Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteBefore an AI agent can touch real systems, define what it may do, log the actions it takes and their effects, and give an authorized operator a way to interrupt it. A chat transcript alone may not show which tools ran, what data they changed, or what happened in the surrounding environment. Least privilege, independent checks for consequential actions, and bounded scope limit harm; monitoring and recovery cannot replace those safeguards.
What to monitor during and after an agent run
Monitor the agent while it is active and retain records for later investigation. Connect each tool call to the identity that initiated it, the target resource, the requested operation and parameters, the applicable authorization or policy decision, any required approval, the result, and a timestamp. Microsoft’s guidance recommends recording tool inputs and outputs, identity, and decision rationale.

Do not rely on the agent’s own account of what happened. NCSC guidance calls for telemetry from both the agent and its operating environment, which can include access logs, proxies, and network traffic. Where possible, protect records against alteration or deletion; NCSC recommends immutable logs when they are available.

Use a connected event record
A useful record should let an incident reviewer follow an action from request to effect. Link the agent’s activity to the human or service identity behind it, the credentials and authorization used, any approval, the target, and the resulting state change. Preserve the tool inputs and outputs in a way that supports investigation while respecting your organization’s data-handling requirements.

Observe effects beyond software logs
Not every tool has the same audit trail. NIST’s August 5, 2025 article on lessons from a tool-use workshop notes that some tools can be monitored through existing logs or transcripts, while others require new ways to observe the effects of tool-enabled actions. Assess each capability for its function, access pattern, criticality, reversibility, reliability, modality, observability, and degree of autonomy. If an agent can control a robot or otherwise affect the physical world, software logs may need to be supplemented with observation of those environmental effects.
Alert on actions and state changes
Alerts are most useful when they identify a consequential action or change, not just text that seems unusual. The following are implementation patterns to adapt to the agent’s permissions and environment; the cited guidance does not prescribe a universal alert taxonomy.
- A tool call targets a resource, destination, or capability outside the task’s permitted scope.
- An agent attempts a write, deletion, or other state change when the task was authorized for reads only.
- Actions arrive in an unexpected burst, repeat without progress, or appear to form a loop.
- A request uses an unexpected identity or destination, or an approval is denied.
- The agent attempts another action after an operator has issued a stop signal.
Set alert severity according to the possible impact and reversibility of the action, not merely how unusual it looks. For example, an unexpected read and an unauthorized destructive change should not automatically receive the same response.

Prevent unsafe actions before execution
Monitoring helps you detect and investigate activity; it does not make an over-privileged agent safe. OWASP’s 2025 guidance on excessive agency and NCSC guidance both support limiting access and checking actions outside the model’s own judgment.
Limit identity, tools, and scope
Give an agent only the functions, data, and permissions needed for its assigned task. Prefer narrowly scoped credentials with the shortest practical lifetime, avoid broad standing identities, and restrict access to external systems. Where possible, do not expose credentials directly to the model or agent. Keep the task’s permitted resources and actions bounded so a mistake cannot automatically spread to unrelated systems.
Gate actions by impact
Classify actions according to their likely impact and how difficult they are to reverse. A low-impact read may need a lighter control than an externally visible message, an administrative change, a financial operation, or a destructive action. OWASP recommends action previews, explicit approval for high-impact or irreversible operations, and independent validation by the component that executes the action.

| Action class | Example | Possible execution gate |
|---|---|---|
| Lower impact and reversible | Reading an authorized record | Permit within the defined scope and log the call and result. |
| State-changing or externally visible | Editing a record or sending a message | Require a policy check; use human review where the context or potential impact warrants it. |
| High impact, destructive, or hard to reverse | Administrative, financial, or destructive operation | Require explicit approval or an independent authorization decision before execution. |
This is a practical way to apply risk tiers, not a universal classification. Bind approval to the exact actor, tool, target, parameters, time, and expiry so that an approval for one action cannot be reused for a materially different one. If the action cannot be classified or its authorization cannot be validated, fail closed rather than guessing.

- Stop the agent process. Use the operator-controlled control point defined for the deployment.
- Restrict further access. Revoke or narrow the agent’s credentials and disable affected tools as needed.
- Contain surrounding connections. Restrict network access or communications with model infrastructure if the incident requires it.
- Confirm containment. Check relevant access and environment telemetry for activity after the stop signal.
NCSC’s guidance on agentic AI advises planning for shutdown that may require more than stopping a process. Test the containment path before connecting an agent to consequential systems, so operators know what the controls actually disable.
Investigate the action and recover system state
Recovery depends on the systems an agent can reach; there is no universal rollback workflow for every tool or environment. NCSC, Microsoft, and OWASP guidance supports interruption, incident response, protected logs, and rollback controls, but restoration must use the recovery mechanisms appropriate to the affected systems.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- Preserve evidence. Retain the relevant audit records and environment telemetry before they are rotated or altered.
- Establish the scope. Identify the agent, human or service identity, credentials, tools, targets, and time period involved. Determine which systems, records, or other state may have been affected.
- Contain ongoing access. Stop further activity and restrict credentials, tools, or connections before attempting restoration.
- Compare with a known-good state. Use an appropriate source of truth to determine what changed and what must be restored.
- Restore and validate. Use the affected system’s established recovery mechanism, then check both the restored state and its permissions.
- Review before resuming. Re-enable the agent only after an authorized reviewer has confirmed that restoration is sound and the cause of unsafe activity has been addressed.
Assign ownership and decide whether autonomy is appropriate
Before deployment, name the system owner, the person or team that approves access, the monitor on duty, the incident reviewer, and the person empowered to stop the agent. Include agent activity in existing security operations monitoring and incident response. NCSC recommends having an incident plan for failures, misuse, and loss of control, and starting with bounded, low-risk pilots before expanding autonomy.
Responsibility does not disappear when a vendor hosts part of the service. Microsoft’s shared-responsibility guidance varies across IaaS, PaaS, and SaaS, but lists human approval for high-impact actions as a customer responsibility in all three models. Treat that matrix as general guidance: check the configuration and terms for the actual service.
When comparing monitoring approaches, assess whether they cover tool calls and resulting state changes; integrate identity and authorization; support real-time alerting and approval; protect retained events; expose sandbox and outbound network activity; and provide usable stop, credential-revocation, and recovery controls. Also consider who operates each control and how the agent can affect state. NIST highlights reversibility and impact alongside permissions as risk-analysis dimensions. Its AI Agent Standards Initiative, created February 17, 2026, and updated August 14, 2026, describes voluntary standards and ongoing identity and security-evaluation activity; it is not a finished mandatory standard.
