How to Perform an AI Safety Risk Assessment Before Deploying a Chatbot

Perform a chatbot safety assessment as a documented lifecycle process, not a final accuracy check. Assign accountable owners, map users and data, test ordinary and adversarial behavior, mitigate or formally accept residual risks, then make a written go/no-go decision with monitoring and rollback authority. Reassess after any model, prompt, tool, data, or vendor change.

What a pre-deployment chatbot safety assessment must establish

A safe-enough decision depends on context. The same model may be acceptable for drafting low-stakes internal notes and unacceptable for medical triage, benefits eligibility, financial decisions, or a public service used by children. Before choosing tests or thresholds, document the chatbot’s intended users, tasks, autonomy, data sensitivity, affected people, and the consequences if it is wrong, manipulated, unavailable, or discriminatory.

As an Amazon Associate I earn from qualifying purchases.

The assessment should answer five practical questions:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Who is accountable for the system, its data, its vendor relationships, and launch approval?
  • What can the chatbot do, what must it never do, and what happens when it is uncertain?
  • Which harms are reasonably foreseeable in normal use and misuse?
  • What evidence shows that controls work under representative and adversarial conditions?
  • Who can pause, roll back, or shut down the system when monitoring detects unacceptable risk?

NIST’s AI Risk Management Framework (AI RMF) organizes this work into Govern, Map, Measure, and Manage. The framework is voluntary guidance for organizations designing, developing, deploying, or using AI systems. NIST’s Generative AI Profile, released July 26, 2024, adds guidance for risks that are particularly relevant to generative chatbots. ISO/IEC 23894:2023, published in February 2023, provides guidance for integrating AI risk management into activities involving AI products, systems, and services.

These frameworks do not set a universal chatbot score, accuracy percentage, incident rate, or pass/fail number. Your acceptance criteria must reflect harm severity, user population, legal and organizational requirements, and the system’s operating context.

1. Govern: establish accountability before testing

Governance prevents a technically successful pilot from becoming an unmanaged production system. Create a named owner for each decision and record the authority to delay or stop launch.

Assign decision owners

At minimum, name:

  • Business owner: accountable for the purpose, benefits, budget, and launch scope.
  • Safety or risk owner: maintains the risk register, severity model, test evidence, and residual-risk decisions.
  • Technical owner: controls prompts, model settings, retrieval, tools, authentication, logging, and rollback.
  • Privacy and security reviewers: evaluate personal-data handling, access controls, threat scenarios, and retention.
  • Operations owner: monitors incidents, handles escalations, and coordinates user support.
  • Independent approver: confirms that evidence is sufficient and that conflicts of interest are managed.

For a small team, one person may hold several roles, but the assignments and separation of approval from implementation should be explicit.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Set policies and escalation rules

Write rules for prohibited content, regulated advice, vulnerable users, disclosure of uncertainty, human handoff, data retention, employee access, third-party use, and emergency response. Define what counts as a critical incident, who receives alerts, and the maximum time allowed to acknowledge and contain one.

Specify escalation triggers before launch. Examples include a confirmed disclosure of another person’s confidential data, an unauthorized external action, a pattern of discriminatory refusals, a prompt-injection path to protected instructions, or an availability failure that affects a safety-critical workflow.

Control suppliers and changes

Inventory every model, hosting provider, moderation service, retrieval index, plug-in, external API, and analytics tool. Contracts and service-level terms should address data use, retention, security notifications, model-change notice, audit evidence, and incident cooperation. Put prompts, policies, evaluation sets, model versions, tool permissions, and configuration under change control. A silent vendor model update is a safety-relevant change, not routine maintenance.

Maintain evidence

Keep a versioned system description, risk register, test plans, raw results, reviewer notes, mitigation decisions, approvals, incident records, and rollback instructions. Record the date, model and vendor versions, environment, test data, evaluator identity, and known limitations for every result.

2. Map the chatbot’s context, boundaries, and impact

Mapping supplies the facts needed for an initial deployment decision. NIST describes this stage as producing enough contextual knowledge to inform an initial “go/no-go decision” about whether to design, develop, or deploy an AI system.

Describe the intended use

Write a plain-language purpose statement and an out-of-scope statement. Include the channels (web, mobile, messaging, voice), languages, hours of operation, expected volume, response-time targets, and whether conversations are anonymous or tied to an account.

List each task separately. “Answer product questions,” “summarize a support case,” and “change an account setting” have different risks. For every task, state whether the chatbot only provides information, recommends an action, drafts an action for human approval, or executes an action automatically.

Identify users and affected people

Document age ranges, languages, disabilities, digital-literacy assumptions, geographic coverage, and authentication status. Include people who do not use the chatbot but may be affected by its outputs, such as customers described in a support ticket, employees evaluated through an automated summary, or family members mentioned in a conversation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Map data flows

Trace data from input to storage and deletion:

  1. What the user enters, including attachments, voice recordings, identifiers, and inferred attributes.
  2. What is sent to the model, retrieval system, moderation service, or external tool.
  3. Where prompts, outputs, logs, embeddings, backups, and analytics are stored.
  4. Who can access each copy and for what purpose.
  5. How long each data type is retained and how deletion requests propagate.
  6. Whether data is used to train, fine-tune, evaluate, or improve a supplier’s service.

