Reliable AI-agent monitoring needs two connected loops. The first records what happened in every production run: model generations, tool calls, handoffs, guardrails, application steps, latency, errors, token use, and cost. The second turns representative runs into repeatable evaluations so prompt, model, tool, and routing changes can be compared against expected behavior.
A final answer and an uptime check are not enough. An agent can return fluent text after choosing the wrong tool, skipping a handoff, violating a policy, or silently increasing cost. Trace inspection explains the failure; task-specific graders and regression datasets show whether a fix continues to work.

What effective agent monitoring actually covers
Monitor an agent as a workflow, not as a single model response. A user-visible task may include several model requests, retrieval operations, tool executions, retries, specialist handoffs, safety checks, and application-side transformations. Each stage can introduce a different failure, so the monitoring record must preserve the sequence.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Use two loops:
- Production observability: collect a complete trace and operational signals for each run, then investigate anomalies while the evidence is available.
- Evaluation and regression testing: grade representative tasks, maintain a dataset of expected behavior, and rerun it after changes.
Tracing is primarily for understanding. Evaluations are for comparison. Keeping those purposes separate prevents a dashboard metric from being mistaken for proof that an agent completed its task correctly.

Instrument a complete run
Create one root trace for the user-visible task and child spans for every meaningful operation. OpenAI’s Agents SDK describes this pattern as recording LLM generations, tool calls, handoffs, guardrails, and custom events during an agent run. Your implementation should capture equivalent detail whether you use that SDK, another framework, or your own telemetry.
Recommended trace structure
| Trace element | What it should identify | Why it matters |
|---|---|---|
| Root trace | User request, run ID, workflow name, deployment version, start and end times | Connects the user-visible outcome to the exact release and execution. |
| Model span | Provider/model, prompt or message version, response status, latency, token counts, finish reason | Shows whether a model response or a prompt change caused the divergence. |
| Tool span | Tool name and version, validated arguments, start/end time, status, response metadata | Distinguishes wrong tool selection from a tool that returned bad or slow data. |
| Handoff span | Source agent, destination agent, handoff reason, context passed, result | Exposes missing, premature, or looping specialist transitions. |
| Guardrail span | Check name, decision, policy version, blocked or allowed status | Shows whether a safety control caused the result or failed to act. |
| Application span | Retrieval, database, queue, formatter, or other important application step | Prevents an integration defect from being misdiagnosed as model behavior. |
| Custom event | Business milestone such as confirmation sent, refund created, or escalation opened | Connects technical events to the actual task outcome. |
Attach stable metadata
Include a stable run identifier in logs, traces, tool records, and user feedback. Add workflow name, agent version, prompt version, model identifier, tool versions, environment, tenant or region where appropriate, and the evaluation-case identifier when a run comes from a test dataset. Version metadata lets you ask whether a regression appeared after a particular deployment rather than searching through unlabelled transcripts.
Record timestamps and duration for every span, along with success or failure status. Preserve the minimum input and output needed to diagnose behavior. Define redaction rules before collecting payloads: remove credentials, payment data, personal identifiers, secrets, and unnecessary conversation history; restrict who can view unredacted content; and set retention periods that match your policy.

OpenAI JavaScript Agents SDK deployment details
If you use the OpenAI JavaScript Agents SDK, verify its runtime behavior instead of assuming tracing is universal. The documentation says tracing is enabled by default in supported server runtimes, disabled by default in browsers and when NODE_ENV=test, and unavailable to organizations using OpenAI APIs under Zero Data Retention. In runtimes without an automatic export loop, queued traces must be flushed before the process exits. These are SDK-specific conditions; other telemetry systems may differ.

