Repair Windows errors before they cause bigger problemsFix Now →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Use two controls together: enforce every proposed tool call before it reaches a business system, and preserve an end-to-end record that shows who or what acted, under which authority, on which data, with what result. Give each agent a distinct identity, deny access by default, validate actions outside the model, require time-limited human approval for high-impact changes, and alert when behavior leaves its baseline. Monitoring can reveal an unauthorized write; only an enforcement point that blocks the write can prevent it.
What monitoring can—and cannot—do
AI agents can read records, call APIs, modify tickets, change configuration, send messages, create users, or trigger financial and operational workflows. The risk is not limited to a model producing a wrong answer. A tool call can make a real change before a person notices the agent’s reasoning.

Monitoring supplies evidence and detection. It answers questions such as which agent ran, which human or workflow delegated authority, what the agent proposed, which policy decision was made, what tool and target were used, and what state changed downstream. Monitoring by itself does not stop an action. A complete log of an unauthorized write does not undo that write.
Prevention requires a control at the execution path: an allowlist, policy service, gateway, orchestrator, or destination system that can deny the operation before the change is committed. The strongest design makes the agent propose an action, has an independent component validate it, obtains approval when required, and only then executes the normalized request.

How do I give an AI agent an identity that can be audited?
Create one identity per agent or tightly defined workflow
Do not let multiple agents share a broad service account. Assign each production agent a distinct identity that can be authenticated, scoped, monitored, suspended, and revoked. Record the owner, business purpose, model and version, tools and plugins, data sources, downstream systems, environment, and lifecycle status for that identity.
A distinct identity lets an authorization system answer whether this particular agent is allowed to perform this particular operation. It also preserves attribution when several agents run in the same orchestration platform. Connect the agent identity to the human, service, or workflow that delegated authority, but keep the agent principal visible in every record.
Make the identity lifecycle explicit
- Register: document the owner, purpose, model, tools, data classes, target systems, and proposed action boundary.
- Approve: have security, system owners, and the business owner accept the initial permissions and risk tier.
- Issue: create credentials or delegated tokens that identify the agent and expire or rotate under normal policy.
- Review: re-evaluate permissions when the model, prompt, tool, plugin, destination, or workflow changes. Version material changes.
- Suspend or revoke: provide an operator action that immediately stops new runs and invalidates outstanding authorization.
- Decommission: disable credentials, remove schedules and webhooks, archive the audit trail, and record the retirement decision.
Microsoft guidance on reducing autonomous-agent risk treats the agent, its tools, and its connected systems as one security boundary. A component that is added without review can silently expand what the identity is able to do.

How can I stop an agent from changing business data without permission?
Write an action boundary before granting access
For each workflow, define the objective and the exact boundary in plain language and machine-readable policy. Include:
- Permitted tools and API methods.
- Target resources, tenants, records, environments, or accounts.
- Allowed operations, such as read, create, update, approve, publish, or delete.
- Fields and parameter ranges the agent may change.
- Data classes it may read or transmit.
- Rate, volume, time-window, and geographic restrictions where relevant.
- Explicitly prohibited actions, including attempts to reach an unlisted endpoint or broaden its own scope.
Start with deny-by-default. Add one permission at a time and test the resulting boundary. Task-specific access is safer than a role that happens to include every operation used by a larger team.
Reduce blast radius through isolation
Separate development, test, and production identities and destinations. Isolate agents that handle unrelated business functions. Restrict network egress to approved services, and keep secrets out of prompts and ordinary logs. If one agent is manipulated, isolation limits the systems and data it can reach while responders investigate.
Use deterministic action schemas
Define a schema for every tool operation. A useful request contains a stable operation name, target resource, normalized parameters, data scope, requested effect, and correlation identifier. Reject requests with missing fields, unknown operations, extra parameters, invalid types, or values outside policy.
{
"operation": "update_invoice_status",
"target": "invoice/…",
"parameters": {"status": "pending_review"},
"data_scope": "accounts_payable/region_eu",
"correlation_id": "run-…"
}
The schema is not a prompt instruction. It is an input contract enforced by code. Normalize values before authorization so that aliases, alternate encodings, or reordered parameters cannot cause the approval decision to differ from the operation that executes.
Why the model must not be the final authorization check
An agent can classify its own request as safe, but its explanation is not proof of permission. The model may misunderstand a policy, follow a malicious instruction in retrieved content, or be induced to call a tool outside its assigned task.