Mark sensitive fields and prohibit unnecessary collection. Test whether the chatbot repeats secrets from context, exposes one user’s records to another, or reveals hidden system instructions.

List dependencies and failure modes

Draw a dependency map covering the model, system prompt, retrieval sources, identity provider, tools, rate limiter, content filter, monitoring, and vendor services. For each dependency, record what happens when it is slow, unavailable, compromised, or changed. A fallback that silently produces fabricated answers may be riskier than a clear outage message.

Consider benefits, harms, and constraints

Record expected benefits alongside foreseeable harms. Harms can include physical or emotional injury, financial loss, privacy violations, security compromise, discrimination, reputational damage, misinformation, loss of access, and over-reliance by staff. Add applicable laws, contracts, sector rules, accessibility duties, organizational policies, and geographic restrictions to the map.

Risk area Example scenario Evidence to collect Typical controls
Accuracy and fabrication The bot invents a policy, diagnosis, citation, or transaction status. Representative question set; groundedness review; uncertainty behavior. Retrieval boundaries, source display, refusal when evidence is missing, human review.
Harmful content The bot provides dangerous instructions or abusive material. Policy-violation prompts, multilingual tests, severity ratings, reviewer decisions. Content filters, safe-completion policy, crisis escalation, rate limits.
Bias and unfair treatment Different groups receive different quality, refusals, or opportunities. Paired prompts, subgroup metrics, accessibility review, qualitative analysis. Data and prompt fixes, targeted tests, human appeal, restricted use.
Privacy leakage The bot reveals personal data from context, logs, retrieval, or another user. Canary secrets, cross-tenant tests, deletion checks, access-log review. Minimization, isolation, redaction, retention limits, authorization.
Security abuse An attacker extracts instructions, credentials, or system details. Threat modeling, red-team prompts, tool-abuse tests, abuse monitoring. Least privilege, secret isolation, input validation, alerting.
Prompt injection and exfiltration Untrusted retrieved text changes behavior or causes data transfer. Malicious documents, indirect-injection tests, tool-call traces. Trust boundaries, content labeling, tool confirmation, egress controls.
Availability Traffic spikes or a supplier outage blocks a critical service. Load and timeout tests, dependency-failure drills. Quotas, queues, graceful fallback, status communication.
Accessibility Keyboard, screen-reader, language, or cognitive barriers prevent safe use. Assistive-technology testing and user review. Accessible interface, plain language, alternate human channel.

Rate impact according to the actual consequence, not merely the probability. A rare event may require strong controls when the outcome is severe or irreversible.

4. Design a pre-launch measurement plan

Define acceptance criteria before looking at results. Every criterion should state the population, test set, method, severity threshold, reviewer, and action if it fails. Use both automated checks and human review; neither is sufficient alone for nuanced harms.

Prepare representative and adversarial prompts

Build a versioned evaluation set from real, de-identified tasks and foreseeable misuse. Include:

  • Common questions, ambiguous requests, incomplete information, and expected follow-ups.
  • Requests in supported languages, dialects, and accessibility modes.
  • Typos, slang, code-switching, long conversations, attachments, and context limits.
  • Attempts to obtain harmful instructions, protected data, credentials, or restricted advice.
  • Prompt injection in user messages, retrieved pages, files, and tool responses.
  • Conflicting instructions, role-play, emotional pressure, and jailbreak patterns.
  • Requests to take external actions, including unauthorized or irreversible actions.
  • Outage, timeout, rate-limit, stale-index, and malformed-tool scenarios.

Keep a holdout set that evaluators and developers do not use for prompt tuning. Re-run it after every material change.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Test accuracy and grounding

For factual tasks, define what constitutes a correct answer and what evidence must be cited or retrieved. Check whether the bot distinguishes “not found” from “false,” asks for clarification when needed, and expresses uncertainty without giving a misleadingly precise answer. Evaluate both answer quality and the risk of a confident error.

Test harmful-content behavior

Grade outputs by severity and examine the complete conversation, not only the final sentence. A refusal that includes actionable harmful details still fails. Test transformations, translations, encoded requests, multi-turn escalation, and attempts to exploit emotional framing.

Test privacy and data separation

Use synthetic canary values and separate test tenants. Attempt to retrieve another user’s conversation, infer hidden attributes, recover deleted content, and expose secrets through summaries or tool calls. Inspect logs and telemetry for accidental retention. Confirm that authorized support staff see only the fields required for their role.

Test security, injection, and tool use

Assume every user message, retrieved document, web page, and tool response is untrusted. Test whether injected text can override system instructions, reveal secrets, alter retrieval filters, or trigger an external action. Require explicit authorization and, for high-impact actions, human confirmation. Record the exact tool arguments and result used for each action.

Test fairness and accessibility

Use matched prompts that vary only a protected or relevant demographic attribute. Compare refusal rates, factual quality, tone, escalation, and time to resolution. Include disability-related requests and assistive-technology workflows. Investigate meaningful disparities rather than relying on one aggregate score.

