Safe AI-agent operations require more than collecting logs. Use monitoring to reconstruct what an agent did, detect behavior that departs from its approved task, and trigger investigation or containment. Use separate execution-time controls—least-privilege tools, downstream authorization, independent validation, rate limits, and approval gates—to stop or constrain harmful actions before they reach a tool or downstream system.
This guide shows what to observe, which behaviors to block or escalate, how to design a useful telemetry trail, and how to test whether monitoring is finding problems without pretending it can guarantee safety.

What monitoring can—and cannot—do
An agent can call APIs, retrieve data, update memory, hand work to another agent, and make externally visible changes. Monitoring makes those activities inspectable. It can reveal an unauthorized tool call, a sudden expansion in data access, repeated failed attempts, or a memory update that changes later behavior. It can preserve the evidence a reviewer needs to understand the task, identity, inputs, outputs, and permissions involved.
Monitoring is not an authorization system. A dashboard that notices a harmful transfer after it occurs has provided detection and response value, but it has not prevented the transfer. OWASP’s guidance treats logging and monitoring as ways to identify undesirable actions and respond; it also states that these measures do not prevent excessive agency. Controls that run before execution must therefore sit alongside observability.

| Control layer | Primary question | Typical mechanisms |
|---|---|---|
| Prevention | Can the agent reach this capability at all? | Narrow tools, least privilege, downstream authorization, complete mediation, network and data boundaries |
| Pre-execution validation | Is this exact proposed action allowed for this task and identity? | Independent policy checks, target and scope validation, approval-token binding, fail-closed behavior |
| Containment | How much can happen before a problem is noticed? | Rate limits, quotas, transaction limits, sandboxing, revocation and pause controls |
| Detection and response | What happened, and what should people do now? | Traces, rules, anomaly detection, alerts, human review, incident procedures |
Design these layers together. A monitor should be able to show why an action was attempted and which control allowed or rejected it; an authorization service should be able to emit the decision into the same trace.
1. Define the agent’s mandate and risk tiers
Write the mandate before choosing dashboards. State the agent’s purpose, permitted resources, identities it may act for, allowed tools, data boundaries, and actions that are out of scope. Define what requires a person’s approval and what must always be denied. This turns “unsafe behavior” into testable conditions.
Threats worth modeling
OWASP’s agent-security guidance highlights tool abuse and privilege escalation, sensitive-data exfiltration, memory poisoning, goal hijacking, excessive autonomy, high-impact action abuse, and cascading failures. Map each threat to an observable event and an execution-time control. For example, an attempted export to an unapproved destination should create a security event and be rejected by the destination’s authorization layer.

Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteUse impact tiers
A practical tiering scheme separates actions by reversibility and consequence:
- Low impact: read-only retrievals or reversible formatting changes within an approved workspace.
- Medium impact: changes to shared records, external messages, or updates that affect another workflow but can be rolled back.
- High impact: financial transactions, access-control changes, production deployment, deletion, publication, legal or safety decisions, and actions that are difficult to reverse.
Document the approval rule for each tier. Do not let the model classify its own action; classify it with an independent policy service using the requested operation, target, identity, and current task context.
2. Constrain capabilities before you instrument them
The safest tool call is one the agent cannot make. Give the agent only the functions and permissions necessary for its assigned task. Prefer a narrowly defined operation—such as “create a support ticket in this queue”—over unrestricted shell execution, a general-purpose URL fetcher, or a tool that accepts arbitrary database commands.
Enforce authorization downstream
Authorization must be enforced where the resource is controlled. If an agent proposes reading a customer record, the records service should check the authenticated principal, scope, purpose, and resource policy. The prompt or model output is not an authorization decision. Use the user’s identity when the agent is acting on that user’s behalf, and ensure every hop preserves the identity and permitted scope.