Place an independent policy or execution component between the agent and the destination. For every proposed operation, it should verify:
- The authenticated agent and any delegated human or workflow principal.
- The current, unexpired permission scope for that identity.
- The operation and target resource.
- Normalized parameters, affected fields, data classification, and volume.
- Whether the action is high impact, destructive, financial, administrative, or externally visible.
- Whether a required approval exists, is bound to this exact request, has not expired, and has not already been used.
- Whether policy and configuration versions are still current.
Only an allow decision from this layer should release the call. The destination system should enforce its own authorization as a second line; a gateway log is not a substitute for downstream denial.
When should an AI agent need human approval?
Classify actions by consequence
Require explicit approval outside the agent’s reasoning for operations that can materially affect money, access, legal or regulatory records, production availability, customer-visible content, sensitive data, or irreversible state. Examples include deleting records, changing payment details, granting administrative roles, publishing externally, rotating a production credential, or executing a migration.
Low-risk read operations and reversible changes may be automated when their scope is narrow and independently enforced. The classification belongs in policy, not in a free-form model response.
Bind approval to the action that will execute
An approval should identify the agent, delegated principal, tool, target resource, normalized parameters, requested operation, approver, timestamp, and expiry. If any material element changes, require a new approval. Do not approve a general intention such as “handle the invoice queue” and then allow the agent to choose a different target or amount later.
Use short-lived authorization artifacts for irreversible operations and reject replay. A one-time approval token should be consumed when the action executes, with a recorded result. If execution fails and a retry would produce a different effect, send the changed request through authorization again.

The review interface should show the normalized operation, affected records, before-and-after values where available, policy reason, and expected downstream effect. The reviewer must be able to deny, expire, or revoke the approval. An approval button that merely records an event while the agent can call the destination independently is not a control.
What should AI agent audit logs capture?
Capture the full path from plan to downstream outcome. The minimum record set is:
| Record element | What to capture | Why it matters |
|---|---|---|
| Identity and delegation | Agent identifier, human or workflow principal, roles, delegated authority, credential or token identifier | Attributes responsibility and supports revocation |
| Run context | Run or correlation ID, parent run, environment, timestamps, model and policy versions | Connects events across orchestrator, tools, and systems |
| Plan and decision | Relevant plan or decision context, input references, allow/deny result, policy rule, approval requirement | Shows why an operation was attempted and how it was evaluated |
| Tool invocation | Tool or API, operation, target resource, normalized parameters, data scope, request and response identifiers | Reconstructs the exact action rather than a vague narrative |
| Approval | Approver, approval artifact, bound action details, issue time, expiry, consumption state | Proves that consent covered the operation that ran |
| Outcome | Allow, deny, timeout, execution result, downstream state change, error, rollback, and recovery result | Distinguishes a blocked attempt from a completed change |
| Security signals | Repeated failures, policy bypass attempts, unusual endpoint or parameter use, data-volume and rate indicators | Feeds detection and response |
Preserve evidence without creating a new data leak
Logs should be protected against tampering and unauthorized deletion. Restrict who can alter retention settings, separate audit writers from investigators, and retain enough history to support incident response and regulatory obligations. Use integrity controls appropriate to the environment, such as append-only storage, access separation, and monitored export.

Do not copy secrets, full credentials, or unnecessary personal and business data into every event. Redact or tokenize sensitive values while retaining a stable reference, field names, hashes, sizes, and decision-relevant metadata. Record enough input, output, and decision context to reconstruct the sequence. NIST IR 8596, the Cybersecurity Framework Profile for Artificial Intelligence, is an initial preliminary draft dated December 2025; it discusses protecting logs from tampering and preserving inputs, outputs, and decision chains. Treat it as draft guidance, not a final standard.

