A useful trace connects the request to every model call, retrieval operation, tool action, state change, handoff, cost, and final outcome. Troubleshooting then becomes a repeatable loop: locate the run, find the earliest divergence, separate agent behavior from infrastructure faults, correct the responsible component, and turn the incident into a regression evaluation.
This guide shows what to collect, how to alert without drowning operators in noise, how to protect sensitive telemetry, and how to investigate failures such as looping tool calls, unexplained cost spikes, missing spans, and plausible but incorrect answers.

Why ordinary service monitoring is not enough
Availability, HTTP errors, CPU, memory, and latency remain necessary. They cannot answer the questions that determine whether an agent is useful and safe:
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- Did the task meet its acceptance criteria, or did the model produce a confident but wrong answer?
- Which retrieval result or tool response led to the final action?
- Why did the agent call the same tool three times?
- Did a permission failure, provider limit, network timeout, or prompt change cause the visible failure?
- Did the agent claim to complete an action that never happened?
- Why did one run consume dramatically more tokens or money than comparable runs?
Health and quality are different dimensions. A service can respond successfully while returning an ungrounded, incomplete, unsafe, or unauthorized result. Agent observability therefore treats one request and its ordered work as the primary unit of inspection.

LangChain’s 2026 survey of more than 1,300 professionals reported that 89% of surveyed organizations had some form of agent observability and 62% reported detailed tracing. Among respondents with agents in production, 94% reported some observability and 71.5% reported full tracing. These are survey results, not a census of all enterprises. The same survey reported offline evaluation at 52.4% and online evaluation at 37.3%; among respondents at organizations with 2,000 or more employees, 24.9% cited security as a major concern.
Model every request as a traceable execution graph
Create one root trace for each user request, job, or event. Give every operation a child span and preserve parent-child relationships across services, queues, and agents. The resulting graph should show both order and causality.
Recommended span types
- Request: request ID, session or conversation ID, tenant or user segment where policy allows, environment, region, deployment version, prompt or policy version, start and end time, and final status.
- Model call: provider and model identifier, relevant parameters, input and output token counts when available, estimated cost, latency, stop reason, tool calls requested, and error details.
- Retrieval: query, index or knowledge source, returned references, scores or relevance signals, filtering decisions, and latency. Capture content only when privacy policy permits.
- Tool action: tool name and version, authorized identity and execution context, redacted arguments, result or error, duration, retry count, and idempotency key where applicable.
- State transition: state read or written, workflow checkpoint, approval status, and the component that performed the change. Avoid recording secrets or unnecessary payloads.
- Handoff: parent agent, receiving agent, task description or identifier, authorization context, and return status.
- Outcome: acceptance checks, policy result, user feedback, correction, abandonment, and whether a human approval was required.
Attach prompt, tool schema, retrieval configuration, and deployment versions to the trace. When behavior changes after a release, those fields let an investigator distinguish a code change from a model, prompt, index, or policy change.
Capture decisions without collecting hidden chain-of-thought
Operational telemetry does not require storing private chain-of-thought. Record observable decisions and actions: which tool was selected, the validated arguments, the retrieved references, safety or policy outcomes, and concise reason codes defined by the application. This provides a diagnostic trail while limiting exposure of sensitive model content. Microsoft guidance warns that content recording can expose prompts, tool arguments, and outputs; choose the least sensitive data that still supports the incident you must investigate.