Watch operational health separately
Operational metrics reveal service degradation and workload changes. They do not prove that an agent made the right decision, so pair them with task-quality evaluations.
| Signal | Useful breakdowns | Questions it answers |
|---|---|---|
| Request volume | By workflow, tenant, region, model, and release | Did traffic or workload mix change, making other metrics look different? |
| Completion and error status | Model, tool, handoff, guardrail, timeout, and application error classes | Where are runs stopping, retrying, or returning partial results? |
| Latency | p50, p95, and p99 for full runs and individual spans | Is a slowdown in the model, a tool, a handoff, or your integration? |
| Token consumption | Input and output tokens by model, workflow, and run | Did context growth, retries, or a routing change increase usage? |
| Cost | Estimated cost by model, workflow, tenant, and release | Which behavior changes are financially material? |
| User or reviewer feedback | Ratings, corrections, escalations, and refunds linked to run IDs | Do technical signals correspond to problems people actually notice? |
Latency percentiles, error rates, token usage, cost breakdowns, and feedback scores are common dashboard measures. They are examples, not universal requirements. Set alert thresholds from your workflow’s baseline, service-level objectives, and risk tolerance; the available documentation does not establish a single pass rate, latency limit, or sample size that applies to every agent.
Evaluate behavior, not just availability
A run can be fast, inexpensive, and free of exceptions while still failing the user. Define checks for the outcomes that matter in your workflow.

Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Turn requirements into graders
Use task-specific graders for conditions such as:
- the correct tool was selected and its arguments were valid;
- the required handoff occurred, with the necessary context;
- the answer contains required fields or calculations;
- the agent followed policy and refused disallowed requests;
- the task reached the intended business state, not merely a plausible response;
- the result was grounded in permitted data and did not expose sensitive content.
A grader may be a deterministic assertion, a schema validator, a comparison against an expected state, a human rubric, or a model-based judge. Document what each grader can and cannot establish. A judge that scores style cannot prove that a payment was actually issued, and a field-level assertion cannot assess whether the advice was safe.

Start with traces, then build a dataset
Use trace inspection while the workflow is still being understood. OpenAI’s evaluation guidance distinguishes trace grading for workflow-level debugging from repeatable dataset-based evaluation. Examine representative successes, failures, edge cases, and near misses; write down the expected behavior; then add those cases to a versioned dataset.
Run the dataset after prompt, model, tool, retrieval, routing, guardrail, or application changes. Compare grader results and distributions across runs when the agent is stochastic. Exact text matching is appropriate only when the task genuinely requires fixed wording; otherwise grade facts, actions, constraints, and outcomes. There is no universal dataset size or pass-rate target. Choose enough cases to cover important paths and set thresholds from the risk of your workflow and its observed baseline.
Read transcripts, not only scores
Anthropic’s evaluation guidance warns that a low score can reflect a faulty grader rather than a failed agent. Review the complete transcript and the grader’s reasoning or evidence. If a valid result was rejected, improve the grader or task specification. If the grader accepted an invalid result, add a stronger check and a clearer example. Sustained investment in evaluation tasks and graders is part of monitoring, not cleanup after launch.

- Define the task contract. Write the allowed actions, required outputs, policy constraints, success state, and escalation conditions for each workflow.
- Create the root trace. Generate a run ID at request start and attach workflow and release metadata.
- Instrument every meaningful span. Record model calls, tool calls, retrieval, handoffs, guardrails, retries, and application milestones with duration and status.
- Apply data controls. Redact sensitive payloads, limit access, choose retention, and document whether data leaves your deployment boundary.
- Build operational views. Chart volume, completion and error classes, latency percentiles, tokens, cost, and linked user feedback. Break down anomalies by version and component.
- Review representative traces. Sample normal runs as well as failures, inspect the first point of divergence, and verify that the final business state matches the response.
- Create and run evaluations. Convert confirmed expectations and incidents into dataset cases with task-specific graders; run them before and after each material change.
- Close the loop. For a confirmed regression, fix the prompt, model, tool, route, guardrail, or integration; rerun the affected cases; then retain the incident as a permanent regression test.
Diagnose failures by the first divergence
The model chose the wrong tool
Inspect the model span’s available-tool context, selected tool, and arguments. Check whether a prompt or routing change altered the choice, and whether the tool description made competing actions distinguishable. Add a case that requires the correct choice and a grader that checks both tool identity and argument validity.