Build a behavioral baseline for each workflow
A useful baseline is specific to an agent and task: normal tools, endpoints, resources, operation types, query shapes, parameter ranges, call rates, data volumes, time windows, and approval frequency. Baselines should account for scheduled peaks and legitimate changes rather than treating every deviation as an incident.
Alert on meaningful deviations
- A new tool, endpoint, tenant, environment, or data class.
- Expansion from read to write, or from one record to bulk operations.
- Parameters outside the approved range, such as an unusually large amount or recipient set.
- Repeated authorization failures, malformed requests, or attempts to bypass an approval gate.
- Unusual query patterns, extraction volume, or output destinations.
- Calls made with stale, mismatched, or unexpectedly delegated credentials.
- Changes to the agent’s prompt, tool definition, policy, model, or runtime configuration followed by new behavior.
Correlate telemetry across the orchestrator, policy layer, tool gateway, and destination application. A signal visible only in one layer can miss the point at which the state actually changed.
Route alerts to people who can contain the action
An alert should identify the run, principal, target, operation, policy decision, and current downstream state. Give responders practical actions: suspend the agent, revoke or expire credentials, stop the run, disable a webhook, block an endpoint, require manual review, and roll back a supported transaction. Define escalation for an action that cannot be reversed.
An end-to-end control architecture
Use the following sequence for every tool call:
- Receive: the agent proposes a typed operation with a run and correlation ID.
- Authenticate: the execution layer verifies the agent identity and delegated authority.
- Normalize: canonicalize the operation, target, parameters, and data scope.
- Authorize: a deterministic policy check evaluates identity, scope, operation, target, parameters, rate, and current configuration.
- Approve: if the risk class requires it, obtain a time-limited approval bound to the normalized request.
- Execute: call the allowlisted tool or destination with the narrow credential and record the request identifier.
- Verify: confirm the downstream result and state change, not merely that a gateway returned success.
- Record: write an integrity-protected event containing the decision, approval, result, and recovery status.
- Detect: compare the event with the workflow baseline and alert on deviations.
Input and output filters remain useful for untrusted content and sensitive data, but they do not replace authorization. A prompt-injection defense that blocks suspicious text while leaving a powerful, unvalidated tool available is incomplete.

A practical implementation sequence
1. Inventory and assign ownership
List every agent, owner, model, tool, plugin, data source, identity, schedule, and downstream system. Mark which components can read, write, delete, publish, administer, or move money. Record a responsible owner for each destination system.
2. Define the action boundary
Write the objective, permitted tools, target resources, allowed operations, data scope, limits, and prohibited actions. Start with no permissions, then add and test each required capability. Separate environments and workflows to limit blast radius.
3. Intercept before execution
Implement typed action schemas, tool allowlists, independent policy checks, and destination-side authorization. Make the deny path fail closed when identity, policy, or approval information is unavailable.
4. Add approval gates
Set risk thresholds for destructive, financial, administrative, and externally visible operations. Bind approvals to exact normalized actions, apply short expiries, prevent replay, and verify that the destination cannot be called around the gate.

5. Build the audit trail
Emit events from the orchestrator, policy service, tool gateway, and destination application. Use one correlation ID and preserve decision, approval, execution, outcome, and recovery records. Protect retention and minimize sensitive content.
6. Establish baselines and response
Measure normal use for each workflow, configure anomaly signals, and route alerts to responders who can stop runs and revoke access. Document who is on call and how a downstream change is reversed.
7. Validate before expanding autonomy
Test prompt injection and other untrusted inputs, unauthorized tool calls, approval bypass, stale credentials, modified tool definitions, excessive data access, and downstream visibility. Verify that a blocked request is denied where the business state would change, not merely recorded at an agent gateway.

Exercise attack and failure cases
- Feed the agent instructions that attempt to override its task or expose hidden tool parameters.
- Request an operation outside its allowlist and confirm that no destination-side state changes.
- Change a target, amount, field, or recipient after approval and verify that authorization is repeated.
- Replay an old approval artifact and confirm rejection.
- Expire or revoke credentials during a run and confirm that subsequent calls fail closed.
- Replace or modify a tool schema and verify that registration and review are required.
- Generate unusual volume or endpoint use and confirm alert delivery and triage data.
Prove the recovery path
For each high-impact operation, document the operator who can suspend the agent, invalidate tokens, stop queued work, disable integrations, and restore supported prior state. Test the sequence with system owners. Record what cannot be rolled back, such as an external message already delivered, so responders know when containment must focus on downstream notification and access revocation.

