How to Monitor AI Agents for Unsafe Actions in Production

By Freedom251 Team · · 11 min read

Monitor an AI agent where its decisions become real side effects: the tool call, authorization check, and downstream system. Capture an attributable record of every attempted action and outcome, send those records to infrastructure the agent cannot alter, alert on policy violations and unusual behavior, and place deterministic approval gates in front of high-impact or irreversible operations. Monitoring gives you detection and evidence; least privilege, authorization, validation, and approval gates provide the prevention.

This design applies to agents that can call APIs, modify files, send messages, change cloud resources, query sensitive data, or delegate work to other agents. It also works for multi-agent systems, provided each side-effecting tool call is covered rather than relying on a single top-level trace.

Start with the action path, not the chat transcript

A prompt and the agent’s final answer are only part of what happened. The safety-critical sequence is the request, the model’s proposed action, the tool invocation, the policy decision, any human approval, the downstream result, and the resulting change. Instrument that complete path.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Request: identify the human or service that initiated the session and record the intended task.
  2. Plan or decision: retain the agent step that led to a tool request, with sensitive content minimized.
  3. Tool call: record the exact tool or action name and a safe representation of its arguments.
  4. Authorization: evaluate the actor, resource, operation, and current policy outside the model’s context.
  5. Approval: if the action is high impact, create an approval request tied to the exact call and current actor.
  6. Execution: perform the call through a controlled component, not through an unmediated model connection.
  7. Outcome: capture the downstream response, status, and resulting change or diff.

OWASP’s DevSecOps guidance calls for logging tool calls, commands, file writes, and network requests with agent identity, initiating user, session, and resulting diff. Microsoft’s guidance likewise treats plans, tool calls, decisions, and outcomes as observability inputs for audit and incident response. The practical implication is simple: a trace that ends when the model emits JSON is incomplete.

Define a forensic event record

Use structured events rather than free-form text. A central event schema lets you correlate a request with the authorization decision and the actual downstream change.

Event data What to capture
Identity Agent identity, initiating human or service, and the effective identity used downstream
Session context Session or run identifier, parent run for delegated agents, and timestamp
Action Tool or operation name, target resource, and an approved action classification
Arguments Exact parameters or a redacted or tokenized representation that still identifies the target and policy boundary
Risk decision Risk class or score when used, policy version, authorization result, and reason for a denial
Approval Whether approval was required, approval identifier, approving actor, decision, and expiry or timeout state
Execution Execution result, downstream status, error, and the system that performed the call
Change evidence Resulting diff, affected records, destination, or other evidence of what changed

Keep the event identifiers stable across services so an investigator can follow one action through a gateway, tool wrapper, and downstream API. Include a policy version with every decision; otherwise, a later policy change can make an old denial or approval impossible to interpret.

Instrument every side-effecting tool

Put the instrumentation next to the component that can cause the effect. A wrapper around a file-write tool should log the write and resulting diff. An API connector should log the method, resource, authorization result, and response. A network-capable tool should record the destination after policy normalization, not only the URL string supplied by the model.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

In a multi-agent chain, repeat this at each custom tool boundary. Top-level input and output guardrails do not necessarily execute at every hand-off. OpenAI’s human-review guidance describes a pattern in which a tool call raises an approval interruption; the application approves or rejects it, then resumes the same run. Treat that as an implementation example, not a requirement to use one platform. The general rule is that the execution component must be able to pause, reject, or modify a call before the side effect occurs.

Record attempted and blocked actions

A denied call is valuable evidence. Log the attempted operation, the policy that rejected it, and whether the agent retried with different parameters. Without denied events, repeated probing and approval-bypass attempts disappear from your telemetry.

Capture downstream truth

The agent’s explanation is not proof of what happened. Query the downstream system for the committed result when possible: a configuration diff, database change identifier, message delivery status, or cloud audit event. Reconcile the tool response with that independent record so a compromised connector cannot silently report success.

Enforce authorization outside the model

Never treat an agent’s confidence, explanation, or self-reported approval as authorization. OWASP’s excessive-agency guidance recommends complete mediation in downstream systems: every request must be checked against policy at the point where it is executed. The check should consider the current actor, operation, resource, tenant, and context, rather than trusting a flag supplied by the model.

A bare user_confirmed value is insufficient. The execution component should validate that the approval belongs to the current actor, refers to the exact action and parameters, has not expired or been reused, and was issued under the current policy version. If any of those checks fail, reject the call and emit a denial event.

Choose a gate for high-impact actions

Classify operations by impact and reversibility before deployment. Examples of high-impact actions include deleting data, changing access controls, moving money, publishing externally, modifying production infrastructure, or exfiltrating sensitive records. Require explicit approval for these operations and make the gate deterministic in orchestrator or policy code, not a reasoning step the model can skip.

