A persistent or unusual symptom is easier to evaluate when you arrive with a concise timeline, a complete medication list, relevant records, and your most important questions. Start a symptom diary, describe how the problem affects daily activities, and decide what you want from the visit—such as a working diagnosis, testing, symptom relief, a referral, or a monitoring plan.
During the appointment, be candid even about embarrassing symptoms or medication difficulties. Ask why each test or treatment is recommended, what it cannot show, what the alternatives are, when results will arrive, and what to do if you worsen. Before you leave, repeat the plan in your own words and write down every next step.

Why preparation matters when a symptom will not go away
Symptoms that last for weeks or keep returning can be difficult to describe from memory. A clinician needs more than the name of a sensation: the timing, pattern, triggers, associated symptoms, medicines, previous results, and effect on your life all change the possible explanations and the safest next step.
Preparation also protects limited appointment time. The NHS and Centers for Disease Control and Prevention (CDC) advise putting your main concern and highest-priority questions first. If you spend the opening minutes on less important details, you may not reach the issue that worries you most.

An unexplained symptom is still a real symptom. NHS patient information says about one in four people who see a general practitioner have physical symptoms that cannot be explained; that page displays information reviewed in 2018. Lack of an immediate explanation does not mean the symptom is fabricated or that nothing can be done. It means the plan may need staged testing, symptom management, monitoring, or referral.
Create a one-page symptom summary
Keep the first page short enough to scan. You can bring a printed sheet, a note on your phone, or a notebook. A longer diary can sit behind the summary, but the first page should let the clinician understand the problem in under a minute.
Write the main concern in one sentence
Use plain language and include duration. For example: “For six weeks I have had episodes of light-headedness that last 10 to 20 minutes, usually after standing, and they are making it hard to work.” This is more useful than a label such as “dizzy,” which can mean spinning, faintness, imbalance, or several sensations.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesIf there is more than one symptom, state which one you most need addressed today. You can list secondary concerns underneath it rather than presenting five problems as equally urgent.
Record the timeline and pattern
Write when you first noticed the symptom, whether it began suddenly or gradually, and how it has changed. Note how often it occurs, how long each episode lasts, and whether there are symptom-free periods. Include the most recent episode and the worst episode, not just an average.
| Diary item | What to record | Useful detail |
|---|---|---|
| Onset | Date and circumstances of the first episode | What you were doing, eating, taking, or feeling at the time |
| Frequency | How often it happens | Several times a day, weekly, or unpredictable |
| Duration | How long each episode lasts | Seconds, minutes, hours, or continuous |
| Sensation and severity | Your own words and a consistent severity scale | Describe pressure, burning, weakness, spinning, nausea, or another sensation; note what “7 out of 10” means to you |
| Triggers | What came before the episode | Movement, posture, food, exertion, heat, stress, sleep loss, or a medicine dose |
| Relieving factors | What made it better | Rest, hydration, changing position, eating, or nothing |
| Associated symptoms | Other changes occurring at the same time | For example, palpitations, stomach symptoms, breathlessness, weakness, numbness, or fatigue |
| Functional effect | What you could not do | Missed work, interrupted sleep, reduced mobility, difficulty driving, eating, exercising, or caring for someone |
The CDC’s 2024 checklist recommends: “Prepare a journal or list describing all of your past or current symptoms, how they have affected your daily activities, and anything that made them feel better or worse.” Record observations rather than trying to prove a diagnosis.