How to compare agent-security approaches and tools
Evaluate categories such as agent identity and access management, policy enforcement or API controls, and security-event monitoring or agent observability against the same questions:
| Evaluation axis | Questions to ask |
|---|---|
| Enforcement point | Can it block the call before the destination changes, or does it only observe? |
| Action granularity | Can policy constrain operation, target, fields, parameters, data scope, rate, and identity? |
| Identity and delegation | Can activity be attributed to a distinct agent and linked to the human or workflow that authorized it? |
| Approval integrity | Can high-risk actions require approval bound to the exact normalized request, with expiry and replay protection? |
| Traceability | Does one correlation record follow the run through orchestration, tools, and downstream applications? |
| Detection and response | Can teams establish baselines, detect deviations, suspend runs, and revoke access quickly? |
| Audit integrity and privacy | Are records protected against tampering while secrets and unnecessary sensitive data are minimized? |
| Operational cost | What engineering effort, review friction, policy maintenance, and ongoing monitoring staffing are required? |
Microsoft’s guidance highlights real trade-offs: deterministic controls and oversight add engineering work and can slow a workflow, while governance and observability require continuing investment. No neutral comparative product test establishes that one category or implementation is universally superior.
Common failure modes and their fixes
Shared credentials hide responsibility
Symptom: logs show a generic service account and investigators cannot identify the agent or delegating principal. Fix: issue distinct identities, preserve delegation context, and make revocation possible per agent.
Approval covers an intention, not an operation
Symptom: a reviewer approves a task while the agent later changes the target, amount, or fields. Fix: normalize the request and bind approval to the exact actor, tool, target, parameters, timestamp, and expiry.
Best Value
The gateway records a denial but the destination still changes
Symptom: an alternate integration, cached token, or direct API path bypasses the monitoring layer. Fix: enforce destination-side authorization, restrict network paths, and test the blocked path end to end.
Logs contain plans but not outcomes
Symptom: the team can see what the model intended but not whether a record changed. Fix: capture downstream request identifiers, resulting state, errors, rollback, and recovery status.
Alert volume obscures the serious deviations
Symptom: responders receive generic “agent ran” events and ignore them. Fix: baseline each workflow, alert on meaningful deviations, include target and impact in the alert, and tune thresholds with system owners.
Free tools Windows power users keep installed
One-click scans. No signup required.
Logs become a second sensitive-data store
Symptom: prompts, credentials, or full customer records are copied into broad-access telemetry. Fix: redact or tokenize secrets and personal data, retain decision-relevant metadata, and separate audit access from routine operations.
What the controls cost—and where to place the effort
Expect three recurring costs. First, engineering teams must build identity registration, schemas, policy evaluation, approval workflows, correlation, and destination verification. Second, reviewers add human friction to high-impact work; narrow risk thresholds and clear review displays reduce unnecessary interruptions without removing the gate where it matters. Third, security and operations teams must maintain baselines, investigate alerts, rotate credentials, review changes, and test recovery.
Spend first on the paths that can cause irreversible or high-impact harm. A read-only agent with a small data scope needs less oversight than an agent that can alter production configuration or approve payments. Reassess the boundary whenever a tool, model, prompt, plugin, endpoint, or business process changes.
A concise operating checklist
- Every agent has a unique, auditable identity and named owner.
- Permissions begin at zero and include only task-required tools, data, operations, and targets.
- Action requests use strict schemas and are normalized before authorization.
- An independent policy or execution layer checks identity, scope, target, parameters, and approval.
- High-risk and irreversible actions require an expiring approval bound to the exact request.
- Destination systems enforce authorization and cannot be reached through an unmonitored bypass.
- Logs capture plan, decision, identity, delegation, tool call, approval, outcome, downstream state, and recovery.
- Audit records are tamper-resistant and avoid unnecessary secrets or sensitive content.
- Baselines cover tools, endpoints, resources, parameters, rates, volumes, and approval patterns.
- Responders can suspend runs, revoke credentials, stop queued work, and contain downstream changes.
- Red-team tests cover prompt injection, bypass, replay, stale credentials, tool changes, and recovery.
Sources and evidence boundaries
This control model follows the OWASP AI Agent Security Cheat Sheet; Microsoft Learn guidance on securing autonomous agentic AI systems, reducing autonomous agentic AI risk, and least privilege for AI agents; Microsoft Security guidance published June 30, 2026 on agents moving from reading to acting; and NIST IR 8596, the December 2025 initial preliminary draft Cybersecurity Framework Profile for Artificial Intelligence. Microsoft Security material is vendor guidance, not an independent product evaluation. The cited material provides design recommendations rather than a quantified, general effectiveness result.
The Bottom Line
Prevent unauthorized agent changes at an independent, deny-by-default execution gate; make approval specific and short-lived; and preserve tamper-resistant, end-to-end evidence through the downstream state change. Monitoring without enforcement is detection after the fact, not protection.