Validate the complete action
Before execution, validate the operation, parameters, target, quantity, destination, and approval status. Bind an approval to those exact details. An approval for “post the drafted announcement to the internal channel” must not be reusable for a different message or a public channel. If any material field changes, require a new decision.
Add human gates for high-impact operations
Require a person to review and approve actions that are externally visible, financially consequential, administrative, destructive, or hard to undo. OWASP uses posting social-media content as an example: the user should review and approve the operation before it runs. Present the proposed action in a form a reviewer can understand, including the target, data to be sent, resulting side effects, and expiration of the approval.
Limit blast radius
Rate limits restrict how many undesirable actions can occur and create time to discover a problem. Add quotas, transaction ceilings, destination allowlists, short-lived credentials, and an immediate revoke or pause mechanism. Complete mediation means every access is checked, not merely the first access in a session.
3. Capture a reconstructable execution trace
A useful log answers three questions: what happened, what task was the agent pursuing, and which capability or permission enabled each step. Singapore’s 2026 guidance lists the core fields teams should consider logging:
The Tool Desk
Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →- Agent actions, including API calls and data retrievals.
- Inputs and outputs for model and tool interactions.
- Internal state changes, such as memory or knowledge-base updates.
- Errors, exceptions, retries, and rejected operations.
- Timestamps, duration, and ordering information.
- Task, session, workflow, user, and agent identifiers.
Capture parent-child relationships so a reviewer can follow a request through model calls, tools, retrieval systems, queues, and agent handoffs. Include the policy decision associated with each consequential call: allowed, denied, escalated, or approved, together with the policy version and decision reason.
Trace across handoffs and standards
The OWASP Agent Observability Standard describes agents as instrumentable, traceable, and inspectable. A trace should identify the models, tools, capabilities, software versions, builders, services, and accessible data behind a running system. OpenTelemetry and OCSF are referenced for tracing and event structure; MCP and A2A are referenced as instrumentation contexts for tool and agent interactions. Whatever standards you use, preserve a stable correlation identifier across every component.
Protect sensitive telemetry
Prompts, tool arguments, outputs, identifiers, and memory contents can contain credentials, personal data, or confidential business information. Decide which fields are necessary for security review, redact or tokenize data that is not needed, encrypt records in transit and at rest, restrict investigator access, and set retention periods based on incident and regulatory needs. Singapore’s guidance specifically advises considering privacy regulations when logging inputs. A trace that becomes a second data-leak channel is not a safety improvement.

4. Detect deviations from the approved task
Detection should combine deterministic rules, tested baselines, and human review. Rules catch known violations; baselines expose drift; people handle ambiguous intent and novel attacks.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsHigh-value detection rules
- Tool calls outside the agent’s declared capability set.
- Attempts to elevate privileges, switch identities, or bypass a policy service.
- Access to data classes, tenants, regions, or destinations not required by the task.
- Changes to memory, retrieval indexes, or instructions that alter future behavior.
- Repeated retries after denial, especially with changing parameters.
- Unusually high call volume, long-running loops, or parallel actions inconsistent with the workflow.
- Output or action patterns that conflict with the task objective, approval details, or user constraints.
- Failures that cascade into retries, duplicate transactions, or handoffs to untrusted components.
Build a baseline from tested behavior
Measure normal sequences for each agent and workflow: common tools, data volumes, latency, handoff counts, error rates, and approval frequency. Alert on meaningful departures rather than every variation. The Singapore addendum recommends continuous monitoring for behavior that drifts from tested baselines and periodic permission audits. Re-baseline deliberately when a model, tool, prompt, policy, or workflow changes; otherwise a legitimate release can look like an attack or an attack can hide in a stale baseline.
Use risk-based alert severity
Severity should reflect potential impact and confidence. An attempted production deletion without an approval should page an on-call reviewer and trigger an automatic block. A small increase in retrieval latency may be informational. Include the reason for the alert, affected identity and task, exact action and target, policy result, and links or identifiers for the surrounding trace.
5. Connect alerts to investigation and response
An alert without an owner is only a notification. Define who reviews each severity, how quickly, and what they can do. The response path should preserve the trace, identify affected tasks and identities, notify the appropriate reviewer, and provide a way to pause the agent, revoke credentials, disable a tool, or quarantine a workflow while facts are established.
A practical investigation sequence
- Preserve evidence. Freeze the relevant trace, policy decisions, model and tool versions, and state snapshots according to your retention policy.
- Confirm scope. Determine whether the event was an attempted action, a completed action, a duplicate, or a false positive. Check downstream records rather than relying only on the agent’s report.
- Contain. Pause the workflow, revoke the affected credential, reduce permissions, or disable the specific tool. Use the narrowest containment that stops further harm.
- Assess impact. Identify data accessed, systems changed, messages sent, and other tasks or tenants that may share the same capability.
- Recover and learn. Roll back reversible changes, notify affected parties when required, update policies and tests, and record the decision so future reviewers can distinguish the known case.
These steps are an operational design pattern, not a universal incident standard. Adapt ownership, notification, and retention to your organization’s legal, safety, and contractual obligations.

