Use clinician-directed diabetic eye care while taking semaglutide (Ozempic or Wegovy). The important distinction is between routine surveillance and an emergency: rapid improvement in blood glucose can temporarily worsen diabetic retinopathy, while a sudden or partial loss of vision can signal the very rare event non-arteritic anterior ischemic optic neuropathy (NAION) and needs same-day ophthalmologic assessment.
Keep your scheduled dilated eye examinations, tell both your prescriber and eye clinician about any retinopathy and recent A1C changes, and report new visual symptoms promptly. Home charts, Amsler grids, eye drops and supplements cannot rule out retinopathy progression or NAION.

What semaglutide means for eye monitoring
Semaglutide improves blood-glucose control. That benefit can coincide with an initial, temporary worsening of diabetic retinopathy when glucose falls quickly. The current U.S. Ozempic prescribing information therefore says that people with a history of diabetic retinopathy should be monitored for progression. The American Diabetes Association (ADA) Standards of Care in Diabetes—2025 likewise says retinopathy status should be assessed when glucose-lowering treatment is intensified, including treatment with a GLP-1 receptor agonist, because rapid A1C reductions can be associated with initial worsening.

On 5 February 2026, the UK Medicines and Healthcare products Regulatory Agency (MHRA) advised that anyone reporting sudden vision loss, including partial loss, while taking semaglutide should be urgently referred for an ophthalmological examination because NAION has been very rarely reported. NAION typically causes sudden, often painless loss of vision, frequently in one eye. It is not something a home screening chart can safely assess.

These issues call for two different responses:
- Planned monitoring: keep individualized dilated diabetic eye examinations and arrange follow-up when treatment is started or intensified, especially if retinopathy is already present.
- Emergency assessment: seek same-day ophthalmologic or emergency care for sudden or partial loss of vision, particularly painless loss in one eye.
What the Ozempic trial data show
The Ozempic label reports diabetic-retinopathy complications in a two-year trial of adults with type 2 diabetes and high cardiovascular risk. The figures are trial results, not a prediction of what will happen to every person taking semaglutide.
| Trial group | Semaglutide | Placebo | Absolute difference |
|---|---|---|---|
| All participants | 3.0% | 1.8% | 1.2 percentage points |
| Participants with baseline diabetic retinopathy | 8.2% | 5.2% | 3.0 percentage points |
| Participants without known diabetic retinopathy | 0.7% | 0.4% | 0.3 percentage points |
The larger absolute difference among participants who already had retinopathy is why the label specifically calls for monitoring in that group. The label also notes that rapid improvement in glucose control has been associated with temporary worsening of diabetic retinopathy. The numbers do not establish that semaglutide permanently damages every person’s eyes, nor do they replace an examination that determines your current retinopathy stage.
How common is the sudden-loss warning?
NAION is considered very rare in people using semaglutide. An EMA Pharmacovigilance Risk Assessment Committee review in 2025 found epidemiologic evidence suggesting an approximately two-fold risk in adults with type 2 diabetes. The underlying event remained very rare, and the observational JAMA Ophthalmology study informing that review cannot prove that semaglutide caused an individual person’s vision loss. A statistical association also cannot tell you whether a particular episode is NAION, retinal disease, stroke, or another emergency; examination is required.
Because the consequence of missing sudden optic-nerve or retinal disease can be serious, the MHRA recommendation is symptom-based: sudden loss, including a partial field defect, warrants urgent ophthalmological examination rather than watchful waiting for a routine appointment.

Before starting semaglutide or increasing a dose
There is no single semaglutide-specific eye-exam interval that applies to everyone. The useful preparation is to make sure your clinicians know your eye and diabetes history so they can decide whether an examination is due and how closely to follow you.
- Tell the prescriber about your eye history. Mention diagnosed diabetic retinopathy, prior retinal laser treatment, prior eye injections and any previous episode of sudden visual loss. Ask the prescriber to record the date and findings of your last dilated examination.
- Tell the eye clinician about the treatment change. Give the ophthalmologist or optometrist the planned semaglutide start or dose increase and the reason for it. The ADA recommends assessing retinopathy status when glucose-lowering therapy is intensified.
- Share factors that can affect follow-up decisions. Include kidney disease, hypertension and recent A1C changes. These details help the treating team judge how quickly glucose is changing and whether an earlier review is appropriate.
- Ask what follow-up interval is right for your retinopathy stage. The interval is individualized according to retinopathy findings and diabetes history. Do not substitute a generic internet schedule for the interval your eye clinician gives you.
- Record a baseline symptom status. Note whether you currently have floaters, blur, distortion or a known missing area of vision. A baseline makes it easier to recognize a genuine change, but it does not replace a clinical examination.
Routine monitoring after treatment starts or intensifies
Keep the eye appointments already assigned to you
Continue the dilated diabetic eye examinations recommended by your eye clinician. A visit may include examination of the retina and other ophthalmic tests selected for your findings. The purpose is to detect progression that you cannot reliably identify by reading small print or checking a home chart. If you miss an appointment, contact the eye clinic for a new date instead of waiting for symptoms.
Pay closer attention during rapid glucose improvement
The first months after a major improvement in glucose control deserve particular attention because rapid A1C reductions can be associated with initial retinopathy worsening. “First months” is not a universal appointment rule; your clinicians may choose a different interval based on your retinal stage, A1C trajectory and treatment plan. Ask how you should report symptoms and which service to call outside office hours.