For an approval workflow, display the normalized target, exact parameters, predicted effect, and expiration to the reviewer. Bind the resulting approval identifier to that immutable request. If a parameter changes, require a new decision. Define timeout behavior explicitly: fail closed for destructive operations, and avoid automatic retries that could turn a transient error into repeated side effects.

Limit blast radius with least privilege

Monitoring cannot undo a destructive action after the fact. Give each agent only the extensions, tools, data scopes, and permissions required for its task. Prefer narrow, purpose-built tools over a general shell or unrestricted API client. Where the business process supports it, execute with the initiating user’s scoped authorization instead of a shared administrator identity.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Separate read-only tools from mutating tools so a read request cannot be upgraded by changing a parameter.
  • Constrain resources, tenants, regions, and destinations in the tool definition and enforce the same limits downstream.
  • Use short-lived credentials and distinct identities for different workflows.
  • Remove unused extensions and disable dynamic tool registration unless it is itself governed and logged.
  • Rate-limit expensive or dangerous operations and require a stronger review path as impact rises.

These controls reduce the consequences of a prompt injection, memory poisoning event, compromised tool server, or faulty model output. OWASP describes monitoring and rate limiting as ways to limit damage and identify undesirable activity, not as substitutes for preventive authorization.

Build detections around high-signal behavior

Begin with a normal baseline for each workflow. A customer-support agent, deployment agent, and data-analysis agent have different call sequences and volumes, so one global threshold will create either noise or blind spots. Tune rules against observed sessions, then test that the alert reaches the on-call path with enough context to investigate.

Scope and destination violations

Alert when a tool requests a resource outside the session’s tenant, region, project, repository, or data classification. Flag unexpected network destinations, attempts to access credential files, and bulk reads that exceed the workflow’s normal shape. Normalize aliases and redirects before comparison so an encoded or indirect destination cannot evade the rule.

Privilege and approval anomalies

Detect elevation from a low-privilege identity, repeated denied calls, approval-bypass attempts, reused or expired approval identifiers, and a sudden change in the approval-to-execution ratio. OWASP’s monitoring guidance specifically calls out approval drift, bypass attempts, and privilege elevation as useful signals.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Invocation and configuration anomalies

Watch for unusual tool-call frequency, sudden increases in high-risk actions, recursive tool use, new or changed tool servers, and modifications to instruction files or CI configuration. A new MCP server or extension should generate a review event before it is allowed to handle production data.

Illustrative thresholds in security examples are not validated universal defaults. Set limits from workflow baselines, document the rationale, and revisit them after material changes. The available guidance does not establish a single threshold or a measured incident-reduction rate for any control combination.

Protect the audit trail and sensitive data

Send events to a central logging or security system outside the agent’s control. An agent that can edit or delete its own audit records cannot support reliable forensics. Restrict write access, separate operational logs from immutable audit storage, and monitor attempts to tamper with either.

Do not place credentials, access tokens, private keys, or full sensitive payloads in plain text logs. Redact or tokenize arguments while preserving the resource identity, operation, and policy-relevant boundary. The guidance establishes the need to avoid secret values but does not prescribe one redaction algorithm or retention period; choose those according to your organization’s data-protection and incident-response policies. Record which redaction rule ran so investigators know what information was intentionally removed.

Protect the logs themselves with access controls and encryption. Limit who can view sensitive arguments, and retain the minimum data needed for audit, detection, and legal obligations. A shorter retention period may be appropriate for high-volume payload traces, while durable event metadata and change identifiers may need longer retention for investigations.

Compare implementation patterns

There is no single architecture that fits every production agent. Evaluate each pattern against coverage, identity propagation, and the ability to stop a call before execution.

Pattern Strengths Risks and design checks
Application or tool wrapper Sees the exact tool arguments and can create an approval interruption immediately before execution Every side-effecting tool must use the wrapper; direct or newly added paths can bypass it
Downstream service enforcement Provides complete mediation even when multiple clients or agents call the same service The service must receive trustworthy actor identity, normalized parameters, and policy context
Central gateway or proxy Offers a common logging, rate-limiting, and policy layer across tools and destinations Out-of-band paths, non-HTTP actions, or opaque payloads may escape coverage; preserve end-to-end identity
Narrow tools with least privilege Reduces blast radius and makes expected call sequences easier to baseline Too little task coverage can push teams toward unsafe workarounds; expand scopes deliberately
Risk-based deterministic review Focuses human attention on irreversible or high-impact actions and binds approval to exact parameters Requires clear classifications, timeout behavior, and protection against approval reuse
Centralized audit and alerting Supports correlation, incident response, policy-version tracking, and tamper resistance Requires secret redaction, access control, and an operational process for alert triage

The Agent Control Standard resource from OWASP describes a framework-portable approach based on middleware hooks and runtime policy enforcement. That description does not establish product maturity or independently measured effectiveness, so treat it as an architectural reference rather than a performance claim.