6. Test whether monitoring actually works
Evaluate monitors against realistic workflows and adversarial scenarios, not only clean demonstrations. Test prompt injection, goal hijacking, unauthorized tool selection, privilege escalation, sensitive-data exfiltration, poisoned memory, runaway retries, malicious handoffs, and changes in model or tool versions.
Measure the right outcomes
- Can investigators reconstruct the full action path from the alert?
- Does the monitor identify the correct task, identity, target, and policy decision?
- Are high-impact actions blocked or held before execution?
- How long does it take to pause or revoke access?
- What proportion of alerts are actionable, duplicated, or false positives?
- Do tests detect behavior that departs from the approved baseline after a release?
A monitor that reviews only after execution can warn and support containment, but it cannot block the first harmful action. Test preventive controls separately by asserting that unauthorized calls are rejected at the tool or downstream service boundary.

What the 2026 OpenAI example shows
In a March 19, 2026 report about internal coding-agent monitoring, OpenAI described an asynchronous monitor that reviews interactions and flags suspicious activity for human review. Approximately 1,000 moderate-severity conversations triggered alerts; many involved deliberate internal red-teaming. The report said the monitor had matched every employee escalation known to OpenAI at that time, while cautioning that subtler issues could remain and that false-negative rates on open-ended, real-world traffic could not yet be quantified confidently. That alert count is a deployment-specific figure, not an industry prevalence rate or a general accuracy score.
OpenAI summarized the design this way: “That’s why we treat monitoring as one layer in a defense-in-depth stack—investing in monitorable reasoning traces and pairing monitors with complementary evaluations and preventative controls.” The lesson is operational: keep improving trace quality and detection, but do not remove execution-time safeguards.
Choosing an agent-observability product
Products marketed for AI agent observability vary from trace collection to evaluation and runtime enforcement. Names in government guidance include MLflow, Databricks, OpenLLMetry, Helicone, Langfuse, LangSmith, Azure Agent Monitoring, and AWS Bedrock Trace Events. Their appearance in guidance identifies relevant categories, not a ranking or endorsement.
Use a proof-of-concept with your own workflows. Compare capabilities rather than assuming that a tracing product blocks unsafe actions.
| Evaluation axis | Questions to ask |
|---|---|
| Trace coverage | Are model calls, tool calls, handoffs, retrievals, memory updates, errors, and policy decisions captured? |
| Context linkage | Can an alert be connected to the complete task, session, user, identity, target, and downstream result? |
| Instrumentation | Which frameworks and standards are supported? Can you add custom events without losing correlation? |
| Data handling | Where is telemetry processed and stored? Are redaction, encryption, access controls, regional deployment, and retention configurable? |
| Alerting and workflow | Are rules, baselines, severity, suppression, escalation, case management, and pause or revoke actions integrated? |
| Enforcement | Does the product only record and evaluate activity, or can it prevent a call through a policy decision point? |
| Operational fit | Can it handle your volume, latency budget, uptime requirements, and release process without creating an unreviewable alert backlog? |
Require vendors to demonstrate a denied high-impact action, an approval bound to exact parameters, a cross-agent handoff trace, and an investigator’s path from alert to containment. If the demonstration stops at a dashboard, treat the product as observability—not as policy enforcement.
Common failure modes and fixes
“The model knows not to do that.”
Models can misinterpret instructions, follow injected content, or pursue a mistaken goal. Move authorization and scope checks to independent services, narrow the tool surface, and fail closed when a decision is missing.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →Logging only the final answer