Report changes without waiting for the next routine visit
Contact your prescriber or eye-care team promptly if you develop new floaters, blurred vision, distortion, a missing area of vision or a noticeable worsening of known retinopathy. Describe when the change began, whether it affects one or both eyes, whether it is constant or intermittent, and whether it is getting worse. Do not drive if your vision is unsafe.

Use the symptom pattern to choose the right urgency
| What you notice | What to do |
|---|---|
| Sudden loss of vision in one eye, even if painless | Seek same-day ophthalmologic or emergency assessment. The MHRA specifically includes sudden loss while taking semaglutide in its urgent-referral advice. |
| A sudden missing or dark area in part of the visual field | Treat as urgent and obtain same-day assessment; partial loss is included in the MHRA warning. |
| New, persistent blur, distortion or floaters | Contact the eye clinician or prescriber promptly for advice and an examination; do not wait for the next scheduled visit if the symptom is new or worsening. |
| Brief, mild fluctuation with no persistent deficit | Contact the prescriber if it recurs or concerns you. Do not change or stop semaglutide solely because of a nonspecific fluctuation without medical advice. |
| Known retinopathy that appears to be worsening | Notify the eye-care team promptly. They can determine whether the change is related to retinopathy progression or another eye problem. |
Sudden or partial loss is an emergency even when there is no pain. Do not wait to see whether the symptom resolves overnight, and do not rely on a home test to decide that it is harmless.
Why a home chart or Amsler grid is not enough
A printed reading chart or Amsler grid can help you notice that something has changed, but official safety guidance does not recommend a consumer device for semaglutide-specific monitoring. A home check cannot examine the retina, optic nerve or blood vessels, measure disease severity, or exclude NAION. A normal result also cannot guarantee that a serious problem is absent.
If you use a grid as a personal reminder, test each eye separately under the same lighting and record the date and any new distortion. Stop the self-check and seek professional advice if one eye shows a new missing or warped area. The grid is a prompt to contact a clinician, not a clearance to continue waiting.

Do not make an unsupervised change for mild, nonspecific symptoms
Semaglutide may be treating diabetes or another approved condition, and abruptly changing the regimen can disrupt glucose control. If the symptom is mild or nonspecific, contact the prescriber and describe it rather than stopping or changing the dose on your own. The prescriber can coordinate an eye assessment and decide whether treatment should continue.

If NAION is confirmed
Regulator communications advise discontinuing semaglutide when NAION is confirmed, in coordination with the treating clinician. Do not interpret that advice as a reason to self-diagnose every episode of blur as NAION. Confirmation requires an ophthalmologic assessment, and your diabetes clinician must help plan an alternative strategy for glucose control when appropriate.
Keep managing the factors your clinicians identify
Continue the glucose, blood-pressure and other diabetes care your clinicians prescribe. Bring recent A1C results, medication changes and home glucose records to appointments if requested. The eye-monitoring decision depends on the pattern and speed of glucose improvement as well as the retinal findings, not on a single home reading.

Special situations that deserve explicit coordination
Existing diabetic retinopathy
Existing disease is the clearest reason to make sure the eye clinician knows about semaglutide. In the Ozempic trial, the absolute difference in reported retinopathy complications was larger among participants with baseline retinopathy than among those without known retinopathy. Your retinal stage, prior treatment and current glucose trend determine how closely you need to be followed.
Recent large A1C reduction
A rapid fall in A1C can be clinically desirable while still being a period in which retinopathy may initially worsen. Tell both clinicians about the timing and size of recent A1C changes. This is why ADA guidance places retinopathy assessment at the point when glucose-lowering therapy is intensified rather than treating the eye examination as unrelated to the medication change.
Best Value
Kidney disease or hypertension
Report kidney disease and hypertension before treatment changes. The ADA identifies these details as relevant to risk assessment and follow-up decisions. They do not, by themselves, tell you whether a new visual symptom is urgent; the symptom pattern still determines whether you need same-day care.
What an eye-care visit can establish
An eye clinician can determine whether symptoms reflect diabetic-retinopathy progression, another retinal problem, optic-nerve disease or a non-ocular cause. The examination may be supplemented by ophthalmic testing chosen for your presentation. That clinical assessment is fundamentally different from checking whether you can read a line of text at home.
Bring the names and doses of your diabetes medicines, the date semaglutide started or was increased, recent A1C values and a description of every visual symptom. If the change was sudden, write down the exact time it began and whether one eye or both eyes were affected. Those details help the receiving service triage you quickly.

Bottom line for daily decisions
Follow the eye-examination schedule set for your diabetic-retinopathy status, and make sure the eye clinician knows whenever semaglutide is started or intensified. Watch more carefully during periods of rapid glucose improvement, report new or worsening symptoms promptly, and treat any sudden or partial vision loss as a same-day emergency. No home chart, supplement or over-the-counter eye product can replace that evaluation.