Record timestamps with enough precision to reconstruct concurrency and queueing. Propagate trace context through asynchronous workers and sub-agent boundaries. If a component cannot propagate context, create an explicit handoff event containing the parent trace ID and the child run ID.
Build layered signals and alerts
Use infrastructure, execution, quality, economic, and security signals together. An alert should identify an owner and the next action; a metric without a response path creates noise.
Infrastructure and provider signals
- Request availability and error rate by provider, model, region, and deployment.
- Timeouts, throttling, rate-limit responses, queue depth, worker saturation, and network failures.
- Downstream tool health, dependency latency, authentication failures, and permission denials.
- Trace ingestion delay, dropped spans, exporter backpressure, and telemetry backend availability.
Agent-behavior signals
- Task-completion rate against an explicit acceptance test.
- Tool error rate, retry count, and repeated identical calls within one trace.
- Unexpected tool selection, invalid arguments, stalled steps, and excessive step count.
- Retrieval relevance, empty-result rate, citation or reference coverage, and context-window pressure.
- Handoff failures, state-transition errors, and incomplete traces.
Experience, cost, and risk signals
- Latency from request to accepted outcome, not only time to first token.
- Tokens and estimated cost per run, by task, tenant, model, and release.
- User corrections, re-prompts, abandonment, escalation to a human, and complaint rate.
- Policy violations, suspicious prompt activity, privileged-tool usage, and unusual access patterns.
Set thresholds from service objectives, risk tolerance, and observed baselines. The available guidance identifies useful metrics and alert categories but does not establish universal values. A customer-support agent, a financial workflow, and an internal coding assistant will have different acceptable error, latency, and cost limits.
Examples of actionable alert definitions
- Loop alert: page the workflow owner when the same tool and materially identical arguments repeat beyond the task’s normal baseline; include the trace ID and first failed result.
- Cost anomaly: notify the FinOps or platform owner when cost per accepted task deviates from the baseline for the same task class; attach token counts and step breakdown.
- Quality regression: block or roll back a deployment when a representative evaluation set falls below its release objective, even if availability and latency improve.
- Security event: route unauthorized privileged-tool attempts to the security owner with the principal, policy decision, and relevant trace context.
Protect telemetry as production data
Agent traces can contain personal information, confidential prompts, customer records, credentials embedded in arguments, and sensitive tool outputs. Apply data governance before instrumentation reaches production.
Redaction and minimization
- Disable content recording in production when full prompts or outputs are not required for the operational use case.
- Redact secrets, tokens, payment data, personal identifiers, and regulated fields before export, not only in the dashboard.
- Prefer reference IDs, hashes, classifications, and bounded previews over full documents.
- Define separate capture policies for development, staging, and production; test redaction with adversarial examples.
Access, retention, and residency
- Restrict trace search and replay by role and tenant. Audit reads of privileged traces as well as writes and exports.
- Set retention and sampling by incident value, regulatory requirement, and storage cost. Keep a sanitized, durable record for examples that become regression tests.
- Document where traces, evaluation data, and backups are stored and which operators or vendors can access them.
- Audit privileged tool actions and preserve the event context needed to investigate suspicious prompt activity or policy bypasses.
Microsoft troubleshooting guidance specifically recommends disabling content recording in production and redacting sensitive data before capture. Government guidance likewise emphasizes privacy-aware logging, permission audits on privileged actions, and monitoring for drift and suspicious prompts.

Keep tracing non-blocking
Telemetry must not become a new dependency for the agent’s business action. Export spans asynchronously through a buffer or queue, batch them, and flush in the background. Define bounded memory and disk limits, backpressure behavior, retry policy, and a drop policy for overload.
As StackGen principal engineer Sabith K Soopy wrote in a CNCF article dated August 4, 2026, “Tool execution should never wait on a synchronous HTTP POST to a tracing backend.” The same article notes that “Traditional APM can’t tell you why your agent spent far more than usual asking the same question three times.” These are practitioner recommendations, not formal CNCF standards.
When the telemetry backend is unavailable, the agent should continue according to its application SLO unless a specific compliance control requires fail-closed behavior. Emit a local health signal for dropped or delayed spans so operators know that a blind spot exists.
A step-by-step troubleshooting loop
- Locate the run. Start with the request or session ID, timestamp, environment, deployment and prompt versions, alert, and user feedback. Confirm that a trace was created and check ingestion delay before declaring it missing.
- Verify trace shape. Check that model, retrieval, tool, state, and handoff spans appear in the expected order. Missing framework spans can indicate that instrumentation was not initialized or enabled. Missing tool spans can indicate that tools were not bound to the model or that graph tool nodes were never configured.
- Find the earliest divergence. Compare the observed sequence with the intended workflow. Look for the first unexpected tool choice, poor retrieval result, repeated failing action, invalid argument, or unexpected state change. The final answer is often only the symptom.
- Separate behavior from infrastructure. Inspect provider errors, runtime limits, permissions, network conditions, queueing, and downstream tool health alongside the model’s decisions. An authorization error while querying telemetry may be an Application Insights or Log Analytics role problem rather than evidence that the trace does not exist.
- Measure impact and expected outcome. State the acceptance criterion for the task, then compare the result and its supporting context with that criterion. Record latency, token and cost impact, user correction, data exposure, and policy implications.
- Preserve a safe reproduction. Save the original trace under the applicable retention policy or create a sanitized representative example. Include the relevant versions, tool responses, retrieval references, and policy decisions, but remove secrets and unnecessary content.
- Apply the narrowest corrective change. Change the prompt, tool schema, retry/idempotency logic, retrieval filter, state machine, permission, provider configuration, or infrastructure component implicated by evidence. Avoid changing several unrelated variables at once.
- Turn the incident into an evaluation. Add the example to a targeted offline test. Define the expected outcome, allowed actions, safety constraints, cost or step budget, and acceptable evidence. Run it before deployment and monitor the corresponding live signal after release.
Common failure patterns and fixes
The agent keeps calling the same tool
Inspect the first tool error and the next model decision. A retry may be appropriate for a transient timeout but harmful for a deterministic validation or authorization failure. Add an explicit retry budget, classify errors as retryable or terminal, use idempotency keys for side effects, and expose the tool’s structured error to the planner. Add a regression case that asserts the agent stops or escalates after the permitted attempts.
The final answer is plausible but unsupported
Compare the answer with retrieved references and the task’s acceptance test. An empty or low-relevance retrieval result, a filter that excluded the authoritative record, or a context-window truncation may have occurred. Log reference IDs and relevance signals, require an application-level evidence check where the domain needs it, and evaluate groundedness using domain reviewers when automated scoring is insufficient.