A final response hides the calls, retries, state changes, and permissions that produced it. Instrument the complete execution path and retain enough input, output, and state context to reconstruct consequential steps.
Best Value
Alerting without containment
If reviewers cannot pause the workflow or revoke access, an alert may arrive after the damage is complete. Exercise the pause, credential-revocation, and tool-disable paths during drills.
Approvals that are too broad
A generic “approved” flag can be replayed for a different target or payload. Bind approval to the exact operation, parameters, destination, identity, and expiration; invalidate it when those details change.
Collecting everything forever
Unbounded telemetry increases privacy and breach risk and makes investigations harder. Define security-useful fields, redact unnecessary content, restrict access, and apply a documented retention schedule.
Confusing a new release with an attack
Model, prompt, tool, and policy changes alter normal behavior. Version your baselines, run regression and adversarial tests before release, and review alert changes after deployment.
A concise implementation checklist
- Document the agent mandate, identities, resources, tools, out-of-scope actions, and approval tiers.
- Replace broad extensions with narrow, purpose-built operations wherever possible.
- Enforce authorization in every downstream system and preserve user identity across handoffs.
- Validate action, target, scope, privilege, and approval independently before execution.
- Require human approval for high-impact or irreversible operations and bind it to exact details.
- Apply rate limits, quotas, transaction ceilings, destination restrictions, and short-lived credentials.
- Capture actions, inputs, outputs, state changes, errors, timestamps, duration, and task or session context.
- Correlate traces across models, tools, retrieval, memory, services, and agent handoffs.
- Protect telemetry with minimization, redaction, access controls, encryption, and retention limits.
- Detect unauthorized calls, scope changes, unusual data access, memory anomalies, retries, volume spikes, and task-inconsistent patterns.
- Route alerts to named reviewers with evidence and a tested pause or revoke procedure.
- Test monitors and preventive controls with adversarial scenarios after every material change.
How current guidance fits together
The NIST AI Risk Management Framework remains a voluntary, cross-lifecycle framework, and NIST says it is under revision. NIST’s February 2026 Agent Standards Initiative focuses on interoperable standards, open protocols, and agent security and identity research. These efforts complement, rather than replace, concrete engineering controls: least privilege, complete mediation, independent validation, traceability, and response.
Frequently Asked Questions
Does monitoring prevent an AI agent from taking an unsafe action?
Not by itself. Monitoring detects and records behavior and can trigger containment; authorization, least privilege, independent validation, rate limits, and approval gates must block or constrain actions before execution.
What events should an AI-agent trace include?
Capture tool and API calls, inputs and outputs, memory or knowledge-base changes, errors and retries, timestamps and duration, and task, session, workflow, user, and agent context. Correlate events across handoffs and include policy decisions for consequential calls.
When should a human approve an agent action?
Require approval for externally visible, financially consequential, administrative, destructive, or difficult-to-reverse operations. Bind approval to the exact operation, parameters, target, identity, and expiration.
How do I compare AI-agent observability tools?
Assess trace coverage, framework and standards support, context linkage, privacy and retention controls, alerting and response integration, deployment model, and whether the product enforces runtime policy or only records activity.