  1. Create a benign session and verify that identity, session, tool, policy version, result, and change evidence correlate across every component.
  2. Generate a denied call for an out-of-scope resource. Confirm that execution does not occur, the denial is logged, and the alert includes the attempted target.
  3. Generate a high-risk call. Verify that the gate pauses execution, presents exact parameters to the reviewer, and resumes only after a valid approval.
  4. Change one parameter after approval and confirm that the old approval cannot authorize the modified call.
  5. Simulate an anomalous sequence, such as repeated credential-file reads or a sudden burst of high-risk operations, and verify alert delivery and triage context.
  6. Cause a downstream failure and confirm that the event records the failure, retry behavior, and whether any partial change occurred.
  7. Attempt to alter or delete an audit record using the agent’s available permissions and verify that the action is prevented and itself recorded.

Include abuse cases named in OWASP’s agent-security testing guidance: prompt override, tool misuse, privilege escalation, memory poisoning, data exfiltration, recursive tool abuse, approval bypass, and multi-agent chaining. Record expected results, observed approvals, denials, timeouts, circuit-breaker behavior, and residual risks with compensating controls.

Operate the system as an incident capability

An alert is useful only if responders can act. Route high-confidence policy violations and destructive-action attempts to the team that owns the affected system. Include the session, initiating identity, agent identity, tool, normalized target, policy version, approval record, downstream result, and related events in the alert.

Prepare containment actions that do not depend on the agent cooperating: revoke its short-lived credentials, disable a tool or connector, block a destination, pause the workflow, or require manual execution for a class of operations. Preserve the central events and downstream audit records before changing access, then reconstruct the sequence from the initiating request through the final system state.

After containment, determine whether the issue was a malicious input, an overly broad permission, a policy gap, a tool defect, an approval-process failure, or an instrumentation blind spot. Update the policy and test case, then rerun the relevant abuse scenarios before restoring autonomy.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Common failure modes and fixes

  • Logging only prompts and final answers: add tool-call, authorization, approval, and downstream-result events.
  • Trusting a model-generated approval flag: validate approval identity, exact parameters, policy version, expiry, and single use at execution.
  • Putting all checks in a top-level guardrail: place checks around every custom side-effecting tool, including delegated-agent paths.
  • Using a shared administrator credential: issue least-privilege identities and propagate the initiating actor.
  • Recording raw secrets for convenience: redact or tokenize sensitive arguments and retain policy-relevant context instead.
  • Applying one alert threshold everywhere: baseline each workflow and tune thresholds with replayable tests.
  • Assuming monitoring prevents damage: combine telemetry with downstream authorization, narrow tools, rate limits, and deterministic approval gates.
  • Ignoring configuration changes: alert on new tools, changed instruction files, policy edits, and CI changes that can alter agent behavior.

A practical rollout checklist

  • Inventory every agent, tool, connector, delegated agent, identity, and side effect.
  • Classify actions by impact and reversibility; mark which require explicit approval.
  • Define the event schema and correlation identifiers before instrumenting individual tools.
  • Implement downstream authorization and parameter validation outside model reasoning.
  • Wrap each side-effecting tool and verify that no alternate path bypasses the wrapper or gateway.
  • Send structured events to centralized, access-controlled storage outside agent control.
  • Redact secrets while preserving target, operation, identity, policy, and result context.
  • Create baseline and anomaly rules for scope, destination, privilege, frequency, approval, and configuration changes.
  • Exercise denied, approved, anomalous, failed, and tamper-attempt scenarios.
  • Document containment, credential-revocation, and rollback procedures and assign alert ownership.
  • Repeat the tests after changes to prompts, tools, memory, retrieval, policies, models, or providers.

What monitoring can and cannot prove

Monitoring can show what an agent attempted, which policy and approval decisions applied, and what the downstream system recorded. It supports detection, audit, and forensics. It cannot by itself prove that an agent will never take an unsafe action or guarantee a particular reduction in incidents.

The reviewed security guidance provides practical controls but no primary comparative study showing that one architecture or control combination reduces incidents by a specific percentage. It also provides no universal alert threshold. Treat effectiveness as workload-specific: measure coverage, false positives, missed scenarios, response time, and the proportion of side-effecting paths that are actually mediated.

“Log and monitor the activity of LLM extensions and downstream systems to identify where undesirable actions are taking place, and respond accordingly.”