Cost or latency suddenly increases
Break the run into model, retrieval, tool, queue, and handoff spans. Look for a prompt or model-version change, context growth, repeated calls, slow downstream dependencies, or a fallback path. Compare against the same task class rather than an all-traffic average. Correct the specific source and keep a cost-per-accepted-task metric so a fast but unsuccessful response is not mistaken for efficiency.
Traces are incomplete
Check exporter health, sampling, context propagation, instrumentation initialization order, and custom operations that lack manual spans. A trace that contains a root request but no tool or retrieval spans is not evidence that those operations did not happen; it may indicate an instrumentation gap.

Microsoft’s documented checks provide a concrete example of how to narrow observability failures:
| Symptom | Checks |
|---|---|
| No traces | Verify the Application Insights connection, generate recent traffic, and allow for ingestion delay. |
| No LangChain or LangGraph spans | Ensure the Microsoft OpenTelemetry distribution and framework instrumentation are initialized and enabled before agent execution. |
| Missing tool-call spans | Verify that tools are bound to the model and that tool nodes exist in the graph. |
| Incomplete spans | Check content-recording and generative-AI semantic-convention configuration, or add manual spans around custom operations. |
| Authorization errors | Verify access control on Application Insights and Log Analytics for the investigator’s role. |
| Sensitive content appears | Disable production content recording and redact before capture. |
Configuration names and procedures change with Microsoft and framework releases. Confirm the current documentation and the versions deployed in your environment before applying a setting.

Use incidents to build evaluations
A trace review is not complete when an engineer explains the failure. Preserve a sanitized example, state the expected behavior, and make it executable as a check.
Best Value
What a useful regression case contains
- The task input and relevant environment or tenant characteristics, minimized and anonymized.
- Expected completion criteria, prohibited actions, and acceptable escalation behavior.
- Required tools, allowed argument patterns, retrieval sources, and evidence requirements.
- Budgets for steps, retries, latency, and cost when those limits are part of the service objective.
- Assertions over both outcome and path: for example, no duplicate side effect, no unauthorized tool call, and use of an authoritative reference.
Run offline evaluations before release and use online checks for live traffic where the risk and latency permit. Human or domain-expert review remains important when correctness depends on context that an automated judge cannot reliably assess. Track user corrections, complaints, and policy breaches as inputs to the same review-and-fix workflow instead of leaving them only in chat or an incident ticket.
Choosing an observability approach
Teams commonly combine framework instrumentation, OpenTelemetry, cloud monitoring, and a dedicated agent-observability service. LangSmith describes framework-agnostic tracing, OpenTelemetry integrations, and managed-cloud, bring-your-own-cloud, and self-hosted options. Langfuse describes native SDK and OpenTelemetry ingestion, framework integrations, trace-linked monitors, user feedback, and evaluations. These are vendor claims; validate current availability, security posture, deployment boundaries, data location, and plan limits for your region.
| Decision axis | Questions to ask |
|---|---|
| Trace fidelity | Can it represent model, tool, retrieval, state, handoff, retries, and ordering? Can engineers add manual spans? |
| Interoperability | Does it work with the existing orchestration framework, cloud providers, model providers, and OpenTelemetry pipeline? |
| Quality workflow | Are trace review, human feedback, online and offline evaluation, regression datasets, and alerts connected? |
| Security and residency | What content controls, redaction, access roles, retention, deployment models, and data locations are available? |
| Operational reliability | Are exports asynchronous? What happens during backend outages, backpressure, sampling, and retention limits? |
| Economics and ownership | How are trace volume, storage, evaluation usage, and cost attribution charged, and how much maintenance remains with your team? |
A small single-call application may need structured logs and basic metrics. Multi-step and multi-agent systems usually need full traces because failures emerge from interactions among model calls, tools, retrieval, and state. No platform is universally best without the workload, compliance, infrastructure, and budget requirements.

A practical rollout plan
Phase 1: establish the minimum viable trace
- Choose a stable trace and span schema.
- Instrument the root request, model calls, tools, retrieval, and final outcome.
- Propagate context across queues and agent handoffs.
- Implement redaction, role-based access, retention, and asynchronous export before broad production rollout.
Phase 2: connect quality and economics
- Define acceptance tests for the highest-risk tasks.
- Attribute tokens, estimated cost, latency, retries, and step count to each run.
- Collect user correction and abandonment signals with privacy controls.
- Create dashboards segmented by task, model, release, environment, and user group where permitted.
Phase 3: operationalize learning
- Assign owners and next actions to alerts.
- Review representative traces on a fixed cadence and after incidents.
- Convert recurring failures into offline evaluations and release gates.
- Compare live behavior with evaluation results and investigate drift, new loops, or changes in retrieval relevance.
The result is a closed system: production behavior creates traces; traces reveal the earliest fault; the fix is tested against a preserved example; and the evaluation plus live signals show whether the change actually improved the agent.
