veins can take longer. |
| Radiotracer injection | A small amount of PSMA-targeting radiotracer is injected through the IV. | The exact tracer and injection workflow are determined by the center. |
| Uptake period | You wait while the tracer circulates and binds to PSMA proteins. | SNMMI describes about one hour; the center may use a different interval. |
| PET/CT acquisition | You lie on a table that moves through the scanner while PET and CT images are obtained. | SNMMI says the scan usually takes about 30 minutes. |
| Completion | Staff check that the required images have been obtained and tell you whether you may leave. | Local workflow determines the exact discharge process. |
Check-in and final screening
At arrival, provide the information the center requests and repeat any concern about medicines, allergies, implants, metal or claustrophobia. Staff may verify your appointment time because the radiotracer schedule is coordinated with imaging. Ask questions before the injection if you are unsure what will happen next.

IV placement and injection
The technologist places an IV and administers a small amount of radiotracer. Tell the technologist immediately if the IV site becomes painful, swollen or unusually uncomfortable. The tracer then circulates through your body during the uptake period.

The uptake wait
SNMMI describes an uptake wait of about an hour. This is not idle time in the appointment: it allows the tracer to distribute and bind to PSMA proteins before images are acquired. The center may ask you to remain in a designated area and may give instructions about drinking or using the restroom. Follow those local directions rather than assuming every facility handles the wait in the same way.
Positioning for PET and CT
For imaging, you lie on a table that moves through the scanner. The practical requirement is to follow the technologist’s positioning instructions and remain as still as you comfortably can while images are collected. Tell staff before the scan if pain, anxiety or another problem could make it difficult to hold the required position; they can explain the options available under that center’s protocol.
How PET and CT complement each other
PET identifies areas where the radiotracer accumulates. CT shows anatomical structures and helps the specialist determine where that activity is located. The combined study gives the interpreting physician both tracer-distribution information and structural context.
How long should you plan to be there?
The two patient-facing estimates most often cited by SNMMI are about one hour for tracer uptake and about 30 minutes for scanning. Those figures do not include travel, registration, health-history review, IV placement, waiting for the scheduled injection, bathroom breaks or any additional images the technologist determines are needed. Treat them as approximate process times, not a promise of total visit length.

Leave enough flexibility that a delayed start will not create a problem. If you have work, transportation or childcare arranged, use the appointment letter’s arrival time and ask the imaging center how much additional time patients should normally allow. A time-sensitive radiotracer schedule means arriving late can require the center to reschedule rather than simply shortening the visit.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Questions to ask before you leave home
- Does this appointment require fasting, or may I eat normally?
- Should I take every regular medicine at its usual time?
- When should I start drinking water, and is there a center-specific amount?
- Do you need a complete medicine and supplement list, prior imaging or treatment records?
- Should I report any implanted device, metal, allergy or previous injection reaction in advance?
- What should I do if I am claustrophobic or may need help staying still?
- If anxiety medicine is prescribed, will I need another person to drive me?
- What arrival time should I use, and how long should I reserve for the entire visit?
Writing the answers in your phone or on the appointment letter can prevent last-minute uncertainty. Do not independently alter medicines or fluid intake to fit a generic checklist.
After the scan: what happens next
Leaving the imaging department
Once the required images are complete, staff tell you whether the appointment is finished and whether there are any center-specific instructions. If you took a medicine for anxiety, follow the driving and supervision instructions you were given. If you did not receive such medicine, your ability to leave and resume activities is governed by the imaging center’s directions and your own condition.
Image interpretation
An imaging specialist, such as a nuclear medicine physician or radiologist, reviews the PET/CT images and writes a report. The report may describe where tracer uptake is seen and how those areas correspond to anatomy. It is not the same as a treatment recommendation.
How your clinician uses the report
Your treating doctor interprets the report together with your broader health information, which may include PSA results, symptoms, previous treatment, pathology and earlier imaging. If a finding is unclear, the clinician may recommend further evaluation. The next step therefore depends on the whole clinical picture, not on a single phrase in the scan report.
Best Value
Why an area of uptake is not automatically cancer
PSMA is present in normal organs and tissues, so normal uptake can appear on a scan. Conversely, very small tumors may not be visible. These two limitations are why the scan must be interpreted by a qualified specialist and discussed with the clinician who knows your history. Do not treat a highlighted area, a preliminary comment or a number viewed in an online portal as a diagnosis without the formal report and clinical explanation.
Preparation problems and practical fixes
You were told not to fast, but the center’s letter says something different
Follow the center’s written instruction and call if it is unclear. General guidance is not a substitute for a protocol tailored to the tracer, equipment or your circumstances.
You forgot to mention a medicine, supplement or allergy
Tell the staff as soon as you arrive, before the IV and injection. Do not conceal an item because you think it is unrelated; the team decides what information affects preparation.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesYou cannot drink the amount suggested
Contact the imaging service or the clinician managing the condition that limits your fluids. Hydration advice must be adapted to individual circumstances; do not force fluids or ignore a restriction.
You may be late because of travel or another appointment
Call the center immediately. The radiotracer and scanner schedule may make the appointment time-sensitive, and staff can tell you whether to continue traveling or arrange a new time.
You are worried you will not tolerate the scanner
Tell the team before the scan. Claustrophobia, pain or difficulty lying still is easier to address before positioning. If medication is considered, clarify supervision and driving requirements in advance.
You are waiting for results and see that tracer uptake was mentioned
Wait for the specialist’s report and your treating clinician’s explanation. Uptake can be normal, and the scan has limits for very small tumors; a single imaging observation is not a diagnosis.
Quick Recap
A concise appointment checklist
- Read the appointment letter and record the arrival time.
- Ask the imaging center whether its instructions differ from general advice about food, medicines or fluids.
- Prepare a list of prescriptions, over-the-counter medicines, vitamins and supplements.
- Report allergies, prior reactions, implanted devices or metal and claustrophobia.
- Confirm that the referring team has your diagnosis, PSA and treatment history, symptoms and relevant prior imaging.
- Drink only as directed by the center and by any clinician who manages a fluid restriction.
- Wear comfortable clothing and allow time beyond the approximately one-hour uptake period and 30-minute scan.
- At check-in, repeat any new health information and ask questions before the injection.
- During imaging, follow positioning instructions and remain still as comfortably as possible.
- Afterward, wait for the formal interpretation and discuss it with the clinician who ordered the examination.