Test robustness and availability

Vary wording, order, conversation length, model temperature, retrieval availability, and network conditions. Exercise quotas, timeouts, retries, failover, and degraded modes. Verify that a failed dependency produces a safe, understandable response rather than fabricated completion.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

5. Apply mitigations and verify that they work

Mitigations should target the failure mechanism, not merely the symptom. Re-test after each major control and document any new trade-off.

Limit capability and authority

  • Use retrieval allowlists and freshness checks for domain answers.
  • Separate system instructions, secrets, user content, and retrieved content by trust level.
  • Give tools the minimum permissions, data scope, and network access required.
  • Require authentication for personalized information and step-up confirmation for sensitive actions.
  • Set rate limits, quotas, session timeouts, and abuse detection.

Improve responses and handoffs

  • Provide concise uncertainty language and show authoritative sources when appropriate.
  • Refuse unsafe requests without repeating dangerous details.
  • Route high-risk, ambiguous, or emotionally distressed cases to trained humans.
  • Offer an accessible human channel and explain how to appeal or correct an answer.

Protect data and operations

  • Minimize collection, redact unnecessary identifiers, encrypt data, and enforce role-based access.
  • Define retention and deletion behavior for chats, prompts, outputs, embeddings, backups, and logs.
  • Monitor model, prompt, retrieval, tool, and vendor versions.
  • Keep a tested rollback configuration and a shutdown procedure that does not depend on the chatbot itself.

6. Make and document the go/no-go decision

Do not treat a green test dashboard as automatic approval. Hold a review with the accountable owners and record a written decision for the exact version, scope, geography, channels, and user population assessed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Approve only when the evidence is sufficient

The decision record should include:

  • Purpose, boundaries, intended users, affected groups, and operating environment.
  • Model, prompt, retrieval, tool, data, and vendor versions.
  • Test coverage, methods, reviewer qualifications, and unresolved limitations.
  • Open risks, severity, likelihood, compensating controls, and named owners.
  • Launch limits, such as a small cohort, human approval, restricted tasks, or geographic exclusion.
  • Monitoring metrics, alert thresholds, review frequency, and incident contacts.
  • Who can pause, roll back, or shut down the system and under what conditions.

Use clear decision outcomes

  • Go: evidence supports the defined scope, residual risks are accepted by the accountable owner, and operational controls are ready.
  • Go with limits: launch only with specified users, tasks, human review, rate limits, or monitoring conditions.
  • No-go: a critical risk lacks an effective control, evidence is incomplete for the intended use, or owners cannot operate the required safeguards.

A no-go is not permanent. It identifies the control, evidence, or design change required for a later review.

7. Monitor continuously after launch

NIST states: “Risk management should be continuous, timely, and performed throughout the AI system lifecycle dimensions.” Production monitoring turns that principle into operating practice.

Track safety-relevant signals

Monitor harmful-output reports, fabricated-answer samples, escalation volume, privacy incidents, injection detections, unauthorized tool attempts, subgroup disparities, latency, outages, abandonment, and changes in user behavior. Sample conversations under a documented privacy-preserving procedure. Separate quality metrics from safety metrics so high engagement cannot conceal rising harm.

Run incident response and rollback drills

Define triage severity, evidence preservation, user notification, containment, supplier notification, regulator or contractual reporting, remediation, and post-incident review. Practice disabling a tool, reverting a prompt or model, isolating a tenant, and switching to a human or static fallback. Verify that operators can act during an outage or compromised monitoring system.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Reassess on change

Trigger a new assessment when the model, system prompt, retrieval corpus, tool, policy, vendor, user population, language, channel, or data classification changes. Also reassess after a serious incident, a new legal requirement, a material drift in performance, or a newly discovered attack technique. Preserve the previous decision so changes remain auditable.

Choosing between NIST and ISO/IEC 23894

The frameworks can be used together, but they serve different organizational needs.

Comparison point NIST AI RMF 1.0 and Generative AI Profile ISO/IEC 23894:2023
Primary scope General AI risk management, with a generative-AI profile released July 26, 2024. Guidance for organizations developing, producing, deploying, or using AI products, systems, and services.
Lifecycle coverage Govern, Map, Measure, and Manage across the AI lifecycle. Integrates risk management into AI activities and organizational processes.
Governance detail Strong emphasis on accountability, context, documentation, and ongoing management. Structured guidance for embedding AI risk practices in an organization’s risk framework.
Testing and evaluation Useful for defining context-specific measurement and generative-AI risk actions. Supports a broader risk-management approach; implementation details must be tailored to the system.
Certification status Voluntary framework; it does not provide a universal chatbot pass score. Guidance standard; certification availability and requirements depend on the relevant conformity program.
Best fit Teams needing a practical, public lifecycle structure for design, deployment, and monitoring. Organizations integrating AI risk into established enterprise or quality-management processes.

Neither framework supplies a universal numeric threshold. Use their structures to justify your own criteria and evidence, then add sector, privacy, security, accessibility, and geographic obligations that apply to your deployment.