The tool returned an error or misleading result
Compare the tool span’s validated request, authorization context, response status, timeout, and returned data with the model’s subsequent reasoning. A successful HTTP response can still contain stale or incomplete business data. Record typed error classes and make the agent’s retry, fallback, or escalation behavior observable.
A handoff was missing, repeated, or premature
Read the handoff sequence and the context passed between agents. Confirm that the source agent had the information needed to decide and that the destination agent received the task contract. A loop usually points to an unclear completion condition, while a missing handoff may indicate routing logic or a classifier change.
A guardrail blocked a valid task or allowed an unsafe one
Compare the guardrail decision, policy version, input presented to the check, and downstream action. Review false positives and false negatives as separate evaluation cases. Do not infer policy compliance from a polite final answer; grade the decision and the resulting action.
The grader reports a failure that the transcript does not support
Reproduce the grader decision against the full trace and a human-reviewed label. If the expected behavior was ambiguous, clarify the task and update the dataset. Treat grader quality as an observable component with its own review and version history.
Best Value
Make regressions durable
Store each evaluation case with an input, relevant context, expected outcome or rubric, grader version, and provenance such as incident, support ticket, or expert-authored example. Tag cases by workflow, risk, language, tool path, and failure mode so coverage gaps are visible.
Use separate sets for development, release checks, and monitoring. Keep difficult historical incidents in the regression set even after they stop occurring. When a change improves one metric but harms another, inspect the affected traces rather than accepting a single aggregate score. For nondeterministic behavior, compare score distributions and failure categories across repeated runs; report the sampling conditions alongside the result.
Choosing a monitoring and evaluation platform
Select a platform by the workflow it must support, not by the number of dashboard widgets. Verify current capabilities and data terms because vendor features and deployment choices change.
| Decision axis | Questions to ask |
|---|---|
| Trace depth | Can it represent nested model calls, tools, retrieval, handoffs, guardrails, retries, and custom application events? |
| Evaluation lifecycle | Does it support manual trace review, graders, versioned datasets, repeatable runs, experiment comparison, and online evaluation where needed? |
| Stack fit | Does it integrate with your agent framework, language runtime, existing logs, and OpenTelemetry pipeline? |
| Data control | Are managed, bring-your-own-cloud, or self-hosted deployments available in the regions and retention model you require? |
| Operational workflow | Can engineers query traces, correlate alerts with releases, collect feedback, and route incidents to owners? |
How commonly described options differ
Braintrust describes a combination of offline evaluation, production observability, and experiment tracking. LangSmith describes tracing, offline and online evaluations, dataset management, framework interoperability, OpenTelemetry support, and managed, bring-your-own-cloud, and self-hosted deployment options. Langfuse is described as a self-hosted open-source alternative. Arize Phoenix and AX are described as open-source tracing and evaluation tools with a SaaS option. These descriptions are starting points for a technical review, not guarantees that every current plan includes every feature.
Privacy, reliability, and rollout checklist
- Payload minimization: capture enough content to diagnose behavior, but redact secrets and unnecessary personal data before export.
- Access control: separate broad operational visibility from permission to read transcripts or tool payloads.
- Retention: define deletion and legal-hold rules for traces, evaluation inputs, grader outputs, and feedback.
- Export reliability: ensure queued telemetry is flushed during graceful shutdown and that dropped spans are counted.
- Test-mode coverage: verify that tracing is actually enabled in the environment where regression tests run.
- Versioning: version prompts, tools, policies, graders, datasets, and routing rules so a comparison has a known cause.
- Human escalation: provide a path for reviewers to label ambiguous cases and feed confirmed incidents back into the dataset.
- Ownership: assign an owner for each alert and failure class; an unowned dashboard is not an operational control.
A small event schema to standardize early
Even when you later adopt a hosted or self-managed platform, a consistent internal event model keeps telemetry portable. A run record should be able to answer these questions without joining unrelated logs:
- Which user-visible task and release produced this trace?
- What was the ordered sequence of model, tool, handoff, guardrail, and application events?
- What inputs and outputs were redacted, retained, or omitted?
- Which grader and dataset case evaluated the run?
- What business outcome and reviewer feedback followed?
Use correlation IDs across services, record explicit status values instead of inferring them from free text, and preserve parent-child relationships between spans. That structure makes it possible to locate the first divergence and to compare the same workflow across releases.
Putting the system into practice
Start with one high-value workflow and instrument it end to end. Establish a baseline from real traces, define a small set of outcome-focused graders, and add every confirmed incident to a versioned regression dataset. Expand coverage as new tools, handoffs, policies, and failure modes appear. The durable standard is not a particular vendor or dashboard: it is the ability to explain an individual run, detect changed behavior across a population, verify that the grader is trustworthy, and prove that a fix survives the next release.