Describe function, not only intensity
A symptom rated “moderate” can still be disabling if it prevents sleep or walking. Explain practical consequences: how many hours you sleep, whether you can finish a shift, how far you can walk, whether you avoid meals or activities, and whether someone must help you. These details help the clinician judge urgency and whether treatment is working.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Include your own concern and expectation
Write what you think might be causing the symptom, while making clear that it is a concern rather than a diagnosis. Also state what you hope to accomplish. Your goal might be to identify dangerous causes, obtain reasonable testing, reduce the symptom enough to work, receive a referral, or agree on monitoring if immediate testing is not useful. NHS guidance specifically encourages patients to share their expectations.
Bring a complete medication and health history
Medication information is essential because a prescription, over-the-counter product, vitamin, herb, or dietary supplement can cause or worsen symptoms, interact with another product, or affect test results. Bring the containers when practical, or prepare an accurate list.
| Include | Record |
|---|---|
| Prescription medicines | Name, strength, dose, schedule, reason for taking it, and the last dose if known |
| Non-prescription medicines | Pain relievers, cold remedies, antacids, sleep products, and anything used occasionally |
| Vitamins, herbs, and dietary supplements | Product name, amount, frequency, and when you started or stopped it |
| Allergies and reactions | The substance and what happened, such as rash, swelling, breathing difficulty, or another reaction |
| Recent changes | New medicines, dose changes, missed doses, discontinuations, or problems taking a product as directed |
AHRQ, NHS, and Mayo Clinic all emphasize bringing non-prescription products and supplements, not only prescriptions. Do not omit a product because it seems harmless or unrelated; tell the clinician how often you actually take it.
Assemble diagnoses, procedures, and test information
List relevant diagnoses, surgeries, hospital visits, scans, laboratory tests, referrals, and their dates. Bring reports or result summaries if you have them. If another clinic ordered a test, ask whether the report and images were transferred; do not assume that every office can see another system’s records.
Include your family history when it relates to the symptom or to conditions your clinician may consider. Note major illnesses in close relatives and, when known, their age at diagnosis. Bring your insurance or identification documents if your clinic requires them, but do not delay care because you cannot assemble a perfect file.

Choose and rank your questions
Write two or three questions and put the most important one first. You can carry a longer list, but tell the clinician which questions must be answered today. The NHS advises patients not to be afraid to ask about anything unclear; asking early is especially important when the appointment is short.
Questions about possible explanations
- What are the most likely explanations for this pattern?
- Which serious causes are you checking for, and what findings would make them more likely?
- Could any medicine, supplement, recent illness, or change in routine be causing or worsening it?
- What has already been ruled out by my history, examination, or previous results?
Questions about tests
- Why do you recommend this test now?
- What can the test show, and what can it not show?
- How should I prepare, and are there risks, discomforts, or costs?
- When and how will I receive the result, and whom should I contact if it does not arrive?
Questions about treatment and monitoring
- What self-care or treatment options are reasonable while we investigate?
- What benefit should I expect, how long might it take to work, and what side effects require a call?
- What alternatives are available, including the option of waiting?
- What symptoms should prompt urgent contact, and when should we review the situation?
If you need a test or treatment for a particular reason, say so. Explaining that your goal is to sleep, work, walk, eat, or feel safe helps the clinician weigh options that have different burdens and benefits.
Arrange communication and practical support
Contact the clinic before the appointment if you need an interpreter, communication support, mobility accommodation, hearing or vision assistance, or a longer visit. Describe the support you need rather than assuming the office will infer it.

Tell the clinician who that person is and what help you want. If you want to record audio, ask about the clinic’s recording rules first; do not assume recording is permitted.
Keep your symptom summary and medication list easy to open. If the visit is by phone or video, place the same information nearby and check how the clinic will send orders, results, or a visit summary.
Explain the symptom clearly during the visit
Use a brief opening statement
Start with your one-sentence concern, then give the timeline. A useful sequence is: what happens, when it began, how often it occurs, what brings it on, what helps, what else occurs with it, and what it prevents you from doing. Hand over the one-page summary if the clinician prefers to read while you speak.

- Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
Use your own words for the sensation. If “pain,” “pressure,” “weakness,” or “dizziness” does not fit, say what it feels like and give an example. Mention changes since you booked the appointment, including improvement, worsening, or a new associated symptom.
Be candid about sensitive information
Mayo Clinic states: “Be honest. In order to fully understand your health, your doctor needs accurate information.” Include symptoms that feel embarrassing, emotional effects, alcohol or other substance use when relevant, sleep changes, diet changes, sexual or reproductive details when relevant, and difficulty taking medication. The purpose is safe care, not judgment. If you are worried about privacy, ask who can see the information and how it will be documented.
Ask for plain-language explanations
When you hear an unfamiliar term, ask what it means in your case. You can say, “Could you explain that without medical jargon?” or “What should I remember from that result?” Ask the clinician to write down medication names, preparation instructions, warning signs, and follow-up dates. A written visit summary can prevent errors after you get home.
Compare tests and treatments by purpose and burden
A choice is not automatically better because it is newer or more extensive. Ask the clinician to compare options using the factors below.
| Comparison factor | What to ask |
|---|---|
| Purpose | What question is this option intended to answer? |
| Expected benefit | How likely is it to clarify the cause or improve the symptom? |
| Limits | What will a normal or abnormal result fail to prove? |
| Risks and side effects | What harms, discomforts, interactions, or false alarms are possible? |
| Burden and cost | How much time, preparation, travel, disruption, or expense is involved? |
| Time to information | When will the result or treatment effect be available? |
| Alternatives | Is there another test, treatment, or monitoring approach? |
| Waiting | What could happen if we wait, and what change would make waiting unsafe? |
For treatment, ask how you will know whether it is helping and what to do if it does not. Do not stop or change a prescribed medicine on your own; ask the prescriber about suspected side effects or a safer adjustment.
Confirm the plan before you leave
Use a repeat-back: summarize the plan in your own words and ask whether you understood it correctly. Confirm each item below.

- How and when results will be delivered, and whom to contact if they do not arrive.
- The warning signs that require urgent contact or emergency care.
- The follow-up date or the specific trigger for reassessment.
- The correct phone number, portal route, or other contact method for problems.
NHS guidance recommends checking that everything on your list was covered and knowing what happens next. Write the dates in a calendar before you leave. If the office promised a summary, referral, or result and it does not arrive within the stated time, contact the office rather than guessing what to do.
Track what happens after the appointment
Continue the symptom diary and medication record. Add the date you started a treatment, each dose change, symptom severity, functional effects, and any new reaction. This creates a before-and-after comparison and gives the clinician usable information at follow-up.
Keep copies of test reports, referral details, and instructions in one place. Record the date a result was expected and the date it actually arrived. If a result is difficult to interpret, ask the ordering office to explain what it means and what action, if any, is required.
Recommended Free Tools
Follow the agreed plan, but contact the clinic sooner if symptoms worsen, a treatment causes a concerning reaction, or you cannot follow the instructions. If you believe you are experiencing a medical emergency, use your local emergency service rather than waiting for a routine appointment or portal reply.
What to do when no cause is found yet
Some persistent physical symptoms—including pain, dizziness, fatigue, chest discomfort, palpitations, stomach problems, breathlessness, weakness, and numbness—can remain unexplained after an initial evaluation. NHS information notes that symptoms lasting more than a few weeks may have no clear explanation at first; that uncertainty does not mean they are faked.
Ask what has been ruled out
Request a clear account of what the history, examination, and completed tests make less likely, what remains possible, and what would change the plan. A normal result may answer one question without explaining every symptom, so ask what the result does and does not establish.
Agree on management and a review point
Even without a final label, you can set practical goals such as improving sleep, mobility, work capacity, or the ability to eat. Ask which symptom-management steps are reasonable, how progress will be measured, and when the situation will be reviewed. Put the review date or trigger in your calendar.

Discuss referral or a second opinion
If the symptom persists, interferes with daily life, or falls outside the clinician’s expertise, ask whether a specialist referral or second opinion is appropriate. Bring your timeline, medication list, previous reports, and the questions that remain unanswered so the next clinician can build on, rather than repeat, the evaluation.
Avoid diagnosing yourself from a symptom list
Online symptom lists cannot provide your examination, history, test interpretation, or risk assessment. Use your diary to describe patterns and ask focused questions, not to assign yourself a condition. Tell the clinician which explanation worries you and why, then let the evaluation determine what is supported.
